강원도 보건소 방문보건사업실태와 업무분석 (A Study on the Status of the Visiting Health Care Services at the Public Health Centers in Gangwon Province)

  • The purpose of this study was to 1) identify the current management status of the Visiting Health Care Services (VHCS) and 2) to analyze the workload of the staff in the VHCS located in the Public Health Centers (PHCs) in the urban and rural areas. Method: A descriptive research design and a prospective, time and motion research design were used. A total of 18 PHCs in Gangwon Province participated in this study. A questionnaire and semi-structured observational sheet were utilized. A total of 650 self report records of the work load from the VHCS personnel were collected for a 10 day period at each of the 18 PHCs. A descriptive analysis was then done. Results: The major results were as follows. 1) The VHCS staff (nurses and nurse aids) was being assigned additional work such as maternal health care, chronic disease care, mental health care and health promotion on top of their VHCS duties. 2) The average number of home visits per client during the past year was 5.8. More specifically, the clients in the severe dependent group received an average of 27.1 visits, those clients in the moderate dependent group received 14.0 visits those clients in the slightly dependent group received 5.0 visits and those clients in the self-care group received 1.6 visits. 3) The time required for the work duties of the VHCS staff totaled 488 minutes per day. The percentage of time for home visits was only 17.4%, and this didn't include travel time. Conclusion: The main problems of VHCS were identified as a lack of personnel and a lack of time for the home visits. Strategies that are directed at the construction of a better infrastructure for VHCS are needed.

노인요양시설 종사자의 업무환경, 직무만족이 서비스 질에 미치는 영향 (Impact of Work Environment and Job Satisfaction on Service Quality among Staff in Elderly Facility)

  • Purpose: This study examined the effect of work environment and job satisfaction on service quality among staff caring for the elderly. Methods: A convenient sample was 192 staff members from 95 elderly facilities. Data collection occurred from June to July, 2011 using a self-administered questionnaire. Data was analyzed using SPSS/WIN 18.0. Results: Among the subscales of Work Environment Scale, the self-realization score was the highest followed by workload, nervousness, and conflict. The mean score of job satisfaction was 71.43. Reliability was the highest followed by assurance, empathy, responsiveness, and tangibles. Predictive factors of service quality included self-realization work environment and job satisfaction. Conclusion: Exploration of strategies to improve the recognition of self-realization work environment and job satisfaction will be necessary in order to increase of service quality among staffs in elderly facility.

포괄간호서비스 병동과 일반병동 입원 환자의 간호만족도와 병원재이용의도 영향요인 비교 (Comparing Satisfaction with Nursing Care and Factors Relevant to Hospital Revisit Intent among Hospitalized Patients in Comprehensive Nursing Care Units and General Care Units)

  • Purpose: The purpose of this study was to compare hospitalized patients in comprehensive nursing care units and general care units as to satisfaction with nursing care and factors influencing their intent to revisit the hospital. Methods: A cross-sectional study was conducted with 178 patients who had been hospitalized in a comprehensive nursing care unit and a general care unit in one hospital. Participants completed self-report questionnaires. Data were analyzed using SPSS 21.0. Results: There was a significant difference between the comprehensive nursing care unit and general care unit for intent to revisit the hospital (p=.036). Factors influencing intent to revisit the hospital for patients in the comprehensive nursing care unit were 'satisfaction with nursing care' (p<.001) and 'use of additional costs for comprehensive nursing care' (p=.041). The factor influencing intent to revisit hospital for patients in the general care unit was 'satisfaction with nursing care' (p<.001). Conclusion: Findings indicate that comprehensive nursing care service in which all care is provided by nursing staff only, without family or private caregivers, increases intent to revisit the hospital. These results can be used as the foundation of reviewing the operation and expansion of comprehensive nursing care service.

간호과정 적용에 따른 간호기록지 평가 (The Nursing Record Evaluation of the Application of the Nursing Process)

  • According to the provious study. it was suggested a need for improvement of nursing care through application of nursing process in Ewha Womans University Hospital With those data. it has been applied to the nursing care at maternity ward by nurses. This study was undertaken to determine the evaluation of the application of the nursing process which is an orderly. systemic min of deforming the patient's/client's nursing problems. This study involved 191 cases with patient chart and was carried out from Feb. 1979 to Feb. 1980 is Ewha Womans University Hospital. The results were as follows : 1. Actual performance of“Assessment”stage was 59%, and 45 % for the nursing diagnosis. 2. It was achieved with specific planning of nursing care for 71.6% and the plan was safely and effective implemented (97.9% ). 3. Afer“Implement”stage it was made of evaluation and feedback process (39%). 4. Nurses in Eha womans University hospital. they showed the Positive attitude toward the application of nursing process. but they saggested that there were lack of manpower and the consideration of time allocation. Recommandation are as follows : 1. All nursing staff must participate in continious education program for nursing process. 2. The results suggest a need for modification for the nursing history formeat and SOAPIER. 3. A need for improvement of physical condition for interview and effective utilization of nursing staff. 4. It will more effective when Dr's medical record replace by problem ariented aedical record( POMR).

스트레스 관리 프로그램이 호텔조리사의 직무스트레스에 미치는 효과 (The Effects of a Stress Management Program on Job Stress in a Hotel Culinary Staff)

  • Purpose: This study was conducted to investigate the effects of a stress management program on Job Stress in culinary staff at a Hotel. Method: This was a quasi-experimental study using non-equivalent control group pretest-posttest design. The subjects of this study were 52 culinary staff members(head culinary. first culinary) working at a S hotel. (25 in the experimental group. 27 in the control group) The experimental group was provided with an individual counseling program for 12 weeks and 30 min/week. The control group was provided with booklets. Data were analyzed with SAS Program using repeated measures ANOVA. Results: The stress management program was effective to improve job stressors and especially to relax interpersonal conflict among the sub-factors of job stress based on the results mentioned above. Conclusion: This program was effective in reducing job stress. However. further studies are required to measure persistent and long-term effects through the application of the program. which is specifically designed for reduce to job stress. for over 12 weeks.

입원 아동의 가족중심돌봄 개념분석 (Family-Centered Care for Hospitalized Children: Concept Analysis)

  • Purpose: This paper is a report on the concept analysis of family-centered care for hospitalized children. Methods: The concept analysis approach of Walker and Avant was used. A search of multidisciplinary literature published between 1960 and 2016 was undertaken using the keyword 'family centered care' or 'family centered nursing' combined with hospitalized children. Attributes, antecedents, and consequences were inductively derived from the citations analyzed (n=19). Results: The attributes of family-centered care included (1) family respect, (2) collaboration, (3) family support, and (4) information sharing. These attributes are influenced by the 'willingness of family to participate', 'competency and willingness of staff,' and 'institution policy and system.' Additionally, family-centered care does significantly impact 'the health of the children', 'family empowerment' and 'work satisfaction and self-confidence of staff'. Conclusion: Family-centered care of hospitalized children as defined by the result of this study will contribute to the theoretical foundation for application in pediatric nursing practice.

간호등급 상향에 따른 직접간호활동, 간호업무성과와 직무만족 비교 (Differences of Upgrading Nurse Staffing in Nursing Care Activity, Work Performance Outcomes, and Job Satisfaction)

  • Purpose: This study examined differences in nursing care activity, work performance outcomes, and job satisfaction associated with upgrading nurse staffing of a nurse-to-patients ratio. Methods: Descriptive design was used in this study. In total, 148 medical and surgical nurses were recruited from one university hospital. Three instruments were used for data collection: Scale of Nursing Care Activity, Nurses' Work Performance Outcome Measurement Scale and Nurses' Job Satisfaction Scale. Data were analyzed by using descriptive statistics, t-test, ANOVA, and paired t-test. Results: There were significant differences in nursing care activity (t=-5.06, p<.001), in work performance outcomes (t=-5.46, p<.001) and in job satisfaction (t=4.61, p<.001) when the grading for the nursing staff was changed from three to two indicating increasing number of nurses. Conclusion: The findings from this study showed that there were more nursing care activities, better work performance outcomes, and higher job satisfaction when numbers of nurses were increased. The changes in the scale to evaluate nursing staff influence nursing activities, work performance and job satisfaction.

심장외과 중환자실 일반간호사의 업무분석 (Job Analysis of the Staff Nurse in Cardiac Surgery Intensive Care Unit)

  • Purpose : This study was conducted to provide for a basic resource, which can be used to set up a efficient management system in Cardiac Surgery Intensive Care Units(CSICU). Method: 1) Questionnaires were administered and observation methods were used, to examine the nursing activities performed in the CSICU after having reviewed related literatures and a review by the experts. Thus, the nursing activities were designating 254 activities and classified into 28 categories. 2)The 22 nurses in the 2 CSICUs filled out questionnaires about nursing activities from 12 April, 2002 to 17 April, 2002. The frequency of the nursing activities in the 28 categories counted and new nursing activities added by directly observing 12 nurses by two trained research staffs for 4 day. 3)In terms of validity, the 264 nursing activities were analysed by the 25 experts. As a result, 231 nursing activities were found valid and remained as appropriate nursing activities to be used for the careful analysis of the nursing activities in CSICUs. Result: The 22 categories are as below: assessment, monitoring, respiration management, nutrition management, elimination/drainage management, mobility management, sanitation management, safety management, temperature management, specimens collection, preparation and assistance of treatment, skin/wound management, infection management, medication management, education/support, dying patient care, recording/keeping, supplies management, environment management, communications, evaluations, professional development Conclusion : The manifest job description of the staff nurse will contribute to improving the efficiency of the nursing activities and to reducing the role conflicts among the medical staffs.

간호근무환경과 간호결과와의 관련성 및 간호사 업무 부담에 대한 내용분석 (The Relationship of the Nursing Work Environment and Nursing Outcome among it's Nurses and Content Analysis of Nurses' Workload)

  • Purpose: The aim of this study was to explore the relationship between nursing work environment and nursing outcome of clinical nurses. Also, the purpose of this study was to identify the nurses' workload. Method: The participants in this study were 246 nurses working at one hospital in a province. Data was collected using a structured questionnaire from October 2011 to November 2011. Open questions were subject to content analysis; closed questions were subject to descriptive statistics and Pearson correlation coefficient with SPSS 18.0 program were used. Result: The number of patients per nurse in day-duty and overtime work had a positive correlation. Nursing performance and the overtime work in night duty nurses had a negative correlation. The results of this study, with a much higher number of patients per nurse, showed an increase in overtime hours. In the nursing delivery system experience of nurses, 13 themes were identified and they were divided into five categories. Conclusion: Therefore, in order to improve nursing performance and increase job satisfaction, efforts must be made to reduce overtime, and nursing personnel placement to secure a substantial nursing staff.

수간호사가 활용하는 근무성적 평정에 관한 조사연구 (A Survey on Head Nurses' Perceptions and Performances on Staff Evaluation)

  • This study was designed and carried out to identify the methodologies of the staff evaluation used by the headnurses and their perception on the purpose of the evaluations, and to measure the degree and type of perceived purpose and the degree of implementation of staff evaluation principles into preactice. A hundred and eleven from four general hospitals with 600 beds and over were conveniently sampled. Data were gathered from August 20, through September 10, 1994 ; demographic data of respondents and 'methodologies used for staff evaluation' by questionaires, level of 'perceived purposes of staff evaluation' and level of implementation of the 'principles of staff evaluation into practice' by Likert-type rating scales developed by the researcher. Raw and percentile scores and means were calculated and data were processed further by t-tests and ANOVA. Results are as follows ; 1. Traditional methodlogies : checklists(90.1%) and ranking methods(45.9%) revealed to be the two mostly prefered and used methods. 2. Of the 20 item-descriptions of the 'purpose of staff evaluation': 10 'control, evaluative' and 10 'non-control, developmental' the first(mean=3.19) to the ninth(mean=2.74) in rank order revealed to be the 'control, evaluative purpose', while the tenth in rank order revealed to be the first among the 'non-control, evaluative' item(mean-1.57). 3. Level of purpose of staff evaluation perceived as 'control ; evaluative'(mean=43.76, S.D.=5.01) revealed to be significantly(t=-3.86, p<.05) highter than 'non-control ; developmental'(mean=40.65, S.D.=3.41). 4. Level of implementation of 'staff evaluation principle into practice' revealed to be moderately high(mean=2.40).
