PURPOSE. To compare the accuracy of marginal and internal adaptation of zirconia (Zr) copings fabricated on anatomic (A), semi-anatomic (SA) and non-anatomic (NA) occlusal surface preparations. MATERIALS AND METHODS. 45 extracted bicuspid teeth were prepared for receiving zirconia crowns, with different occlusal preparation designs A=15, SA=15 & NA=15. The Zr copings were fabricated by using CAD4DENT, CAD/CAM. The copings were adjusted, cemented and were cross sectioned centrally from buccal cusp tip to lingual cusp tip into mesial and distal halves. The copings were examined under electron microscope at ${\times}200$ magnification and the measurements were recorded at 9 predetermined areas in micrometers. RESULTS. Overall mean gap values for the three groups was found to be $155.93{\pm}33.98{\mu}m$ with Anatomical Occlusal preparation design having the least gap value of $139.23{\pm}30.85{\mu}m$ showing the best adaptation among the groups. Post Hoc Tukey's test showed a statistically significant difference (P=.007) between the means of gap for A & NA preparation designs. Measurements recorded at 9 predetermined points showed variations for the three groups. CONCLUSION. Anatomical occlusal preparation designs resulted in better marginal and internal adaptation of Zr copings. There is a considerable variation between the measured marginal and internal gap values for the Zr copings fabricated by the (CAD4DENT-CAD/CAM). This variation may be associated with the lack of standardization of the preparation of teeth, computerized designing of the coping for each tooth, cement used, uniform pressure application during the cementation of the copings, sectioning of the copings and the microscopic measurements.
PURPOSE. To compare marginal and internal gaps of zirconia substructure of single crowns with those of three-unit fixed dental prostheses. MATERIALS AND METHODS. Standardized Co-Cr alloy simulated second premolar and second molar abutments were fabricated and subsequently duplicated into type-III dental stone for working casts. After that, all zirconia substructures were made using $Lava^{TM}$ system. Marginal and internal gaps were measured in 2 planes (mesial-distal plane and buccal-palatal plane) at 5 locations: marginal opening (MO), chamfer area (CA), axial wall (AW), cusp tip (CT) and mid-occlusal (OA) using Replica technique. RESULTS. There were significant differences between gaps at all locations. The $mean{\pm}SD$ of marginal gap in premolar was $43.6{\pm}0.4{\mu}m$ and $46.5{\pm}0.5{\mu}m$ for single crown and 3-unit bridge substructure respectively. For molar substructure the $mean{\pm}SD$ of marginal gap was $48.5{\pm}0.4{\mu}m$ and $52.6{\pm}0.4{\mu}m$ for single crown and 3-unit bridge respectively. The largest gaps were found at the occlusal area, which was $150.5{\pm}0.5{\mu}m$ and $154.5{\pm}0.4{\mu}m$ for single and 3-unit bridge premolar substructures respectively and $146.5{\pm}0.4{\mu}m$ and $211.5{\pm}0.4{\mu}m$ for single and 3-unit bridge molar substructure respectively. CONCLUSION. Independent-samples t-test showed significant differences of gap in zirconia substructure between single crowns and three-unit bridge (P<.001). Therefore, the span length has the effect on the fit of zirconia substructure that is fabricated using CAD/CAM technique especially at the occlusal area.

Objectives: The purpose of this study was to compare the marginal and internal fit of nano-composite CAD-CAM restorations. Materials and Methods: A full veneer crown and an mesio-occluso-distal (MOD) inlay cavity, which were prepared on extracted human molars, were used as templates of epoxy resin replicas. The prepared teeth were scanned and CAD-CAM restorations were milled using Lava Ultimate (LU) and experimental nano-composite CAD/CAM blocks (EB) under the same milling parameters. To assess the marginal and internal fit, the restorations were cemented to replicas and were embedded in an acrylic mold for sectioning at 0.5 mm intervals. The measured gap data were pooled according to the block types and measuring points for statistical analysis. Results: Both the block type and measuring point significantly affected gap values, and their interaction was significant (p = 0.000). In crowns and inlays made from the two blocks, gap values were significantly larger in the occlusal area than in the axial area, while gap values in the marginal area were smallest (p < 0.001). Among the blocks, the restorations milled from EB had a significantly larger gap at all measuring points than those milled from LU (p = 0.000). Conclusions: The marginal and internal gaps of the two nano-composite CAD/CAM blocks differed according to the measuring points. Among the internal area of the two nano-composite CAD/CAM restorations, occlusal gap data were significantly larger than axial gap data. The EB crowns and inlays had significantly larger gaps than LU restorations.
PURPOSE. The aim of this stuldy was to compare the clinical marginal fit of CAD-CAM inlays obtained from intraoral digital impression or addition silicone impression techniques. MATERIALS AND METHODS. The study included 31 inlays for prosthodontics purposes of 31 patients: 15 based on intraoral digital impressions (DI group); and 16 based on a conventional impression technique (CI group). Inlays included occlusal and a non-occlusal surface. Inlays were milled in ceramic. The inlay-teeth interface was replicated by placing each inlay in its corresponding uncemented clinical preparation and taking interface impressions with silicone material from occlusal and free surfaces. Interface analysis was made using white light confocal microscopy (WLCM) (scanning area: 694 × 510 ㎛2) from the impression samples. The gap size and the inlay overextension were measured from the microscopy topographies. For analytical purposes (i.e., 95-%-confidence intervals calculations and P-value calculations), the procedure REGRESS in SUDAAN was used to account for clustering (i.e., multiple measurements). For p-value calculation, the log transformation of the dependent variables was used to normalize the distributions. RESULTS. Marginal fit values for occlusal and free surfaces were affected by the type of impression. There were no differences between surfaces (occlusal vs. free). Gap obtained for DI group was 164 ± 84 ㎛ and that for CI group was 209 ± 104 ㎛, and there were statistical differences between them (p = .041). Mean overextension values were 60 ± 59 ㎛ for DI group and 67 ± 73 ㎛ for CI group, and there were no differences between then (p = .553). CONCLUSION. Digital impression achieved inlays with higher clinical marginal fit and performed better than the conventional silicone materials.
The aim of this document is to evaluate the effect of bite balance on physical fitness and motor capacity to determine the importance of the occlusal stability as a possible action mechanism of occlusal appliance including mouthguards on physical performances. We all remember that day when the prominent athlete was in the news wearing a mouthguard to improve human athletic performance. I once had investigated the determinants of athletic performance in all sorts of sports. Most of the studies had overinterpretation of results and lack of evidence to support the information. However, I discovered great expectations for new possibility in this field and settled for a more academic approach to this intriguing subject. The followings are some examples of the subject 1. Archers who wore occlusal appliances to increase the vertical dimension of occlusion by 2~3mm increased their ability to focus and to maintain good sense of balance. Their accuracy rate has been improved. The appliance was made of acrylic resin (a type of plastic). 2. Canoe players who wore occlusal appliances to increase the vertical dimension of occlusion by 5mm surpassed longtime competitor and won the race with 10 seconds gap. 3. A cycle rider who wore an occlusal appliance to treat his malocclusion surpassed his old record from 10.8 seconds to 10.3 seconds. His muscle reflexes (anaerobic exercise) and endurance (aerobic exercise) has been enhanced. 4. Occlusal appliances had a good effect on athletic performance in most sports such as swimming, diving, and weightlifting. As for the clenching on dentition, people clench their teeth when they push their physical and mental limits during sports activities. Clenching can be induced by physical exertion when maximum muscle strength is retained and this is similarly found in different types of exercises. In addition, restraint of respiratory function is observed. Therefore, creating a occlusal balance with occlusal appliance or mouthguard promoted a favorable influence when an athlete clenches.

PURPOSE. The purpose of this study was to evaluate the reliability of computer-aided replica technique (CART) by calculating its agreement with the replica technique (RT), using statistical agreement analysis. MATERIALS AND METHODS. A prepared metal die and a metal crown were fabricated. The gap between the restoration and abutment was replicated using silicone indicator paste (n = 25). Gap measurements differed in the control (RT) and experimental (CART) groups. In the RT group, the silicone replica was manually sectioned, and the marginal and occlusal gaps were measured using a microscope. In the CART group, the gap was digitized using optical scanning and image superimposition, and the gaps were measured using a software program. The agreement between the measurement techniques was evaluated by using the 95% Bland-Altman limits of agreement and concordance correlation coefficients (CCC). The least acceptable CCC was 0.90. RESULTS. The RT and CART groups showed linear association, with a strong positive correlation in gap measurements, but without significant differences. The 95% limits of agreement between the paired gap measurements were 3.84% and 7.08% of the mean. The lower 95% confidence limits of CCC were 0.9676 and 0.9188 for the marginal and occlusal gap measurements, respectively, and the values were greater than the allowed limit. CONCLUSION. The CART is a reliable digital approach for evaluating the fit accuracy of fixed dental prostheses.
Background: Temporomandibular joint (TMJ) ankylosis can be accompanied by various degrees of functional and esthetic problems. Adequate mouth opening, occlusal stability, and harmonious facial form are the main goals of treatment for ankylosis. Distraction osteogenesis has proven to be an excellent treatment for lengthening the ramus-condyle unit. However, various timings for distraction have been suggested, and there is no consensus on selection criteria for performing the procedure in stages or simultaneously with other treatments. Case presentation: In this case report, concomitant intraoral distraction and gap arthroplasty was planned to treat TMJ ankylosis and associated facial asymmetry. After gap arthroplasty and 23 mm of distraction, the ramus-condyle segment was successfully lengthened and mouth opening range was significantly increased. The resultant interocclusal space was stably maintained with an occlusal splint for 4 months after distraction. Finally, good occlusion was achieved after prosthetic treatment. The remaining mandibular asymmetry was corrected with osseous contouring and augmentation surgery. The mouth-opening range was maintained at 35 mm 24 months after treatment. Conclusion: Gap arthroplasty with intraoral distraction as a one-stage treatment and subsequent contouring surgery can be applied to correct ankylosis with moderate malocclusion and facial asymmetry.
The purpose of this study was to compare the adaptation to tooth structure of light - cured glass ionomer cement with that of self -cured glass ionomer cement. In this study, class V cavities were prepared on the buccal surfaces of 10 extracted human premolar teeth, and teeth were randomly assigned 2 groups of 5 teeth each. The cavities of self-curing glass ionomer cement group were restored with the Fuji n. and the cavities of lightcuring glass ionomer cement group were restored with the Fuji II LC. The surfaces of glass ionomer cements were applied with All-Bond 2 adhesive, and cured with visible light. The restored teeth were stored in 100% relative humidity at $37^{\circ}C$ for 24 hours. And then. the roots of the teeth were removed with the tapered fissure bur and the remaining crowns were sectioned occlusogingivally through the center of glass ionomer restorations. Adaptation at tooth-restoration interface was assessed occlusally. axially, and gingivally by scanning electron microscope. The results were as follows : 1. On the occlusal margin, the group of self - curing glass ionomer cement showed closer adaptation to both enamel and dentin than the group of light-curing glass ionomer cement showing 5/lm gap between cement and tooth structure. 2. On the axial wall. the group of light-curing glass ionomer cement showing 5-$7{\mu}m$ gap between cement and dentin showed closer adaptation to dentin than the group of self -curing glass ionomer cement showing 10-$15{\mu}m$ gap between cement and dentin. 3. On the gingival margin, the group of light-curing glass ionomer cement showing 2-$5{\mu}m$ gap between cement and dentin(X 1200) showed closer adaptation to dentin than the group of self-curing glass ionomer cement showing 20pm gap between cement and dentin(X 600). 4. The group of self -curing glass ionomer cement showed closer adaptation on the occlusal margin than on the gingival margin, and the group of light-curing glass ionomer cement showed similar adaptation on both occlusal and gingival margins.
PURPOSE. The aim of this study was to measure the changes on the marginal and internal adaptation of zirconia based anterior fixed partial dentures after the porcelain firing process. MATERIALS AND METHODS. A total of 34 anterior fixed partial dentures using LAVA CAD/CAM system (3M ESPE, Germany) were applied. Two silicone replicas were obtained: one is obtained before porcelain firing process (initial) and the other is obtained after porcelain firing process (final), followed by the examination under a binocular stereomicroscope. Kruskal Wallis and Wilcoxon Signed Ranks tests were used for the statistical analysis (P<.05). RESULTS. No statistically significant difference was found between initial and final marginal gap values (P>.05). At the internal gap measurements, final marginal area values ($59.54{\mu}m$) were significantly lower than the initial marginal area values ($68.68{\mu}m$)(P<.05). The highest and the lowest internal gap values were observed at the incisal/occlusal area and at the marginal area, respectively. In addition, lower internal gap values were obtained for canines than for central incisors, lateral incisors and premolars at the incisal area (P<.05). CONCLUSION. The firing cycles did not affect the marginal gap of Lava CAD/CAM system, but it is controversial for the internal gap.
