비만 청소년에서 체질량지수와 한의변증의 상관성 (Relation between Body Mass Index and Pattern Identification in Obese Adolescents)

  • Objectives : This study was conducted to analysis correlation of pattern identification and body mass index (BMI) in obese adolescents. Methods : A total of 228 middle school students, 226 students were agreed and included study and two were excluded because of disagreement. It was conducted cold-heat pattern questionnaire, phlegm pattern questionnaire and after height and weight were measured. Firstly, out of 24 who did not respond to all of questionnaires. Secondary, out of 159 who are normal range on international obesity standard(BMI < $30kg/m^2$) Results : The average height and weight of 43 subjects was $159.65{\pm}9.45cm$, $70.10{\pm}8.47kg$ respectively. The average score of pattern identification was cold pattern score(CPS) was $2.70{\pm}1.61$, heat pattern score(HPS) was $3.30{\pm}1.99$, phlegm pattern score(PPS) was $3.95{\pm}5.02$. There was no significant correlation with BMI and CPS, HPS, PPS. Conclusions : In this study, there was no significant correlation of pattern identification and BMI in obese adolescents.

대전지역 중학생의 혈압 분포와 비만지수 및 일부 환경인자와의 관계 (Blood Pressure Distribution of Adolescents in Taejon City and its Relationship with Obese Index and Several Environmental Factors)

  • Blood pressure distribution of adolescents and its several related factors including obesity were investigated through blood pressure measuring by automated oscillometric method, anthropometry, and questionnaire. Subjects were 480 female and 480 male middle school students living in Taejon. Mean values of systolic(SBP)/diastolic(DNP) blood pressure of male and female students were 118.4$\pm$11.0/69.5$\pm$9.6 mmHg and 116.9$\pm$10.4/69.5$\pm$8.6 mmHg, respectively. Their blood pressures, particularly SBP, increased with age and showed higher value in the males than in the females. All frequency distributions of SBP and DBP, by sex and age, showed normal curves and their 50th, 90th, and 95th percentiles were presented. As obese index increased, both SBP and DBP were elevated and the prevalence of hypertension which was classified by Task Force Report(1987) increased, particularly in male students. Body weight and BMI were positively correlated with both SBP and DBP, and hight was also positively correlated with SBP or DBP. Our data inicated that blood pressure and hypertension prevalence of adolescents tended to increase and various internal and external environmental factors affected them. It was emphasized that blood pressure measurement should be done in every medical examination of adolescents and the nationwide Korean standard of blood pressure for adolescents have to be prepared. 소아 및 청소년에서 비만과 사구체여과율과의 관계 (The Relation between Obesity and Glomerular Filtration Rate in Children and Adolescents)

  • 목 적 : 최근 소아 및 청소년의 비만이 빠르게 증가하고 있다. 고도 비만아들에서 단백뇨 및 FSGS가 발현될 수 있고, 그 기전으로 과여과, 신정맥압 증가, 사구체비대, 고지혈증, angiotensin II, insulin, leptin, $TGF-{\beta}1$의 합성 증가 등이 보고되고 있다. 본 연구에서는 BMI와 비만도에 따른 GFR의 변화를 조사하여 과여과 여부를 알아보았고, 혈청 cystatin C 농도를 측정하여 비만과의 상관관계와 유용성을 알아보고자 하였다. 방 법 : 2004년 5월부터 12월까지 중앙대학교 용산병원을 방문하여 혈액검사를 시행받은 6세 이상의 환아들 중에서 임상적으로나 검사상으로 명백한 신질환의 증거가 없는 115명의 환아들을 대상으로 BMI와 비만도를 측정하였고, Schwartz's formula로 GFR을 추정하였다. 혈청 cystatin C 농도는 particle enhanced nephelometric immunoassay를 이용하여 Behring Nephelometer II로 측정하였다. 결 과 : GFR은 BMI로 구분한 경우 비만군이 정상군에 비해 유의하게 높았다. GFR은 비만도로 구분한 경우에는 비만군과 정상군 사이에 유의한 차이를 보이지 않았지만 고도 비만군은 정상군에 비해 유의하게 높은 값을 나타내었다. BMI와 GFR은 양의 상관관계를 보였으나, 비만도와 GFR은 유의한 상관관계를 보이지 않았다. 혈청 cystatin C 농도는 BMI로 구분한 경우와 비만도로 구분한 경우 모두 비만군과 정상군 사이에 유의한 차이를 보이지 않았고, BMI나 비만도와 통계학적으로 유의한 상관관계를 나타내지 않았다. 결 론 : 비만으로 인해 과여과와 같은 신기능의 변화가 초래되고 단백뇨 및 FSGS와 같은 신병증이 발현될 수 있으므로 비만에 대한 적절한 관리와 치료가 필요할 것으로 생각된다.

Association Between Trp64arg Polymorphism of the β3 adrenoreceptor Gene and Female Sex in Obese Turkish Children and Adolescents

  • Purpose: The ${\beta}3-adrenergic$ receptor (ADRB3) is expressed in visceral adipose tissue and has been speculated to contribute to lipolysis, energy metabolism, and regulation of the metabolic rate. In this study, we aimed to investigate the association of polymorphism of the ADRB3 gene with the sex of children with obesity and related pathologies. Methods: ADRB3 gene trp64arg genotyping was conducted in 441 children aged 6-18 years. Among these subjects, 264 were obese (103 boys; 161 girls) and 179 were of normal weight (81 boys; 98 girls). In the obese group, fasting lipids, glucose and insulin levels, and blood pressure were measured. Metabolic syndrome (MS) was defined according to the modified World Health Organization criteria adapted for children. Results: The frequency of trp64arg genotype was similar in obese and normal weight children. In obese children, serum lipid, glucose, and insulin levels; homeostasis model assessment of insulin resistance (HOMA-IR) scores; and MS were not different between arg allele carriers (trp64arg) and noncarriers (trp64trp). In 264 obese children, genetic analysis results revealed that the arg allele carriers were significantly higher in girls than in boys (p=0.001). In the normal weight group, no statistically significant difference was found between genotypes of boys and girls (p=0.771). Conclusion: Trp64arg polymorphism of the ADRB3 gene was not associated with obesity and MS in Turkish children and adolescents. Although no relationships were observed between the genotypes and lipids, glucose/insulin levels, or HOMA-IR, the presence of trp64arg variant was frequent in obese girls, which can lead to weight gain as well as difficulty in losing weight in women.

청소년의 비만도에 따른 건강행위 실천과 건강관련 삶의 질과의 관련성 (Factors Related to Health Behavior and Health-Related Quality of Life Among Obese High School Youths)

  • Objectives: This study examined to explain the practical health behaviour and health-related quality of life, and their influencing factors in high school students. Methods: Total of 718 high school students from 1 school in Seoul were assessed with a self-administered questionnaire regarding general characteristics, health related characteristics, obesity index(Height and weight calculated by using the relative weight law: obesity group>20%, overweight group $10{\sim}20%$, normal weight group $-10{\sim}10%$, under weight group <-10%), health behaviour in school-aged children(eating, exercise and weight control) and health-related quality of life(PedsQLTM4.0 Generic Core Scale: physical health, emotional functioning, social functioning, school functioning). Results: Major results were as follows. 1. The rate of obesity by obesity index was 5.3% of high school students. Obesity incidence in adolescents was mainly associated with gender and parents whether obesity. 2. Perceived health status was lower in obese adolescents than in normal adolescents. 3. The rate of miss a breakfast was 37.9%, and obesity group than normal weight group were fruits, vegetables and milk intake at least, a lot of fastfood intake. During the past week, followed by intense physical activity, and overweight consumed a lot of time for TV and the Internet. Overall, under weight group and normal weight group belong to the students evaluated fatter than themselves. Weight control for weight loss, gain and maintain was grater in obesity group than in normal weight group. Weight loss showed highest scores in overweight group which appeared significant difference. 4. Obese adolescents compared with other groups, reported lower total QOL score and all QOL in domain, and especially social functioning showed significant differences. 5. Factors influencing the adolescents's QOL were found to be gender, perceived health status and exercise. Conclusions: High school girls were aware of their bad health status and likely to improve the QOL by practicing health behaviour. But obese adolescents were likely to degrade the quality of life by reducing the practice of health behaviors. So further school-based education about proper practical health behaviors and obesity prevention is necessary.

전남 여수지역 청소년들의 비만도와 식생활습관에 관한 연구 (A Study on Obesity and Food Habit of Adolescents in Yeosu, Jeonnam Area)

  • This study was to investigate obesity and food habit of adolescents in Yosu, Chonnam area. It was surveyed using questionnaires with 551 adolescents consisted of 280 boys and 271 girls. The questionnaire included general charac-teristics, obesity index, eating behavior and snack intake pattern. The results were as follows. The proportion of obese subjects was 10% by Rohrer index but the rate of obesity by body mass index was 3.3%. The self perception of body shape were optimal (47.9%), fat (29.8%), slightly lean (14.5%), obese (4.7%) and lean (3.1%). Generally, the propor-tion of girls responded them as “fat” was greater than boys. The greater percentage of boys responded them as “lean” than girls. The subjects don't take breakfast regularly were 50.8% and the main reason for skipping breakfast was the lack of time (55.2%). The main type of breakfast was cooked rice (78.9%). The most favorite snacks hun out to be cookies (36.5%) and 45.7% of subjects take snacks during the rest. The reason for taking snacks was “feeling hungry” (52.6%) and the taste was the most important factor of snack choice. The results of this study showed that the most of subjects were not in a serious obesity condition, and their eating habits were generally satisfactory. Also, this study has found that it is necessary to educate the students the importance of regular intake of a balanced meal. (Korean J Community Nutrition 8(2) : 129∼137, 2003)

비만 청소년의 비만 중재 프로그램에 대한 국내외 연구 분석: 혈관내피 기능을 중심으로 (Analysis of Obesity Intervention Programs in Adolescents: Focused on Endothelium Functions)

  • Purpose: The purpose of this study was to determine the current status of obesity intervention programs, and its effects on endothelium function in adolescents. Method: This was a descriptive research study, that investigated domestic and international literature 2009-2018. Using databases inside and outside of Korea to search for 'adolescent', 'obese adolescent', 'exercise', 'program', or 'intervention', and 'endothelium function', a total of 14 literature have been selected for 6 domestic research and 8 international research, excluding overlapping, case studies, literature reviews, and unrelated studies. Results: Interventions for enhancing endothelium function were exercise program, diet, vitamin D3 treatment, and Orlistat intake relative to international research, and combined exercise and purple sweet potato intake, in most domestic research. Dependent variables included FMD, RHI, NMD, EPCs, and EMPs, for measuring endothelium function in international research, and PWV, FMD and RHI, in domestic research. Conclusion: Results of this study confirmed that application of obese intervention programs, may improve endothelium function in adolescents. Further studies are required, to develop nursing intervention, that would enhance endothelium function in adolescents.

저체중과 비만 청소년의 식습관, 운동, 체형인식과 체중조절 비교 (Comparison of Dietary Habits, Exercise, Recognized Body Shapes and Weight Control between Obesity and Underweight of Adolescents)

  • This study was conducted to determine major factors underweight and obesity by comparing and analyzing dietary habits, exercise and weight control of obesity and underweight of Adolescents. The subjects were 26,039 students and they were divided according Body Mass Index(BMI). The collected data was analyzed through frequency, percentage, chi-square test. The results were as follows. 1) Among the male students 67.3%, 32.7% and among the female ones 82.7% and 17.3% were included in the underweight and the obese groups, respectively. 2) For dietary habits, the rate of skipping breakfast(12.5%, 13.6%) was considerably higher than those of skipping lunch and dinner. 3) The females and the obese group received more stress, felt more unhappiness, grief, depression and thought more suicide than the males and the underweight group, respectively. 4) The underweight group had higher rate of strengthening exercise of males and females. 5) For self-recognized body shape, while in the underweight group had a wrong image of their body shape, while more males in the obese group had a distorted self body image. 6) Measures to control weight were regular exercise, reduced amount of diet, and the obese group controled their weight more actively than the underweight group of the male and the female students. 7) From these results, appropriate mental health programs and education programs about health knowledge, properly self-recognized body shape and measures to control weight were considered to be necessary.

가상착의 시스템을 활용한 하체 비만 여자 청소년의 슬랙스원형 설계 (Slacks pattern development for the female adolescents with lower-body obesity using virtual simulation system)

  • This study analyzes the body shapes of lower-body obese female adolescents and proposes a slacks pattern suitable for their body type. Lower-body obesity is a prevalent type of teenage obesity, and our proposals aim to improve consumer satisfaction in ready-to-wear clothes across this demographic. We first observe characteristics of obese lower bodies, noting significantly above-average thigh and hip circumference. These figures indicate a high degree of curvature in obese lower bodies, along with a large drop value. Leveraging this data, we develop a novel slacks pattern using 3D avatars in a virtual simulation system. The formulas for the main areas of the pattern are as follows: front waist girth W/4+0.75cm+0.5cm, back waist girth W/4+0.5cm-0.5cm, front hip girth H/4+1.25cm-0.5cm, back hip girth H/4+2cm+0.5cm, front crotch extension H/16+0.5cm, back crotch extension H/8+1cm. Results from appearance evaluations show that this pattern minimizes strain rate on the waist and hips, and its average score is significantly higher than that of an alternative pattern that was also evaluated. The minimized strain rate and high average score indicate that our pattern assigns a sufficient amount of space to the appropriate areas. Based on these results, we expect our research to inform slacks pattern development and production for obese consumers of all types.

Serum alanine aminotransferase levels are closely associated with metabolic disturbances in apparently healthy young adolescents independent of obesity

  • Purpose: Liver metabolism plays a pivotal role in the development of metabolic disorders. We aimed to investigate the clinical and laboratory risk factors associated with alanine aminotransferase (ALT) levels in young adolescents from an urban population in Korea. Methods: A population of 120 apparently healthy adolescents aged 12-13 years was included in the cross-sectional design study; 58 were overweight or obese and 62 were of normal weight. We estimated anthropometric and laboratory measurements, including waist-to-height ratio, blood pressure, insulin sensitivity, aspartate aminotransferases (AST), ALT, and lipid profiles. Results: The mean ages of the overweight or obese and normal weight participants were $12.9{\pm}0.3$ and $13.0{\pm}0.3years$, respectively. Height, weight, body mass index, waist circumference, waist-to-height ratio, systolic and diastolic blood pressure, AST, ALT, total cholesterol, low-density lipoprotein-cholesterol, triglyceride, insulin, and the homeostatic model assessment of insulin resistance (HOMA-IR) score were significantly higher and the high-density lipoprotein-cholesterol and quantitative insulin-sensitivity check index were significantly lower in the overweight/obese participants in comparison to the normal-weight participants (all P<0.05). In multivariate linear regression analysis, waist-to-height ratio, systolic blood pressure, and HOMA-IR score were independently and positively associated with serum ALT levels. Conclusion: Screening for ALT levels in adolescents may help to differentiate those at risk of metabolic abnormalities and thus prevent disease progression at an early age.
