A Case Report on Parkinson's Disease Soyangin Patient with Abdominal Pain and Constipation (만성복통 및 변비를 호소하는 소양인 파킨슨병 환자의 치험1례)

  • Objectives The purpose of this case was to report significant improvement of abdominal pain, constipation and a slight improvement of tremor after treatment by Sasang Constitutional Medicine to the patient with Parkinson's disease.Methods The patient was diagnosed as Soyangin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications and acupuncture. The progress was evaluated with the Global Assessment Scale (GAS).Results The abdominal pain was reduced from Numeric Rating Scale (NRS)9 to NRS1 and headache was reduced from NRS5 to NRS3. The tremor was reduced to GAS 10. Unified Parkinson Disease Rating Scale (UPDRS) was reduced from 40 to 27. The difference of heat distribution between left and right side was reduced and the abdomen was more warmer on Digital Infrared Thermal Imaging (DITI).Conclusions Sasang constitutional medicine can be a solution for the non motor symptoms caused by Parkinson's disease.

  • Drug-induced dyskinesia is an involuntary muscle movement caused by various dopamine receptor-blocking drug exposure, such as antipsychotics, antidepressants, and antiemetics. Causative drug removal is the main treatment for drug-induced dyskinesia whenever possible because its pathophysiology lacks a universally accepted mechanism; however, the symptoms can persist for years or decades in many patients even after causative drug removal. Herein, we present a case of drug-induced dyskinesia in a 61-year-old female patient who consumed medication for approximately 10 years for her depression, anxiety, and insomnia. Cervical and facial dyskinesia was suggested to be related to perphenazine and levosulpiride administration. The patient received acupuncture, pharmacopuncture, herbal medicine, and chuna treatment for 81 days during hospitalization. The symptoms were evaluated using the Abnormal Involuntary Movement Scale, Toronto Western Spasmodic Torticollis Rating Scale, Tsui's score, and Numeric Rating Scale, which revealed remarkable improvement, suggesting the effectiveness of combined Korean medicine for drug-induced dyskinesia.

A Rehabilitation for Ankle Fracture in Korean Medicine: A Report of 4 Cases (발목 골절 수술 후 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고)

  • The objective of this study was to propose a rehabilitation protocol for ankle fracture in Korean Medicine and to report its effectiveness. Four patients who had undergone ankle fracture by car accident were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the ankle joint, numeric rating scale, Olerud-Molander score, and walking state. After the treatment, we observed that ankle movement and muscular strength were improved. Moreover numeric rating scale, Olerud-Molander score, and walking state of patients were improved. A rehabilitation in Korean Medicine can be effectively used for ankle fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine.

Three Case Reports of Patients Treated with Korean Medicine after Rotator Cuff Repair

  • Objectives : This study examined the effects of Korean medicine treatment in three patients following rotator cuff repair. Methods : Patients were treated with acupuncture, bee venom, moxibustion, and herbal medicine. Treatments were performed for an average of 3 weeks. A numeric rating scale (NRS) and range of motion (ROM) were used for evaluation of treatment effects. Results : The NRS score decreased and the ROM increased after treatment. In Case 1, the NRS score decreased from 10 to 6; flexion increased from $25^{\circ}$ to $180^{\circ}$ and abduction increased from $35^{\circ}$ to $180^{\circ}$. In Case 2, the NRS score decreased from 10 to 7; flexion increased from $30^{\circ}$ to $125^{\circ}$ and abduction increased from $15^{\circ}$ to $100^{\circ}$. In Case 3, the NRS score decreased from 10 to 3; flexion increased from $40^{\circ}$ to $120^{\circ}$ and abduction increased from $60^{\circ}$ to $95^{\circ}$. Conclusion : Korean medicine treatment following rotator cuff repair was effective in decreasing the NRS score and increasing ROM. Although the study only involved 3 cases, Korean medicine treatment may reduce the duration of rehabilitation.

A Case Report on the Effects of Gamiguibi-tang Combined with Sweet Bee Venom to Improve Raynaud's Disease (봉약침 치료와 병행한 가미귀비탕으로 호전된 레이노병 환자 치험 1례)

  • Objectives: The aim of this study was to report the clinical effects of a combination of Gamiguibi-tang and sweet bee venom on a patient with Raynaud's disease. Methods: The patient with Raynaud's disease was treated with Gamiguibi-tang three times a day for 43 days and with sweet bee venom daily. The effects on Raynaud's disease were measured on both hands using a cold stress test and an infrared thermometer and by digital infrared thermographic imaging and a NRS (Numeric Rating Scale). We conducted the cold stress test at 6-8 day intervals from 2016.04.27 to 2016.06.08. Results: After treatment, the symptoms of pain were decreased in both hands and digital infrared thermographic imaging (DITI) confirmed a rise in the temperature of the fingers. The fingertip temperature increased from 29.6 to 30.4 degrees and the cooling/rewarming ratio (CRR) increased steadily. No adverse events were found at discharge. Conclusions: Gamiguibi-tang combined sweet bee venom may improve symptoms in patients with Raynaud's disease.

Effects of Daoyin Exercise Therapy Combined with Korean Medicine Treatment on the Pain and Function Improvement of Low Back Pain Patients Diagnosed with Lumbar Disc Herniation : A Retrospective Observational Study (한방 복합치료를 병행한 도인운동요법이 요추추간판탈출증을 진단받은 요통 환자의 통증 및 기능 개선에 미치는 영향 : 후향적 관찰연구)

  • This retrospective observational study was aimed to evaluate that Daoyin Exercise therapy improves the symptoms related to lumbar disc herniation. We analyzed the medical records of 7 patients, who satisfied with inclusion and exclusion criteria. The subjects of the study were the patients who were prescribed Daoyin Exercise Therapy and Korean medical treatments for lumbar intervertebral disc herniation at Dunsan Korean Medicine Hospital of Daejeon University from July 25th, 2016 to March 31th, 2017. The effects of Daoyin Exercise Therapy was evaluated by comparing before and after taking Daoyin Exercise Therapy through 0-10 Numeric Rating Scale(NRS), Roland & Morris Disability Questionnaire(RMDQ), Trunk Extension Flexion(TEF) Program, EuroQol five Demension Questionnaire(EQ-5D, EQVAS) of the symptoms including lower back pain. The Daoyin Exercise Therapy combined with Korean medicine treatment reduced NRS($3.357{\pm}3.038$) and RMDQ($9.50{\pm}6.364$), and improved Muscle endurance($44.429{\pm}45.136$), EQ-5D($0.09{\pm}0.12$) and EQVAS($6.571{\pm}19.260$) of the 7 patients without side effects. These results implied that Daoyin Exercise Therapy might helps to improve symptoms of patients with lumbar disc herniation by reducing the symptoms of lower back pain and improving muscle endurance, quality of life, strength enhancement and core muscles.

Integrative Oriental Medical Treatments after Hip Fracture Surgery: Two Case Report (고관절 골절 수술 후 한방 통합 치료를 시행한 2례 보고)

  • Objectives : The purpose of this study is to show the clinical effects of integrative oriental medical treatments for femur pain and disfunction after hip fracture surgery. Methods : Two patients were treated using acupuncture, pharmacopuncture, herbal medication and other oriental medical treatments include moxibustion, cupping therapy, physical rehabilitation for 2~3 months. And the effects of these integrative treatments were measured with numeric rating scale(NRS) and functional recovery score(FRS). Results : NRS of femur pain was decreased from 8 to 4~5 in case 1 and from 6 to 4 in case 2. FRS was improved in both cases. Conclusions : Integrative oriental medical treatments maybe effective to the patients complaining pain and functional limitations after hip fracture surgery.

Clinical Effectiveness of Korean Medical Rehabilitation Treatment after Patellar Fracture: A Report of 4 Cases (무릎뼈 골절 후 한방 재활치료의 임상적 효과: 증례 보고)

  • The aim of this study was to propose our rehabilitation protocol involving Korean medicine for patients with patellar fracture and report its effectiveness. Further, for the same patellar fracture patients, we created different protocols for those who underwent surgical treatment and those who required conservative treatment. Among patients who received inpatient treatment at our hospital for three years from August 2017 to August 2020, we included three patients who underwent surgical treatment for patellar fracture and one patient who required conservative treatment for patellar fracture. The degree of improvement of the patients was evaluated using physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After each treatments, we observed that the range of motion of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state improved in the patient groups. Even with the same patellar fracture, we observed that different rehabilitation treatments involving Korean medicine should be applied to patients who underwent surgical treatment and those who required conservative treatment. However, further studies are needed to design a rehabilitation protocol involving Korean medicine.

Seven Cases Report on the Foot Drop Patients with Herniated Intervertebral Lumbar Disc by Muscle Energy Technique & Korean Traditional Medicine Therapy (근에너지기법 및 한방치료를 적용한 족하수를 동반한 요추추간판탈출증 환자 치험 7례)

  • Objectives : The present study reports seven foot drop patients with herniated intervertebral lumbar disc treated by Muscle Energy Technique and Korean traditional medicine therapy. Methods : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and were assessed by Manual Muscle Test, numeric rating scale, and oswestry low back pain disability index. Results : Among seven foot drop patients, two cases showed complete recovery, three cases showed recovery over good grade, and two cases showed recovery over good grade after temporary weakness. numeric rating scale and oswestry low back pain disability index of all cases showed improvement. Conclusions : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and showed improvement. It is anticipated that this report benefits the future in depth study and clinical treatments on the foot drop symptom in korean medicine.

The Clinical Study of the ShinBaro Pharmacopuncture and acupuncture treatment on Plantar Fasciitis (신바로약침과 침치료를 이용한 족저근막염 환자 4례에 대한 임상증례보고)

  • Objects : This study is a report on treatment effect of the case that treated by using of Shinbaro pharmacopunture and acupuncture to plantar fasciitis patients. Methods : The four patients diagnosed with Plantar fasciitis were treated by using of Shinbaro pharmacopunture and acupuncture, Algometer Pressure and Numeric rating scale(NRS) were used as the evaluation tool of treatment effect. Results : Shinbaro pharmacopunture and acupuncture showed improvement of symptoms for all patient groups and brought about significant change on numerical value of Algometer Pressure and Numeric rating scale(NRS). Conclusions : The result shows that Shinbaro pharmacopunture and acupuncture can be effective treatment to Plantar fasciitis. Merely later, further study is needed.
