한국 성인의 지방산 섭취와 복부비만 및 고혈당과의 관련성 -1998~2007 국민건강영양조사 자료에 근거하여- (Fatty Acids Intake and Its Association with Abdominal Obesity and Hyperglycemia in Korean Adults: Korea National Health and Nutrition Survey, 1998~2007)

  • Abdominal obesity is an important factor of metabolic syndrome and is known as an independent risk factor of cardiovascular disease. The purpose of this study was to examine the fatty acid intake pattern as well as the association between fatty acid intake and abdominal obesity and hyperglycemia in Korean adults. This study was based on data from the Korea National Health and Nutrition Examination Surveys (KNHANES) I, II, III, and IV. After excluding subjects who had missing data for dietary intakes, anthropometric measurments, and laboratory tests, a total of 12,320 subjects aged 20~64y (5,266 men and 7,054 women) were included in the study. Trends in individual consumption of dietary fatty acids by year were analyzed by One-way ANOVA test, and the association between fatty acid intake and abdominal obesity and hyperglycemia was analyzed by logistic regression analysis after adjusting for age, energy intake, marital status, job, education level, alcohol intake, and smoking status. In men, intakes of TFA, SFA, MUFA, FUFA, and n-6 PUFA significantly increased with year. On the other hand, intakes of TFA, SFA and FUFA significantly increased with year in women. Logistic regression analysis showed that the risk of abdominal obesity was significantly associated with intakes of PUFA, LNA and n-3 PUFA in women. However, there was no association with any fatty acid intakes in men. The risk of hyperglycemia was significantly and positively associated with intakes of TFA, SFA, PUFA, and LNA in men, but, there was no significant association with any fatty acid intakes in women. These results show that abdominal obesity and hyperglycemia are significantly associated with individual intake of fatty acids, suggesting that fatty acid type may be associated with risk of abdominal obesity and hyperglycemia. Futher, the macronutrients of measl must be properly balanced.

  • Purpose: This study was aimed to develop and evaluate an obesity-management program for middle-aged obese women. Methods: Two 12 week session of the obesity-management programs constituted of weekly education and exercise such as aquarobic, pilates & yoga, and power-walking during 60 min for 3 days a week from June to December, 2006. The subjects included 47 middle-aged obese women between 30 and 60 yr. The effectiveness of the program was evaluated according to the change of body weight, body component, abdominal circumference, and serum lipid concentration. Data were measured by Inbody, tape measure, and blood test. Data were analyzed by descriptive statistics, and a paired t-test with an SPSS/PC. Results: There were statistically significant decreases in body weight, body mass index, body fat mass, Hemoglobin, abdominal circumference, total cholesterol level, and low density lipoprotein after the program was completed. But there were not statistically significant difference in fat free mass, muscle mass and triglyceride. Conclusion: The results of this study showed that an obesity-management program have positive effects in body composition, abdominal circumference, and lipid metabolism for middle-aged obese women.

Association of Sleep Duration and Obesity According to Gender and Age in Korean Adults: Results from the Korea National Health and Nutrition Examination Survey 2007-2015

  • Background: This study aimed to investigate associations between self-reported sleep duration and general and abdominal obesity in Korean adults stratified according to gender and age. Methods: Data from 41,805 adults, 18-110 years of age, collected by the Korea National Health and Nutrition Examination Survey (KNHANES) in 2007 and 2015, were analyzed. Multivariable logistic regression was used to calculate adjusted odds ratios (AORs) and 95% confidence intervals (CIs) for obesity and abdominal obesity by sleep duration after controlling for sociodemographic and lifestyle variables. Results: Among individuals 30-49 years of age, there was an increased AOR for obesity only for sleep duration ${\leq}5hour/day$ compared with sleep duration 6 to 8 hour/day, both in men (OR, 1.25; 95% CI, 1.02-1.54) and women (OR, 1.56; 95% CI, 1.29-1.90), after controlling for covariates. Regarding women, there was increased AOR for abdominal obesity for sleep duration ${\leq}5hour/day$ (OR, 1.45; 95% CI, 1.18-1.78) and ${\geq}9hour/day$ (OR, 1.38; 95% CI, 1.09-1.76) compared with sleep duration 6 to 8 hour/day. However, for elderly individuals (${\geq}65years$), there was a negative association between sleep duration ${\leq}5hour/day$ and obesity, but not with abdominal obesity, in both men and women. Conclusion: This study demonstrated a significant association between sleep duration and obesity, which varied according to gender and age.

젊은 비만 여성의 심박변이도 특성에 대한 연구 (A Study on Characters of Heart Rate Variability in Young Overweight and Obese Woman)

  • Objectives Obesity is the major risk factor of the heart disease and the metabolic disease. The autonomic nervous system is a key contributor in the regulation of energy balance, so the blunted activity may contribute to the maintenance of the obese state. So we evaluated the function of the autonomic nervous system in young overweight and obese women with heart rate variability. Methods The subjects were 26 overweight and obese young women(BMI〉$23kg/m^2$) and 25 lean women who visited Kangnam Kyunghee hospital for obesity management from March 2006 to April 2008. Bioimpedance Analysis (BIA), short-term spectral analysis of heart rate variability (HRV) were performed. Results HRV of obese young group was lower than the lean young group, but there is no statistical significance. BMI had significantly a negative correlation with Low Frequency(LF) of HRV. LF power is mediated by sympathetic nervous system activity. These results indicate a decrease of sympathetic modulation in overweight and obese young women. Conclusions Overweight and obese young women have decreased sympathetic nervous system activity. In clinical practice, an assessement of HRV would be noninvasive and sensitive methods for sympathetic nervous system of young overweight and obese women.

어머니의 영양지식에 따른 유아기 자녀의 비만도 및 식습관의 차이 (Mothers' Nutrition Knowledge and Their Preschoolers' Obesity and Dietary Habits)

  • The principal objective of this study was to determine the effects of mothers' nutrition knowledge on preschoolers' obesity and dietary habits. The subject population included 456 mothers and their preschoolers, aged 3 to 5 years, who were enrolling in kindergartens located in Seoul and KyongGi-Do, Korea. The results of this study are as follows: 1. The majority of mothers responded correctly to those questions concerning obesity-related nutritional knowledge, but responded incorrectly to questions regarding basic nutritional knowledge. 2. The mothers' nutritional knowledge was associated significantly with the mother's age (p<0.05) and degree of education (p<0.05). 3. The mothers' nutritional knowledge was significantly and negatively correlated with their obesity, but not with their preschoolers' obesity. 4. Mothers' nutritional knowledge was determined to be positively associated with higher interest in nutritional information and food selection in regard to weight management strategies for their kids. 5. In general, preschoolers whose mother had excellent nutrition knowledge evidenced better dietary habits than preschoolers whose mothers had poor nutrition knowledge. In particular, one question regarding serving size was significantly associated with mothers' general level of nutritional knowledge(p<0.05).

체중과 심혈관 질환에 대한 폐경기 호르몬 요법의 효과 (Menopausal Hormone Therapy for Preventing Weight Gain and Cardiovascular Disease)

  • Estrogen is crucial in regulating food intake, energy expenditure, glucose metabolism, and lipid metabolism. During menopause, the decline in estrogen levels predisposes women to weight gain, abdominal obesity, insulin resistance, type 2 diabetes, hypertension, and cardiovascular disease (CVD). Menopausal hormone therapy (MHT) prevents weight gain, improves lipid metabolism by lowering low-density lipoprotein cholesterol while raising high-density lipoprotein cholesterol, and delays the onset of type 2 diabetes in menopausal women. The effect of MHT on CVD in menopausal women remains controversial. The Women's Health Initiative study was terminated prematurely after it revealed that hormone administration increased the risk of myocardial infarction, stroke, and thromboembolism. However, some studies have found that MHT had no effect or decreased the risk of CVD. The inconsistent results were likely due to multiple factors, including the timing of hormone therapy initiation, duration of therapy, type and dosage, and presence or absence of CVD risk factors at the start of treatment. Despite its benefits in terms of managing weight gain and reducing the risk of type 2 diabetes, dyslipidemia, and CVD associated with obesity, it is not recommended as the primary therapy for weight loss or diabetes prevention. MHT is primarily indicated for postmenopausal women, who are likely to benefit from its potential to prevent weight gain and improve lipid metabolism.

청소년기 비만지표와 초기 성인기 경동맥 내중막 두께와의 관련성: Kangwha Study (The Association between Obesity Indices in Adolescence and Carotid Intima-media Thickness in Young Adults: Kangwha Study)

  • Objectives : The aim of this study is to investigate the association between obesity indices(body mass index, weight, waist-hip ratio and waist circumference) in adolescents and the carotid intima-media thickness (C-IMT) in early adulthood. We also wanted to identify the best predictor for C-IMT among these obesity indices. Methods : This study used community-based prospective cohort study, known as the Kangwha Study, and the data we used were from subjects who were 16-years old in 1996 (defined as "adolescence") and 25 years-old in 2005 (defined as "early adulthood"). The 256 subjects (113 men and 143 women) who were used for analysis participated in both follow-ups, and they underwent B-mode ultrasonography of the carotid arteries at the early adulthood follow-up. Obesity indices were defined as the body mass index, weight, waist-hip ratio and waist circumference. The C-IMT was defined as the mean of the maximal IMT of each common carotid artery. The C-IMT and obesity indices associations were evaluated via multivariable regression, logistic regression and the receiver-operator characteristic curve analyses. Results : In men, all the obesity indices in adolescence were showed to have statistically significant positive association with C-IMT in early adulthood. However, no such relationship was showed in women. On multiple regression and logistic regression analysis, the waist-hip ratio showed the biggest relationship with the C-IMT among the 4 obesity indices. However, there were no statistical significant differences and no best predictor was found. For the women, the obesity incidences and C-IMT showed no relationships. Conclusions : This study suggested that obesity in adolescence was related to an increase C-IMT in healthy young Korean men.

젊은 성인에서 비만이 심혈관대사질환 위험인자 군집에 미치는 영향 (Impact of Obesity on Cardiometabolic Disease Risk Factor Clusters among Korean Young Adults)

  • Background: This study investigated the association between obesity and cardiometabolic disease clusters in Korean adults aged 19-39 years. Methods: We included 3,785 participants (1,767 men and 2,018 women) aged 19-39 years who participated in the Korea National Health and Nutrition Examination Survey (2020-2022). The risk factors for cardiometabolic disease were defined as follows: high blood pressure; hypertriglyceridemia; hyperuricemia; and high low-density lipoprotein cholesterol, low high-density lipoprotein cholesterol, high fasting plasma glucose, elevated alanine aminotransferase levels. Results: The prevalence of participants with ≥ 1, ≥ 2, and ≥ 3 cardiometabolic disease risk factors was 95.2%, 84.2%, and 65.6% in men and 90.0%, 70.5%, and 43.0%, in women with class 2-3 obesity, respectively. Compared to those for young adults with underweight/normal weight, the odds ratios (ORs) and 95% confidence intervals (CIs) for the clustering of cardiometabolic risk factors were 17.26 (8.32-35.80) for ≥ 1; 19.43 (12.18-31.00) for ≥ 2; and 22.93 (14.15-37.15) for ≥ 3 factors in men with class 2-3 obesity and 14.67 (8.00-26.91) for ≥ 1; 20.88 (12.15-35.88) for ≥ 2; and 36.26 (20.20-65.09) for ≥ 3 factors in women with class 2-3 obesity. Conclusion: Although the patients were young, the prevalence and ORs for cardiometabolic disease risk factor clusters in young adults with obesity were prominently high. Prevention and management of obesity in young Korean adults are urgently needed at both individual and public levels.

A Study on Body Image Perception and Obesity Stress by the Degree of Obesity in College Women

  • The principal objective of this study was to assess the weight control behavior, body shape satisfaction, and obesity stress depending on the degree of obesity in college students who applied for a weight control program. The average BMI of the participants was $21.4\;kg/m^2$ which was in the normal range, and 76.3% of the participants were of normal body weight. Ideal BMI in this participant was $18.1\;kg/m^2$, which is in the underweight range. In terms of body perception, participants generally overestimated their body weight. The overweight and normal weight groups were less satisfied with their body shape than was the underweight group. The overweight group also expressed fears of showing their bodies to others. Additionally, the overweight and normal groups exhibited higher obesity stress than the underweight group. 97.0% of participants were interested in weight control as a way to improve their appearance. They usually obtained their weight control information from the internet and mass media. More than 80% of participants had weight control experience, having undergone weight control attempts for duration of less than a month. However, after the discontinuation of weight control efforts, these participants regained the weight. As a consequence, they were generally unsatisfied with the outcomes of weight control programs. This study demonstrated that the college women who had applied for the weight control program were unsatisfied with their body shape owing to distorted notions of the ideal body shape, and these women had generally undertaken frequent efforts to control their body weights. These results underline the importance of educating college-aged women on proper body perception and the maintenance of healthy body weight and shape.

The prevalence of obesity and the level of adherence to the Korean Dietary Action Guides in Korean preschool children

  • The objective of this study was to investigate the associations between the prevalence of overweight and obesity and the degree of adherence to the Korean Dietary Action Guides for Children (KDAGC). In a cross-sectional study based on a child care center-based survey in Seoul, Korea, we collected parental-reported questionnaires (n = 2,038) on children's weight and height, frequency of fruit and vegetable consumption, and the quality of dietary and activity behaviors based on the 2009 KDAGC Adherence Index (KDAGCAI) which was developed as a composite measure of adherence to the KDAGC. Overweight and obesity were determined according to age- and sex-specific BMI percentile from the 2007 Korean national growth chart. Associations were assessed with generalized linear models and polytomous logistic regression models. Approximately 17.6% of Korean preschool children were classified as overweight or obese. Obese preschoolers had lower adherence to the KDAGCAI compared to those with lean/normal weight. Preschoolers with a high quality of dietary and activity behaviors had a 51% decreased odds ratio (OR) of being obese (highest vs. lowest tertile of KDAGCAI-score, 95% CI 0.31, 0.78; P = 0.001); the associations were more pronounced among those who were older (P = 0.048) and lived in lower income households (P = 0.014). A greater frequency of vegetable consumption, but not fruit, was associated with a borderline significant reduction in the prevalence of obesity. Our findings support the association between obesity prevention and high compliance with the Korean national dietary and activity guideline among preschool children.
