파킨슨병 환자의 영양 상태에 따른 식사의 질 및 삶의 질에 관한 연구 (Study on the qualities of diet and life in Parkinson's disease patients according to their nutritional status)

  • Assessment regarding the nutritional status of Parkinson's disease (PD) patients is important because their nutritional status influences the outcome of disease. The purpose of this study investigated the dietary habits, dietary quality, and quality of life for Korean PD patients according to their nutritional status. Seventy PD patients were recruited from K University Hospital (KMC IRB#0918-07) from February 2010 to October 2011. To diagnose PD, the United Kingdom Parkinson's Society Brain Bank standards were used by a neurology physician. The participants were interviewed and assessed using various tools, including a general questionnaire, anthropometrics, questionnaires for dietary habits, dietary diversity score (DVS), dietary variety score (DVS), diet quality index-international (DQI-I), beck depression inventory (BDI), the Korean version of mini mental state examination (K-MMSE), and 36-item short-form health survey (SF-36). The results of the study were as follows: 1) Most of the PD patients (69.0%) were at risk of malnutrition, and 8.5% and 22.5% of the subjects were malnourished and well-nourished, respectively. 2) Clinical symptoms (swallowing and chewing difficulties) were obvious, and the quality of diet (DDS, DVS, DQI-I) was low in the malnourished group (p < 0.01). 3) The malnourished group had significantly lower scores regarding SF-36 (quality of life) as well as that of cognitive function (p < 0.05). 4) The nutritional status of PD patients was significantly correlated with their depression (p < 0.05) and quality of life (p < 0.01). These results indicate that appropriate nutritional management will improve the nutritional status and quality of life as well as the cognitive function in Parkinson's disease patients.

  • This study was conducted to examine the nutritional status, quality of diet and quality of life in postmenopausal women with mild climacteric symptoms based on their food group intake patterns. The data for nutritional status were obtained using 3-day records. Quality of diet was assessed by INQ, NAR, MAR, DDS, DVS, DQI-I. Climacteric symptoms were analyzed by the questionnaire of Kupperman's index and MENoL. The subjects were classified into the five groups, GMVDF, GMVdF, GMVDf, GMVdf, GmVDF according to their food group intake patterns. Analysis of nutrient intakes showed that the GMVDF group took significantly higher levels of kcal, carbohydrate, protein, fat, vitamin A, thiamin, riboflavin, folate, vitamin C, vitamin E, calcium, phosphorous, sodium, iron, zinc and fiber than GMVdf group did (p < 0.05). INQ of Ca and Fe appeared to be higher in GMVDF than in GMVdf groups (p < 0.05). Analysis of NARs showed that missing milk groups took lower riboflavin, Ca and P than other groups did as the same result with MAR (p < 0.05). Analysis of DDS and DQI showed that GMVdf group had the lowest quality of diet (p < 0.05); however, no difference was found on DVS. The GMVdf group showed the worst climacteric symptoms compared with those of the other groups (p < 0.05). However, we couldn't observe any differences in menopause-specific quality of life among the groups. In conclusion, it would be beneficial to meet all five food groups to increase the quality of diet and to reduce the climacteric symptoms in postmenopausal women.

노인의 건강기능식품 섭취 실태와 영양지수를 이용한 식사의 질 및 영양 평가 (Evaluation of Dietary Quality and Nutritional Status based on Nutrition Quotient and Health Functional Food Intake in the Korea Elderly)

  • This study aim to evaluate dietary quality and nutritional status according to the consumption of health functional food using Nutrition Quotient for Korean elderly (NQ-E) for 288 elderly people attending senior welfare centers in Gyeonggi-do. The questionnaire consisted of items about general information, health functional food, and Nutrition Quotient for Korean elderly (NQ-E). Chi-squared test, Fisher's exact tests, and Analysis of Covariance (ANCOVA) were performed using the SAS program ver. 9.4. Among the male and female subjects, the female subjects consumed more health functional food. The results of the dietary quality and nutritional status difference according to the intake of health functional food showed significant differences only in the areas of variety and abstinence among nutrition quotient factors for men, while no significant differences were observed in any of the nutrition quotient factors for women. In conclusion, focused-nutrition education program and useful guideline is needed for promoting adequate consumption of health functional foods in elderly.

메타분석을 이용한 임상영양서비스의 비용-효과성 평가 (Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. 영양교육 프로그램이 서울 지역 노인의 체력, 영양상태, 건강관련 삶의 질에 미치는 영향 (The Effect of Nutrition Education Program in Physical Health, Nutritional Status and Health-Related Quality of Life of the Elderly in Seoul)

  • This study was performed to investigate the effects oi nutrition education program in physical health, nutritional status and health-related quality of life (HRQoL) of the Elderly in Seoul. Nutrition education program was consisted of healthy eating, prevention and diet therapy of obesity, diabetes, cardiovascular disease, and osteoporosis. Seventy eight free-living elderly people (13 male, 65 female), aged ${\geq}60 $ years participated in this program. Before and after nutrition education program, we surveyed the general characteristics, physical health, general health, nutrition status, and health-related quality of life to the subjects. All the subjects were divided into program completers (N=47) and noncompleters (N=31). All the data were analyzed by student t-test, chi-square test, paired t-test, and marginal homogeneity test using SPSS 9.0 version at p<0.05. After nutrition education program, physical activity and ADL maintained, however IADL improved in program noncompleters. In eating habits, 'slow eating' significantly improved in program completers in program completers. Nutrition knowledge and recognition scores were significantly increased in both groups, and accuracy score was significantly increased in program completers. However, nutrient-intakes of %RDA were not significantly changed in both groups, and it seemed to be more influenced by other factors such as 'family income' or 'family type' than by the nutrition education program. In HRQoL, social functioning was improved after nutrition education program in both groups (p<0.05). The nutrition education program has more effects on the program completers than on the noncompleters, and it is also needed social supports for the Elderly to fulfill their nutrient requirements.

Metabolomics Analysis of the Beef Samples with Different Meat Qualities and Tastes

  • The purpose of this study was to investigate the meat metabolite profiles related to differences in beef quality attributes (i.e., high-marbled and low-marbled groups) using nuclear magnetic resonance (NMR) spectroscopy. The beef of different marbling scores showed significant differences in water content and fat content. High-marbled meat had mainly higher taste compounds than low-marbled meat. Metabolite analysis showed differences between two marbling groups based on partial least square discriminant analysis (PLS-DA). Metabolites identified by PLS-DA, such as N,N-dimethylglycine, creatine, lactate, carnosine, carnitine, sn-glycero-3-phosphocholine, betaine, glycine, glucose, alanine, tryptophan, methionine, taurine, tyrosine, could be directly linked to marbling groups. Metabolites from variable importance in projection plots were identified and estimated high sensitivity as candidate markers for beef quality attributes. These potential markers were involved in beef taste-related pathways including carbohydrate and amino acid metabolism. Among these metabolites, carnosine, creatine, glucose, and lactate had significantly higher in high-marbled meat compared to low-marbled meat (p<0.05). Therefore, these results will provide an important understanding of the roles of taste-related metabolites in beef quality attributes. Our findings suggest that metabolomics analysis of taste compounds and meat quality may be a powerful method for the discovery of novel biomarkers underlying the quality of beef products.

대구지역 성인의 가당음료 섭취와 영양지수를 활용한 식사의 질과의 관련성 (Association between Sugar-Sweetened Beverage Intake and Dietary Quality using Nutritional Quotient among Adults in Daegu, Korea)

  • Objectives: This study investigated an association between sugar-sweetened beverage (SSB) intake and the dietary quality of adults in Deagu, Korea. Methods: A questionnaire survey was conducted in 1,022 adults aged 19 ~ 49 years (502 men and 520 women) in the Deagu area of Korea. Daily intake of SSB was obtained by the food frequency questionnaire, and the dietary quality was assessed using the nutrition quotient (NQ) for Korean adults. Multiple logistic regression analysis was used to estimate the association between dietary quality and daily intake of SSB in adults. Results: Daily intake of SSB was 463.6 mL/d for total subjects, and the highest intakes were sweetened coffees (192.7 mL/d), followed by carbonated drinks (77.1 mL/d). Higher intake of SSB was associated with higher intake frequency of fast food or sweet and greasy bread, processed beverage, ramyon, eating out or delivery food and night snack, and also associated with lower frequency of water, breakfast intake and nutrition label checking in men or women. Men and women who had a higher intake SSB had significantly greater odds for being in the low grade of NQ (P for trend = 0.0006 for men, P for trend = 0.0007 for women), especially in the moderation factor (P for trend < 0.0001 for men and women). Conclusions: This study showed that high SSB intake was significantly associated with low dietary quality among adults. These study results suggest that nutrition education programs and guidelines should be provided to adults for improving their consumption of SSB and related diets.

  • The purpose of this study was to evaluate nutrition intake and diet quality according to beverage consumption status in elementary school students (ESS = 317), middle-school students (MSS = 431), and high-school students (HSS = 373). We analyzed data from the combined 2007-2008 Korean National Health and Nutrition Examination Survey (KNHANES). Subjects were divided into two groups, the non-beverage group and the beverage intake group according to beverage intake from drink types (fruit-vegetable drinks, carbonated drinks, and dairy drinks). Intake of dairy drinks was significantly lower in the carbonated drinks intake (CDI) group, compared with the non-CDI group in the MSS group. Intake of vitamin C was significantly higher in the fruit-vegetable drinks intake (FVDI) group, compared with the non-FVDI group. Intake of Ca and P was significantly lower in the CDI group, compared with the non-CDI group. Intake of vitamin B2, Ca, and P was significantly higher in the dairy drinks intake (DDI) group, compared with the non-DDI group. The mean adequacy ratio (MAR) of the FVDI group and the DDI group was significantly higher than those of the non-FVDI and non-DDI group. In diet quality, nutrients less than 1 on the index of nutritional quality (INQ) were significantly higher in the CDI group, compared with the non-CDI group. In conclusion, consumption of carbonated drinks dropped the diet quality however, consumption of fruit-vegetable drinks and dairy drinks improved the diet quality of micronutrients. Therefore, a well-planned diet must be used for replacement of nutrients lost from excessive intake of carbonated beverages during a time in life when growth is especially prominent.

Determining Quality Criteria for Online Health Information: A Qualitative Study

  • The Web is an important source of information for health care consumers, and the resources they find on the Web have a direct affect on their health outcomes. Despite the enormous benefits of online health care, the quality of health information on the Internet is an area of increasing concern. Therefore, there's a need to develop quality assessment tools that can filter out poor quality online health information. The purpose of this study is to explore the critical attributes for assessing website quality and for developing quality assessment measurements. We completed three focus group discussions with 24 participants that were administered by a moderator and based on specifically focused group questions. The results suggest that the most important quality criteria, as identified by the respondents, were related to issues of credibility and accuracy. To determine the credibility of Internet health information, the respondents stated one must consider the following: the information source, disclosure of the author's or organization's credentials/qualifications, disclosure of ownership and the updating of the content. For the accuracy of content, elements such as a statement of purpose, evidence-based information, relevance and completeness should be considered. Interactivity, accessibility, and design were additional quality criteria.

성인 남녀에서 영양표시 활용 정도에 따른 영양섭취 및 식사의 질 평가: 2010~2011 국민건강영양조사 자료를 이용하여 (Evaluation of nutrient and food intake status, and dietary quality in Korean adults according to nutrition label utilization: Based on 2010-2011 Korean National Health and Nutrition Examination Survey)

  • 본 연구에서 국민건강영양조사 데이터를 활용하여 우리나라 성인에서 성별 영양표시의 활용 정도에 따른 식습관, 생활 습관 및 식사 섭취 상태에 미치는 영향을 분석한 결과, 영양표시를 읽지 않고, 영양표시의 영향을 받지 않는 성인의 경우 음주 빈도가 높거나 (남성), 라면의 섭취 빈도는 높은 반면 우유, 요구르트, 두유와 같은 바람직한 식품의 섭취 빈도는 낮은 것으로 나타났다. 또한 영양표시를 활용하지 않는 대상자에서 비타민 $B_2$, 비타민 C 및 칼슘과 같은 미량영양소 섭취의 질이 낮게 나타난 의미있는 결과를 도출하였다. 이와 같은 결과는 영양표시의 이용 및 영향 여부와 같은 적절한 식태도가 영양소 섭취의 질 및 식품 선택에도 영향을 미칠 수 있음을 제안할 수 있는 근거자료가 될 수 있으며, 향후 영양표시 이용에 대한 세분화된 영양교육시 기초자료가 될 수 있을 것으로 사료된다.
