자동차 바퀴에 의한 소아 아래다리의 압궤 손상 (Car-tire-related Crushing Injury of the Lower Leg in Children)

  • Purpose: Crushing injuries by car tires result from a combination of friction, shearing, and compression forces and the severity of injury is influenced by the acceleration. Because car-tire injuries of the lower leg in children are common these days but they have received little attention; thus, our purpose was to look closely into this problem. Methods: A retrospective analysis was conducted of data from children under 15 years old age who visited an emergency department because of a car-tire-related crushing injury to the lower leg in pedestrian traffic accident from January 2008 to September 2012. The patient's age, sex, site of injury, degree of injury, associated injuries, type of surgery, and complications were reviewed. Results: There were 39 children, the mean age was 8.0 years, and 71.8% were boys. The dorsal part of the leg was involved most frequently. According to the severity classification, 15 children were grade I, 6 were grade II, and 18 were grade III. Among 24 patients, 13 were treated with skin graft and 3 were treated using a sural flap. Twelve patients developed complications, such as hypertrophic scarring, contractures, and deformities with significant bone loss. Conclusion: Various degrees of skin or soft tissue defects were caused in children by car tires. In this study, patients were often also had tendon or bone damage. Proper and timely initial treatments are needed to reduce the incidence of infection, the number of operative procedures, and the hospital stay.

  • 본 사례는 중형차 시장의 선도 브랜드인 기아자동차 K5가 어떻게 중형차 시장에 성공적으로 포지셔닝 하였는지를 분석하고 있다. 기아자동차가 K5를 출시하기 전까지 쏘나타와 SM5는 각 브랜드를 선호하는 고객들의 지지를 기반으로 중형차 시장에서 1, 2위를 다투면서 오랫동안 소비자의 사랑을 독차지해 왔다. 경쟁사들이 지속적으로 새 브랜드나 신 모델을 출시함에 따라 쏘나타와 SM5도 새로운 버전의 차를 출시해야 한다는 압박을 받게 된다. 하지만, 시장의 새로운 트랜드를 따라 잡기위해 출시된 YF 쏘나타는 기존 디자인에 비해 너무나 달라 이질감을 느끼게 하였으며, New SM5는 달라진 것이 거의 없는 구형차 라는 이미지로 받아들여졌다. 두 브랜드가 고객들을 제대로 만족시키지 못하고 있을 때, 기아자동차 K5는 중형차 소비자를 만족시킬 수 있는 제품에 대한 기회를 포착하고 중형차 시장에서 현재 제공되는 가치와 소비자들이 원하는 가치의 차이를 채워줌으로써 준중형 고객의 업그레이드 수요와 중형차 대체 수요를 만족시키는 성과를 거두었다. K5는 고객들이 원하는 가치를 찾고 그 가치를 성공적으로 제안하였다. 본 사례는 포화상태에 있는 자동차 산업에서 성공적인 포지셔닝 전략을 통해 신 시장을 개척하는 과정을 잘 보여주고 있다. 기아자동차 K5의 성공적인 포지셔닝 전략을 크게 디자인 혁신, 시장분석, STP, 그리고 출시전략의 4가지 축으로 설명하고 있다. 기아자동차 K5의 성공적인 포지셔닝 전략은 신 시장 개척을 위한 포지셔닝 전략을 계획하고 있는 많은 기업에게 의미 있는 시사점을 제공해 준다.

  • THE CARBODY AND BOGIE FRAME OF AN URBAN RAILWAY VEHICLE CONSIST OF MULTI-LAYERED WELDING STRUCTURE. IN KOREA ENDURANCE LIMIT OF AN URBAN RAILWAY VEHICLE IS STSTED IN THE RULE OF MANAGING URBAN RAILWAY VEHICLE UNDER THE LAW OF URBAN RAILWAY. IN KOREA AN URBAN RAILWAY VEHICLE IS DESIGNED AND MADE TO KEEP ITS QUALITY OVER 25 YEARS. WHEN THE RAILWAY VEHICLE BECOMES 25 YEARS OLD, CORROSION OF CARBODY AND UNDER FRAME OF A RAILWAY VEHICLE IS EVALUATED ACCORDING TO THE NON-DESTRUCTIVE TESTING. IT CAN BE USED AS LONG AS 40 YEARS. IT IS STATED IN THE ARTICLE 4 'THE METHOD AND STANDARDS OF PRECISE DIAGNOSIS' UNDER THE RULE OF MANAGING RAILWAY VEHICLE IN KOREA. SO, IN THIS STUDY, WE HAVE INVESTIGATED PERFORMANCE OF PULSED EDDY CURRENT TESTING METHOD BY MEASURING THICKNESS VARIATION OF FABRICATE OF CARBODY AND UNDER FRAME FOR URBAN RAILROAD CAR. AND THEM, THE PROCESS OF EVALUATING REMAINING LIFE ACCORDING TO TESTING OF CORROSION AMOUNT IS INTRODUCED.

A Study on Annoyance of Interior Noise on Town-Bus

  • In these days, the size of urban is growing and the function of city becomes complicated. And also, in city, people lives a lot. The life of urban is getting closer and linked with neighboring people in many parts. Especially, when peoples are exposing during using public transportation, even though does not be known, in they were living. Seoul is the most crowded place in Korea. In Seoul,The village buses have been serviced to the narrow streets. And people who use this bus, wants to seek the comfort of the ride, air quality and noise during in vehicle. In this paper, we determine the degree of annoyance with the noise inside the town bus in dB scale. And, such a situation was confirmed annoyance see their effect. The Interior noise did not see a big difference in the new car and the old car. Annoyance but also according to the skill of the bus driver remains the difference was confirmed.

  • 저자들은 시야 결손 등의 시력 변화를 주 증상으로 내원한 환자에서 소세포 폐암과 이로 인한 부종양성증후군으로 동반된 암 관련 망막증을 진단하고 항암화학요법을 시행하여, 원발 병변의 부분 관해 보였으나, 시야 결손은 회복되지 않았던 1예를 경험하였기에 문헌 고찰과 함께 보고 하는 바이다.

  • 최근 철도정책이 KTX 개통 이후 속도 향상, 편의성 개선, 역 중심의 공간창출 등 이용자 서비스 중심으로 전환하는 중에 있다. 이는 철도의 차별화 서비스 전략으로 녹색교통수단의 입지를 강화하고자 하는 데 있다. 이러한 철도 유틸리티 개선 노력에도 불구하고 국민의 자동차 의존적 교통행동양식 때문에 자동차에서 철도로의 전환에 한계가 있다. 본 논문에서는 수도권 통근자 대상으로 자동차 의존성을 분석하였다. 자동차 의존성 분석결과 환경의식은 대중교통만족도와 함께 자동차 의존성에 영향을 미치는 중요한 변수로 작용하였고, 이는 철도 서비스 개선과 함께 환경의식을 이용한 사회마케팅이 자동차의존성을 약화시킬 수 있는 정책대안이 될 수 있다 것을 확인하였다.

  • Currently, research on more convenient and safer cars for drivers and passengers, the intelligent cars, are being actively conducted. The researches involve designing systems that intelligently minimize physical damages caused by car accidents, not just pursuing passive safety measures like airbags or safety belts. However, there are many cases around the world where airbags cannot be activated in times of accidents and it is difficult for consumers to check if their airbags will be activated, especially for the old-style cars. Thus, in this paper, a base technology for an application that can determine whether the airbags will actually be actuated at a critical moment based on the mechanical calculations is being introduced.

경기도 운행차 저공해화 사업의 성과분석 및 발전방안 (Evaluation and Improvement of Diesel Retrofit Program in Gyeonggi-do)

  • The purpose of this study is to evaluate the diesel retrofit program which have been proceeded for the last 7 years in Gyeonggi-do. Diesel retrofit programs in Gyeonggi-do consist of the attachment of DPF (Diesel Particulate Filter), p-DPF (partial-DPF) and DOC (Diesel Oxidation Catalyst), switching to LPG engine, scraping an old cars. Emission reduction by diesel retrofit program was estimated $N_2O$ 8,313.4 ton/year, PM10 3,626.4 ton/year, VOC 8,078.5 ton/year in 2010. The benefit-cost analysis shows that the 964 billion wons of benefits are greater than the 853 billion wons of total costs. Diesel retrofit programs could be one of the most effective measures to improve PM10 concentration in metropolitan area. But retrofit programs also need to be properly maintained by each vehicle.

  • T-cell-based cancer immunotherapies, such as immune checkpoint blockers (ICBs) and chimeric antigen receptor (CAR)-T-cells, have significant anti-tumor effects against certain types of cancer, providing a new paradigm for cancer treatment. However, the activity of tumor infiltrating T-cells (TILs) can be effectively neutralized in the tumor microenvironment (TME) of most solid tumors, rich in various immunosuppressive factors and cells. Therefore, to improve the clinical outcomes of established T-cell-based immunotherapy, adjuvants that can comprehensively relieve multiple immunosuppressive mechanisms of TME are needed. In this regard, recent studies have revealed that metformin has several beneficial effects on anti-tumor immunity. In this mini-review, we understand the immunosuppressive properties of TME and how metformin comprehensively enhances anti-tumor immunity. Finally, we will discuss this old friend's potential as an adjuvant for cancer immunotherapy.

교통사고 소아환자 121례를 통한 임상적 고찰 (The Clinical Study on 121 Traffic Accident Child Patients)

  • Objectives The purpose of this study was to find out the characteristics of children patients who had been treated in the Oriental Medical Hospital due to traffic accidents. Methods The study was carried out with 121 children patients who visited the Oriental Medical Hospital from April 2011 to March 2012 and was analyzed by reviewing the patients' charts. Results In the distribution of sex the ratio between boys and girls were similar, but regarding the age, 0~6 year-old group was more than that of 7~15 year-old group. In the traffic accident patterns, 87.6% of the entire children patients were injured by car crash. The highest type in the collision between cars was rear-ending impact. The most patients (48.8%) visited our hospital for oriental medical treatment in less than 3 days after the traffic accident. The number of the first visitors in the Oriental Medical Hospital were 77 (63.6%) and those who visited other western hospitals before and were 44 (36.4%). The most frequent duration of admission and treatment in OPD was less than 7 days. Sleeping disorders (37.1%) were diagnosed the highest in 0~6 year-old group and C-spine sprain (49.0%) was diagnosed the highest at 0~7 years-old group. Herb medications were prescribed to relieve anxiety for 0~6 year-old group and for 7~15 year-old group, those were prescribed to activate blood eliminating phlegm and to normalize Qi flow. The highest effective treatment period was less than 7 days. Conclusions This data suggested that the oriental medical care could be more effective in treating children victims of traffic accidents by confirming many different symptoms depending on the age.
