The osseointegrated implants react biomechanically in a different pattern to occlusal force, due to lack of the periodontal ligament unlike the natural teeth. The implants show markedly less movement and limited tactile sensitivity compared with the natural teeth. The implant occlusion concept aims to avoid overloading on the implants and to direct occlusal loads along the longitudinal axis of the implants, in order to compensate for the different biomechanics of the implants. Although many guidelines and theories on implant occlusion have been proposed, few have provided strong supportive evidence. Moreover, the outcome of treatment often quite successful in spite of different concepts of occlusion and there is an increasing tendency to doubt about the strict theoretical implant-specific occlusion concept. The purpose of this article is to review the previous reports about the concept of implant occlusion and discuss clinical occlusal considerations in implant rehabilitations.
The significance of occlusion has regained its popularity in dentistry with the introduction of implant therapy. Literature has reported that the clinical success and longevity of dental implants can be achieved by biomechanically controlled occlusion. Occlusal overload is known to be one of the main causes for implant failure. Evidences have suggested that occlusal overload contribute to early implant bone loss as well as deosseointegration of successfully integrated implants. Unlike natural teeth, osseointegrated implants are ankylosed to surrounding bone without the periodontal ligament (PDL) which provides mechanoreceptors as well as shock-absorbing function. Moreover, the crestal bone around dental implants may act as a fulcrum point for lever action when a force (bending moment) is applied, indicating that implants/implant prosthesis could be more susceptible to crestal bone loss by applying force. Hence, it is essential for clinicians to understand inherent differences between teeth and implants and how force, either normal or excessive force, may influence on implants under occlusal loading. The purposes of this paper are to review the importance of implant occlusion, to establish the optimum implant occlusion with biomechanical rationale, to provide clinical guidelines of implant occlusion and to discuss how to manage complications related to implant occlusion.
Objective: The aim of this systematic review was to assess the occlusal outcome and duration of fixed orthodontic therapy from clinical trials in humans with the Objective Grading System (OGS) proposed by the American Board of Orthodontics. Methods: Nine databases were searched up to October 2016 for prospective/retrospective clinical trials assessing the outcomes of orthodontic therapy with fixed appliances. After duplicate study selection, data extraction, and risk of bias assessment according to the Cochrane guidelines, random-effects meta-analyses of the mean OGS score and treatment duration were performed and 95% confidence intervals (CIs) were calculated. Results: A total of 34 relevant clinical trials including 6,207 patients (40% male, 60% female; average age, 18.4 years) were identified. The average OGS score after treatment was 27.9 points (95% CI, 25.3-30.6 points), while the average treatment duration was 24.9 months (95% CI, 24.6-25.1 months). There was no significant association between occlusal outcome and treatment duration, while considerable heterogeneity was identified. In addition, orthodontic treatment involving extraction of four premolars appeared to have an important effect on both outcomes and duration of treatment. Finally, only 10 (39%) of the identified studies matched compared groups by initial malocclusion severity, although meta-epidemiological evidence suggested that matching may have significantly influenced their results. Conclusions: The findings from this systematic review suggest that the occlusal outcomes of fixed appliance treatment vary considerably, with no significant association between treatment outcomes and duration. Prospective matched clinical studies that use the OGS tool are needed to compare the effectiveness of orthodontic appliances.

Objectives: This study sought to provide guidelines in order to decrease the incidence of nerve injury during mandibular ramus bone harvesting, and to improve understanding of the anatomical structure of the inferior alveolar canal (IAC) to include its distance from the exterior buccal cortex. Materials and Methods: In January and February 2009, 20 patients who visited the Wonkwang University Department of Oral and Maxillofacial Surgery reporting various conditions underwent cone beam computed tomography and were included in this study. Patients with missing left or right mandibular first molars or incisors, or who had jaw fracture or bone pathologies, were excluded. The reference point (R point) was defined as the point where the occlusal plane reached the anterior ramus of the mandible. The position of the IAC in relation to the R point, the buccal bone width (BW), the alveolar crest distance (ACD), the distance from the alveolar crest to the occlusal plane (COD), and the distance from the IAC to the sagittal plane (CS) were determined using proprietary image analysis software which produced cross-sectional coronal and axial images. Results: The distance medially from the R point to the IAC along the axial plane was $6.19{\pm}1.21mm$. The HD from the R point, posteriorly to IAC, in the lateral view was $13.07{\pm}2.45mm$, the VD from the R point was $14.24{\pm}2.41mm$, and the ND from the R point was $10.12{\pm}1.76mm$. The pathway of the IAC was positioned almost in a straight line along a sagittal plane within $0.56{\pm}0.70mm$. The distance from the buccal bone surface to the IAC increased anteriorly from the R point. Conclusion: Marking osteotomy lines in the retromolar area in procedures involving bone harvesting should be discouraged due to the risk of damage to IAC structures. Our measurements indicated that the area from the R point in the ramus of the mandible to 10 mm anterior can be safely harvested for bone grafting purposes.
광범위한 보철 수복 치료는 수복 부위별로 다양한 고려 요소를 필요로 한다. 특히, 상악 전치부의 교합 계획은 다른 부위보다 더 많은 지식과 기술을 요한다. 전방 유도(anterior guidance)를 결정하는 상악 전치부 설면 외형을 제대로 형성하지 못하면, 기능적인 불편감과 함께 전체 치열의 불안정성을 야기한다. 임시 수복물 장착한 상태에서 적절한 조정을 통해 조화로운 전방 유도를 얻었다면 임시 수복물의 설면 형태를 최종 보철물로 정확하게 구현하는 방법에 주의를 기울여야 한다. 본 증례에서는 CAD/CAM (computer-aided design / computer-aided manufacturing) 시스템의 복제 기법을 활용하여 지대치의 디지털 이미지와 임시수복물의 디지털 이미지를 중첩시켜 보다 간편하게 보철물 형태를 복제하였다. 기존의 방법에 비해 술자는 진료실 시간을 줄일 수 있고, 환자는 기능과 심미 모두 만족할 만한 결과를 얻어 이를 보고하고자 한다.
Objective: The absence of a guideline to refer to developing malocclusions appropriately, may be a contributing factor to the inadequacy of timely interceptive orthodontics provision. This study aimed to develop and validate a new orthodontic grading and referral index to be used by dental frontliners to prioritize the orthodontic referral of developing malocclusion in children based on its severity. Methods: A cross-sectional study involving clinical assessment with 413 schoolchildren aged between 8.1 and 11.9 years was conducted in 2018. All the presenting malocclusion was listed and graded based on a few dental guidelines to produce the draft index. The validity and reliability of the draft index were tested using twenty study models. Face and content validation was carried out using the content validation index and Modified Kappa Statistics. Results: Fourteen dental and occlusal anomalies were identified as components of malocclusion and three grades of referral (monitor, standard, urgent) were included in the final index. The scale-level content validity index average value of 0.86 and 0.87 was obtained for content and face validation, respectively. There was moderate to excellent agreement in the Modified Kappa Statistics for both validations. Excellent inter- and intra-assessor agreement was obtained. The new index displayed valid and reliable scores. Conclusions: The Index for Interceptive Orthodontics Referral was developed and validated for the dental frontliners to identify and prioritize the developing malocclusion in children based on its severity and refer for orthodontic consultation to increase the possibility for interceptive orthodontics.
Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar. 본 연구의 목적은 전후방 및 수직적 골격형에 따라 선험적으로 9 개의 유형 (type)으로 분류된 정상교합자의 치아치조부 보상기전의 양상을 분석하고 이를 임상적으로 개별 부정교합자에게 적용이 가능한 신뢰성 있고 간편한 골격 유형의 감별기준을 도출하고자 한 것이었다. 이를 위하여 정상교합자 294명의 측모두부방사선계측치 중 치아치조부 계측 항목의 기술 통계량을 구하고 각 유형간의 특징을 비교하기 위하여 분산 분석과 다중 비교를 시행하였으며, 골격 계측 항목과 치아치조부 계측 항목간의 상관관계를 분석하였다. 또한 이러한 유형을 부정교합자의 개별 골격형에 적용할 수 있도록 판별 분석을 시행하여 골격형 감별의 효율성과 정확성을 가늠하였다. 그 결과 갈 골격 유형별로 특징적인 치아치조부 보상기전을 확인할 수 있었으며, 상/하악 전치의 위치 및 교합 평면을 나타내는 변수에서 높은 상관관계가 관찰되었다. 판별 분석 결과 9개의 골격 유형을 분류할 수 있는 4개의 변수(AB-MP, SN-AB, PMA)를 구할 수 있었으며, 이들 4개의 변수로 이루어진 판별 계수로 전체 표본의 $87.8\%$를 정확하게 분류할 수 있었다. 결론적으로, 이러한 연구 결과를 통해 개별 부정교합자의 개별화된 진단 및 치료 지침 수립시 기초적 정보를 제공할 수 있을 것으로 예견되었다.
본 연구는 인레이 와동에서 인상을 채득하기 전 지각과민처리제 (Isodan)과 2단계 전부식 접착시스템 (One-Step)과 단일단계 자가부식 접착시스템 (All-Bond SE)으로 처리한 후, 복합레진 인레이를 자가 접착형 레진시멘트인 BisCem으로 합착시키는 경우 레진 인레이의 상아질에 대한 미세인장결합강도와 주사현미경적인 분석을 통하여 비교하였다. 본 연구의 결과, 지각과민처리제인 Isodan만을 사용하는 경우 레진 인레이의 결합강도를 감소시킬 수 있으므로 단독 사용보다는 단일 단계 자가부식 시스템 (All-Bond SE)과 같이 사용하는 것이 추천된다.
