The effect of Semen Sinapsis Albae Pressing on Dysmenorrhea of Women`s University Students (백개자(白芥子) 피내침(皮內鍼)이 여대생의 월경곤난증(月經困難症)에 미치는 효과)

  • Objective : This study was to vertify the effect of Semen Sinapsis Albae Pressing at Sanyinjiao(SP6), Umnungchon(SP9) and Shinnsu(B23) on dysmenorrhea of Women's University Students. This study was an one group pretest-posttest experimental design. Methods : The subjects were 50 patients who was suffering for dysmenorrhea visited to D University Health center located in B city, from January to May, 2004. They had agreed to take part in this experiment, with didn't take any anodyne drugs. Results : The results of the study are as follows 1. Hypothesis 1 that "Semen Sinapsis Albae pressing in SP6, SP9 and B23 is decreased dysmenorrhea" is adapted(t=12.777, p=0.000). 2. Semen Sinapsis Albae pressing is more fast manifested in alleviation of pain, longer in duration of alleviation of pain, and had a less dysmenorrhea than anodyne drugs. Semen Sinapsis Albae pressing is probably more effective than who are dosed anodyne instead of pressing.

Comparison of Sugammadex and Neostigmine on First Spontaneous Breathing and Adverse Effects for Laparoscopic Cholecystectomy (Laparoscopic Cholecystectomy 수술 후 Sugammadex와 Neostigmine 간에 첫 자발호흡과 부작용 발현 연구)

  • Objective: The purpose of the study was to investigate the time from the injection of muscle relaxants to the first spontaneous respiration between sugammadex and conventional reversal for patients undergoing laparoscopic cholecystectomy. Methods: This study was retrospectively conducted on patients who were diagnosed with gallbladder stone (N802) between January 2014 and April 2017. The data were collected from the electronic medical records of a total of 186 patients (84 patients in the neostigmine group and 102 patients in the sugammadex group). Results: The time required for the first spontaneous respiration in the sugammadex group was shorter than that in the neostigmine group (3.6 min vs 4.9 min; p<0.05). After the injection of intermediate muscle relaxants, the comparison of heart rate and mean arterial pressure in the sugammadex and neostigmine groups revealed that the heart rate in the neostigmine group was higher than in the sugammadex group after 5 min (p<0.05). The mean arterial pressure in the neostigmine group was higher than in the sugammadex group after 10 min (p<0.05). A significant adverse effect of tachycardia was observed in the neostigmine group (p<0.05), but the frequency of rescue antiemetic in the sugammadex group was significantly higher than in the neostigmine group (p<0.05). Conclusion: In this study, the unwanted effect of neostigmine group was tachycardia; therefore, in the case of patients with hemodynamic instability, sugammadex is recommended. At 12 hours after the injection of sugammadex to patients, more antiemetics were required than in the neostigmine group; therefore, more research should be conducted on postoperative nausea and vomiting.

The Estimated Evacuation Time for the Emergency Planning Zone of the Kori Nuclear Site, with a Focus on the Precautionary Action Zone

  • Background: The emergency planning zone (EPZ) of the city of Busan is divided into the precautionary actions zone (PAZ) and the urgent protective action planning zone; which have a 5-km radius and a 20-km to 21-km radius from the nuclear power plant site, respectively. In this study, we assumed that a severe accident occurred at Shin-Kori nuclear unit 3 and evaluated the dispersion speed of radiological material at each distance at various wind speeds, and estimated the effective dose equivalent and the evacuation time of PAZ residents with the goal of supporting off-site emergency action planning for the nuclear site. Materials and Methods: The total effective dose equivalent, which shows the effect of released radioactive materials on the residents, was evaluated using the RASCAL 4.2 program. In addition, a survey of 1,036 residents was performed using a standardized questionnaire, and the resident evacuation time according to road and distance was analyzed using the VISSIM 6.0 program. Results and Discussion: According to the results obtained using the VISSIM and RASCAL programs, it would take approximately 80 to 252.2 minutes for permanent residents to move out of the PAZ boundary, 40 to 197.2 minutes for students, 60 to 232.2 minutes for the infirm, such as elderly people and those in a nursing home or hospital, and 30 to 182.2 minutes for those temporarily within the area. Consequently, in the event of any delay in the evacuation, it is estimated that the residents would be exposed to up to $10mSv{\cdot}h^{-1}$ of radiation at the Exclusion Area Boundaries (EAB) boundary and $4-6mSv{\cdot}h^{-1}$ at the PAZ boundary. Conclusion: It was shown that the evacuation time for the residents is adequate in light of the time lapse from the initial moment of a severe accident to the radiation release. However, in order to minimize the evacuation time, it is necessary to maintain a system of close collaboration to avoid traffic congestion and spontaneous evacuation attempts.

A study on the attitude toward dying and hospice (임종태도와 HOSPICE에 관한 연구)

  • This research is focused on the attitudes toward dying and hospice. 4subjects groups are Ca. patient, Ca. patien's family, elderly, medical personnel. A 40 questionare was filled out by each participant. For this study chi-square and T-test was done. The result were as follows : 1. Telling the truth 61.2% of all subject groups agreed upon telling the impending death. About 40% of elderly groups and cancer patient group were disagreed which is the highest percentage in all groups. Particularly medical personnel group were remarkably high in telling the truth. 2. Attitudes of medical personnels 43.3% of all groups agreed upon medical personnels prefer to avoid dying patient. In medical personnel group. 44% of respondents disagred comparably higher than other group. But 37.7% of medical personnel agreed. It showed that medical personnels admitted their negative feelings toward dying patient in considerable degree. 3. Attitudes toward mechanical assistance for life-expanding or hopeless patient. 44.8% of all groups disagreed upon mechanical assistance for hopeless case. Elderly (54.9%) and medical personnels (50%) disagreed, which is higher than cancer patient (33.3%) and (22.8%) of cancer patient's family. 4. Special facility and educational preperation for dying patient. 67.4% of all groups agreed upon the needs of special facility for dying, 81.3% of medical personnels agreed which is highest percentage in all group. 5. Attitudes of family members of dying patient. 82.3% of all groups agreed upon the family members feel annoyed at dying patient. 34% of cancer patient's family member and 48% of cancer patients group agreed, but elderly group showed highest percentage(84%). 6. Perception of dying patient about imminent death. 58.3% of all group thought dying patients are aware of their impanding death even though they had not beent told. 77.3% of medical personnel agreed which is highest percentage in all group.

Differences in the Needs According to Gender or Major for Development of the Prevention Program for Dating Violence of University Students (대학생의 데이트폭력 예방 프로그램 개발을 위한 성별 및 전공계열에 따른 요구의 차이)

  • The purpose of this study is to analyze whether there is a difference in the needs of the prevention program for dating violence according to the gender and major of University students. The subjects of the study were the humanities, social science and health care students in U university, which included 220 male students, 131 female students, and a total of 351 students. The research tool produced and surveyed the questionnaire about the timing, content and method of education about prevention of dating violence through the review of previous studies. For data analysis, independent sample t-test and cross-analysis were conducted to analyze the differences in the demands for dating violence prevention programs according to the gender and major of University students. The results of the study were as follows: First, there was a significant difference in the demands of teaching methods of dating violence prevention programs according to the gender in personal counseling and experience activities using open KakaoTalk. The women were significantly higher than men in all sub-factors of dating violence prevention programs. Second, there was a significant difference in the demands of teaching methods of dating violence prevention programs according to the major in personal counseling and experience activities using open KakaoTalk. The health care students were significantly higher in all sub-factors of dating violence prevention programs than in humanities and social sciences students.

Comparison of Safety Perception between Patients and Nurses and Factors Affecting Nurses Safety Management Activities in Tertiary Hospitals (상급종합병원 환자와 간호사의 안전 인식 비교와 간호사의 환자안전관리 직무수행 영향요인)

  • This study is a descriptive research study to compare the safety perception between patients and nurses and to identify the factors affecting the nurses' safety management activities in tertiary hospitals. Data were collected with structured questionnaires from 147 patients and 147 nurses from a university hospital in J city, Gyeongnam, from July 24 to September 23, 2019, and analyzed using SPSS 23.0. The result showed that the patient's safety perception was significantly higher than the nurse's perception. The significant factors affecting patient management activity were nurse's educational level, position, effective communication, patient safety perception, and perception of importance on patient safety management. In the multiple regression analysis, knowledge about 'effective communication', and 'concerned about patient safety management', explained 54.8% of the nurses' patient management activity. Based on the results of this study, it is emphasized that nurses need to develop strategies to improve the communication and attention between patients and hospital members to improve patient safety management activities in tertiary hospitals.

The Effect of the Possibility of Job-related Risk, Emotional Labor, and Emotional Leadership on Turnover Intention of Caregivers in Long-term Care Facilities (장기요양기관 내 위험발생가능성, 감정노동, 감성리더십이 요양보호사의 이직의도에 미치는 영향)

  • The purpose of this study was to examine the effect of the possibility of job-related risk, emotional labor, and emotional leadership on the job turnover intention of caregivers working at the long-term care facilities in Korea. For the study, survey data of 434 caregivers at senior nursing homes were collected and analyzed using hierarchical multiple regression method. The results showed that high level of caregivers'perception of both the possibility of management risk and employee risk led to increased turnover intention. On the other hand, perception of the possibility of social-disaster risk did not give significant impact on turnover intention. Although the perception of the possibility of service user risk gave statistically significant effect on the turnover intention, the direction was the opposite to the one set at the research hypothesis. Frequent camouflage behavior("surface acting") of caregivers, which is the kind of emotional labor led to increased turnover intention. However, caregivers'behavior consistent with their inner warm heart("deep acting") did not lead to a statistically significant result. Lastly, the more caregivers perceive their superiors'emotionally supportive leadership, the lower possibility of turnover intention. Based on the above result, the researchers suggested several intervention methods in order to prevent turnover of caregivers and improve service quality of long-term care institutions.

A Study on the Sufficiency of Anticipated Effect of Order Communication System Introduced to Medium-Sized Hospitals (Focused on the case of S Hospital) (중소병원의 처방전달시스템 도입효과분석에 관한 연구 (S병원의 사례를 중심으로))

  • The purpose of this study is to ascertain whether the effect of introduction of OCS(Order Communication System) to the hospital is satisfied or not comparing the anticipated effect with the actual effect. For this purpose, a domestic hospital which has introduced and has been operating OCS for several years was chosen. Based on the internal data of S Hospital prepared before introducing OCS, researcher has analyzed the basic direction, design standard and status of operation after the introduction of OCS, etc. After analyzing the status of operations of several departments using OCS and interviewing with the chiefs of pertinent departments, a survey form was designed. Actual survey and interviews were conducted by the researcher for weeks to know whether doctors, nurses, medical technicians and clerks of the patient management dept. were satisfied with OCS and to find if they have any recommendations to improve OCS. Based on the analysis of survey, the effect of OCS was evaluated whether it has satisfied the anticipated effectiveness. For the question if they feel convenient in using OCS, doctors, nursing staffs in charge of ward and the staffs of billing dept. has answered that they were all satisfied(100%). The answers for the same question were relatively high in the case of nurses in charge of outpatient and staffs of radiography. Of course, there have been some nurses and staffs who complained for the inconvenience. However, overall satisfaction was high on the average. Some common problems occurred after the introduction of OCS were frequent errors due to instability of OCS system, paralysis of function of hardware on data back-up system and redundant investment due to erroneous choice of DB program in setting DB. It was also pointed out that lack of computer education and low participation of medical staffs has resulted in failure of developing effective software. As a result, it has lowered the efficiency of OCS. For example, some works have to be done by hands even after OCS. Based on the result of this research, recommendations to maximize the effect of OCS were presented as follows. First, strong leadership of CEO and active cooperation of doctors are mandatory. Second, all the process of hospital work should be analyzed and be redesigned in more efficient ways. Third, OCS should be designed to be user-based system which can be used efficiently by all staffs of the hospital. Forth, prior to the operation of OCS, proper tests of the program and trainings of the pertinent staff are required. Fifth, prior to the selection of hardware, BMT(Bench Marking Test) should be conducted. Sixth, before introducing OCS, staffs in charge of OCS should visit many hospitals operating the OCS system and take their cases into account.

Influences of Type of Leadership on Hospital Organizational Members' Job Satisfaction, Organizational commitment, and Turnover Intentions (리더십유형이 병원 조직구성원의 직무만족, 조직몰입 및 이직의도에 미치는 영향)

  • It is important how the leader leads the organizational members with leadership appropriate to environment changes as the leader has great influence on the management of hospital organization. These data were collected from the questionnaire surveyed, from February 14 through February 28, 2003, to 218 people among those in the field of doctorship, management administration, nursing, technology, medical engineering, management engineering, and general affairs, working for three hospitals under S medical foundation in Pusan. The principal findings of this research are as below : 1. Type of leadership and job satisfaction had positive correlation in terms of both transformational and transactional leadership, with former having more relation than the latter. 2. Type of leadership and organizational commitment had positive correlation in terms of intellectual stimulus and situational rewards, having little relation statistically. 3. Type of leadership and turnover intentions had negative correlation only in individual consideration, situational rewards, and exceptional management, showing that transactional leadership had more relation than transformational leadership. 4. In terms of leadership and its effect on the organizational performance and turnover intentions, transformational leadership had more correlation than transactional leadership. But transactional leadership also turned out to have effect on organizational performance. When transformational leadership used, turnover intentions were lower than when transactional leadership used. However, both transformational and transactional leadership had negative correlation to turnover intentions. 5. Effect that type of leadership according to job classification, status, age, and academic career has on organizational performance and turnover intentions was differed by those factors; especially, in the occupation of doctor, both transformational and transactional leadership had positive correlation to turnover intentions. 6. Regression analysis on the factors of organizational performance and turnover intentions showed that higher age had positive correlation to the organizational performance in terms of socio-demographical features and that the more leadership was used for charismatic and situational rewards and the less leadership was used for intellectual stimuli, the higher organizational performance was likely. In short, hospital operators and managers should examine each manager's qualification, type, and style of leadership in the hospital organization, and they must develop lasting programs for effective education and training for developing characteristics of leadership are in needed to have managers with appropriate managerial mind to respective post and status. It should be kept in mind that manager's leadership especially has great effect on members' goal-setting, goal-achievement, organizational performance, and turnover intentions as it is a decisive factor of organizational members' attitude and motivation.

Correlation between Regular mealtime, Oral health knowledge and Oral health of college students (대학생의 식사규칙성과 구강건강지식 및 구강건강 간의 관계)

  • The purpose of this study was to investigate the correlation between regular mealtime, oral health knowledge and oral health to prevent dental impairment of college students. This research design is correlation study. Data of 903 were collected from June 1 to June 30, 2010. Data were analyzed using the SPSS PASW Statistics 18.0 Program. More than 50% of respondents were the irregular mealtime. Higher percentage of correct answer in the oral health knowledge showed periodontal disease prevention(82.6%). There were a statistical significant in grade, major, smoking, drinking, and wishing for dental care between general characteristics and regular mealtime respectively(p<.05). Between general characteristics and oral health knowledge were a statistical significant in gender, age, grade, smoking, and dental checkup respectively(p<.05). On the other hand, there was a statistical significant wishing for dental care between general characteristics and oral health(p<.001). Regular mealtime was negatively associated with oral health(p<.01). Education for oral health should include regular mealtime, prohibit of smoking and alcohol, right toothbrushing and so on.
