공원녹지의 특성과 신체활동 및 건강의 상호관련성 - 창원시를 대상으로 - (Associations between Characteristics of Green Spaces, Physical Activity and Health - Focusing on the Case Study of Changwon City -)

  • 도시의 공원녹지는 지역 주민들의 신체활동과 건강증진을 위해서 중요한 역할을 담당할 수 있기 때문에, 본 연구에서는 공원녹지의 다양한 특성과 신체활동 및 건강증진 목적의 공원녹지 이용의 상호관련성을 분석하고자 한다. 설문조사는 경상남도 창원시에 거주하는 541명의 주민들을 대상으로 공원녹지의 이용패턴과 주관적인 근린환경 인식을 파악하기 위해서 실시하였다. 사례지역에 대한 공원녹지의 접근성과 물리적 근린환경에 대한 공간정보를 구축하기 위해서 지리정보시스템(GIS)을 이용하였다. 다중회귀분석은 공원녹지의 특성과 신체활동 목적의 공원녹지 이용횟수, 자가인식 건강수준, 그리고 체질량지수(BMI)와의 상호관련성을 규명하기 위해 수행하였다. 거주지로부터 200m 이내에 분포하는 공원녹지의 출입구와 공원의 개수, 집 주변에서 운동하는 사람을 많이 볼 수 있거나, 저렴하게 이용할 수 있는 운동시설이 많다고 느낄수록 신체활동의 증가에 긍정적인 영향을 미치는 것으로 나타났다. 집 주변 공원녹지까지의 거리가 가깝고, 공원녹지의 개수가 많고, 면적이 넓을수록, 보행이 편리할수록, 공동주거지역의 비율이 높을수록 자가인식 건강수준(perceived health level)에 긍정적인 영향을 미치는 것으로 나타났다. 거주지로부터 400m 이내에 분포하는 공원녹지의 개수, 보행환경의 안전성, 공동주거지역의 비율, 도로비율, 교차로 밀도 등이 BMI와 상호관련성이 있는 것으로 나타났다. 독립변수인 공원녹지의 특성과 종속변수인 신체활동 목적의 공원녹지 이용횟수 및 자가인식 건강수준 사이의 다중회귀분석 결과, 유의수준 10% 이내에서 의미가 있는 회귀모형이 도출되었다. 본 연구는 공원녹지와 근린환경의 특성이 지역주민들의 신체활동과 건강에 미치는 영향을 규명함으로써, 향후 신체활동 목적의 공원녹지의 이용을 증진하고 비만을 감소시키기 위한 목적의 조경계획을 수립하는데 활용 가능할 것이다.

과체중·비만인에서 필라테스 운동 순응도에 따른 식생활 변화, 체구성, 신체 균형도 및 대사지표 개선효과 (Changes in body composition, body balance, metabolic parameters and eating behavior among overweight and obese women due to adherence to the Pilates exercise program)

  • 본 연구는 과체중 및 비만 또는 복부비만 성인을 대상으로 8주간 중강도 수준의 필라테스 운동 후 필라테스 운동의 순응도에 따른 신체조성, 균형능력 및 기초대사 지표의 변화를 확인하였고, 식생활습관 개선교육후 영양지수의 변화도 함께 살펴보았다. 필라테스 운동에 대한 순응도가 높은 군이 순응도가 낮은 군에 비해 신체의 균형도와 체구성에 더 긍정적인 변화를 보였고 더불어 혈관노화지수도 유의적으로 개선되었고 공복혈당과 당화혈색소가 유의적으로 개선되었다. 그러나 필라테스 운동에 대해 순응도가 낮은 군이라도 실시하지 않았던 것에 비해서 신체균형도, 체질량지수 및 피하지방 면적은 유의적으로 개선되었다. 이는 순응군과 비순응군 모두에서 영양지수가 개선된 것과, 순응도 여부에 관계없이 전체 대상자에서 식생활 교육 후에 하루 식사횟수, 아침식사횟수, 식사의 규칙성, 과식 및 외식횟수 등이 긍정적인 방향으로 변동된 것과 관련이 있을 것으로 판단된다. 따라서, 본 연구는 필라테스 운동과 식생활 개선교육을 포함한 전반적인 생활습관의 개선을 통해 과체중 이상의 여성에서 유의미한 개선 결과가 있음을 보여주고 있다. 더불어 식생활습관 개선 교육만으로도 어느정도 비만관련 지표의 개선에 영향을 줄 수 있을 것이며, 필라테스 운동을 함께 실시했을 때에는 그 개선 효과가 좀더 잘 관찰되는 것으로 판단된다.

서울지역에 거주하는 95세 이상 남녀 초고령 노인의 체격지수와 식습관 및 영양섭취 실태 (Anthropometric Index, Dietary Habits and Nutrient Intake of the Oldest-old Population Aged 95 and Over Living in Seoul)

  • The population aged 95 years and older in Seoul approximately increased to five-fold over the past 10 years, while nationwide rates increased to three-fold. In order to examine the dietary habit and nutritional status of oldest-old population living in Seoul, we recruited 87 subjects (25 males and 62 females) aged 95 years and older. The prevalence of underweight (BMI < 18.5 $kg/m^2$) and obesity (BMI ${\geq}25kg/m^2$) were 18.2% and 18.2% in males, and 20.8% and 9.4% in females, respectively. In self-assessment of health, only 25.3% answered to be unhealthy. More males exercised regularly and reported a wide range of activities than females. The average of %Kcal from carbohydrate, protein and fat (C : P : F) was 64.9 : 13.8 : 21.2 in males and 68.1 : 14.2 : 17.7 in females. The average daily energy intake was 1,307 kcal in males and 1,304 kcal in females. More than 75% of subjects were taking under estimated average requirements (EAR) for vitamin $B_1$, $B_2$ & C and Ca. The average of mean adequacy ratio (MAR) was 0.66 in males and 0.70 in females, and 28.8% of males and 12.9% of females were in MAR < 0.50. Based on MAR, 32.0% of males and 14.5% females were classified as normal and 16.0% of males and 25.8% of females were classified as malnourished. Our subjects were taking more animal food, especially milk and its products, compared to those living, in rural areas. However, a significant proportion did not meet the EAR for vitamin $B_1$, $B_2$ & C and Ca.

일부 보육교사의 영양지식과 식생활태도 및 영양교육 요구 조사 (A Study on Nutrition Knowledge, Dietary Attitudes and Nutrition Education Needs among Child-Care Teachers)

  • This study was conducted on 175 child-care teachers, who participated in in-service education, to research the methods to improve child-care teacher's nutrition management capability for infants and children. Investigated results of child-care teachers' nutrition knowledge, dietary attitude status, and needs on nutrition education in child-care centers are as follows: The score of child-care teachers' nutrition knowledge was 10.83 points out of 15, which is about 72%. Total score increased as teachers' age but not significantly different from their career duration, since teachers who have a child-care career less than 5 years acquired 10.91 points, which is higher than 10.64 points of teachers having more than 5 years of child-care career. Teachers' average recognition to the nutrition knowledge was 90.6%, increased significantly by the older they are, and decreased according to the accumulation of their career. The average accuracy of the nutrition knowledge was 79.7%, increased in proportion to the teachers' age. The marks of child-care teachers' dietary attitude were 41.3 points (possible score range 5-50) and 83%, older teachers tended to have more desirable dietary attitude. As indicated by the increment of child-care career, the score of emotional attitude tended to be increased but which of cognitive and behavioral attitude showed a declining tendency. Nutrition information which child-care teachers were mainly interested in were correct selection of food (58.1%), obesity and weight management (52.7%), and nutrient content of food (44.9%). Nutrition education contents which child-care teachers needed were 'nutritious food and menu for child' (72.2%), 'health management of child' (69.2%) and meal management of child (40.2%). Nutrition education methods, which child-care teachers considered as of desirable ones, were cooking class of small scale (31.8%), visiting class at child-care center (26.5%). In consequence, the nutrition knowledge and dietary attitude of child-care teachers were not good and showed different issues by age and career duration. Therefore, it is requisite to intensify nutrition management courses in child-care teachers' qualification and in-service education courses which has actual necessity and suitability based on teachers' age, career, and the type of child-care center, and to disseminate these through public health centers and child-care & education information centers to pursue the efficient balance of nutrition education programs.

비만아(肥滿兒)의 신체발육(身體發育)과 식습관(食習慣)에 관(關)한 연구(硏究) (A Study on the Physical Growth and Food Habits of Obese Children)

  • For the purpose of ascertaining the status of physical growth and food habit of obese children, a study was made of 1086 primary school children ranging between 2nd and 3ra grades living in Kwang-ju City in Korea. 66 subjects were selected from an anthropometrical point of view and they were classified into two groups, an obese group and a control group, according to their relative body weight. Food habits of each group were observed by means of questionnaires. In the statistical analysis of the data, percentage calculation and T-score test were used. The results of the study can be summarized as follows; 1) Viewed from relative body weight, 3.0% of the 1,086 subjects turned out to be obese, and the measure of skinfold showed 2.4%. 2) Physical growth of the obese growth is superior to that of the control group in all aspects. 3) The average score of food preference by means of Hedonic scaling method by the obese group is 3.90, while that of food preference by the control group is 3.76. The most favored food by the obese group is ice cream (4.91), while that of most favored food by the control group is banana (4.75). The statistically meaningful foods of the two groups are cucumber (0.02>p>0.01), orange (p<0.02), peach(0.05>p>0.02), watermelon (p <0.01) and ice cream (p<0.01). The score of food preference for protein foods by the obese group is 3.78 and that of food preference by the control group is 3.57(0.05>p>0.02). The excessive food intake per meal of the obese group is 45.4%, while that of the control group is 19.7% (0.05>p>0.02). Ratio of taking severely unbalanced food by the obese group is 15.1%, while that of the control group is 50.0% (p<0.01). 4) 30.3% of mothers of the obese group is fat, while 15.1% of mothers of the control group is fat (p<0.01). Concerning body weight at birth, 90.9% of the obese group and 63.6% of the control group are above-average weight. (p <0.01). From the results shown above, the obesity in children is almost due to food. It is hoped, therefore, that overeating be discouraged through the nutrition education.

여성 골다공증의 위험요인에 관한 환자-대조군 연구 - 서울지역 1개 병원 내원환자를 중심으로 - (A Case-control Study on Risk Factors of Osteoporosis in Some Korean Outpatient Women of One General Hospital of Seoul)

  • Until now there are few available epidemiologic data of osteoporosis in Korea, and the severity of osteoporosis-related health problem has not been widely recognized yet. But the numbers of the old people are increasing in Korea, and in 2000, the proportion of people over 65 will be up to about 6.8% of total population. Therefore, osteoporosis, one of the most common metabolic bone disease among the old people, will be one of the most important public health problem. On this background this study was performed to find out risk factors of the development of osteoporosis in Korean women through case-control approach. The subject of this study were selected among the women who visited menopause clinic in one general hospital in Seoul and were checked bone density from Sep. 1988 to Sep. 1993. Those who were diagnosed to have hypertension, diabetes mellitus, thyroid disease, breast disease, or liver disease, which are thought to influnce bone density, were excluded. Also excluded those who are age-unknown. Finally the subjects were 2,139 women aged between 18 and 79. We operatively defined patient group as those whose bone density is below $1.039/cm^2$, 90% of average bone density of women of 4th decade who visited the same hospital. And we defined control group as whose bone dendity is above $1.15g/cm^2$. We randomly selected 201 women from the patient group and 202 from the control. As independent variables we chose age, menarche age, menopause age, menopause type, the number of siblings, the number of pregnancies, body mass index, taking oral pill or not, feeding type, and educational state. Multiple logistic regression analysis was done to see the influence of these variables on the risk of osteoporosis. Results are as follows; 1. menopausal status was statistically significant risk factor to all women irrespective of her age, while obesity and later menopause age were found to be statistically significant protective factors. 2. The more siblings and pregnancies, the greater tile risk of osteoporosis, but these factors were not statistically significant. This result is not consistent with other studies. Further studies are strongly needed.

글로벌 플러스 사이즈 파워 블로그에 나타난 여성 패션 연구 -미국 시장을 중심으로- (The Analysis of Fashion Styles from Global Plus-size Woman's Power Blog -Focused on Analysis of USA Market-)

  • The world wide increase of obesity and demands for various plus-size fashion are expanding the global plus-size fashion market. This study is to understand the market status of the US (the biggest plus-size fashion market) as well as analyze women's fashion shown in power blogs on the plus-size fashion trend. For research methods, photos from the top 10 globally ranked plus-size power blogs on Alexa.com were collected and divided into 5 plus-size body types based on: design factors, style, color, item, and texture. Pants with tops or completed outfits including pants, tops, and outer were the most common for casual styles; in addition, a tendency to pursue comfortable and naturally fitted clothes was also indicated. As for colors, the most common were blue colors and white or pastel toned colors; in addition, soft, hard, and transparent were all evenly used for materials. One-piece items were the most popular formal style that were mostly a one-tone color made with hard materials indicated by the pursuit of the fanciness and formality of a dress for a formal occasion. Black was the most common color, and the color variation was less diverse compared to that of casual styles. The most common for semi-formal styles were outfits with movability and more fanciness such as wearing a casual outer on top of a formal one-piece. When examining the fashion in plus-size blogs, there are differences in the frequency of design factors due to the diversity of body-types; in addition, different items were shown to be preferred in accordance with styles. The results of this study will help fashion companies who want to enter the global plus-size women's fashion market (including the US market); in addition, research on plus-size fashion that is changing the fashion and aesthetic paradigm is expected to contribute to academia.

부산, 경남지역 청각장애 고등학생의 영양지식, 식행동 및 영양소 섭취 상태 (A Study on the Nutrition Knowledge, Dietary Behaviors and Nutrient Intakes of the Deaf-mute High School Students)

  • This study was conducted to provide basic information on the nutritional status and dietary behaviors of the deaf teenagers. The subjects of this study were 87 deaf-mute high school students in Pusan and Kyung-nam area and, in comparison,90 general high school students in Ham-an area. The survey was investigated by using a self-administered questionnaire. The results were as follows: Deaf-mute group was consisted of 63.2% male and 36.8% female, and mean of height and weight of male were 171.2 cm and 61.9kg, female were 158.0cm and 51.7kg. Obesity index and body mass index (BMI) were normal range in both deaf-mute and normal groups. The average nutrition knowledge score of the deaf-mute group was 6.8 $\pm$ 1.5 out of possible 10 points, that was significantly lower than 7.3 $\pm$ 0.8 of the normal group (p<0.01). The mean of self-control and conscious control scores in the eating behaviors of the deaf-mute group were 2.6 $\pm$ 1.2 and 2.4 $\pm$ 1.3 out of possible 5 points from each item, which was significantly higher than 2.1 $\pm$ 1.3 and 1.8 $\pm$ 1.4 of the normal group (p< 0,01). Nutrients consumed below 90% of Korean RDA were energy (79.9%), Ca (71.5%) for deaf-mute male students and Ca (88.5%) for deaf-mute female students. Energy (71.4%), protein (87.8%), Ca (74.8%), vitamin B$_1$ (83.4%) intake of normal male students and energy (72.8%), Ca (71.2%), Fe (78.7%) intake of normal female students were below 90% of Korean RDA. Energy, protein, fat, vitamin B$_1$, niacin intake of deaf-mute male students were significantly higher than normal male students and all nutrients intake of deaf-mute female students were significantly higher than normal female students. By the correlation of nutrients intakes with nutrition knowledge, there was positive correlation with the intakes of Ca, Fe, vitamin A, vitamin B$_2$, and vitamin C in the deaf-mute group, while there was negative correlation with the nutrients intakes (except for protein and fat) in the normal group. The amount of meal, breakfast, regularity of meal time, frequency of snacks showed a positive relation to nutrient intakes in deaf-mute group and amount of meal, breakfast, regularity of meal time, frequency of overeating showed a positive relation to nutrient intakes in normal group. The nutrition knowledge had no correlation with food habits or eating behaviors in both groups.(Korean J Nutrition 35(9) : 982~995, 2002)

Creating a school nutrition environment index and pilot testing it in elementary and middle schools in urban South Korea

  • BACKGROUND/OBJECTIVES: The role of a school's nutrition environment in explaining students' eating behaviors and weight status has not been examined in an Asian setting. The purpose of this study was to create a school nutrition environment index and to pilot test the index in elementary and middle schools in urban South Korea. SUBJECTS/METHODS: This study used a mixed-methods approach. Environment assessment tools were developed based on formative research, which comprised literature reviews, in-depth interviews, and focus group discussions. Key elements from the formative research were included in the assessment tool, which consisted of a structured survey questionnaire for school dietitians. Fifteen school dietitians from 7 elementary and 8 middle schools in Seoul completed the questionnaire. RESULTS: The formative research revealed four main sections that guided a summary index to assess a school's nutrition environment: resource availability, education and programs, dietitians' perceptions and characteristics, and school lunch menu. Based on the literature reviews and interviews, an index scoring system was developed. The total possible score from the combined four index sections was 40 points. From the 15 schools participating in the pilot survey, the mean school nutrition-environment index was 22.5 (standard deviation ${\pm}3.2$; range 17-28). The majority of the schools did not offer classroom-based nutrition education or nutrition counseling for students and parents. The popular modes of nutrition education were school websites, posters, and newsletters. CONCLUSIONS: This paper illustrates the process used to develop an instrument to assess a school's nutrition environment. Moreover, it presents the steps used to develop a scoring system for creation of a school nutrition environment index. As pilot testing indicated the total index score has some variation across schools, we suggest applying this instrument in future studies involving a larger number of schools. Future studies with larger samples will allow investigation of the validity and reliability of this newly developed tool.

병원 유형 및 지역에 따른 임상영양서비스에 대한 간호사의 인식 (Nurses' Perception on Clinical Nutrition Services by Types of Medical Institution and Area)

  • The purpose of this study was to evaluate nurses' perception of clinical nutrition services. A cross-sectional survey design was performed. The research was accomplished by using questionnaires developed for this study and administered from September 12 to December 31, 2013 to 343 nurses at 43 tertiary hospitals and 20 general hospitals. The percentage of nurses who recognized clinical nutrition certificate as issued from nation was 27.8%. The mean scores of perceived usefulness on clinical nutrition services was 4.23/5.00, whereas that of perceived implementation was 3.76/5.00. The mean scores of necessity of disease-specialized clinical dietitian at capital hospitals were significantly higher for obesity (P<0.01), cancer (P<0.05), and infant & childhood disease (P<0.01) than at local hospitals. The rates of nurses' experience in group education on cancer at capital hospitals (21.7%) was significantly higher than that at local hospitals (10.3%) (P<0.05). The mean scores of perceived importance of clinical nutrition services were 4.46/5.00 for 'group nutrition education', 4.46/5.00 for 'individual consultation', and 4.40/5.00 for 'nutrition management for enteral nutrition (EN) patients'. The most common reason why clinical nutrition services are important was 'improving malnutritional status'. To activate clinical nutrition services especially at local hospitals, clinical dietitians should give systematic assistance to patients and also institutional supports are needed.
