두부자세에 따른 두경부 근활성 및 교합접촉양태의 변화 (Effects of Head Posture on Resting EMG Activity of Craniocervical Muscles and on Occlusal Contacts)

  • This study was performed to investigate influence of the changes of head posture on resting electromyographic (EMG) activity in anterior temporalis, masseter, sternocleidomastoid muscle and trapezius, and on status of occlusal contacts. For this study twenty-nine patients with temporomandibular disorders(TMD) and thirty dental students without any masticatory symptoms were selected as patients group and control group, respectively. EMG activity($\mu$V) at rest was observed in four kind of head postures such as natural or normal head posture(NHP), forward head posture(FHP), upward head posture(UHP), downward head posture(DHP), and in NHP and FHP, EMG activity with flat occlusal splint was also checked. BioEMG$^\textregistered$(Bioelectromyograph, Bioresearch Inc., USA) was used to record EMG activity in the above four muscles with eight locations on both sides. The author used T-Scan$^\textregistered$(Tekscan Co., USA) system to investigate the changes of oclusal contats on clenching in the four head postures about number, force, time(duration) and total left-right statistis(TLR, occlusal stability crossing left-right dental arch on clenching). For taking in upward or downward head posture, head was inclined $10^{\circ}$ upward or downward and CROM$^\textregistered$ (cervical-range-of motion, Performance attainment Inc., USA) was used to maintain same posture during the procedure. The results obtained were as follows : 1. For resting EMG activity, anterior temporalis did not show any difference by change of head posture, but masseter and sternocleidomastoid muscle showed higher value of EMG activity in FHP and UHP, and trapezius showed higher value of EMG activity in FHP and DHP. 2. EMG activity of trapezius was higher than that of any other muscles in NHP, FHP, and DHP, but in UHP, the activity was the lowest reversely. 3. Patients group showed higher EMG activity than control group did in all the muscles in NHP. And significant difference between the two groups were also observed in anterior temporalis in FHP, in sternocleidomastoid muscle in UHP, and in sterno-cleidomastoid muscle and trapezius in DHP with higher activity in patients group. 4. There was no change of EMG activity in NHp with splint, but EMG activity in anterior temporalis and masseter was decreased in FHP with splint. 5. In general, status of occlusal contacts was not changed with head posture in all subjects, and difference between patients group and control group was only noted for number and force of tooth contact in UHP and DHP with more value in control group. 6. Correlationship between EMG activity and number ad force of tooth contacts was shown negatively with regard to masseter in NHP, and trapezius in UHP and DHP.

전치부 개교를 동반한 골격성 III급 부정교합 환자의 악교정 수술 후 교합평면의 변화와 안정성에 관한 연구 (Postoperative Stability and Occlusal Plane Alternation by Orthognathic Surgery of Skeletal Class III Malocclusion with Anterior Open Bite)

  • 악골의 골격적 부조화를 가진 환자의 교정치료는 악교정 수술을 필요로 하고, 악교정 수술에 의해 교합평면각은 변화된다. 특히 전치부 개교를 동반한 III급 부정교합 환자에서 치아안면 기형을 치료하기 위한 교합평면각의 변화에 대하여 많은 논란이 있어 왔다. 교합평면각을 증가시키는 하악골의 시계 방향 회전(clockwise rotation)은 전치부 개교에 대한 적절한 치료법으로 추천되어 왔고 하악골의 반시계 방향의 회전(counterclockwise rotation)은 하악지 고경을 증가시키는 방향으로 하악골을 회전시킨다는 개념으로 인해 재발(relapse)을 유발하는 불안정한 수술 방법으로 인식되어 왔다. 본 연구는 전치부 개교를 동반한 골격성 III 부정교합 환자에서 교합평면의 반시계 방향 회전을 동반한 하악지 시상분할 골절단술 시행 후 교합평면각의 변화와 술후 안정성과의 관계를 평가 하고자 하였다. 하악지 시상분할 골절 단술과 rigid fixation으로 치료받은 환자 25 명 (평균연령 20.6세)을 대상으로 하여 수술 직전(T1), 술후 2주내(T2), 그리고 술후 6 개월 이후(T3)에 촬영한 측모 두부계측 방사선 사진을 통계분석한 결과(Paired t-test, Pearson correlation analysis), 다음의 결론을 얻었다. 1. 악교정 수술 후(T2) 하악평면각은 $2.9^{\circ}$ 감소하였고 SN 평면에 대한 하악 교합평면의 각도는 $2.7^{\circ}$ 감소하였다. 술후 6 개월경과 후(T3) 후안면 고경의 감소로 인해(P<0.01) 하악 평면각은 $1.0^{\circ}$ 증가하였으나 하악 교합평면의 경사도는 변화되지 않았다. 2. 악교정 수술 후 시간 경과(T3)에 의해 발생한 수평적 재발은 하악골의 전방 이동량이 B점에서 1.6 mm로 수술시 전체 후방 이동량의 약$22\%$였다. 전안면 고경에서는 수직적 재발이 발생하지 않았으나,후안면 고경은 감소하였다. (P<0.01). 3. 수평적 재발과 상관관계를 보이는 항목은 하악평면각 이었고(P<0.01) 수술 6 개월경과 후 후안면 고경의 감소와 상관관계가 있는 요소로는 수술시 하악골의 후방 이동량(P<0.01), 하악지 고경의 증가량(P<0.01), 그리고 하악평면각의 감소량(P<0.01) 등이 있었다. 4. 수술시 하악 교합평면 경사도의 변화량과 술후 재발과는 상관관계가 없었다.

의치의 안정성을 증대시키기 위한 고려 요소: 증례보고 (Considerations for increasing denture stability: a case report)

  • 총의치 장착시, 환자는 생리적으로 안정된 범위 내에서 자연스럽게 기능하기를 원하며 이를 위해서는, 안모의 회복(심미적 효과), 생물학적으로 안정된 인공치 배열과 외형, 의치 동요가 없는 교합접촉이 반드시 필요하다. 이에 본 증례보고에서는 총의치를 제작하기에 불리한 조건을 가진 환자를 대상으로 인상면, 연마면, 교합면에 있어서의 의치의 안정을 증대시키기 위해, 기능적 변연 형성을 통한 인상채득, 중립대 확인, 생리적 중심위 재확립, 안정된 교합을 형성시켜 주었으며, 임상적으로 기능과 심미적인 측면에 있어서 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

Positional changes in the mandibular proximal segment after intraoral vertical ramus osteotomy: Surgery-first approach versus conventional approach

  • Objective: To compare postoperative positional changes in the mandibular proximal segment between the conventional orthognathic surgery (CS) and the surgery-first approach (SF) using intraoral vertical ramus osteotomy (IVRO) in patients with Class III malocclusion. Methods: Thirty-eight patients with skeletal Class III malocclusion who underwent bimaxillary surgery were divided into two groups according to the use of preoperative orthodontic treatment: CS group (n = 18) and SF group (n = 20). Skeletal changes in both groups were measured using computed tomography before (T0), 2 days after (T1), and 1 year after (T2) the surgery. Three-dimensional (3D) angular changes in the mandibular proximal segment, condylar position, and maxillomandibular landmarks were assessed. Results: The mean amounts of mandibular setback and maxillary posterior impaction were similar in both groups. At T2, the posterior portion of the mandible moved upward in both groups. In the SF group, the anterior portion of the mandible moved upward by a mean distance of 0.9 ± 1.0 mm, which was statistically significant (p < 0.001). There were significant between-group differences in occlusal changes (p < 0.001) as well as in overjet and overbite. However, there were no significant between-group differences in proximal segment variables. Conclusions: Despite postoperative occlusal changes, positional changes in the mandibular proximal segment and the position of the condyles were similar between CS and SF, which suggested that SF using IVRO achieved satisfactory postoperative stability. If active physiotherapy is conducted, the proximal segment can be adapted in the physiological position regardless of the occlusal changes.

Full Arch Restoration through Orthognathic Surgery after Implantation on the Patients with Mandibular Prognathism and Loss of Posterior Teeth: A Case Report

  • In case of loss of many teeth due to dental caries or periodontal disease, improvement of masticatory function and aesthetics can be obtained through implant treatment. However, if the patient does not have a normal intercondylar relationship, it is difficult to achieve an ideal occlusal relationship with only prosthetic treatment. In particular, oral reconstruction with orthodontic treatment or orthognathic surgery is necessary for patients with mandibular prognathism. However, if the posterior occlusion collapses due to severe caries or periodontal disease, orthognathic surgery may be difficult. The occlusal vertical stop is very important for the stability of the mandibular position during occlusal reconstruction through orthognathic surgery. The patient in this case had posterior occlusion collapsed due to the caries of a large number of posterior teeth, and showed mandibular prognathism and long face. We planned a full arch restoration with orthognathic surgery and extracted the hopeless teeth. To secure the vertical stop required for orthognathic surgery, the implant was placed before surgery. After the orthognathic surgery and the final prosthesis application, the results were satisfactory for the improvement of the aesthetics and the restoration of the masticatory function.

안정적인 교합접촉을 잃은 환자에서의 전방유도를 고려한 치료증례 (Maxillary anterior prosthetic treatment concerning anterior guidance of a patient who lost stable holding contact)

  • 최근 들어 치아의 심미적인 개선을 위해 전치부 보철 치료를 하는 경우가 많다. 하지만 전치부 심미 보철치료이후 환자가 발음, 저작 등 기능적으로 불만족스럽게 생각하는 경우가 종종 있다. 전치부는 심미적으로 중요할 뿐만 아니라 교합과 관련하여서도 중심위 다음으로 중요한 전방유도가 성립되는 부분으로, 잘못된 전방유도 설정으로 인해 구치부의 교합간섭을 초래할 수 있다. 또한 새롭게 설정된 전치부 보철물의 전방유도가 환자의 기능로(envelope of function)과 조화를 이루지 않을 시 환자가 불편감을 나타낼 수 있다. 본 증례는 전치부 심미 보철 이후 부정확한 발음 및 안정적인 접촉이 없어서 불안정한 교합, 전반적인 불편감을 호소하는 환자에서 체계적인 진단과 치료과정을 통해 보철물을 재수복하여 환자와 술자 모두에게서 심미적으로, 그리고 기능적으로 만족스러운 결과를 얻었기에 보고하는 바이다.

지르코니아 및 티타늄 임플란트를 사용한 지지골 및 임플란트 유지 수복물의 응력 분포 비교: 3차원 유한 요소 분석 (Comparison of stress distribution in bone and implant-supported dental prosthesis with zirconia and titanium implants: a 3-dimensional finite element analysis)

  • Purpose: Zirconia is differentiated from other ceramics because of its high resistance to corrosion and wear, excellent flexural strength (900~1400 MPa), and high hardness. Dental zirconia with proven mechanical/biological stability is suitable for the manufacture of implants. However, there are limited in vivo studies evaluating stress distribution in zirconia compared with that in titanium implants and studies analyzing finite elements. This study was conducted to evaluate the stress distribution of the supporting bone surrounding zirconia and titanium implants using the finite element analysis method. Methods: For finite element analysis, a single implant-supported restoration was designed. Using a universal analysis program, eight occlusal points were set in the direction of the occlusal long axis. The occlusal load was simulated at 700 N. Results: The zirconia implant (47.7 MPa) von Mises stress decreased by 5.3% in the upper cortical bone compared with the titanium implant (50.2 MPa) von Mises stress. Similarly, the zirconia implant (20.8 MPa) von Mises stress decreased by almost 4% in the cancellous bone compared with the titanium implant (21.7 MPa) von Mises stress. The principal stress in the cortical and cancellous bone exhibited a similar propensity to von Mises stress. Conclusion: In the supporting bone, the zirconia implant is able to reduce bone resorption caused by mechanically transferred stress. It is believed that the zirconia implant can be a potential substitute for the titanium implant by reinforcing aesthetic characteristics and improving stress distribution.

의치상 레진의 중합 방법에 따른 크기의 안정성 및 표면 형태에 관한 연구 (DIMENSIONAL STABILITY AND SURFACE MORPHOLOGY OF VARIOUS DENTURE RESINS)

  • The purpose of this study was to investigate the dimensional changes and surface morphology of dentures processed by various polymerization conditions. The measurements were done by taking radiograph and using vernier calipers and each specimen was observed on scanning electron microscope. Results obtained were as follows. 1. The difference of dimensional stability was not recognized between various polymerization conditions(heat-cured resin, pour-type resin, microwave-cured resin, and injection molding resin). 2. There were expansion and shrinkage in the occlusal dimension, shrinkage in the frontal dimension, and expansion in the lateral dimension. 3. Scanning electron microscope pictures of heat-cured resin showed dense and regular surface morphology. 4. Microwave-cured resin surface appeared more regular and smooth than pour-type resin but less dense and more irregular than heat-cured resin. 5. Scanning electron microscope pictures of pour-type resin with the lowest dimensional change showed the most irregular surface morphology.

성장기 하악골 열성장 환자의 Berlin standard activator를 이용한 부정교합 치료: 증례보고 (Berlin standard activator in the treatment of growing patients with mandibular deficiency: Case report)

  • Activator is a removable functional appliance used for correcting the skeletal Class II malocclusion in children with the mandibular deficiency. Berlin standard activator modified from Andresen activator has following characters; do not cover the palatal surface for tongue space, relief on lingual surface of mandibular incisors and resin capping 1/3-1/2 of crown height on mandibular incisors for preventing labioversion of mandibular incisors, L-hook between maxillary lateral incisor and canine for anterior high pull headgear, relief on mandibular posterior bite block for differential eruption of posterior teeth. Two cases presented here had a mandibular deficiency and slight maxillary protrusion. First case (an 11-year-old girl) treated with Berlin standard activator and anterior high pull headgear for 13 months followed by fixed orthodontic appliance for another 29 months. Second case (a 12-year-old boy) treated with Berlin standard activator for 6 months followed by fixed appliance for another 24 months. Treatment results showed a significant improvement in sagittal skeletal and occlusal relationship without premolar extraction. Mandibular condyles were concentric in TMJ [ossa, and masticatory muscle activities were normalized after treatment. In the retention period facial harmony and occlusal stability was maintained.

Management of dental erosion induced by gastro-esophageal reflux disorder with direct composite veneering aided by a flexible splint matrix

  • Dental erosion is frequently overlooked in clinical practice. The management of erosion-induced damage to the dentition is often delayed, such that extensive occlusal rehabilitation is required. These cases can be diagnosed by a careful clinical examination and a thorough review of the patient's medical history and/or lifestyle habits. This case report presents the diagnosis, categorization, and management of a case of gastro-esophageal reflux disease-induced palatal erosion of the maxillary teeth. The early management of such cases is of utmost importance to delay or prevent the progression of damage both to the dentition and to occlusal stability. Non-invasive adhesively bonded restorations aid in achieving this goal.
