건강검진 수진 노인의 대사증후군 유병상태 및 관련 요인 (Prevalence and Related Factors of Metabolic Syndrome among Korean Older Adults)

  • Objectives: The purpose of this study is to identify prevalence and related factors of the elderly, who took health examination, with metabolic syndrome. Methods: The health examination and lifestyle survey were performed for 21,512 adults at 60 years of age or older who took health examination in H health promotion center during January-March 2009. Results: The prevalence of metabolic syndrome for the subject was 24.0%. Of the subject with metabolic syndrome, the prevalence of the diseases was obesity 60%, abdominal obesity 78.5%, hypertension 82.6%, dyslipidemia 89.7% and diabetes 51.9%. In comparison of the relationship between metabolic syndrome and other diseases, the male subject with metabolic syndrome were significantly higher in BMI, waist circumference, systolic/diastolic blood pressure, hemoglobin, AST, ALT, $\gamma$-GTP, TG, AC glucose, creatinine than normal male(p<0.001). In comparison of the relationship between metabolic syndrome and lifestyle, more drinking frequency and amount in male and more drinking frequency in female were associated with increased risk of metabolic syndrome(p<0.01). Regardless of exercise intensity, practice of exercise contributed to reduce the risk of metabolic syndrome(p<0.01). Conclusion: In conclusion, TLC program, focused on lifestyle behaviors which is strongly associated with the prevalence of metabolic syndrome, should be developed for the improvement of life quality in the elderly with metabolic syndrome.

국민 영양, 이대로 좋은가? -풍요 속에 심각해지는 영양문제- (Deteriorating Nutritional Status in an Increasingly Prosperous Society)

  • Health outcomes are strongly associated with dietary factors. Poor dietary intake increases health risks and can have adverse affects throughout all stages of live and can be passed onto next generations. Korea faces a double burden of nutritional problems. One one side health problems are related to an inadequate diet including underweight, osteoporosis or anemia etc. On the other side the health problems are related to over consumption, including obesity, hypertension, diabetes, cancer, cardiovascular disease and etc. Improving the national health and nutritional status requires adjustments of nutrition policies that have primarily focused on under nutrition, to accommodate all nutrition concerns within the country. Nutrition research specific to Korean people must be promoted to produce scientific evidences on which to base nutrition policies. The creation of a nutrition-specialized institute is an important starting point.

  • Polyunsaturated fatty acids (PUFAs) are the major components of brain and retina, and are the essential fatty acids with important physiologically active functions. Thus, PUFAs should be provided to children, and are very important in the brain growth and development for fetuses, newborn infants, and children. Omega-3 fatty acids decrease coronary artery disease and improve blood flow. PUFAs have been known to have anti-inflammatory action and improved the chronic inflammation such as auto-immune diseases or degenerative neurologic diseases. PUFAs are used for metabolic syndrome related with obesity or diabetes. However, there are several considerations related with intake of PUFAs. Obsession with the intake of unsaturated fatty acids could bring about the shortage of essential fatty acids that are crucial for our body, weaken the immune system, and increase the risk of heart disease, arrhythmia, and stroke. In this review, we discuss types, physiologic mechanism of action of PUFAs, intake of PUFAs for children, recommended intake of PUFAs, and considerations for the intake of PUFAs.

사상체질병증 임상진료지침: 예방 및 위험인자 (Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.

수풍순기환 투여가 고지방, 고탄수화물 식이로 유발된 비만형 제2형 당뇨병 동물모델에 미치는 영향 (The Effects of Supungsunki-hwan on High Fat, High Carbohydrate Diet-induced Obese Type 2 Diabetic Mouse Model)

  • Objective : Obesity is an important cause of diabetes, and lipotoxicity causes insulin resistance. Recently a lot of research is being done on PPAR-${\alpha}$. TNF-${\alpha}$. adiponectin, and leptin, which are important obesity related factors. In this study, we investigated the effects of Supungsunki-hwan on high fat. high carbohydrate diet-induced obese type 2 diabetic mouse models. Methods: Diabetes was induced in ICR male mouse (30${\pm}$5g) with Surwit's high fat, high sucrose diet. Mice were divided into 4 groups(n=10) of Normal. Control. Supungsunkj-hwan group. and acarbose group. The Supungsunki-hwsn group was given 10% Supungsunkj-hwan in their diet. and the acarbose group was given 0.5% acarbose in their diet. After 6 weeks. body weight. food intake, FBS and OGTT. lipid profile and liver enzymes, epididymal fat weight, and gene expression of leptin, adiponectin, TNF-${\alpha}$ and PPAR-${\alpha}$ were measured. Leptin. adiponectin. tumor necrosis factor(TNF)-${\alpha}$ and peroxisome proliferator-activated receptor (PPAR)-${\alpha}$ were evaluated by reverse transcription-polymerase chain reaction. Results : Supungsunkj-hwan increased the gene expression of PPAR-${\alpha}$, which reduces lipotoxicity and insulin resistance. Supungsunkj-hwan also significantly reduced triglyceride. AST. ALT serum levels. and 1 hour oral glucose tolerance levels. Conclusion : These results show that Supungsunkj-hwan improves insulin resistance in the liver and muscles, by reducing triglyceride levels and lipotoxicity through increased PPAR-${\alpha}$ gene expression. This is supported by the fact that Supungsunkj-hwan significantly reduces 1 hour oral glucose tolerance levels. Therefore we suggest that Supungsunkj-hwan would be an effective treatment for obese type 2 diabetic patients.

CTRP9 Regulates Growth, Differentiation, and Apoptosis in Human Keratinocytes through TGFβ1-p38-Dependent Pathway

  • Impairment of wound healing is a common problem in individuals with diabetes. Adiponectin, an adipocyte-derived cytokine, has many beneficial effects on metabolic disorders such as diabetes, obesity, hypertension, and dyslipidemia. C1q/TNF-Related Protein 9 (CTRP9), the closest paralog of adiponectin, has been reported to have beneficial effects on wound healing. In the current study, we demonstrate that CTRP9 regulates growth, differentiation, and apoptosis of HaCaT human keratinocytes. We found that CTRP9 augmented expression of transforming growth factor beta 1 ($TGF{\beta}1$) by transcription factor activator protein 1 (AP-1) binding activity and phosphorylation of p38 in a dose-dependent manner. Furthermore, siRNA-mediated suppression of $TGF{\beta}1$ reversed the increase in p38 phosphorylation induced by CTRP9. siRNA-mediated suppression of $TGF{\beta}1$ or p38 significantly abrogated the effects of CTRP9 on cell proliferation and differentiation while inducing apoptosis, implying that CTRP9 stimulates wound recovery through a $TGF{\beta}1$-dependent pathway in keratinocytes. Furthermore, intravenous injection of CTRP9 via tail vein suppressed mRNA expression of Ki67 and involucrin whereas it augmented $TGF{\beta}1$ mRNA expression and caspase 3 activity in skin of type 1 diabetes animal models. In conclusion, our results suggest that CTRP9 has suppressive effects on hyperkeratosis, providing a potentially effective therapeutic strategy for diabetic wounds.

경기지역 일부 대학생들의 식생활 관련 심혈관대사질환 건강위험도 평가 (Diet-Related Health Risk Appraisal for Cardiometabolic Diseases of the College Students in Gyunggi-do)

  • This study was conducted to assess diet-related health appraisal for cardiometabolic diseases of the college students in Gyeonggi-do. The survey data obtained by 21 questionnaire(6 general characteristics and 15 food behaviors) given to 90 students, 41 males and 49 females. The average age of the subjects was 22.2 years old for the male students and 20.9 years old for the female students(p<0.001). Among the subjects, 35 males and 47 females' height, weight and percentage of body fat were measured using bioelectrical impedance(Inbody 720). The average height, weight, Body Mass Index and percent body fat of the subjects were 175.7cm, 69.5kg, 22.4 and 16.9% for males and 162.0cm, 53.5kg, 20.4 and 27.7% for females, respectively (p<0.001). Male students had a higher prevalence of smoking compared with female students(p<0.001). Females had higher intake frequency of fruits than males(p<0.05). Males had higher intake frequency of fishes than females(p<0.05). Male students had higher number of side dishes at mealtime compared with female students. There were no difference between the sexes in intake frequency of dairy, ramyeon, rice with whole grain, 3-layered pork belly, processed meat, soft drinks, soy products and fast food. Also, there were no difference between sexes in number of vegetables per day, frequency of eating out, prevalence of eating kimchi at every meal. Cardiometabolic risks were calculated by 'Diet-related health risk appraisal'. Male students had higher cardiometabolic risks(hypertension, hyperlipidemia, diabetes, obesity, abdominal obesity) than female students(p<0.001). Appropriate nutritional education for college male students may help to prevent cardiometabolic diseases in the future.

Chamaeneron angustifolia 추출물이 3T3-L1 Preadipocyte의 지방대사 및 분화에 미치는 영향 (Effects of Chamaeneron angustifolia Extract on Lipid Metabolism and Differentiation of 3T3-L1 Preadipocyte)

  • Objectives : Obesity is an imbalance between energy intake and consumption due to overeating and lack of exercise, and if it persists, it increases non-infectious diseases such as high blood pressure, diabetes, and hyperlipidemia. In this study, we tried to investigate the possibility of using Chamaeneron angustifolia (CA) as a material for anti-obesity by confirming the effect of inhibiting lipid differentiation. Methods : We measured the effects of CA extract on oil-red-o staining, cell cytotoxicity evaluation activity using 3T3-L1 cells. Additionally, we assessed fat decomposition and metabolism-related protein expression through Western blot analysis. Results : In this study, the anti-obesity effects of CA extract were experimentally assessed. Results showed significant inhibition of adipocyte differentiation and accumulation at concentrations of 0.05, 0.1, and 0.2 mg/ml of oil-red-o staining, with reductions of 80% or more. CA notably increased the phosphorylation of AMPK protein expression compared to the control group across all concentrations. Additionally, phosphorylation of ACC significantly increased at a concentration of 0.2 mg/ml compared to the control. PPAR-γ, which regulates adipogenesis, exhibited a significant decrease compared to the control, while protein expression of CPT-1, involved in fatty acid oxidation, showed a concentration-dependent increase across all groups. Therefore, CA extract demonstrates potential as a functional material for anti-obesity by increasing the expression of proteins related to fat decomposition and synthesis while decreasing others. Conclusions : These results suggest that CA may also be useful as an anti-obesity functional substance.

Sinapic acid induces the expression of thermogenic signature genes and lipolysis through activation of PKA/CREB signaling in brown adipocytes

  • Lipid accumulation in white adipose tissue is the key contributor to the obesity and orchestrates numerous metabolic health problems such as type 2 diabetes, hypertension, atherosclerosis, and cancer. Nonetheless, the prevention and treatment of obesity are still inadequate. Recently, scientists found that brown adipose tissue (BAT) in adult humans has functions that are diametrically opposite to those of white adipose tissue and that BAT holds promise for a new strategy to counteract obesity. In this study, we evaluated the potential of sinapic acid (SA) to promote the thermogenic program and lipolysis in BAT. SA treatment of brown adipocytes induced the expression of brown-adipocyte activation-related genes such as Ucp1, Pgc-1α, and Prdm16. Furthermore, structural analysis and western blot revealed that SA upregulates protein kinase A (PKA) phosphorylation with competitive inhibition by a pan-PKA inhibitor, H89. SA binds to the adenosine triphosphate (ATP) site on the PKA catalytic subunit where H89 binds specifically. PKA-cat-α1 gene-silencing experiments confirmed that SA activates the thermogenic program via a mechanism involving PKA and cyclic AMP response element-binding protein (CREB) signaling. Moreover, SA treatment promoted lipolysis via a PKA/p38-mediated pathway. Our findings may allow us to open a new avenue of strategies against obesity and need further investigation.

소형견종에서 Homocysteine과 비만, 당 관련 인자, 지방 관련인자의 상관관계에 대한 연구 (The Relationship between Homocysteine, Obesity, Glucose and Lipid Profiles in Small-Breed Dogs)

  • 본 연구는 건강한 개에서 혈장 homocysteine농도가 비만이나 비만 관련인자(예, 지방, 인슐린, 혈당, 글루카곤, fructosamine)들과 어떤 상관관계가 있는지를 확인하기 위해 실시되었다. 이를 위해, 절식시 혈당, 지방지수(예, total triglycerides [TG], total cholesterol [TC], high-density lipoprotein cholesterol [HDL-C]과 low-density lipoprotein cholesterol [LDL-C]), fructosamine, insulin 및 glucagon 농도를 각각 측정하였다. 실험에 사용된 개들은 body condition score (BCS)에 따라 분류하였다. 평균 혈장 homocysteine 농도는 비만한 개 집단이 정상이나 마른 개 집단에 비해 상당히 높았다. 또한 혈장 homocysteine농도는 HDL-C농도 뿐아니라 TG, TC, 혈장 glucagon 및 fructosamine 농도와도 밀접한 상관관계를 가지고 있었다. 반대로 LDL-C 농도, 절식시 혈당농도 및 insulin농도는 아무런 상관관계가 확인되지 않았다. 본 연구 결과, homocysteine의 농도 상승이 비만견에서 생물학적으로 중요한 역할을 하는 것으로 추정된다.
