Sleep alters both breathing pattern and the ventilatory responses to external stimuli. These changes during sleep permit the development or aggravation of sleep-related hypoxemia in patients with respiratory disease and contribute to the pathogenesis of apneas in patients with the sleep apnea syndrome. Fundamental effects of sleep on the ventilatory control system are 1) removal of wakefulness input to the upper airway leading to the increase in upper airway resistance, 2) loss of wakefulness drive to the respiratory pump, 3) compromise of protective respiratory reflexes, and 4) additional sleep-induced compromise of ventilatory control initiated by reduced functional residual capacity on supine position assumed in sleep, decreased $CO_2$ production during sleep, and increased cerebral blood flow in especially rapid eye movement(REM) sleep. These effects resulted in periodic breathing during unsteady non-rapid eye movement(NREM) sleep even in normal subjects, regular but low ventilation during steady NREM sleep, and irregular breathing during REM sleep. Sleep-induced breathing instabilities are divided due primarily to transient increase in upper airway resistance and those that involve overshoots and undershoots in neural feedback mechanisms regulating the timing and/or amplitude of respiratory output. Following ventilatory overshoots, breathing stability will be maintained if excitatory short-term potentiation is the prevailing influence. On the other hand, apnea and hypopnea will occur if inhibitory mechanisms dominate following the ventilatory overshoot. These inhibitory mechanisms include 1) hypocapnia, 2) inhibitory effect from lung stretch, 3) baroreceptor stimulation, 4) upper airway mechanoreceptor reflexes, 5) central depression by hypoxia, and 6) central system inertia. While the respiratory control system functions well during wakefulness, the control of breathing is commonly disrupted during sleep. These changes in respiratory control resulting in breathing instability during sleep are related with the pathophysiologic mechanisms of obstructive and/or central apnea, and have the therapeutic implications for nocturnal hypoventilation in patients with chronic obstructive pulmonary disease or alveolar hypoventilation syndrome.
Purpose: Mandibular advancement device (MAD) is widely recognized as an important treatment option for obstructive sleep apnea (OSA) and is readily accepted than any other treatment options owing to its simplicity and ambulatory nature. At this time, there are a multitude of MAD designs and their efficacies may be influenced by adjustment and retention mechanism. The MAD with the anterior connector (anteriorly adjustable mandibular advancement device, AAMAD) was newly developed in the Department of Oral Medicine, Dankook University Dental Hospital (Cheonan, Korea) and was prescribed for the OSA patients including snoring patients. Thus, this study was aimed to objectively investigate the effectiveness of the AAMAD on the OSA patients using the self-applied portable device (ApneaLink), and evaluate the treatment outcomes among patients with various severity of OSA level. Methods: Results of the treatment of fourteen patients (13 male, 1 female) with the AAMAD were retrospectively analyzed. Each patient underwent home sleep test before treatment and were divided into two groups, i.e., those with mild (apnea-hypopnea index [AHI] ${\geq}5$ and <15) to moderate OSA (AHI ${\geq}15$ and <30) and severe OSA (AHI ${\geq}30$). After treatment, home sleep test was conducted again and treatment outcomes were compared between mild to moderate and severe OSA patients. Results: Of all patients, 78.6% showed more than 50% AHI reduction. We found a significant reduction (85.3%) of AHI in the severe OSA patients. Patients with mild to moderate OSA showed the reduced AHI (56.1%). Conclusions: We concluded that AAMAD is an effective oral appliance for the majority of OSA patients.
The baroreflex is one kind of homeostatic mechanisms to regulate acute blood pressure (BP) changes by controlling heartbeat interval (HBI). To quantify the effect of baroreflex, we suggested a new approach of analyzing Granger causality between systolic BP (SBP) and HBI. The index defined as baroreflex effectiveness (BRE) was generated by the hypothesis that more effectual baroreflex would be related to more effective Granger causal influence of SBP on HBI. Six obstructive sleep apnea (OSA) patients (apnea-hypopnea index, AHI ${\geq}5$ events/hr) and six normal subjects participated in the study. Their SBP and HBI during nocturnal sleep were obtained from a non-invasive continuous BP measurement device. While the BRE ($mean{\pm}SD$) of normal subjects was $47.0{\pm}4.0%$, OSA patients exhibited the BRE of $34.0{\pm}3.8%$. The impaired baroreflex function of OSA patients can be explained by the physiological mechanism associated with recurrent hypoxic episodes during sleep. Thus, the significantly lower BRE in OSA patients verified the availability of Granger causality analysis to evaluate baroreflex during sleep. Furthermore, the range of BRE obtained from normal subjects was not overlapped with that obtained from OSA patients. It suggests the potential of BRE as a new helpful tool for diagnosing OSA.
Objectives: The aim of this study was to develop a predicting model for the optimal continuous positive airway pressure (CPAP) for obstructive sleep apnea (OSA) patient with obesity by using a machine learning. Methods: We retrospectively investigated the medical records of 162 OSA patients who had obesity [body mass index (BMI) ≥ 25] and undertaken successful CPAP titration study. We divided the data to a training set (90%) and a test set (10%), randomly. We made a random forest model and a least absolute shrinkage and selection operator (lasso) regression model to predict the optimal pressure by using the training set, and then applied our models and previous reported equations to the test set. To compare the fitness of each models, we used a correlation coefficient (CC) and a mean absolute error (MAE). Results: The random forest model showed the best performance {CC 0.78 [95% confidence interval (CI) 0.43-0.93], MAE 1.20}. The lasso regression model also showed the improved result [CC 0.78 (95% CI 0.42-0.93), MAE 1.26] compared to the Hoffstein equation [CC 0.68 (95% CI 0.23-0.89), MAE 1.34] and the Choi's equation [CC 0.72 (95% CI 0.30-0.90), MAE 1.40]. Conclusions: Our random forest model and lasso model ($26.213+0.084{\times}BMI+0.004{\times}$apnea-hypopnea index+$0.004{\times}oxygen$ desaturation index-$0.215{\times}mean$ oxygen saturation) showed the improved performance compared to the previous reported equations. The further study for other subgroup or phenotype of OSA is required.
Objectives: This was a retrospective case series about the clinical effect of integrated approach with behavior therapy for obstructive sleep apnea (OSA). Methods: Medical records of twelve patients with OSA who were treated with behavior modification including nasal breathing with oral appliances and sleep in lateral position, oral administration of herbal medicines and acupuncture treatment between January and September of 2013 were reviewed. Apnea-hypopnea index (AHI), day time sleepiness, apneas and hypopneas counts during sleep, risk indicator (RI), oxygen desaturation index, average saturation during sleep, lowest desaturation, lowest saturation, snoring events ratio and number of desaturations (%) were assessed before and after treatments with the ApneaLink device, which is a portable diagnostic apparatus for monitoring airflows of the patient's breath at home. Results: After an average 62.67 (SD 37.16) days of treatment, AHI (from 17.67, 12.79 to 8.75, 8.25, p=0.007), RI (from 22.00, 13.26 to 12.09, 8.03, p=0.004), oxygen desaturation index (from 17.33, 12.17 to 8.17, 7.86, p=0.005), and number of desaturations (from 7.00 times, 9.79 to 0.92 times, 1.39, p=0.044) showed significant improvement. Daytime sleepiness improved from 6.5 (3.2) to 3.8 (1.8) but there was no significant difference after treatment (p=0.17). No adverse events related to treatment were observed during participation in the treatment. Conclusion: From this case series, we found that behavior modification with herbal medication and acupuncture may be effective for improving sleep apnea without serious adverse events. Future randomized controlled trials with larger sample size will be necessary for concrete evidence on the benefit of this integrated treatment for OSA.
Positive airway pressure (PAP) is currently recommended as a primary treatment for obstructive sleep apnea syndrome (OSAS) and positively affects various subjective and objective parameters related to OSAS, such as the apnea-hypopnea index, excessive daytime sleepiness, and blood pressure. However, PAP also exhibits various adverse effects, including skin breakdown, pressure intolerance, claustrophobia, unintentional mask removal, mouth leaks, and dryness. Especially, unintentional mask removal due to nasal obstruction may result in poor PAP compliance. A 47-year-old male patient with severe OSAS who had low PAP compliance due to nasal obstruction underwent nasal surgery. After the surgery, nasal obstruction was corrected and the patient experienced improved PAP compliance (from 30.4% to 86.7%). This case demonstrates that nasal surgery may be useful for improving PAP compliance in OSAS patients with nasal obstruction.
배 경:수면은 크게 비렘수면(non-REM sleep)과 렘수면(REM sleep)으로 나눌 수 있으며 렘수면 중에는 근력의 소실로 상기도 확장 근육의 긴장도가 저하되고 상기도 저항은 증가하여 무호흡증이 발생하거나 악화될 수 있다. 폐쇄성 수면무호흡증 자체에 대해서는 이미 많은 부분이 알려져 왔으나 렘수면에만 특이적으로 나타나는 렘수면 의존성 폐쇄성 수면무호흡증(REM-dependent OSA)은 그 중요성이 간과되어 왔다. 본 연구에서는 REM-dependent OSA 환자들의 임상 특성과 수면 변인을 살펴보고자 하였다. 방 법:2004년 3월부터 2007년 2월까지 시립은평병원 수면검사실을 방문하여 경도나 중등도의 폐쇄성 수면무호흡증으로 진단받은(5$53.7{\pm}16.7$, 남성:42명)를 대상으로 하였다. REM-dependent OSA(AHIREM/AHINREM${\ge}$2) 환자군과 No REM-dependent OSA(AHIREM/AHINREM<2) 환자군의 임상적 특성과 수면 관련변인을 비교하였다. 통계분석은 Statistica 6.0 프로그램을 사용하였으며, 유의수준은 p<0.05로 하였다. 결 과:대상군 56명 중 21명(37.5%)가 REM-dependent OSA으로 진단되었다. 두 군사이의 연령, 성별, BMI (Body Mass Index)는 유의한 차이를 보이지 않았다. 성별, 연령, BMI, PLMI를 통제한 후, 수면 관련 변인 중 REMdependent OSA 환자군에서 AHI(Apnea Hypopnea Index), ODI(Oxygen Desaturation Index)가 유의하게 낮았다($12.1{\pm}6.4$ vs. $35.9{\pm}24.7$, p=0.010; $6.0{\pm}5.3$ vs. $24.0{\pm}20.4$, p=0.006, respectively, ANCOVA). REMdependent OSA 환자군에서유의하게 높은 2단계 수면이 관찰되었다($58.5{\pm}10.3$ vs. $51.2{\pm}13.2$, p=0.031, ANCOVA). REM-dependent OSA군으로 대상군을 한정하였을 때, 성별, 연령, BMI, PLMI를 통제한 후, 렘수면중 AHI는 전체 수면중 AHI, 비렘수면중 AHI, ODI와 유의미한 양의 상관관계를 보였다(r=0.720, p=0.001;r=750, p=0.001;r=0.749, p=0.001, partial correlation, respectively). 결 론 :본 연구에서는 폐쇄성 수면무호흡증 환자 중 37.5%를 REM-dependent OSA 환자로 진단할 수 있었다. REM-dependent OSA 환자는 더 경한 정도의 수면 무호흡증을 보이는 경향이 있었으며, 2단계 수면 분율의 증가 같은 수면구조의 변화와 연관되어 있었다. 또한, 렘수면중의 무호흡은 산소포화도 저하와 연관되어 있었다.
목 적 : 폐쇄성 수면무호흡증(obstructive sleep apnea)은 수면 시 호흡을 하려고 하는 노력에도 불구하고 기도의 부분적/완전 폐색이 반복되는 수면 관련 호흡 질환이다. 일반적으로 렘수면에서 근긴장도의 저하가 나타나게 되므로 렘수면에서 수면무호흡증의 악화가 예상되지만 많은 경우에 있어 비렘수면에서의 무호흡/저호흡 지수(apnea-hypopnea index, AHI)가 렘수면에서의 AHI보다 높은 경우가 관찰되었다. 그러나 그런 현상의 분명한 이유는 밝혀져 있지 않았기에 렘/비렘수면 의존성 수면무호흡증에 대한 특징을 종합적으로 알아보고자 하였다. 방 법 : 560명의 성인 수면무호흡증 환자를 대상으로 후향적 연구를 진행하였다. 모든 환자는 렘 수면시의 AHI (REM-AHI), 비렘수면시의 AHI (NREM-AHI)의 비율에 따라 세군으로 분류하였다. 렘수면 의존성 수면무호흡증(REM sleep dependent OSA, REM-OSA)는 REM-AHI/NREMAHI > 2, 비렘수면 의존성 수면무호흡증(NREM sleep dependent OSA, NREM-OSA)는 NREM-AHI/REM-AHI > 2, 수면 단계 비의존성 수면무호흡증(Sleep stage independent OSA, IND-OSA)로 분류하였다. 수면다원검사 이외에도 주관적인 수면의 질, 낮시간 졸림, 정서와 연관된 설문지를 함께 시행하였다. 카이제곱 검정, 일원배치분산분석, 공분산분석을 시행하였다. 결 과 : 세 군 사이에서 연령의 차이는 보이지 않았다. REM-OSA는 대체로 경도의 수면무호흡증 환자가 많았고 여성의 비율이 가장 높았다. 또한 주관적인 수면의 질이 가장 저하되어 있다고 느꼈으며 기분 상태 평가 척도인 K-POMS의 점수는 가장 높았다. IND-OSA는 BMI, 목둘레(neck circumference, NC), 복부 둘레(abdominal circumfefence)가 REM-OSA에 비해 높았고 주간 졸림의 호소는 세군 중 가장 컸고, 무호흡 관련 변인의 값이 가장 높아 중증도가 심한 군이었다. 공변량을 통제하고 시행한 공분산 분석의 결과에서 NREM-OSA는 앙와위에서의 AHI가 가장 높았고 수면 중 측와위의 비율이 가장 컸다. 또한 수면 효율이 가장 떨어졌고 렘수면 잠복기가 가장 길었으며 입면 후 각성 시간의 비율이 가장 컸다. 결 론 : REM-OSA는 여성이 많고 부정적인 정서를 가장 많이 호소하는 군으로, 경도의 수면무호흡증을 가장 많이 포함하였고 IND-OSA는 중증의 수면무호흡증을 반영하였다. NREM-OSA는 자세 변화와 가장 밀접한 관련은 지니고 있었으며 수면 효율이 가장 떨어졌다. 세 군의 수면무호흡증이 일련의 중중도 차이만이 아닌 또 다른 특성을 반영하는 것으로 보이고 이를 통해 폐쇄성 수면무호흡증의 이해를 넓힐 수 있을 것이다.
렘수면 의존성' 폐쇄성 수면무호흡증(REMOSA)에 대한 연구는 일반적인 폐쇄성 수면무호흡증(OSAS)에 대한 연구에 비해 지난 10여 년 동안 활발한 편이 아니었으며, 그 중요성 역시 간과되어 왔다. REM-OSA가 남자에서보다 여자에서 좀 더 흔히 일어난다고 보고한 연구들은 있었으나, 아직까지 여자 환자군을 대상으로 한 연구는 없었다. 따라서, 이 연구에서는 폐쇄성 수면무호흡증 여자 환자군을 대상으로 하여 렘수면 의존성 무호흡증군(REM-OSA)을 렘수면 의존성이 없는 무호흡증군(SND-OSA)과 비교 연구하여 이 질환의 특성에 대해 살펴보고자 하였다. 방 법 : 2004년 10월부터 2006년 2월까지 서울대학교 병원 수면의학센터에서 야간 수면다원검사를 시행하여 폐쇄성 수면무호흡증으로 진단을 받은(AHI>5 ; AHI:apneahypopnea index) 여자 환자 53명중 비교 연구를 위해 경도(52와 AHI-NR<15(AHI-R : AHI during REM sleep, AHI-NR : AHI during non-REM sleep)을 동시에 만족하도록 기준을 설정하였고, 렘수면에 의존하지 않는 무호흡증환자(SND-OSA)군과 제반 변인들에 관해 비교 분석을 하였다. 그리고 산술적인 진단을 적용한 환자군과 육안판독으로 진단한 환자군도 비교하였다. 결 과 : 총 44명 중 산술적인 진단기준을 통해 REMOSA로 진단된 환자는 28명(63.6%), 육안 판독으로 진단된 환자는 24명(54.5%)이었다. 산술적 진단기준에 따른 REM-OSA군과 SND-OSA군 비교에서 두 군간에 저호흡지수, 무호흡-저호흡지수, 비렘수면 무호흡-저호흡지수, 총 수면시간, 총각성시간, 수면효율, 1단계 수면 분율, 2단계 수면 분율, 렘수면 단계 분율, REM 잠복시간에서 통계적으로 유의한 차이(p<0.05)가 있었다. 진단에 관여하는 변인인 무호흡-저호흡지수 비율(AHI-R/AHI-NR)에 영향을 주는 변인은 렘수면 단계 분율(B=0.537, p=0.002)이었으며, REM-OSA는 중증도가 경할수록(${\chi}^2=13.117$, p<0.001), 렘수면 단계 분율이 클수록(${\chi}^2=13.325$, p=0.001) 더 흔하게 진단되었다. 그리고, 산술적인 진단기준으로 REM-OSA를 진단한 결과와 육안 판독으로 진단한 결과 간에는 통계적으로 유의한 차이가 없었다(p=0.157). 결론 : 이 연구에서 살펴본 바로는 REM-OSA 환자군과 SND-OSA 환자군 간에 야간 수면다원검사로 산출한 수면 변인과 수면 구조의 유의한 차이가 있었다. 이 연구는 폐쇄성 수면무호흡증 여자 환자군을 대상으로 REM-OSA를 국내외 최초로 분석한 점에서 의의가 있다.
Background and Objectives: Obesity is one of the most important risk factors for obstructive sleep apnea (OSA). There is limited evidence regarding the obesity-related anthropometric characteristics of Korean patients. Materials and Method: Medical records of 984 patients referred to 3 tertiary referral hospitals for habitual snoring or sleep apnea were analyzed. We defined OSA as apnea-hypopnea index (AHI) ${\geq}5$ and analyzed data to determine the anthropometric characteristics of patients with OSA such as neck circumference (NC), waist circumference (WC), hip circumference (HC), and waist to hip ratio (WHR). Results: A total of 952 patients (719 men) were included in the analysis. The main findings were: 1) BMI, WC, NC, HC, and WHR were greater among patients with OSA than among controls (AHI <5); 2) for both sexes, the proportion of patients with an OSA diagnosis increased with age; it increased steeply for women aged >50 years; 3) WC and WHR were most strongly correlated with AHI for men and women, respectively. Conclusion: OSA is associated with anthropometric characteristics, although different patterns were observed between men and women. OSA was more strongly associated with NC or WC among men and with WHR among women.

② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
