Systematic Literature Review and Meta-Analysis on the Effect of Exercise Intervention Programs on Blood Lipids in Obese Adolescents in Korea according to Sex (국내 비만 청소년을 대상으로 실시한 운동 중재 프로그램의 혈중지질 효과에 대한 체계적 문헌고찰과 메타분석: 성별 차이 분석)

  • Purpose: The authors used a meta-analysis approach to investigate the effects of exercise on gender-specific triglyceride (TG) and high-density lipoprotein cholesterol (HDL-C) in obese adolescents. Methods: This study is a systematic review and meta-analysis study to confirm the effect of exercise intervention according to gender for obese adolescents. PICO were obese a adolescents, exercise intervention, comparative intervention (control and general group),TG and HDL-C. Results: The group of obese male adolescents showed a statistically significant decrease of 51.72 in TG (n=175, MD=-51.72; 95% CI -52.44 to -51.00).There was a statistically significant increase of 4.44 in HDL-C (n=175, MD=4.44; 95% CI 3.79 to 5.10) in the group of obese male adolescents. Conclusion: Triglyceride and high-density lipoprotein cholesterol were analyzed to determine the effect of exercise interventions in obese adolescents according to gender differences. The results of this study can be used as basic data for the design of future exercise intervention studies, subjects, exercise methods, and intervention periods.

Twelve-year Study on Body Mass Index Changes of Obese Adolescents (비만 청소년의 과거 12년간 Body Mass Index 변화 - 서울시내 일부 고등학생을 대상으로 -)

  • The purpose of this study is to observe the longitudinal changes in BMI (Body Mass Index) of obese and non-obese 3rd. grade high school students in Seoul for 12 years and to see the trends of overt weight gain in obese adolescents. The results are as follows; 1. The average annual increasing rates of body mass Indices in male students were $1.14kg/m^2$ in obese group and $0.59kg/m^2$ in non-obese group. In female students, the average annual increasing rates of body mass indices were $0.93kg/m^2$ in obese group and $0.53kg/m^2$ in non-obese group. 2. The change rate of BMI for 12 years was significantly higher in obese group than non-obese group. 3. Puberty had less influence on the change rate of BMI in obese group compared to non-obese group. 4. In obese group, 71.8% of the variance in BMI at 17 can be predicted by BMI at 16 years in male students. In female students 44.4% can be predicted by BMI at age 16. 5. Among the 17-year-old obese students, 58.8% of the males and 56.2% of females were found not to have been obese at 7 years of age. 6. Among the 17-year-old obese students, those who were obese at 7 years of age were found to have higher BMI at later ages than those who were in the non-obese group. Obese adolescents were more likely to be obese in their childhood than non-obese group. There was no optimal age for the significant weight gain and the increasing rate of BMI was constantly higher in obese group than in non-obese group. Due to the fact that child obesity in early age contributes to obesity in adolescence, close observation is advised. On the other hand, a large proportion of obese adolescents can be preventable by early interventions, because about 50% of obese adolescents were not obese in early elementary school age.

Stages of Change, Processes of Change, and Decisional Balance for Weight Control Associated with Body Mass Index in Female Adolescents (여자 청소년의 체질량지수에 따른 체중조절행위 변화단계, 변화과정과 의사결정균형 비교)

  • Purpose: The purpose of this study was to compare variables related to weight control between normal weight and overweight/obese female adolescents. Methods: This study is a supplementary analysis using a total of 293 female adolescents in Seoul. Data were collected through self-report questionnaires on the topics of stages of change, processes of change, and the decisional balance pertaining to weight control. Body weight and height were also measured. Results: There was a significant difference in the stages of change between the normal weight and overweight/obese groups. Of 12 processes of change, 9 processes were significantly higher in the overweight/obese group than in the normal weight groups. Also, female adolescents who were overweight or obese had significantly higher pros and eating efficacy scores comparing to those in the normal weight group. Conclusion: The findings of this study suggest that overweight/obese female adolescents, especially those in the action stage need supports to continue their weight control behaviors. In addition, a weight control program for female adolescents should emphasize their participation in physical activities in addition to their dietary control efforts.

  • Objectives : The purpose of this study is to investigate the effect of fasting therapy on the changes of body composition in obese adolescents. Methods : We examined the body composition of obese adolescents three times; before fasting, on the second refeeding day, two or four weeks later. Results : Body weight, body mass index(BMI), basal metabolic rate(BMR) and lean body mass (LBM) were decreased during the fasting, whereas percent-fat was increased. After fasting, patients have the dietary period. During this period BMR and LBM were increased and the decrease of percent-fat was continued. Conclusions : It seems that fasting therapy with keeping up well-controlled dietary period and receiving oriental medical management can be useful to decrease percent-fat in obese patients.

The Relationship among Insulin Resistance, Blood Profiles and Nutrient Intake in Overweight or Obese Children and Adolescents (과체중 및 비만인 소아 청소년의 인슐린 저항성, 혈액 특성 및 영양소 섭취량과의 관계)

  • The purposes of this study were to investigate blood profiles and nutrient intakes of groups that are different in obese levels, and to find the credible predictor of insulin resistance. The subjects were classified as normal weight (%IBW${\leq}$ 110), obese without MS and obese with MS according to IDF definition of the risk group in metabolic syndrome (MS). Subjects of this study were included 137 (59 boys, 78 girls) free living children and adolescents (mean age $12.6{\pm}3.4$ years) in Gangneung area, South Korea. %IBW of normal weight (94.9%), obese without MS (123.8%) and obese with MS (131.5%) were significantly different among groups. HOMA-IR had positive correlations with TG (r = 0.634), waist circumference (r = 0.553), atherogenic index (r = 0.513), %IBW (r = 0.453) and ALT (r = 0.360), but showed negative correlations with HDL cholesterol (r = -0.417, p < 0.001). HOMA-IR showed positive correlation with polyunsaturated fatty acid intake (p < 0.05). The energy intake of obese with MS was 1762 kcal/day which was not significantly different from those of normal weight and obese without MS. Total fatty acid intakes of two obese groups were significantly higher than that of normal weight. The results of this study suggest that waist circumference and ALT as well as TG, atherogenic index and weight can be credible indices to predict the insulin resistance in children and in adolescents. In addition, nutrition education and adequate diet should be provided to prevent MS in children and in adolescents.

Accuracy of predictive equations for resting energy expenditure (REE) in non-obese and obese Korean children and adolescents

  • Weight-controlling can be supported by a proper prescription of energy intake. The individual energy requirement is usually determined through resting energy expenditure (REE) and physical activity. Because REE contributes to 60-70% of daily energy expenditure, the assessment of REE is very important. REE is often predicted using various equations, which are usually based on the body weight, height, age, gender, and so on. The aim of this study is to validate the published predictive equations for resting energy expenditure in 76 normal weight and 52 obese Korean children and adolescents in the 7-18 years old age group. The open-circuit indirect calorimetry using a ventilated hood system was used to measure REE. Sixteen REE predictive equations were included, which were based on weight and/or height of children and adolescents, or which were commonly used in clinical settings despite its use based on adults. The accuracy of the equations was evaluated on bias, RMSPE, and percentage of accurate prediction. The means of age and height were not significantly different among the groups. Weight and BMI were significantly higher in obese group (64.0 kg, $25.9kg/m^2$) than in the non-obese group (44.8 kg, $19.0kg/m^2$). For the obese group, the Molnar, Mifflin, Liu, and Harris-Benedict equations provided the accurate predictions of > 70% (87%, 79% 77%, and 73%, respectively). On the other hand, for non-obese group, only the Molnar equation had a high level of accuracy (bias of 0.6%, RMSPE of 90.4 kcal/d, and accurate prediction of 72%). The accurate prediction of the Schofield (W/WH), WHO (W/WH), and Henry (W/WH) equations was less than 60% for all groups. Our results showed that the Molnar equation appears to be the most accurate and precise for both the non-obese and the obese groups. This equation might be useful for clinical professionals when calculating energy needs in Korean children and adolescents.

The Evaluation of Simple Indices of Abdominal Visceral Fat Area Among Korean Obese Adolescents (비만 청소년에서 복부 내장지방량을 반영하는 비만지표에 대한 연구)

  • Objectives : The purpose of this study was to determine what is useful index of abdominal visceral fat area among Korean obese adolescents. Methods : 35 obese adolescents who had visited Kirin Oriental Hospital from August 2004 to May 2006 were included in the study. Weight, height, waist and hip circumferences were measured. Abdominal visceral fat areas at L4-5 level were measured by computed tomography. The subjects were divided into two groups, according to sex. In each group, we investigated the correlation among waist circumference, waist/hip ratio, waist/height ratio, obesity degree and abdominal visceral fat area and evaluated that which one of these kinds of indices is the most useful predictor for visceral fat area in Korean obese adolescents. Results : There were significant correlation between waist circumference, waist/hip ratio, waist/height ratio and obesity index in both group. In boys waist/hip ratio had correlation with abdominal visceral fat area and in girls obesity index had significant correlation with abdominal visceral fat area. Conclusions : Our study suggests that waist/hip ratio in boys, obesity degree in girls can be a relatively good parameter for predicting abdominal visceral fat area.

The Influences of Academic Factors on Body Weight Loss Efforts among Obese Adolescents (학업요인이 비만청소년의 체중감량 노력에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the factors related to the academic burden on weight loss efforts among adolescents aged 12-18 in Korea. Methods: This study analyzed 29,069 obese adolescents (10.8%) using raw data of the 9th~12th the Korea Youth Risk Behavior Web-based Survey collected from 2013 to 2016. The data was analyzed using descriptive statistics, Chi-square and multivariate logistic regression. Results: Weight control efforts were significantly higher among girls, middle school students, those who are participating in nutrition education, and individuals with perceived fatness or perceived stress. Additionally, a long sitting time and high academic performance were found to be variables affecting the weight control efforts of adolescents. Conclusion: Obese adolescents in Korea spent a lot of time in their studies due to their academic burdens and this caused a lack of time to adjust their weight. Therefore, it is needed to establish a policy by considering these social and cultural factors for obese adolescents in Korea.

A Study on SOD Activity and Serum Antioxidant Mineral Concentrations in Obese Adolescents (비만 청소년의 SOD 활성도 및 혈청 항산화무기질 농도에 관한 연구)

  • The purpose of this study was to investigate the relationship between obesity and, erythrocyte SOD (superoxide dismutase) activity and serum antioxidant mineral (Fe, Zn, Cu, Mn and Se) concentrations of adolescents. Subjects were assigned to one of two groups such as obese ($BMI{\geq}25$, 32 boys, 24 girls) and normal group (18.5 < BMI < 23, 27 boys, 30 girls) Subjects were evaluated based on anthropometric measurements, 24-hr dietary recalls and blood analysis. The mean age of the total subjects was 13.8 years. The mean weight (p < 0.001), BMI (p < 0.001) and body fat (p < 0.001) of obese were higher than those of normal group. There was no significant difference in nutrient intake between obese and normal groups. SOD activity of obese group was not significantly different from normal groups, in both males and females. However, in the males, serum Cu concentration of obese were significantly lower than those of normal group. In the females, Serum Mn concentration of obese were significantly lower then those of normal group. In the correlation analysis, BMI of the subjects had significantly negative correlations with serum Cu, Zn and Mn. To summarize the results, increase of obesity may lead to decrease of serum antioxidant minerals such as Cu, Zn and Mn.

Physical activity level, total daily energy expenditure, and estimated energy expenditure in normal weight and overweight or obese children and adolescents (소아청소년의 비만여부에 따른 신체활동수준, 1일 총에너지소비량 및 에너지필요추정량의 평가)

  • The purposes of this study were to assess the physical activity level (PAL) and the total daily energy expenditure (TEE) as well as to evaluate the validity of prediction equation for the estimated energy requirement (EER) in normal weight and overweight or obese children and adolescents. The subjects comprised of 100 healthy Korean students aged between 7-18. The anthropometric data was collected. PAL was calculated from the physical activity diary by the 24-hour recall method, and the resting metabolic rate (RMR) was measured by an open-circuit indirect calorimetry using a ventilated hood system. Daily energy expenditure was PAL multiplied by RMR. EER was calculated by using the prediction equation published in KDRIs. There was no significant difference in the means of age and height between the 46 obese subjects and 54 nonobese subjects. The weight and BMI of the obese group (60.2 kg, $25.3kg/m^2$) were significantly higher than those of the nonobese group (42.4 kg, $18.4kg/m^2$). However, PAL was not significantly different between the two groups (nonobese 1.45, obese 1.46). TEE of the obese group (2,212 kcal/day) was significantly higher than that of the nonobese group (1,774 kcal/day). EER (individual PA) and EER (light PA) were significantly higher than TEE (p < 0,001); however, EER (sedentary PA) was not significantly different with TEE in the two groups. These results showed that the levels of physical activity were the same as the sedentary activity both in the nonobese and obese Korean students; moreover, the predictive equation for EER published in KDRI overestimated the TEE of Korean children and adolescents. Therefore, in further research, a new predictive equation for EER should be developed for Korean children and adolescents through the doubly labeled water method.
