Associations between Psychological Distress and Body Mass Index among Law Enforcement Officers: The National Health Interview Survey 2004-2010

  • Objectives: To investigate the association between psychological distress and obesity among law enforcement officers (LEOs) in the United States. Methods: Self-reported data on psychological distress based on six key questions were obtained from LEOs who participated in the National Health Interview Survey (2004-2010). We used Prochaska's cut-point of a Kessler 6 score ${\geq}5$ for moderate/high mental distress in our analysis. Mean levels of body mass index (BMI) were compared across three levels of psychological distress. Results: The average age of LEOs (n = 929) was 39.3 years; 25% were female. Overall, 8.1% of LEOs had moderate or high psychological distress; 37.5% were obese (BMI ${\geq}30$). Mean BMI increased with increasing psychological distress (no distress, BMI = $27.2kg/m^2$; mild distress, $27.6kg/m^2$; and moderate/high distress, $33.1kg/m^2$; p = 0.016) after adjustment for age, race, income, and education level among female officers only. Physical activity modified the association between psychological distress and BMI but only among male LEOs (interaction p = 0.002). Among male LEOs reporting low physical activity, psychological distress was positively associated with BMI ($30.3kg/m^2$ for no distress, 30.7 for mild distress, 31.8 for moderate/high distress; p = 0.179) after adjustment, but not significantly. This association was not significant among males reporting high physical activity. Conclusion: Mean BMI significantly increased as psychological distress increased among female LEOs. A longitudinal study design may reveal the directionality of this association as well as the potential role that physical activity might play in this association.

  • The purpose of this study was to analyze the physical development and health status of 5 year-old children for more effective health management of preschool children. This study carried out examinations of height, weight, hemoglobin, visual acuity & dental examination for 5 years-old preschool children in Kwang-ju up, Kwang-ju gun, Kyunggi-Do from January to February, 1996. For height and weight, the data were compared with the 1985's Korean standard and the hemoglobin and visual acuity data were compared with the Korean standard. The results were as follows. The average height for 5 year-old male children was 106.3${\pm}$4.7(cm) and that of 5 year-old female children was 105.3${\pm}$4.3(cm). Sixty seven point eight percent of male subjects registered below the 50th percentile for the Korean standard height and 79.5% of the female subjects registered below the 50th percentile for female. The average weight for 5 year-old children was 18.6${\pm}$2.6(kg) for females. Also, it is estimated that obese subjects totaled 2.7%. For the visual acuity, 18.0% of subjects had weak vision in the left eye and 16.9% in the right eye. As a result, it was advised that these children undergo further examination. It was remarkable that there were so many anemic children. Children with a hemoglobin content of less than 11.9(g/dl) totaled 47.8%. It is anticipated that the results of this study will contribute to the on-going evaluation and subsequent planning for the children health management program within the health promotion program of public health center in Kwang-ju gun.

초등학교 아동의 혈청 지질 상태 및 지방 섭취에 관한 연구 (A Study on Serum Lipid Levels and Dietary Fat and Fatty Acid Intakes in Primary School Children)

  • The purpose of this study was no investigate serum lipid levels of primary school children and to estimate their intakes of total fat and fatty acids. Subjects fasting blood samples were obtained and analyzed for serum triglycerides(TG), total cholesterol(Chol) and high density lipoprotein-cholesterol (HDL-Chol). Low density lipoprotein cholesterol (LDL-Chol), LDL-Chol/HDL-Chol ratio(LPH), and atherogenic index(AI) were calculated,. Dietary intake of nutrients was assessed by means of a 24-hour recall method using food models and other measuring tools . The serum levels of TG, Chol and LDL-Chol in girls were higher than those in boys, but the serum HDL-Chol level of girls was lower than that of boys. As the degree of obesity increased, the serum TG level of girls was lower than that of boys. As the degree of oesity increased , the serum TG level of girls increased. The serum LDL-Chol level was higher in obese boys than in normal ones. Percentage s of subjects at risk of cardiovascular disease based on corresponding criteria of TG, Chol,HDL-CHol and LDL-Chol were 25.9%, 7.6%, 20.7% and 10.1%, respectively. The serum TG level of children provided with the national school lunch program(NSLP) was lower than that of children without NLSP. The total fat intake of boys was higher than that of girls, but calorie-adjusted fat intake became similar between the two groups. Intakes of polyunsaturated fatty acids(PUFA), monounsaturated fatty acids(MUFA) and saturated fatty acids(SFA) were 13.3$\pm$9.5g, 16.1$\pm$9.3g and 21.4$\pm$14.2g in boys, respecitively , and 12.6$\pm$11.3g, 15.3$\pm$9.7g and 19.9$\pm$13.1g in girls, respectively . The ratios of polyunsaturated /monoun-saturated /saturated /saturated fatty acids(P/M/S) in boys and girls were 0.7/0.8/1.0 and 0.8/0.8/1.0 respectively. The ratios of $\omega$6/$\omega$3 fatty acids in boys and girls were found to be 12.1 /1.0 and 8.6/1.0 , respectively. These results indicated the urgent need of nutritional education in primary schools to prevent further increase in childhood obesity and hyperlipidemia . Therefore, this study will contribute to the establishment of dietary guidelines and health recommendation for school children.

Child health promotion program in South Korea in collaboration with US National Aeronautics and Space Administration: Improvement in dietary and nutrition knowledge of young children

  • BACKGROUND/OBJECTIVES: Childhood obesity has become a global epidemic. Development of effective and sustainable programs to promote healthy behaviors from a young age is important. This study developed and tested an intervention program designed to promote healthy eating and physical activity among young children in South Korea by adaptation of the US National Aeronautics and Space Administration (NASA) Mission X (MX) Program. SUBJECTS/METHODS: The intervention program consisted of 4 weeks of fitness and 2 weeks of nutrition education. A sample of 104 subjects completed pre- and post- surveys on the Children's Nutrition Acknowledgement Test (NAT). Parents were asked for their children's characteristics and two 24-hour dietary records, the Nutrition Quotient (NQ) at baseline and a 6-week follow-up. Child weight status was assessed using Korean body mass index (BMI) percentiles. RESULTS: At baseline, 16.4% (boy: 15.4%; girl: 19.2%) of subjects were overweight or obese (based on $BMI{\geq}85%tile$). Fat consumption significantly decreased in normal BMI children ($48.6{\pm}16.8g$ at baseline to $41.9{\pm}18.1g$ after intervention, P < 0.05); total NQ score significantly increased from 66.4 to 67.9 (P < 0.05); total NAT score significantly improved in normal BMI children (74.3 at baseline to 81.9 after the program), children being underweight (from 71.0 to 77.0), and overweight children (77.1 at baseline vs. 88.2 after intervention, P < 0.001). CONCLUSIONS: The 6-week South Korean NASA MX project is feasible and shows favorable changes in eating behaviors and nutritional knowledge among young children.

과체중 대학생에서 저열량식이 또는 저열량식이와 운동병합에 의한 체중 감량 효과 (The effects of weight loss by a low-calorie diet and a low-calorie plus exercise in overweight undergraduate students)

  • Korean undergraduate students attempt to lose weight but often fail due to utilizing ineffective weight loss strategies. Some diet programs have succeeded, yet, they have not provided adequate skills for long-term weight maintenance. The aim of the study was to determine the effect of a low calorie diet and exercise with nutritional education on weight loss, serum lipid profiles, nutrient intakes, and dietary behavior modification in overweight and obese undergraduate students. The subjects in the low calorie diet group (LCD; n = 12) and the low calorie diet plus exercise group (LCDE; n = 13) had the same goal of losing 4 kg during a 12 week program. Nutrient intakes were assessed by the 24 hour recall method. Also, food habits and dietary behaviors were investigated by self-administered questionnaires before and after the weight control program and one month after completing the program. LCD and LCDE groups lowered body weight by 2 kg and 1 kg, respectively, although they decreased calorie intake by 355 and 287 kcal per day compared to intakes prior to the study. Body fat mass decreased in both the LCD and LCDE groups; however, the decrease was greater in the LCDE group. In addition, only the LCDE group increased muscle mass. The LCD group had a slightly better effect in reducing body weight, body fat, and waist circumference than the LCDE group. However, their decrease was reversed after the mid-study check in the LCD group; the reduction was better maintained and decreased more in the LCDE group. However, serum lipid profiles were already in borderline prior to the study; moreover, they were not modified after losing weight. The dietary behavior program helped students to develop better dietary habits. In conclusion, the combination of a low calorie diet and exercise is necessary in order to maintain longer weight loss by increasing muscle mass and decreasing body fat.

도시지역 초등학생 비만 수준 및 관련 요인에 대한 연구 -사회경제적 요인을 중심으로- (Study on the Relationship between Childhood Obesity and Various Determinants, including Socioeconomic Factors, in an Urban Area)

  • Objectives: This study was conducted to investigate the prevalence of childhood obesity, the association between the undesirable lifestyles and socioeconomic factors, the association between childhood obesity and various risk factors, including socioeconomic factors, and the agreement between the body mass index (BMI) classification and the body fat percentage. Methods: The study subjects were all the 5th grade students from all the elementary schools in Gunpo City, Kyunggi Province, South Korea (4043 children at 22 schools). The subjects were measured for their height, weight and percent body fat etc. and they were also surveyed by questionnaire from March 18th to April 25th, 2005. To determine whether the children were within normal limits or not, standardized BMIs for each age group were used. The data was analyzed by logistic regression analysis using SAS 9.0 version. Results: The prevalence of childhood obesity prevalence was 25.1 %. Boys had a higher prevalence of obesity (27.5%) than did the girls (22.5%). Children had tendencies of having undesirable lifestyles and getting obese if they had a lower socioeconomic status. The risk factors for childhood obesity were low paternal education (OR: 1.17, 95% CI : 0.97-1.42) and non-parental caregivers (OR: 1.34, 95% CI : 0.98-1.82). Other risk factors for childhood obesity were a high birth-weight, longer TV/computer-using time, a lower fruit-eating frequency, short sleeping hours and parental obesity. The agreement rate between the BMI classification and the body fat percentage was 93.1 %. Conclusions: This study showed the children had a higher prevalence of obesity: further, not only individual lifestyles, but also socioeconomic factors could influence childhood obesity. Childhood obesity was especially more problematic for children with a low socioeconomic status.

비만관련 습관 및 비만도 변화에 영향을 미치는 관련요인 분석 (The Associated Factors with Obesity-related Habits and Obesity index Change)

  • Obesity in children is a major concern of public health. This study was performed to illuminate its effect on weight control program and the associated factors of obesity-related habits and obesity index in primary school obese children. Weight control program consisted of behavioral modification, nutrition education and exercise during 17 weeks. The sample consisted of treatment group(n=42) and control group(n=41). There was no statistical difference between the two groups in obesity index, socioeconomic status and grade. To assess the effects of weight control program, the subjects were given pre-test and post-test such as the measurement of anthropometric values and self-reporting questionnaire. This result of this study were as follows; 1. After weight control program was applied, there was a significant decrease in obesity index among the treatment group. Obesity-related habits score of the treatment group increased significantly, While there was not much difference between the pre-test and the post-test among the control group. But exercise habit didn't increase significantly in the both groups. 2. Correlation between obesity-related habits and obesity index were not evident. 3. After application of weight control program, the factors associated with change of eating habit were children's past experience of weight control, motivational change toward weight control program and friends' support for treatment group. The factors associated with change of exercise habit were post-test motivation score and friends' support. Motivational change toward weight control and pre-test self-efficacy of exercise behavior were counter-related to exercise habit. For change of other obesity-related habits, initial obesity index, motivational change, post-test self-efficacy score of exercise behavior and paternal educational status were closely associated. But post-test self-efficacy score of eating behavior was unrelated. 4. Only the factor of experience of weight control was associated with change of obesity index. 5. For the both groups, the factors associated with change of eating habits were post-test self-efficacy of eating behavior and family's support. The factors associated with change of exercise behavior were self-efficacy changes of exercise behavior and friends' support. The factors associated with change of other obesity-related habits were self-efficacy change of eating behavior. Initial obesity index was associated with change of obesity index. 6. The rate of dropouts from weight control program was 28.6% (12/42) in treatment group. Initial obesity index, other obesity-related habits except eating exercise habits, friend's support were associated with dropout. In conclusion, these results indicated that weight control program in primary school settings was effective. Direct exercise regimen and practice was demanded. In addition to the program itself, much of the success is dependent on the degree of motivation of the children involved and support provided by their parents and friends. Further study need to be performed under the condition that the weight control progrom is applied for a longer period.

대전지역 직장 중년 남성의 건강 및 영양 상태 조사 (The Health and Nutritional Status of Middle Aged Men at Worksite in Taejon)

  • This study was conducted to investigate the health and nutritional status of 123 middle aged men at their worksite in Taejon. The results of this study on the factors that influence their health and nutritional status were as follows : 1) 74.8$\%$ of the subjects had history in the order of alimentary, heat, liver, diabetic and pulmonary diseases. 30.3$\%$ stopped smoking at 42.3% yrs. and 74.5$\%$ smoked more than 10 cigarets per day. Also 71.9$\%$ drank 2-3 times per week and 35.3$\%$ drank 1-2 times per week. 91.4$\%$ exercised more than 30min every day. 2) 54.4% showed concerns about their health whereas 20.3$\%$ were afraid that they night get sick. 3) 90.4$\%$ ate regularly and 54.5$\%$ worried about their cholesterol, salt, fat and MSG intakes. 48.7$\%$ ate out 1-2times per week and their favorite foods eaten outside were Korea. 4) 41.5$\%$ were classified as 'normal A', 30.9$\%$ 'normal B' group and high blood pressure and liber diseases in 'doubtful for disease' group were pointed out from their 1996 health check ups. 5) By Broca index, 39.8$\%$ were overweight and 9.8% were obese however by BMI only 23.6% were overweight. According to the relationship between calculated and self recognized obesity, 62.4$\%$ categorized themselves into the right weight range but 34.3$\%$ thought they were thinner than they were. 6) 43.9$\%$ were border line in cholesterol intake and 12.1$\%$ needed medical care for high blood cholesterol. 7) The Average energy intake was 1970.6㎉(80.9$\%$ RDA) with a 65 :19 : 16 ratio of carbohydrate : protein : fat. Protein, Fe, thiamin, riboflavin, niacin and Vit. A. 8) Occupation, regularity of meals, partner's job, income. smoking, alcohol drinking, health concerns and eating out were the factors that influenced the subject's nutrient intakes and health status. from this study, it was found that middle aged men needs to know their health and nutritional status and to be educated correct health and nutritional information through formal or informal channel. The worksite is the vest place to do this and we want these results to be used to develop the nutrition education program for middle aged men at the worksite.

경기 일부지역 초등학생의 채소와 과일 섭취 및 관련 인식, 자아효능감, 영양지식과 식행동 (Intakes and Beliefs of Vegetables and Fruits, Self-Efficacy, Nutrition Knowledge, Eating Behavior of Elementary School Students in Kyunggi Area)

  • The purpose of this study was to examine vegetable & fruit (V/F) intakes, beliefs and self-efficacy regarding V/F consumption, nutrition knowledge and eating behavior of elementary students. A survey was conducted to the 4th graders (n = 234) at two elementary schools in Guri, Kyunggi-do. About one-fourth of subjects were overweight or obese. Subjects had 4.2 servings of V/F a day, consuming 340.2 g of V/F. Girls consumed significantly more amounts of vegetables than boys (p < 0.05). Girls were more favorable regarding V/F consumption (p < 0.01), and believed more strongly on advantages of having V/F such as “good for skin” constipation prevention (p < 0.001) and cancer prevention (p < 0.05). Boys felt more strongly in disadvantages or barriers of eating V/F, including mother's cooking time constraints (p < 0.01), lack of past experience of eating V/F, and family members' disliking of V/F (p < 0.05). Girls felt more confident in eating V/F (p < 0.05) than boys; they also felt more confident in specific items of "eating fruits/salads instead of cookies/chips for snack" and "eating fruit juice/vegetable juice instead of soda"(p < 0.01). Subjects showed low level of nutrition knowledge, especially in items such as balanced meals, recommended servings of V/F and vitamin deficiency. Compared to boys, girls had more desirable eating behavior such as eating adequate amount of meals (p < 0.001), having a variety of foods, eating fruits daily, and having fatty foods less frequently (p < 0.05). Intakes of Ca and K were quite below the recommended level, while the intakes of protein, Na and vitamin A intake were much above the Dietary Reference Intakes (DRI) for 9-11 old children. Nutrient intakes expressed as %DRI was higher in girls for vitamin A (p < 0.01), energy and riboflavin (p < 0.05). High V/F consumption group (${\geq}5$ servings of V/F a day) compared to the counterparts showed higher self-efficacy and had better eating behaviors. Nutrition education for children should focus on increasing consumption of V/F, by helping them to increase self-efficacy for eating V/F and to recognize the benefits and reduce the barriers of eating V/F, especially in boys. It is also needed to provide nutrition information for balanced meals or increasing V/F consumption, and help the children to adopt desirable eating behavior.

제주 지역주민기반 대사성질환 개선 프로그램 중재 효과 (The Effect of Community-based Health Intervention Program to Improve Metabolic Disease in Jeju Island)

  • 본 연구는 대사성질환 관련 위험 인자를 가지고 있는 제주 지역주민을 대상으로 마을 기반 건강프로그램의 효과성을 입증하기 위해 수행되었다. 50명의 실험군은 BMI가 25 이상인 지역주민으로 3주 동안 고유수용성신경근촉진(PNF) 트레이닝과 스트레칭을 포함한 운동요법 7회, 영양관리 식습관 개선을 위한 쿠킹테라피 프로그램 4회 그리고 대체의학활용 프로그램인 힐링터치 마사지 프로그램 3회의 건강중재프로그램에 참여하였다. 건강프로그램 중재의 효과성 평가를 위해 프로그램 중재 전과 후에 체 성분, 혈액의 지질 프로파일, 혈당 및 허리둘레를 측정하여 비교하였다. 건강프로그램 중재 후에는 중재 전에 비해 TC, HbA1C, 이완기혈압, 체지방 그리고 허리둘레가 크게 감소하여 거의 정상수치가 되었고 특히 HbA1C, 체지방 그리고 허리둘레는 유의한 수준으로 감소 (P<0.001)되었다. 그러나 반대로 건강프로그램 중재를 받지 않은 대상군의 경우 HbA1C, 체지방 및 허리둘레가 유의한 수준으로 증가 (P<0.001)하는 부정적 결과를 보였다. 이상의 결과로부터 대사성질환 관련 인자들은 건강프로그램의 중재로 개선되어 호전될 수 있음을 알 수 있었다. 본 연구에서는 건강중재프로그램인 운동과 쿠킹테라피 효과를 구분하여 조사하지는 않았지만 3주간의 단기간에 보여진 효과가 두 가지를 병행해서 수행함으로써 나타난 결과임을 암시하며 식사조절과 운동을 병행하는 것이 대사성질환 개선에 보다 효과적임을 제시하고자 한다.
