자동긴급제동장치의 고령운전자 추돌사고 감소 효과 추정 (Estimation of Road Crash Reduction by Installing Automatic Emergency Braking Systems for Elderly Drivers)

  • 고령 운전자는 다른 연령대에 비해 사망사고 유발 가능성이 높다. 2021년 교통사고 통계에 따르면 고령 운전자 1만 명당 유발한 사망자수가 1.77로 30대 운전자 0.55에 비해 2.67배 높다. 본 연구는 자동긴급제동장치(Automatic Emergency Braking System, AEBS) 설치로 고령 운전자의 추돌사고 유발 가능성을 얼마나 낮출 수 있는지 파악하기 위해 시도되었다. 이를 위해 자동차보험 데이터를 활용하여 자동긴급제동장치가 장착한 차량이 그렇지 않은 차량에 비해 얼마나 추돌사고 발생률이 낮은지 통계적 기법으로 분석하였다. 그 결과 자동긴급제동장치를 장착한 차량과 그렇지 않은 차량의 교통사고 유발 오즈비가 0.75에 그쳐 사고감소 효과가 분명히 있는 것으로 나타났다. 성별로는 남성 운전자가 0.78로 여성 운전자 0.81에 비해 사고감소 효과가 컸다. 연령별로도 65세 이상의 오즈비가 0.76으로 분석되어 교통사고 감소 효과가 있는 것으로 나타났다. 특히 남성 고령운전자가 0.49로 감소효과가 가장 높았다. 향후 고령운전자를 대상으로 자동긴급제동장치 장착 차량을 운전할 경우 보험료 할인 등 혜택을 부여한다면 추돌사고를 예방하는데 도움이 될 것으로 보인다.

A Proportional Odds Mixed - Effects Model for Ordinal Data

  • This paper discusses about how to build up mixed-effects model for analysing ordinal response data by using cumulative logits. Random factors are assumed to be coming from the designed sampling scheme for choosing observational units. Since the observed responses of individuals are ordinal, a proportional odds model with two random effects is suggested. Estimation procedure for the unknown parameters in a suggested model is also discussed by an illustrated example.

인구집단의 비교를 통한 폐금속광산 지역 주민의 카드뮴 노출수준 및 건강영향평가 (Health Impact Assessment for Cadmium Exposure: Comparison of Residents around Abandoned Mines with the General Population)

  • Objective: We compared the level of blood cadmium exposure with health impacts by using data from a survey of residents near an abandoned mine and a national health survey. Methods: For this study, we used data from 7,046 individuals who participated in the Health Effects Survey of Abandoned Metal Mines (AMS, 2008-2011) and 6,871 individuals who participated in the Korea National Health and Nutrition Examination Survey IV-VI (KNHANES, 2008-2013). To evaluate the health impacts, the quartiles (S1 to S4) were classified according to blood cadmium concentration, and then the odds ratios of S2 to S4 over S1 for exceeding the reference values of renal function, blood pressure, and bone density were compared. Similarly, the odds ratio of AMS over KNHANES was confirmed. Results: In the AMS, adjusted for general characteristics, the geometric mean of blood cadmium concentration was 1.34 ㎍/L, which was statistically significantly higher than that of the KNHANES 1.22 ㎍/L (p<0.001). In the integrated data of AMS and KNHANES, the estimated odds ratio of S4 over S1 for exceeding the reference value was found to be 1.70 for serum creatinine, 1.71 for hypertension, and a T-score 2.02 for the tarsal bone. They were all statistically significant. Conclusion: Residents around abandoned metal mines had a higher blood cadmium concentration than the general population, and the odds for exceeding the reference values were higher for some health indicators. Continuous biomonitoring should be conducted for vulnerable areas such as around abandoned metal mines, and measures to reduce cadmium exposure and management of chronic diseases are needed.

  • Background: Many chronic diseases are associated with the lifestyle such as smoking, alcohol drinking and exercise. Attention is increasingly paid on the effect of exercise for the management of chronic disease these days. Objective: The purpose of this study was to evaluate the status of the smoking habit, alcohol drinking and physical exercise of the patients with chronic diseases. Methods: Total 793 persons(normal:422, chronic disease patients:371) in a hospital were questioned regarding the habits of smoking and alcohol drinking, and a practice of physical exercise in 1999. The patients with chronic illness were divided into three groups(Group I included the patients of hypertension and diabetes mellitus. Group II includes respiratory disease and cancer. Group III included liver disease). The rate of exercise, the rate of smoking cessation and the rate of abstinence were assessed. In logistic regression analysis with each independent variables in each disease groups (I, II and III, odds ratio for the presence of disease was controlled for age and education. Results: The rate of exercise, the rate of smoking cessation and the rate of abstinence was 31.5%, 27.3% and 9.5% in the patients with chronic disease, respectively. In control group, those were 31.5%, 21% and 2.1%, respectively. In logistic regression analysis with exercise as independent variable, odds ratios(95% CI for age and education were significantly high, with smoking cessation, odds ratio for age was high and with abstinence, odds ratio for disease with high in all disease groups. Conclusion: It was suggested that an effort for proper changes of lifestyle related to disease such as smoking, alcohol drinking and exercise should be intervened in the patients with chronic diseases.

생애전환 초기 노인의 성별에 따른 낙상경험 및 내재적 요인 비교 (Comparisons of the Falls and Intrinsic Fall Risk Factors according to Gender in the elderly at Ttransitional Periods of Life)

  • 연구목적: 본 연구는 생애전환기를 맞이한 초기노인을 대상으로 낙상경험과 낙상의 내재적 요인을 성별에 따라 비교하였다. 연구방법: 연구대상자는 2008년 1월부터 12월까지 국민건강보험공단에서 실시한 66세 생애전환기 건강진단검사를 받은 수검자 중 자료가 불충분한 것을 제외한 255,505명을 대상으로 2차분석한 서술적 단면연구이다. 연구결과: 생애전환기를 맞이한 66세 초기노인의 낙상경험은 전체 10.6%로 남자 8.5%, 여자 12.4%로 여성노인이 남성노인에 비해 낙상비율이 높았다. 남녀에서 낙상위험에 영향을 미치는 요인은 배뇨장애 6.2-6.6배, 우울 1.5-1.8배, 보행장애 1.3-1.5배, 및 실명 1.3-1.4배 순서로 나타났다. 연구결론: 비록 내적 낙상위험요인이 다양할지라도 효과적인 낙상예방을 위해서 배뇨장애가 중요한 내재적 예측요인이며, 이는 초기노인의 낙상에 영향을 줄 수 있다.

Gender Differences in Marital Disruption among Patients with Cancer: Results from the Korean National Health and Nutrition Examination Survey (KNHANES)

  • Background: Cancer is a major life event that imposes huge economic and mental burdens on patients and families. In addition, the diagnosis of cancer also causes significant family discordance that can lead to marital problems such as divorce or separation. The aim of this study was to investigate the association and any related gender differences between cancer diagnosis and marital disruption among cancer survivors. Materials and Methods: We used the recent cross-sectional Korea National Health and Nutrition Examination Survey ($4^{th}$ and $5^{th}$; Years 2008-2012). The study participants were 623 married cancer survivors over the age of 19. A multivariate logistic regression analysis was conducted to estimate odds ratios. Results: After adjusting for socioeconomic status and health-related behaviors, the odds ratio of marital disruption among female cancer survivors compared with male cancer survivors was 3.94 (95%CI 1.30-11.94; p=0.02). The odds ratio of marital disruption for the below-average economic level compared with the above-average economic level was 5.64 (95%CI: 1.03-31.02; p=0.05). When compared with the non-smoking cancer survivors, the smoking cancer survivors had an OR of marital disruption equal to 2.94 (95%CI: 1.08-8.00; p=0.03). Conclusions: The findings of this study suggest that the odds of marital disruption among female cancer survivors are higher than those among their male couterparts. Medical practitioners should be sensitive to early signs of marital discord in couples affected by a cancer diagnosis. Early identification and psychosocial intervention might reduce the frequency of divorce and separation and thus improve quality of life and quality of care for cancer survivors.

Diseases having an influence on inhibition of angiogenesis as risk factors of osteonecrosis of the jaw

  • Objectives: The objective of this study was to retrospectively investigate the association of diseases having an influence on inhibition of angiogenesis such as hypertension, diabetes mellitus type II, hypercholesterolemia, and rheumatoid arthritis (RA) with the development of osteonecrosis of the jaws. Materials and Methods: The 135 patients were allocated into 4 groups of bisphosphonate-related osteonecrosis of the jaw (BRONJ) group (1A); non-BRONJ group (1B); osteonecrosis of the jaw (ONJ) group (2A); and control group (2B), according to histologic results and use of bisphosphonate. This retrospective study was conducted with patients who were treated in one institute from 2012 to 2013. Fisher's exact test and logistic regression analysis were used to analyze the odds ratios of diseases having an influence on inhibition of angiogenesis for development of ONJ. Results: The effects of diabetes and hypertension were not statistically significant on development of ONJ. When not considering bisphosphonate use, RA exhibited a high odds ratio of 3.23 (P=0.094), while hyperlipidemia showed an odds ratio of 2.10 (P=0.144) for development of ONJ. More than one disease that had an influence on inhibition of angiogenesis showed a statistically significant odds ratio of 2.54 (P=0.012) for development of ONJ. Conclusion: Patients without diseases having an influence on inhibition of angiogenesis were at less risk for developing ONJ.

Outcomes of Critical Pathway in Laparoscopic and Open Surgical Treatments for Gastric Cancer Patients: Patients Selection for Fast-Track Program through Retrospective Analysis

  • Purpose: The aim of this study is to investigate the clinical factors affecting on the cure rate by invasive and open surgery for gastric cancer and to establish a subgroup of patients who can be applied by the early recovery after surgery program through this retrospective analysis. Materials and Methods: In this retrospective study, we analyzed 425 patients who underwent gastric cancer surgery between January 2011 and December 2011 and were managed with conventional clinical therapies. This clinical algorithm was made when the patient was in minimally invasive surgery group and discharged from hospital one day faster than them in open surgery group. Results: The completion rate of the clinical pathway was 62.4%. Despite the different applications of clinical pathway, completion rate in minimally invasive surgery group was significantly higher than that of open group (P<0.001). In multivariate analysis, the surgical procedure of minimally invasive surgery (odds ratio=4.281) was the most predictable factor to complete clinical pathway. Additionally, younger patients (odds ratio=1.933) who underwent distal gastrectomy (odds ratio=1.999) without combined resection (odds ratio=3.069) were predicted to accomplish the clinical pathway without any modifications. Conclusions: We concluded that high efficacy of the clinical pathway for gastric cancer surgery was expected to selected patients through retrospective analysis (expected completion rate=85.4%). In addition, these patients would become enrolled criteria for early recovery program in gastric cancer surgery.

Variation in Meal-skipping Rates of Korean Adolescents According to Socio-economic Status: Results of the Korea Youth Risk Behavior Web-based Survey

  • Objectives: To identify and evaluate the trend of meal-skipping rates among Korean adolescents with their contributing causes and the influence of household income level on meal skipping. Methods: Using 2008, 2010, and 2012 data from the Korea Youth Risk Behavior Web-based Survey of 222 662 students, a cross-sectional study with subgroup analysis was performed. We calculated odds ratios for skipping each meal 5 or more times in a week by household socio-economic status using a multiple logistic regression model. The secular change in the meal-skipping rates by the students' family affluence scale was analyzed by comparing the meal-skipping students within each subgroup and odds ratios for the same event over time. Results: Through 2008 to 2012, most of the meal-skipping rates generally showed a continuous increase or were almost unchanged in both sexes, except for breakfast skipping in several subgroups. Students in low-income households not living with both parents had the highest meal-skipping rates and odds ratios for frequent meal skipping. In a time-series subgroup analysis, the overall odds ratios for the same event increased during 2008 to 2012, with a slight reduction in the gap between low and higher income levels with regard to meal skipping during 2010 to 2012. Conclusions: Household socio-economic status and several other factors had a significant influence on Korean adolescent meal-skipping rates. Although the gap in eating behavior associated with household socio-economic differences is currently decreasing, further study and appropriate interventions are needed.

Status and Determinants of Health Literacy among Adolescents in Guangdong, China

  • Background: Previous studies for non-communicable disease cotrol, including cancer, have mostly relied on health literacy in adults. However, limited studies are available for adolescents. This study aimed to assess the status and determinants of health literacy in in-school adolescents in Guangdong, China. Materials and Methods: A total of 3,821 students aged 13-25 years were selected by multi-stage cluster sampling. After the questionnaire of health literacy was answered, the total scores for health knowledge (18 questions), skills (5 questions) and behaviors (14 questions) were determined. The total scores for health literacy and each subscale were recoded into adequate and inadequate subgroups, and logistic regression models were used to identify factors associated with each outcome variable. Results: The prevalence of adequate health literacy was 14.4%, and the prevalences for adequate knowledge, skills and behavior were 22.4%, 64.7% and 6.6%, respectively. Students coming from prestigious schools and having parents with higher education had higher odds of having adequate knowledge, skills and behaviors. Female students had higher odds of having adequate knowledge and behaviors. Students in grade 7-8 had higher odds of having adequate knowledge and skills. The health knowledge was positive associated with health skills (odds ratio [OR] =2.1, 95% confidence interval [CI] 1.7-2.5) and behaviors (OR=3.0, 95%CI 2.3-4.0), and health skills were positive associated with health behaviors (OR=2.6, 95%CI 1.8-3.8). Conclusions: Further efforts should be made to increase adolescents' health knowledge and behaviors, especially for low grade and male students in non-prestigious schools.
