병원간호사의 조직몰입에 영향을 미치는 요인 (Influencing Factors of the Organizational Commitment among Hospital Nurses)

  • Purpose: The study aimed to identify influencing factors associated with the organizational commitment (OC) among hospital nurses. Methods: A descriptive correlational design was utilized. The subjects of this study were 447 female nurses working for over 6 months in Seoul. The study was based on the data from a self-reported survey using structured questionnaires. The data were collected from September 10 to 30, 2009 and analyzed using Pearson's correlation coefficients and stepwise multiple regression analysis. Results: The mean score of the OC was 2.45, slightly higher than the intermediate level. The mean score of the three-dimensional domain among the OC, affective commitment (AC) was 2.50, continuance commitment (CC) was 2.59, and normative commitment (NC) was 2.29. The influencing factors of nurses' AC were perceived organizational support (POS) (${\beta}$=.31, p<.001), supervisory trust, job burnout, nursing professionalism, and age, which accounted for 49.7%. The influencing factors of the CC were supervisory trust (${\beta}$=.20, p<.001), POS, and job burnout whereas the influencing factors of the NC were POS (${\beta}$=.40, p<.001), supervisory trust, nursing professionalism, and job burnout. Conclusion: It is necessary to develop supportive strategies that increase OC, which also improve the POS, nursing professionalism, and supervisory trust, and alleviate job burnout in hospital nurses.

A Pilot Examination of Oxidative Stress in Trichotillomania

  • Objective Trichotillomania is a relatively common illness whose neurobiology is poorly understood. One treatment for adult trichotillomania, n-acetyl cysteine (NAC), has antioxidative properties, as well as effects on central glutamatergic transmission. Preclinical models suggest that excessive oxidative stress may be involved in its pathophysiology. Methods Adults with trichotillomania provided a blood sample for analysis of compounds that may be influenced by oxidative stress [glutathione, angiotensin II, ferritin, iron, glucose, insulin and insulin growth factor 1 (IGF1), and hepcidin]. Participants were examined on symptom severity, disability, and impulsivity. The number of participants with out-of-reference range oxidative stress measures were compared against the null distribution. Correlations between oxidative stress markers and clinical measures were examined. Results Of 14 participants (mean age 31.2 years; 92.9% female), 35.7% (n=5) had total glutathione levels below the reference range (p=0.041). Other oxidative stress measures did not have significant proportions outside the reference ranges. Lower levels of glutathione correlated significantly with higher motor impulsiveness (Barratt Impulsiveness Scale sub-score) (r=0.97, p=0.001). Conclusion A third of patients with trichotillomania had low levels of glutathione, and lower levels of glutathione correlated significantly with higher motor impulsiveness. Because NAC is a precursor for cysteine, and cysteine is a rate limiting step for glutathione production, these results may shed light on the mechanisms through which NAC can have beneficial effects for impulsive symptoms. Confirmation of these results requires a suitable larger follow-up study, including an internal normative control group.

  • 목적: I형 뮤코다당증 (MPS I)은 ${\alpha}$-L-iduronidase 효소의 결핍으로 인하여 발생하는 리소좀 축적 질환으로, 광범위한 양상으로 다기관에 영향을 미친다. 저신장과 성장 속도의 감소는 MPS I의 중요한 특징이다. 본 연구에서는 효소 보충 요법이 MPS I 환자들의 성장에 미치는 효과에 대해 알아보기 위하여 단일 기관의 환자들을 대상으로 분석하였다. 방법: 2세에서 15세 사이에 효소 보충 요법을 시작하여 최소 3년 이상의 치료를 시행 받은 10명의 한국 MPS I 환자들의 키 측정치를 후향적으로 분석하였다. 효소 보충 요법 시작시의 평균 나이는 7년 7개월 이였으며, 남아는 6명, 여아는 4명 이였다. 키는 표준 편차(SDS)로 표현되었다. 효소 보충 요법 전과 후의 연간 성장 속도를 계산하였으며, 구분회귀모델을 이용하여 치료 전과 후의 키 z-score를 분석하였다. 표현형[(중증(Hurler) versus 경증(Hurler-Scheie, Scheie)]이 성장에 미치는 영향에 대해서는 개별 분석을 시행하였다. 결과: 효소 보충 요법 전 1년 동안의 연간 성장은 3.3 cm (z-score=-0.21) 였으며, 효소 보충 요법 후 1년, 2년, 3년에서는 각각 6.2 cm (z-score=0.17), 5.8 cm (z-score=0.07), 3.8 cm (z-score=-0.4)이였다. 회귀분석 결과, 효소 보충 요법 전에 비하여 치료 후 기울기에 유의한 호전을 보였다(기울기 차이=0.04; P=0.022). 중증과 경증 표현형 간의 치료 전(P=0.001)과 후(P<0.0001)의 기울기 차이는 통계적으로 유의하였으나, 표현형에 따라 분석하였을 때 통계적으로 유의한 차이는 보이지 않았다. 결론: MPS I 환자들의 키 성장에 있어 aldurazyme 효소 보충 요법이 긍정적인 효과를 미치는 것으로 보인다.

인도네시아 아동들의 아침식사하기에 영향을 미치는 요인 분석 -건강신념모델을 이용하여- (Analysis of Factors Affecting Breakfast Eating Behavior of Children in Indonesia: An Application of the Health Belief Model)

  • Objectives: This study investigates the current state of consuming breakfast among elementary school students residing in Malang, East Java, Indonesia, and to identify factors that influence breakfast behavior. Methods: The research model was set up as per the health belief model, and slightly modified by adding the subjective normative factors of the theory of planned behavior. The survey was conducted from July 17 to August 15, 2017 using a questionnaire, after receiving the permission PNU IRB (2017_60_HR). Results: The subjects were 77 boys (49.4%) and 79 girls (50.6%) suffering from malnutrition with anemia (21.2%) and stunting ratio of Height for Age Z Score (HAZ) (11.5%). Furthermore, moderate weakness (14.8%) and overweight and obesity (12.3%) by Body Mass Index for Age Z Score (BMIZ) were coexistent. According to the results obtained for breakfast, 21.8% did not eat breakfast before school, with 18.8% of the reasons for skipping breakfast being attributed to lack of food. Even for subjects partaking breakfast, only about 10% had a good balanced diet. The average score of behavioral intention on eating breakfast was 2.60 ± 0.58. The perceived sensitivity, perceived severity, perceived benefits, and self-efficacy of the health belief model correlated with breakfast behavior. Of these, self-efficacy (β=0.447, R2=0.200) and perceived sensitivity (β=0.373, R2=0.139) had the greatest effect on breakfast behavior. Mother was the largest impact person among children. Conclusions: In order to increase the level of breakfast behavior intention among children surveyed in Indonesia, we determined the effectiveness by focus on education which helps the children recognize to be more likely to get sick when they don't have breakfast, and increase their confidence in ability to have breakfast on their own. We believe there is a necessity to seek ways to provide indirect intervention through mothers, as well as impart direct nutrition education to children.

한국어판 일상 생활 스트레스 척도(K-DSI)의 표준화 연구 (Standardization of Korean Version of Daily Stress Inventory(K-DSI))

  • 연구목적 : 본 연구에서는 일상 생활에서 흔히 경험하는 사소한 스트레스 요인들을 측정하는데 민감한 척도인 일상생활 스트레스 명가 척도의 한국어판(K-DSI)올 표준화함으로써 정상인에서의 규준표를 제시하고자 하였다. 방법 : 19세 이상의 정상인 524명을 대상으로 한국어판 일상생활 스트레스 평가 척도의 58개 문항을 일주일 동안 연속적으로 매일 작성하도록 하였다. 마지막날에 상태-특성 불안 척도(STAI)와 Beck 우울척도(BDI), 사회 재적용 평가 척도(SRRS)를 작성하도록 하였다. 인구학적 변인에 따라 K-DSI의 사건, 영향, 영향/사건 비율 점수를 비교 분석하였으며(ANOVA or t-test). 변량 분석의 사후 검증 방법으로 Scheffe's test를 사용하였다. 내적 일치도를 통해 신뢰도를 검증하고. BDI, STAI, SRRS 점수와의 상관 분석을 통해 수렴 타당도를 검증하였다. K-DSI 점수들의 백분율 분포에 근거하여 규준표를 작성하였다. 결과 : K-DSI 점수는 남성에 비해 여성에서, 나이가 젊을수록 학력이 높을수록 높았으며, 5개의 하부 요인 범주 중 인지적 스트레스 요인들이 가장 많은 것으로 나타났다. K-DSI의 내적 일치도는 Chrombach $\alpha$값이 .99로 높게 나타났으며, BDI, STAI, SRRS 점수와 K-DSI의 일일 및 주간 사건, 영향 접수는 유의한 상관관계를 보여주었다. 백분율 분포에서 75%에 해당되는 사건 접수는 17점, 영향 접수는 57점, 영향/사건 비율은 3.48~3.49점이었으며, 1주일 사건 접수는 118~124점이고, 1주일 영향 점수는 368~389점, 영향/사건 비율은 3.47~3.48점이었다. 또한 95%에 해당하는 사건 점수는 57점, 영향 점수는 151~161점, 영향/사건 비율은 4.72~4.86점이었으며, 1주일 사건 점수는 405점이고, 1주일 영향 접수는 1038~1122점, 영향/사건 비율은 4.46~4.56점이었다. 결론 : DSI의 신뢰도와 타당도가 만족스러운 수준을 보였고 인구학적 특성에 따라 흥미로운 결과가 나타남으로써 임상적 유용성이 검증되었다. 무엇보다도 규준을 스트레스 경험의 평가에 대한 객관적인 비교기준이 될 수 있는 규준자료를 제시하였다는데 큰 의의가 있다고 할 수 있다.

계획적 행위이론을 적용한 근로자의 보호구 착용행위 분석 (Analysis of the Use of Personal Protective Equipment Based on the Theory of Planned Behavior)

  • This study was purposed to apply Aizen's Theory of Planned Behavior to workers using personal protective equipment. For this purpose, occupational health nurses conducted a survey of 173 male workers who were working at 6 different factories during the period from August to October 2003. The score of each variable related to the Theory of Planned Behavior was calculated: attitude (3.74); subjective norm(4.10); and perceived behavioral control (3.48). Four preceding factors were also assessed: behavioral belief(3.63); outcome evaluation(4.05); normative belief (4.20); and motivation to comply(4.03). Analysing the results, we found that two factors of the Theory of Planned Behavior are important in this case, which were subjective norm and perceived behavioral control that were found to be in a strong positive linkage with the use of personal protective equipment. Thus enhancing subjective norm and perceived behavioral control must be the most important goal of health education for workers who must use personal protective equipment. In addition, though behavioral intention, an intermediate factor of the Theory of Planned Behavior, was not measured in this study, subsequent studies are expected to include the factor in their models.

보건소 영양사의 직무만족도가 조직몰입과 이직의도에 미치는 영향 (Job Satisfaction of Nutritionist: Effect on Organizational Commitment and Turnover Intention in Public Health Center)

  • This study examined the job satisfaction of nutritionists, and its effect on organizational commitment and turnover intention in public health centers. The response data of 174 nutritionists were analyzed. The overall average of job satisfaction was 3.94 out of 7 points, which was determined to be below 'normal'; however, the job suitability of sub-factors scored 5.26, which was more than 'normal'. The variables affecting job satisfaction that were provided by the respondents included job-related licenses (P<0.01) and co-nutritionists (P<0.01). Organizational commitment ranked 'normal' with a score of 4.35. Normative commitment (4.90) of the sub-factors was 'normal' or more, and differed when considering 'age' (P<0.01), 'employment status' (P<0.01), 'salary per year' (P<0.05), and 'working area' (P<0.05). The turnover intention was analyzed to be below 'normal' with 3.88 points, and the variables affecting turnover were significantly higher for subjects in their 20s (P<0.01), less than two job-related licenses (P<0.05), and less than two co-nutritionists (P<0.01). The four variables of job satisfaction, 'business discretion' (P<0.05), 'work environment' (P<0.01), 'job suitability' (P<0.01), and 'reward' (P<0.01), positively affected the organizational commitment. Moreover, organizational commitment had a mediating effect (P<0.01) on job satisfaction and turnover intention. Taken together, our results suggest that public health centers need to develop improvement plans for business discretion, work environment, job suitability and reward.

한국어판 매일 스트레스 평가서 개발을 위한 예비연구 (A Preliminary Study for the Development of the Korean Version of the Daily Stress Inventory(DSI))

  • As a preliminary step to develop the Daily stress Inventory(DSI) for assessing minor stressors frequently experienced in everyday lift. We examined the reliability and validity of the DSI. The Daily Stress Inventory(DSI), the Beck Depression Inventory(BDI), the State-Trait Anxiety Inventory(STAI), the Social Readjustment Rating Scale(SRRS) and the Minnesota Multiphasic Personality Inventory(MMPI) were administered to 111 college students. Compared with the weekly DSI Event, Impact and I/E ratio scores of the American college students, the weekly event and I/E ratio scores of the Korean college students were significantly higher but weekly Impact score was lower. The internal consistency reliability of the DSI as computed by Cronbach alpha was .98. The DSI I/E ratio scores were significantly correlated with the scores of State Anxiety$({\gamma}=.27)$ and Trait Anxiety${\gamma}=.24)$, but not with BDI scores. And the SSRS scores were not significantly correlated to the DSI Event or Impact and In ratio scores. Significant correlations were revealed between the DSI Impact scores and the Sc and Ma scales of the MMPI. And the correlation between the DSI I/E ratio scores and the L, D, Hy, Pd and Pa scales of the MMPI was also significant. But no significant correlation was found between the DSI Event scores and all the scales of the MMPI. This study suggests that the DSI can be a reliable and valid tool to assess the minor stress frequently experienced in everyday life future study should be directed to obtain normative data based on more varied and larger population. Especially gathering additional evidences for the validity of the DSI using other minor stressor scale will be required.

제도적 압력이 중국 기업의 순응 수준 및 기업 가치에 미치는 영향: 제도이론과 이해관계자이론을 중심으로 (The Effect of Institutional Pressure on Firm's Compliance and Financial Performance in China: Focused on Institutional Theory and Stakeholder Theory)

  • This study aimed to understand the relationships among the institutional pressure and the level of compliance and corporate value. The research focuses on four main variables, which are regulatory pressure, normative pressure, and cultural-cognitive pressure as the institutional pressure, and the CSR score as the level of the firm's compliance. We examined the impact of the institutional pressure on the firm's compliance-level, together with the effect of compliance level on the corporate value. We analyzed the 3,792 CSR data listed in China's A market and 31 province and city-level data from China. Results showed that institutional pressure had a positive influence on the firm's compliance level. The corporate value was greater with a high compliance level when the institutional pressure was high. The firm's compliance level negatively influenced corporate value when the institutional pressure was low. This study took into account a level of institutional pressures in three dimensions when investigating the effect of CSR compliance level on the corporate value. Thus, this study has a unique academic contribution by demonstrating that CSR activities can have a positive or negative effect depending on the institutional environment for each firm. The findings of this study also provide valuable insights to industry practitioners by suggesting the importance of considering the institution-specific condition when deciding to comply with the institutional pressure.

주의력결핍과잉행동 장애, 아스퍼거 장애, 학습 장애 아동의 실행기능 비교 (Comparison of Executive function in Children with ADHD, Asperger's Disorder, and Learning Disorder)

  • Objectives : This study was conducted to investigate the deficits of executive function in children with ADHD, Asperger's Disorder(AD), and teaming disorder (LD), and to identify the differential characteristics of executive function deficits among three groups. Methods : The clinical group consisted of 46 children between the ages of 7 and 15 (16 ADHD, 16 LD, 14 AD). Neuropsychological tests for measuring cognitive function, attention and executive function were individually administered to children, and their performance scores were calculated based on the age norm for each test. Results : There was no significant difference in FSIQ, VIQ, and PIQ among the three groups. However, the AD group tended to show higher scores on the subtests of Information, Vocabulary and Digit Span, and lower score on Comprehension subtest than the ADHD and LD groups, while the LD group tended to show the lowest scores on the Information and Vocabulary subtests. On ADS, the ADHD group showed the highest omission and commission errors. All groups showed poor performances belonging to below 25 percentile ranks on executive function tests when compared to the age norms of normative group. The number of completed category on WCST was the smallest in the ADHD group, while the working memory score was the lowest in the LD group. Conclusion : These results suggest that ADHD, LD, and AD children have executive function deficit in common. However, the specific deficit areas in executive function are different for each group.
