Granulatous nodules on abdominal serosa of chick infected with Aspergillus species (Aspergillus sp 감염에 의한 닭 복강장막의 육아종성 결절 발생 례)

  • Aspergillosis, caused by infection with Aspergillus fumigatus and less commonly by other Aspergillus species, is a prevalent and costly respiratory disease of poultry, In a flock of chicks, the number of birds, 4∼5 months old, had become gradually emaciated and subsequently died. Gross necropsy revealed multiple granulomatous masses on the abdominal serosa. The masses, 4∼15 mm in size, were attached on mainly intestinal wall. Also, the smaller masses in size were on mesentery and pancreas. However, only a few small white nodules were scattered throughout liver and lung in few samples. Microscopically, the mass were granulomatous with a central area of necrosis containing numerous septate, branched fungal hypae consistent with Aspergillus sp. These were surrounded by macrophages, giant cells, lymphocytes and fibrous tissues. Nodular lesions of liver and lung were seemed to spread hemotogenously from intestine and the possible route of infection was speculated by oral. This report is a Aspergillus-induced granuloma limited to the serosa of abdominal cavity, especially of intestinal wall.

Bronchoscopic Strategies to Improve Diagnostic Yield in Pulmonary Tuberculosis Patients

  • In cases where pulmonary tuberculosis (PTB) is not microbiologically diagnosed via sputum specimens, bronchoscopy has been the conventional method to enhance diagnostic rates. Although the additional benefit of bronchoscopy in diagnosing PTB is well-known, its overall effectiveness remains suboptimal. This review introduces several strategies for improving PTB diagnosis via bronchoscopy. First, it discusses how bronchoalveolar lavage or an increased number of bronchial washings can increase specimen abundance. Second, it explores how thin or ultrathin bronchoscopes can achieve specimen acquisition closer to tuberculosis (TB) lesions. Third, it highlights the importance of conducting more sensitive TB-polymerase chain reaction tests on bronchoscopic specimens, including the Xpert MTB/RIF assay and the Xpert MTB/RIF Ultra assay. Finally, it surveys the implementation of endobronchial ultrasound with a guide sheath for tuberculomas, collection of post-bronchoscopy sputum, and reduced use of lidocaine for local anesthesia. A strategic combination of these approaches may enhance the diagnostic rates in PTB patients undergoing bronchoscopy.

Left Atrial Myxoma; Report of a case (좌심방 점액종의 치험 1례)

  • Myxoma is a benign growth constituting nearly 50% of all primary cardiac tumors. It is important because it can be abolished by surgical therapy and is usually fatal if unrecognized and untreated. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We report a case of left atrial myxoma which was diagnosed by echocardiogram before surgery, and was successfully removed with the aid of extracorporeal circulation. The patient was a 24-year old woman who had suffered from mitral valvular symptoms for 2 months before admission. At operation, a tumor, measuring 5.2 x 4.3 x 4.7 cm, was extremely friable, villous gelatinous mass and it was removed from its origin near the closed fossa ovalis, including its stalk and a portion of the septum. The postoperative course was uneventful. The patient is clinically well and without symptoms of heart disease.

  • The racehorses that under arthroscopic surgery due to be injured his limbs were studied during exercise or training at Busan Race Park from 2005 to 2010. Rate of arthroscopic surgical treatments was 1.4% (63/4,642). Affected bones were radius, radial carpal bone, third carpal bone, proximal phalanx, third metacarpal bone, femur, tibia, proximal sesamoid bone and intermediate carpal bone. The lesions were fracture, chip fracture, slap fracture, osteochonrosis, and osteochondrotitis dissencans. Number of patients under arthroscopic surgery were 63. Success horses of returned to racetrack or tried to return to racetrack were 58, and 5 horses were in training or resting at the time of publication. Success horses of returned to their previous use in the patients were 49 horses (84.4%) and no returned to the racetrack were 9 horses (15.6%) in 58 horses.

Brain metastasis in human epidermal growth factor receptor 2-positive breast cancer: from biology to treatment

  • Overexpression of human epidermal growth factor receptor 2 (HER2) is found in about 20% of breast cancer patients. With treatment using trastuzumab, an anti-HER2 monoclonal antibody, systemic control is improved. Nonetheless, the incidence of brain metastasis does not be improved, rather seems to be increased in HER2-positive breast cancer. The mainstay treatment for brain metastases is radiotherapy. According to the number of metastatic lesions and performance status of patients, radiosurgery or whole brain radiotherapy can be performed. The concurrent use of a radiosensitizer further improves intracranial control. Due to its large molecular weight, trastuzumab has a limited ability to cross the blood-brain barrier. However, small tyrosine kinase inhibitors such as lapatinib, has been noted to be a promising agent that can be used as a radiosensitizer to affect HER2-positive breast cancer. This review will outline general management of brain metastases and will focus on preclinical findings regarding the radiosensitizing effect of small molecule HER2 targeting agents.

  • Clinical neuropsychology which belongs to the necuroscience field is concerned with relationship between human behaviors and the brain structure. Clinical neuropsychology has grown to be a specialized separate field within psychology over the last twenty years. Clinical neuropsychology offers an objective methodology to consider the mind-body interaction and evaluate the behavioral consequences and functional deficits associated with brain lesions. Clinical neuropsychological assessment is composed of cognitive, perceptual, motor and emotional function through various neuropsychological examinations such as Halsted-Reitan and Luria-Nebraska batteries, and computerized neuropsychological test such as PCIS Vienna Test System and Stim. The goals of neuropsychological evaluation are to identify of neuropsychological dysfuncitions, to develop execute and monitor treatment plans, and to make rehabilitation programs. Recently, the neuropsychiatric patients are increasing in number and 15-20% of acute psychiatric patients suffer from organic mental problems. Moreover, clinical neuropsychology has an increasingly important role in both neurobehavioral foundation and clinical application. So, psychiatrists must play a major role in the development of clinical neuropsychology in psychiatry.

Butanol fraction of Panax ginseng head was significantly effective on gastritis and ulcer models in rats. and also had anti-oxidative properties in the previous study. It has been well established that gastric ulcer is induced by imbalance between aggressive factors and protective factors. and the oxidative reaction makes the lesions on gastric mucosal injury severer.   • Endometrial carcinoma is one of the most common gynecologic malignancies and which is predominant in postmenopausal women. Clinically many patients are hospitalized in early stage due to clinical sign and symptom such as vaginal bleeding and in this case, patient's prognosis is known to be good. However, considerable number of patients with advanced and relapsed disease reveal poor prognosis. Therefore, exact staging work up is essential for proper treatment as is primary lesion detection. $^{18}F-FDG-PET$ has been widely used for the evaluation of gynecologic malignancies such as cervical carcinoma and ovarian cancer. In contrast, FDG PET application to endometrial carcinoma is limited until now and there is no sufficient data to validate the usefulness of FDG PET for this disease yet. However, several studies showed promising results that FDG PET is sensitive and specific in detection of recurrent or metastatic lesions. Therefore further active investigation in this field can facilitate the use of FDG PET for endometrial carcinoma.

  • Tuberculosis (TB) is an infectious disease transmitted by aerosol droplets and characterized by forming granulomatous lesions. Although the number of people infected in the population is high, the vast majority does not exhibit symptoms of active disease and only 5-10% develop the disease after a latent period that can vary from weeks to years. The bases of the immune response for this resistance are unknown, but it depends on a complex interaction between the environment, the agent, and the host. The analysis of cellular components of M. tuberculosis shows important host-pathogen interactions, metabolic pathways, virulence mechanisms, and mechanisms of adaptation to the environment. However, the M. tuberculosis proteome still remains largely uncharacterized in terms of virulence and pathogenesis. Here, we summarize some of the major proteomic studies performed to scrutinize all the mycobacterial components.

Comparative Evaluation between 1.5T vs 3.0T MRI in Brain Metastasis According to its Size

  • The purpose of this study was to compare the detection rate of brain metastasis according to size of nodule between 1.5T and 3.0T MRI 대상 및 방법: We reviewed 44 patients with primary tumors and clinical symptoms suggesting brain metastasis. After administration of double dose gadolinium-DTPA, MR imaging was performed with 3D SPGR sequence by 3.0T MRI and then with T1 SE sequence by 1.5T MRI. Consequently, comparison was done in 1.5T T1 SE sequence and 3.0T 3D SPGR sequence. With use of the signal intensity (SI) measurements in the metastatic nodules and adjacent tissue, metastatic nodule-to-adjacent tissue SI ratio were calculated. In each patient, the number of metastatic lesions detected in 1.5T and 3.0T, and their size were assessed qualitatively by three blinded readers.
