베체씨 병에서의 동종 이식편을 이용한 대동맥 근위부 치환술 - 1례 보고 - (Aortic Root Replacement with Homograft in Behcet's Disease -A Case Report-)

  • 구강 생식기의 궤양, 안구와 피부 병변을 특징으로 하는 베체씨 병의 예후는 중추 신경계, 위장관, 혈 관계의 합병증에 의존된다. 심장의 병변 특히 대동맥 판막 폐쇄부전증은 매우 드물고, 일반적으로 혈역 학적 불안정성은 개심술을 통하여 치료된다. 그러나 많은 환자에서 인공 판막의 이탈, 판막 주변 누출. 전도 장애, 가성 동맥류 등의 심각한 합병증이 보고되어 왔다. 이런 합병증을 막기 위하여 많은 노력이 있었으며 그중 하나는 동종 이식편읜 사용이었다. 서울대학교병원 흉부외과에서는 베체씨 대동맥염으로 인해 인조 판막 이탈이 있었던 39세 남자 환자에서 동종 이식편을 이용한 대동맥 근위부 치환술을 시도하여 좋은 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다.

Hemifacial Spasm : A Neurosurgical Perspective

  • Hemifacial spasm (HFS) is characterized by tonic clonic contractions of the muscles innervated by the ipsilateral facial nerve. Compression of the facial nerve by an ectatic vessel is widely recognized as the most common underlying etiology. HFS needs to be differentiated from other causes of facial spasms, such as facial tic, ocular myokymia, and blepharospasm. To understand the overall craniofacial abnormalities and to perform the optimal surgical procedures for HFS, we are to review the prevalence, pathophysiology, differential diagnosis, details of each treatment modality, usefulness of brainstem auditory evoked potentials monitoring, debates on the facial EMG, clinical course, and complications from the literature published from 1995 to the present time.

  • Orbital injuries are common with facial trauma. Direct injuries to the globe are not rare but it can result in complications such as chemosis, subconjunctival hemorrhage and hyphema. Periorbital trauma or injuries to the extraocular muscles and blow-out fracture may result in lid edema, ecchymosis & ptosis and diplopia or limitation of ocular motion respectively. Indirect injuries to the optic nerve come up without any injuries but its complication is irreversible and severe such as loss of vision. The aim of this study is to review the literature on blindness or ptosis following facial trauma and present the cases of blindness after facial trauma and ptosis after mandibular fracture without specific clinical findings.

  • Patient with facial paralysis may develop opthalmic complications. Poor eyelid closure, ectopion and lagophthalmos place the patinet at increased risk for development of corneal problems such as epitheilail defects, stromal thinning, bacterial infection, and even perforation. Inilital treatment should be conservative and include the use of ocular lubricants and taping of the lower eyelid into the proper position. Surgical intervention may be required in patients who have failed medical therapy or in whom the facial paralysis is not expected to improve. Gold weight implantation in upper eyelid and lower lid tightening in lower eyelid has become a popular procedure to correct upper eyelid retraction and lower eyelid laxity and to improve corneal coverage. We describe technique for placement of a gold weight in the upper lid, with attention of the maintenance of symmetric eyelid crease and susture canthopexy to correct malpositioned lower eyelid.

Ossifying fibroma in the maxilla and orbital floor: report of an uncommon case

  • Benign fibro-osseous lesions occur when normal bone is replaced by cellular fibrous connective tissue and mineralized structures. One rare type of these lesions is the ossifying fibroma (OF). The aim of this study is to report an unusual case of OF in a 57-year-old female. Physical examination showed facial asymmetry without any tenderness, fluctuation, ocular pain, or ophthalmoplegia. Imaging exams revealed a solid mass involving the left maxilla and orbital floor. Surgical resection was performed without any complications or sequelae, and the histopathological results confirmed OF. Although recurrence is rare in this condition, the patient remains under follow-up.

Late reconstruction of post-traumatic enophthalmos and hypoglobus using three-dimensional implants: a case series

  • Post-traumatic enophthalmos and hypoglobus are common sequelae of facial bone fractures, even after reduction surgery. They are associated with functional and esthetic issues, which may lower the quality of life. These deformities frequently present late, and adequate correction is difficult. We report three cases of late inferior orbital rim reconstructions with three-dimensional printed implants to help resolve these problems. The average duration between the traumatic event and surgery was 3 years and 4 months. One patient was treated with a completely absorbable implant and exhibited satisfactory results until the implant started to biodegrade at 1 year and 9 months after surgery. Two patients were treated with a permanent implant and demonstrated satisfactory results. However, longer follow-up periods were needed. There were no complications such as infection, diplopia, or restriction of ocular motion and the patients were satisfied with the esthetic results.

Gamma Knife Radiosurgery for Cancer Metastasized to the Ocular Choroid

  • Objective : Choroidal metastases (CMs) are the most common intraocular tumor. Management is mainly radiation therapy with goals of pain control and visual improvement. However, many radiation-related complications are reported. Since gamma knife radiosurgery (GKS) for CM was first reported in 1995, few cases have been reported. We report 7 cases of CMs treated with GKS. Methods : From April 2011 to November 2014, 7 patients with CM underwent GKS. Their median age at treatment was 64 years (range, 51-71 years). Four males and three females were treated. Lung cancer was the most common primary pathology, followed by renal cell carcinoma and stomach cancer. Four patients had multiple cerebral lesions and were treated simultaneously for choroidal lesions. The median marginal dose of 20 Gy (range, 15-25 Gy) was administered at the 50% isodose line. Results : Median follow-up period after GKS was 8 months (range, 2-38.3 months). Four patients expired due to underlying malignancy progression. Except for two patients who were not followed with magnetic resonance image after GKS, all patients showed size reduction in the treated lesions, but a new choroidal lesion appeared in one patient and one recurred. Six of seven patients reported subjectively improved visual symptoms. Visual acuity improved in 2 patients, and 2 were stable upon objective examination. One patient showed no improvement in visual acuity, but ocular pain was relieved; another patient showed improved vision and tumor remission, but visual deterioration recurred. Conclusion : GKS was shown to be safe and effective and should be considered for CM treatment.

  • Introduction: The enlargement and deformation of the orbit give rise to a visible enophthalmos. As a consequence, a disturbance of eye motility together with double images is likely to occur. This study examined the degree of enophthalmos according to the extent of orbital wall fracture and volume of herniated orbital tissue in blowout fractures of the medial and inferior orbital wall. Materials and Methods: This study was performed on patients diagnosed with medial and inferior orbital wall fractures at the Department of Oral and maxillofacial surgery, Chonbuk National University Hospital from 2007 to 2009. The patients' age, gender, etiology of fracture and degree of enophthalmos were investigated. The changes in the degree of enophthalomos, diplopia and ocular motility restriction after operation were examined. Results: The degree of enophthalomos increased with increasing extent of orbital wall fracture and volume of herniated orbital tissue. Conclusion: Whether to perform the operation is decided after measuring the extent of the orbital wall fracture and volume of herniated orbital tissue using computed tomography (CT), time for the decision of operation can be shortened. This can cause a decrease in the complications of orbital wall fractures.

나환자의 눈합병증과 외부측정 (Ocular Complication and Dimension in Leprosy Patients)

  • 나환자 111명을 대상으로 외안각 간격(IOCD), 내안각 간격(IICD), 눈꺼풀틈새 폭 (WPF) 그리고 백내장, 각각, 눈꺼풀 이상을 조사한 결론은 다음과 같다. 외안각 간격의 평균치는 낭자 89.8 mm, 여자 88.4 mm이었고, 내안각 간격은 남자 36.9 mm, 여자 35.8 mm이었으며, 눈꺼풀틈새 폭은 남자 52.9 mm, 여자 52.6 mm이었다. 연령에 따른 연화는 외안각 간격과 눈꺼풀틈새 폭이 좁아졌지만, 내안각 간격은 차이가 없었다. 나병의 눈합병증은 각막이상 37.8%, 눈꺼풀 이상 19.4% 그리고 백내장 29.7%으로 각막 병변이 주증상 이었다. 이들 중상은 남, 여 차이는 없지만 나이가 많아짐에 따라 발병빈도가 높아져 실명과 저시력 환자가 증가하였다.

  • Orbital roof and supraorbital ridge fractures are frequently associated with high energy concomitant craniofacial trauma. When a displacement of the orbital roof and supraorbital rim occurs, exploration, stabilization and reconstruction are warranted to limit the ocular complications. The management of fractures involving the frontal sinus must consider the possible need for obliteration or cranialization of the sinus. Many incisions have been described and used to approach these fractures, such as a coronal incision, eyebrow incision, and an incision through the laceration. We report 3 cases of orbital roof and supraorbital ridge fracture patients with a review of the relevant literature.
