The Role of Forensic Engineering in the Diagnosis of Electrocution Fatalities: Two Case Reports

  • The increase in the number of fatal electric accidents over the years has escalated the demand for specialized forensic engineers to determine their relevant technical causes. Likewise, the complexities associated with identifying the causes of electrocution accidents have prompted the General Department of Forensic Science and Criminology at Dubai Police to adopt a new methodology to diagnose electrocution accidents, consisting of an approach that involves medico-legal examination, electrical diagnosis of the evidence, and trace evidence analysis. This paper will discuss the application of the adopted method in further detail by unfolding two case reports. The first report outlines a case in which a worker got electrocuted at a construction site while attempting to turn on a lamp. The second case report involves the death of a technician in a workshop after trying to disconnect a washing machine from its plug. The methodology was utilized during the investigation of both cases, which were attended by the appointed forensic engineers and showed promising results.

건설업에서의 감전재해 감소 방안을 위한 연구 (A Study on the Decreasing Method of Electric Shock Accident in the Construction Industry)

  • Regarding the characteristics of fatal electric shock accident during the past 10 years, fatalities occurred more frequently in July & August, and AM 10-12 PM & 14-16. Also, the electric shock accident is occurred more frequently in the age group from 30 to 40. Especially, the fatal electric shock accident in construction industry covered more than half of total industry. Therefore, it is strongly requested for the protection technology of an electric shock from the low voltage electric path, grounding method and workers. The aim of this study is to propose the policy about the equipment performance standards and/or worker's safety standards to revise the standards for preventing electric shock accidents on safety workings.

  • The military has adopted and implemented risk assessment since 2013 to reduce fatalities and accident rates at construction sites. However, risk assessment is carried out as a voluntary preventive activity and is maintained as a superficial administrative process for owners. Risk assessment must comply with the Occupational Safety and Health Act and the Enforcement Guideline of Risk Assessment. However, the scope of the assessment covers many industries, including manufacturing and distribution, making it difficult to apply to construction sites. As a result, the guidelines are interpreted and applied differently at each construction site. This study aims to improve the risk assessment system so that it is applicable to construction projects and proposes the improvement of the risk assessment system by analyzing the current status of serious accidents in military construction projects.

이동식 크레인 사망재해의 인지공학적 대안 (Alternative Prevention on Human Error of Fatal Injuries by the Mobile Cranes)

  • According to the statistics, occupational fatal injuries by mobile cranes were about 12 per year in whole industrial. Mobile cranes are widely used in various parts of industries to improve the efficiency of the work. However considerable number of fatal injuries happen each year during the operation of the machines. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the mobile crane showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, among the mainly five machines caused the accidents, mobile crane was higher for the second showed 0.6% for number of fatalities compared to number of mobil cranes and for the third showed 11% for number of fatalities compared to number of injuries. Second, main cause of cognitive engineering agenda was visibility, responsibility, affordance. As the measures to prevent accidents before starting operation, alternative revision for the fool proof including visibility, responsibility, affordance etc. for the fool proof measures was proposed. Third, alternative revision as cognitive accident prevention for the fail safe measures was proposed.

사망재해의 원인분석에 관한 연구 (A Study on Analysis of Causes for Fatal Accidents)

  • This study is conducted to analyze exact causes of occupational fatalities and results are as follows : 1) The number of fatalities in establislment which employed less than 50 workers is 605(40.1% ). Age of sixties and seventies is occupied by 10 percent with there being old-aged trend. Fatal workers who were employed less than 3 months are 39.5 percent and fatal workers who were employed more than 10 years are 11.9 percent. Safety training is necessary for newly recruited and long employed workers. 2) Accident type is, in sequence, fall from elevation, traffic accident, disease, contact with electric current, and hazardous condition is, in order, inadequate guard, hazardous method or procedure, public hazard, and agency of accident is, in sequence, temporary structure/building/structure, powered transport vehicle, passenger vehicle, hoisting or lifting apparatus, and unsafe act is, in order, driving error, failure to secure or warn. 3) The results of cross-tabulation for hazardous condition and accident type is, in sequence, traffic accident by public hazard, fall from elevation by hazardous method or procedure, fall from elevation as well as contact with elctric current by inadequate guard. The result of cross-tabulation for agency of accident and accident type is, in order, fall from elevation by temporary structure/building/structure, traffic accident of passenger vehicle and powered transport vehicle, contact with electric apparatus. The result of cross-tabulation for agency of accident and hazardous condition is, in sequence, public hazard of passenger vehicle, hazardous method or procedure of temporary structure/building/structure, publit hazard of powered transport vehicle, inadequate guard of temporary structure/building/structure. The result of cross-tabulation for unsafe act and accident type is, in sequence, traffic accident by driving error, fall from elevation by inattention to footing or surrounding, contact with electric current by secure or warn.

소득수준 대비 산재사망지수 비교를 통한 건설분야 산업재해 분석 및 저감대책 (A Comparison of Income Level and Work-Related Fatalities for Finding Causes and Measures for Construction Sector)

  • 소득수준이 높으면 이와 함께 사회 경제 문화적 기대수준도 함께 향상되는 것이 일반적이며, 산업안전에 대한 기대수준도 함께 높아질 것이다. 그러나 우리나라는 소득수준은 그동안 많이 향상되었으나, 산재사망률은 OECD국가 중 가장 높은 수준이며 산재사망이 지난 10여년간 전혀 줄어들고 있지 않고 있다. 본 연구는 우리나라의 소득수준 대비 산재사망률을 관련 국가들과 비교하고 그 차이점을 분석하여 근로자 중심의 산재예방 대책의 문제점을 파악하고 이에 대한 적절한 대책을 제시하는 것을 목적으로 하여 진행되었으며, 이를 위해 소득반영 산재사망지수를 제시하였다. 그 결과 우리나라의 경우 비교국가 중에 그 수치가 가장 높게 나타났으며, 그 원인에 대한 분석의 일환으로 산업재해에 대한 법집행실적에 대한 비교를 한 결과 위반 사업주에 대한 법집행에 중점을 두는 영국의 경우와는 달리 우리나라의 법집행 실적은 상대적으로 낮은 편으로 나타나 건설공사의 안전성 향상을 위해서는 향후 법집행 강화의 필요한 것으로 나타났다.

선박 탱크 내부 도장작업 중 발생한 화재·폭발 사고원인 분석 (An Accident Investigation of Fire and Explosion Case During Painting Work in the Tank of the Crude Oil Carrier)

  • 한국의 조선업은 짧은 기간에도 불구하고 규모나 질적 면에서 놀라운 성장을 이루어냈다. 하지만 최근 7년간의 산업재해 통계를 분석했을 때, 조선업 평균 사고사망만인율이 1.48로 전업종 0.66대비 2배가 넘고, 재해율 또한 0.93%로 전업종 평균 0.57% 대비 높은 것으로 나타나 세계조선 강국이라는 말을 무색하게 하고 있다. 따라서 2017년 유조선 탱크 내부에서 도장 작업 중 발생한 사고에 대한 분석을 통해 한국 조선업의 현재를 진단하고자 하였다. 사고 요인 도출을 위해 ECFC(Effects and causal factors charting)를 이용하였으며, 점화원 및 가연물에 대한 분석을 위해 실험, 측정 및 CFD를 이용한 시뮬레이션을 실시하였다.

재해예방을 위한 건축공사 설계단계 안전 위험성 평가 체크리스트 (Occupational Health and Safety Risk Assessment Checklist for Preventing Accidents During Building Design Phase)

  • 지금까지 건설재해를 예방하기 위한 건설공사의 안전관리는 시공단계에서만 활발히 수행되어 왔다. 하지만 사망재해의 15%가 작업자 안전에 부정적인 영향을 미치는 부적절한 설계에 의해 발생하였다는 점을 감안할 때, 설계(안)에 존재하는 안전 위험성을 조기에 발굴 및 평가하여 제거 및 최소할 필요가 있다. 본 연구는 '안전을 위한 설계(Design for Safety)' 개념을 활용하여 설계(안)에 존재하는 안전 위험요소를 발굴 및 평가할 수 있는 체크리스트의 개발을 목적으로 하고 있다 이를 위해 영국 산업안전보건청(Health and Safety Executives)에서 활용하고 있는 설계단계 안전 위험성 평가 체크리스트의 구조, 활용절차 및 문제점에 관한 문헌연구를 수행하였다. 이를 통해 공간, 부위 및 공종별 안전 위험요소와 원인의 분류, 안전수준의 정량적 평가, 안전위험요소의 제거 및 최소화를 위한 안전설계지침 및 관련 안전기준을 반영한 새로운 안전 위험성 평가 체크리스트를 제안하였다. 설계단계 안전 위험성 평가 체크리스트는 건축공사 전반에서 발생할 수 있는 재해를 조기 예방하는데 있어 유용한 도구로 사용될 수 있다.

Human Error Probability Assessment During Maintenance Activities of Marine Systems

  • Background: Maintenance operations on-board ships are highly demanding. Maintenance operations are intensive activities requiring high man-machine interactions in challenging and evolving conditions. The evolving conditions are weather conditions, workplace temperature, ship motion, noise and vibration, and workload and stress. For example, extreme weather condition affects seafarers' performance, increasing the chances of error, and, consequently, can cause injuries or fatalities to personnel. An effective human error probability model is required to better manage maintenance on-board ships. The developed model would assist in developing and maintaining effective risk management protocols. Thus, the objective of this study is to develop a human error probability model considering various internal and external factors affecting seafarers' performance. Methods: The human error probability model is developed using probability theory applied to Bayesian network. The model is tested using the data received through the developed questionnaire survey of >200 experienced seafarers with >5 years of experience. The model developed in this study is used to find out the reliability of human performance on particular maintenance activities. Results: The developed methodology is tested on the maintenance of marine engine's cooling water pump for engine department and anchor windlass for deck department. In the considered case studies, human error probabilities are estimated in various scenarios and the results are compared between the scenarios and the different seafarer categories. The results of the case studies for both departments are also compared. Conclusion: The developed model is effective in assessing human error probabilities. These probabilities would get dynamically updated as and when new information is available on changes in either internal (i.e., training, experience, and fatigue) or external (i.e., environmental and operational conditions such as weather conditions, workplace temperature, ship motion, noise and vibration, and workload and stress) factors.

건설 현장 CCTV 영상을 이용한 작업자와 중장비 추출 및 다중 객체 추적 (Extraction of Workers and Heavy Equipment and Muliti-Object Tracking using Surveillance System in Construction Sites)

  • 건설업은 업무상 재해 발생빈도와 사망자 수가 다른 산업군에 비해 높아 가장 위험한 산업군으로 불린다. 정부는 건설 현장에서 발생하는 산업 재해를 줄이고 예방하기 위해 CCTV 설치 의무화를 발표했다. 건설 현장의 안전 관리자는 CCTV 관제를 통해 현장의 잠재된 위험성을 찾아 제거하고 재해를 예방한다. 하지만 장시간 관제 업무는 피로도가 매우 높아 중요한 상황을 놓치는 경우가 많다. 따라서 본 연구는 딥러닝 기반 컴퓨터 비전 모형 중 개체 분할인 YOLACT와 다중 객체 추적 기법인 SORT을 적용하여 다중 클래스 다중 객체 추적 시스템을 개발하였다. 건설 현장에서 촬영한 영상으로 제안한 방법론의 성능을 MS COCO와 MOT 평가지표로 평가하였다. SORT는 YOLACT의 의존성이 높아서 작은 객체가 적은 데이터셋을 학습한 모형의 성능으로 먼 거리의 물체를 추적하는 성능이 떨어지지만, 크기가 큰 객체에서 뛰어난 성능을 나타냈다. 본 연구로 인해 딥러닝 기반 컴퓨터 비전 기법들의 안전 관제 업무에 보조 역할로 업무상 재해를 예방할 수 있을 것으로 판단된다.
