Descriptions of Four New Species of Actinolaimoidea (Dorylaimida:Nematoda) from Korea (한국산 Actinolaimoidea상과의 (Dorylaimida:Nematoda) 4신종 기재)

  • Four new species of predatory nematodes belonging to the Actinolaimoidea, Dorylaimida are described. Neoactinolaimus gosungensis n. sp. is 2.2-2.4 mm long, b = 4.3-4.9; c = 10.7-11.2; species 60-62 ${\mu}{\textrm}{m}$ long and is distinguishable by low lip region, asymmetrical odontostyle and its wider lumen. Egtitus arcuatus n. sp. is 1.5-1.8 mm long, b = 3.6-4.2; c = 14-15 and is characterized by having dorsally arcuate odontostyle and presence of cardiac glands at base of oesophagus. Paractinolaimus tuberculantus n. sp. is 1.9-2.5 mm long, b = 3.3-4.0, c = 7.2-8.8, spicules 66-69 ${\mu}{\textrm}{m}$ long and is unique in having coiled body posture and a tubercle at the base of oesophagus. Carcharolaimus koriensis n. sp. is 1.7-1.9 mm long, b = 3.6-4.5, c = 65-76 and is characterized by having very thick body cuticle and larger odontostyle aperture.

Psychometric Analysis of a Persian Version of the European Organization for Research and Treatment of Cancer OG25 Quality of Life Questionnaire in Oesophagogastric Cancer Patients

  • Background: Health-related quality of life (HRQL) is a fundamental outcome in oncology patients and quality of life (QOL) assessment requires clinically relevant questionnaires. The purpose of this study was translation and definition of measurement properties and the clinical validity of Quality of Life Questionnaire (QLQ)-OG25 module in Persian patients with oesophagus, oesophagogastric junction (OGJ) or gastric cancers. Materials and Methods: The translation procedure followed European Organization for Research and Treatment of Cancer (EORTC) guidelines. Both EORTC QLQ-OG25 and a core questionnaire (EORTC QLQ-C30) were administered to patients with oesophagus (150), OG junction (93) and gastric (32) cancer undergoing multi-modal treatments. Convergent and discriminant validity, Cronbach's alpha coefficient and known-groups comparisons were used to examine reliability and validity. Results: In all, 275 patients (mean age 62 years) completed both questionnaires. Compliance rate was high and the questionnaire module was well accepted. We found good reliability for multi-item subscales of QLQ-OG25 (Cronbach's alpha coefficients 0.76-0.89). About 73% had TNM staging and scales distinguished between clinically distinct groups of patients. However, patients in palliative group experienced compromised functional status and worse treatment-associated symptoms than those in the potentially curative group. Test-retest scores were consistent. Multi-trait scaling analysis demonstrated good convergent and discriminant validity. Conclusions: Overall, the Persian version of QLQ-OG25 demonstrated psychometric and clinical validity that supports its application as a supplement to the original tool (EORTC QLQ-C30) when assessing HRQL in patients with upper-gastrointestinal (GI) cancer both in curative and palliative phases.

Auto-segmentation of head and neck organs at risk in radiotherapy and its dependence on anatomic similarity

  • Purpose: The aim is to study the dependence of deformable based auto-segmentation of head and neck organs-at-risks (OAR) on anatomy matching for a single atlas based system and generate an acceptable set of contours. Methods: A sample of ten patients in neutral neck position and three atlas sets consisting of ten patients each in different head and neck positions were utilized to generate three scenarios representing poor, average and perfect anatomy matching respectively and auto-segmentation was carried out for each scenario. Brainstem, larynx, mandible, cervical oesophagus, oral cavity, pharyngeal muscles, parotids, spinal cord, and trachea were the structures selected for the study. Automatic and oncologist reference contours were compared using the dice similarity index (DSI), Hausdroff distance and variation in the centre of mass (COM). Results: The mean DSI scores for brainstem was good irrespective of the anatomy matching scenarios. The scores for mandible, oral cavity, larynx, parotids, spinal cord, and trachea were unacceptable with poor matching but improved with enhanced bony matching whereas cervical oesophagus and pharyngeal muscles had less than acceptable scores for even perfect matching scenario. HD value and variation in COM decreased with better matching for all the structures. Conclusion: Improved anatomy matching resulted in better segmentation. At least a similar setup can help generate an acceptable set of automatic contours in systems employing single atlas method. Automatic contours from average matching scenario were acceptable for most structures. Importance should be given to head and neck position during atlas generation for a single atlas based system.

Relationship between EGFR Over-expression and Clinicopathologic Characteristics in Squamous Cell Carcinoma of the Esophagus: A Meta-analysis

  • Over-expression of epidermal growth factor receptor (EGFR) has been identified as a common feature associated with clinical outcome in many types of cancer, including squamous cell carcinoma of the oesophagus (SCCO). However, the clinical importance of EGFR over-expression in SCCO remains unsettled as conflicting results exist. Therefore we carried out the present meta-analysis of published studies for clarification. A total of 13 studies including 1, 150 patients were enrolled. EGFR over-expression was positive in 722 of these cases. With EGFR over-expression, patients had higher depth of invasion, vascular invasion, and poor prognosis. However, expression had no relation with degree of differentiation, histological grade, lymph node metastasis, clinical stage or lymphatic invasion. EGFR over-expression is probably a valuable predictor for the T stage, vascular invasion and OS, and it could be used as a poor prognosis indicator for the esophageal SCC patients. Targeting therapy to EFGR should be considered to the combined treatment in SCCO.

Identification and prevalence of trematode(Gen Azygia) and nematode(Gen Pingus) in Ophicephalus argus (가물치의 위장내 기생하는 흡충(Azygia속) 및 선충(Pingus속)의 동정과 감염상황)

  • A survey of the internal parasites in 115 fishes of Ophicephalus argus was carried out in Kyungpook province during the period from January 1995 to November 1997. A species of trematode from the stomach and a species of nematode from the caeca of the fishes was discovered respectively. The trematode was identified as Azygia hwangtsinyi (Tsin, 1993) and the nematode as Pingus sinensis ($Hs\ddot{u}$, 1993). The fluke was pressed at thick of 0.1mm~0.2mm between slide glasses and fixed in 70% ethyl alcohol solution. The fluke was washed with tap water after fixation and stained with hematoxylin-carmin and mounted in balsam through routine methods. The nematode was fixed in 5% formalin solution and mounted with lactophnol or glycerine jelly. Morphology : Arygia hwantsinyi ; The fluke is elongate body with approximately parallel margins and rounded extremities in pressed preparations(Fig 1). The cuticle is unarmed. The oral sucker is ventro-subterminal in position. The ventral sucker is slightly smaller than oral sucker and positioned at 14.2% of body length from the oral sucker. The cirrus sac and genital pore is in mid-ventral line, slightly anterior to the ventral sucker. The oral sucker is succeeded by a well developed muscular pharynx. It is succeeded by a short inverted Y-shaped oesophagus. The inner wall of oesophagus is consisted of many fine folded membrane. The fine tubes of esophagus pass into the intestinal heads which are distended and presented a marked borderline between the fine tube and intestinal heads. The Intestinal heads start at level of pharynx and pass caudad in a zigzag course to end quite near caudal margin. The uterine coils occupy the intercecal area between the level of the cephalic margin of the ovary and about the level of the ventral sucker. The uterus is filled with many eggs. The testes are round or oval and placed one behind the other generally a little diagonally. The posterior testis is placed at level of 75.1% of body length. The ovary is oval, placed ahead the anterior testis. The shell gland not discernible outline is contacted with ovary ahead it. The vitelline glands are consisted of small oval vesicles. The vesicles occupy extracecal regions and between a little posterior of the ventral sucker and near caudal margin. Pingus sinensis : Female ; The cuticle is smooth and the cervical alae are well developed. The mouth opening is small, triangular, and without lips. The anterior end bears four sub-medial papillae. The oesophagus is divided into a muscular and glandular portion. The nerve ring is situated just at the junction of the muscular and glandular portions of the oesophagus. The vulva which has a prominent flap, is placed behind the middle of the body about one-third of the length of the worm from the posterior extremity. The vagina is strongly muscularised and after running anteriorly about $68{\mu}$, divides itself into two broad uterine tubes with an anterior and a posterior one. There are a few eggs in the uterine tubes. The tail is slender, straight and pointed. The ovary extends almost to the posterior extremity. Male : Male is smaller than female. The tail is pointed, curved and carrying well-developed caudal alae. A prominent muscular precloacal sucker is characteristic. The spicules are equal and short. There are four pairs of pedunculated pre-anal papillae, of which the most anterior pair are situated just at the level anterior to the muscular sucker. Two pairs of pedunculated papillae are present before the anus. There are five pairs of post-anal papillae of which the most anterior pair and the second counted from the posterior end are ventral and more marked than the ones which are situated laterally.

Ultrastructure of the digestive tract of Korean Leech (Erpobdella lineata) (한국산 거머리 (Erpobdella lineate) 소화관의 미세구조)

  • This investigation has been carried out to examine the structure of digestive tract from Korean Leech, Erpobdella lineata, using light and electron microscope. The digestive tract is composed of mouth, pharynx, Oesophagus, six-chambered stomach, three-chambered intestine, rectum and anus. Stomach and intestine have not gastric or intestine ceca and consist of only straight tube. All digestive tracts from pharynx to rectum are covered with simple columnar epithelial cells. While the surfaces of endothelial cell of pharynx and rectum are covered with cuticular layer of about $0.3{\mu}m$ in thickness, stomach and intestine are covered with estimated $0.2-0.3{\mu}m$ and $0.5{\mu}m$ microvilli respectively. Circular folds were found only in first and second chambers of stomach, intestine and rectum, but not in pharynx and the other chambers (third to sixth) of stomach. The granules of $0.3-0.8{\mu}m$ and $0.5-1.0{\mu}m$ in diameter were observed in the cytoplasm of stomach endothelial cell. These granules were demonstrated to contain protein which showed a positive reaction to ninhydrin. It was also found that there are well-developed microvilli in the apical portion of intestine endothelial cell in which endocytosis occurs actively.

  • Acrobeles ciliatus (von Linstow, 1877) and Plectus parietinus (Bastian, 1865) are newly collected from Korea. Acrobeles is similar to those nematode belongs to Rhabditidae and Panagrolaimidae by having terminal bulb without medium bulb but different by having complex, vines shaped labial attachment. A. ciliatus differed from other Acrobeles species by lateral fields with two incisures, primary axils U-shaped, and secondary axils V-shaped. Head region set off with the neck, three high labial probolae present, each having 5 tines at inner margins and 5-7 at outer margins, and two elongate apical tines. Also, it differs from the longer female body. Plectus parietinus is distinguished from all other species of Plectus by the prominent hypodermal glands, the relatively small amphid, the distinct and well set-off lips and by the subdorsal distal caudal seta on the left side of the tail. Stoma without stylet. Oesophagus about 1/4 of total body length with terminal bulb. Anus ca. $100{\mu}m$ from posterior extremity. Vulva is at middle of body. Have spinneret on tail tip.

Processed Meat Consumption and Squamous Cell Carcinoma of the Oesophagus in a Large Case-Control Study in Uruguay

  • Background: The role of processed meat in the aetiology of squamous cell oesophageal cancer has been explored in detail. Methods: In the time period 1990-2005, a case-control study was conducted in Montevideo, Uruguay including 2,368 participants (876 cases of oesophageal cancer and 1,492 controls). Relative risks, approximated by the odds ratios, were estimated by multiple unconditional logistic regression. Results: Processed meat was positively associated with oesophageal cancer (upper quartile vs lower quartile OR 2.30, 95%CI 1.72-3.07), whereas salted meat intake was positively associated with squamous cell oesophageal cancer (OR 3.82, 95%CI 2.74-5.33). Finally other cured meats were positively associated with oesophageal cancer (OR 1.65, 95%CI 1.22-2.22). Conclusions: It could be concluded that processed meat consumption could be an important risk factor for the aetiology of squamous cell oesophageal cancer in Uruguay.

Calculation of Dose Conversion Coefficients in the Anthropomorphic MIRD Phantom in Broad Unidirectional Beams of Monoenergetic Photons (MIRD 인형팬텀의 넓고 평행한 감마선빔에 대한 선량 환산계수 계산)

  • The conversion coefficients of effective dose per unit air kerma and equivalent dose per unit fluence were calculated by MCNP4A code for antero-posterior(AP) and postero- anterior(PA) incidence of broad, unidirectional beams of photons into anthropomorphic MIRD phantom. Calculations have been performed for 20 monoenergetic photons of energy ranging from 0.03 to 10 MeV. The conversion coefficients showed a good agreement with the corresponding values given in the draft publication of joint task group of ICRP and ICRU within 10%. The deviations may arise from the differences of geometry in the MIRD phantom and the ADAM/EVE phantoms, and the differences in the codes and cross-section data used. Inclusion of a specific oesophagus model results in effective dose slightly different(5% at most) from the effective doses obtained by adopting the equivalent doses for the thymus or pancreas. Deletion of the ULI from the remainder organ appeared not to be significant for the cases of photon dosimetry covered in this study.

Esophagectomy with Operating Mediastinoscope (종격동경을 이용한 식도절제수술 -1례-)

  • Esophagectomy has a high morbidity rate, mainly related to pulmonary complications. The respiratory morbidity of open esophagectomy is high, ranging from 6% to 10%. This high morbidity is partially responsible for the 6∼15% mortality rate of esophagectomy. Many techniques of esophagectomy without thoracotomy have been described since the initial report of Orringer and Sloan. Endoscopic microsurgical dissection of the esophagus was clinically introduced in 1989. Endoscopic microsurgical dissection of the esophagus was developed as a minimally invasive procedure that avoids thoracotomy and provides precise vision during the operation in order to reduce mediastinal trauma and to improve the peri- and post-operative situation. A 20 year-old women who accidentally swallowed about 150 cc of glacial acetic acid underwent an esophagectomy using the operating mediastinoscopy, cervical esophagogastric anastomosis, pyloromyotomy, and feeding jejunostomy tube placement for esophageal stricture. The postoperative course was uneventful and the patient was discharged on the 17th postoperative day. Our clinical experience shows that endoscopic microsurgical dissection of esophagus is a safe and feasible method because it offers excellent optical control and enables the surgeon to operate in a minimally invasive manner.
