The objective of this study was to investigate the effects of a nutritional education and physical exercise program on obese children (47 boys and 36 girls). The nutritional education and physical exercise program was provided for 6 weeks and dietary habits, nutrient intake and hematological profiles were measured before and after the program. The BMI (body mass index, $kg/m^2$) in male children was significantly reduced to $24.58{\pm}2.81$ from $25.89{\pm}3.27$ (p<0.01) and it was significantly reduced to $24.29{\pm}3.51$ from $25.21{\pm}3.59$ in female children after the completion of nutritional education and exercise program. Body fat in male children was remarkably reduced to $32.69{\pm}7.74%$ after education from $37.38{\pm}9.21%$ (p<0.001). After the program total cholesterol concentration in male children was reduced to $176.48{\pm}29.10\;mg/dL$ from $196.03{\pm}29.10\;mg/dL$, whereas there was no significant difference in females before and after the nutritional education and exercise program. The grasping power of male children (p<0.05) and backmuscle strength of male (p<0.05) and female children (p<0.001) were significantly increased after the exercise program. The nutrient intakes that were different before and after the nutrition education were calories (p<0.05) and carbohydrate (p<0.05) in male children, and a significant increase in intake of vitamin A (p<0.05) was found in female children. There was a significant difference in meal time regularity before and after program. The findings of this study showed that there was a significant decrease in both body fat and weight, in addition to normalization of hematological profiles after the completion of nutrition education and physical exercise programs.

Objective : To analyze the clinical courses and outcomes after anterior lumbar interbody fusion (ALIF) for the treatment of postoperative spondylodiscitis. Methods : A total of 13 consecutive patients with postoperative spondylodiscitis treated with ALIF at our institute from January, 1994 to August, 2013 were included (92.3% male, mean age 54.5 years old). The outcome data including inflammatory markers (leukocyte count, C-reactive protein, erythrocyte sedimentation rate), the Oswestry Disability Index (ODI), the modified Visual Analogue Scale (VAS), and bony fusion rate using spine X-ray were obtained before and 6 months after ALIF. Results : All of the cases were effectively treated with combination of systemic antibiotics and ALIF with normalization of the inflammatory markers. The mean VAS for back and leg pain before ALIF was $6.8{\pm}1.1$, which improved to $3.2{\pm}2.2$ at 6 months after ALIF. The mean ODI score before ALIF was $70.0{\pm}14.8$, which improved to $34.2{\pm}27.0$ at 6 months after ALIF. Successful bony fusion rate was 84.6% (11/13) and the remaining two patients were also asymptomatic. Conclusion : Our results suggest that ALIF is an effective treatment option for postoperative spondylodiscitis.
Radiation dose estimation on the newborn and infants during radiation examinations, unlike for the adults, is not actively being progressed. Therefore, as an index to present exposure dose during radiation examinations on newborn and infants, entrance skin dose was measured, and the result was compared with results of monte carlo simulation to raise reproducibility of entrance skin dose measurement, and it was proved that various geometry implementation was possible. The resulting values through monte carlo simulation was estimated using normalization factors for entrance skin dose to calibrate radiation dose and then normalized to a unit X ray radiation field size. Average entrance skin dose per one time exposure was $78.41{\mu}Gy$ and the percentage error between measurement by dosimeter and by monte carlo simulation was found to be -4.77%. Entrance skin dose assessment by monte carlo simulation provides possible alternative method in difficult entrance skin dose estimation for the newborn and infants who visit hospital for actual diagnosis.
Purpose: This study aimed to investigate the clinical significance of serum alanine aminotransferase (ALT) levels in children with nonalcoholic fatty liver disease (NAFLD) and the effect of lifestyle intervention on NAFLD. Methods: The clinical data of 86 children diagnosed with NAFLD were reviewed retrospectively. Forty-six patients belonged to the elevated ALT group and 40 to the normal ALT group. The clinical parameters of patients with NAFLD were also compared based on the status of ALT levels after lifestyle intervention. Results: Patients with elevated ALT had significantly higher body mass index (BMI) scores than those with normal ALT (P<0.05). Of all the patients with elevated ALT, 89% exhibited moderate or severe degree of fatty change in the liver on ultrasonographic examination, whereas most patients with normal ALT exhibited mild or moderate degree changes. Liver biopsy was performed in 15 children with elevated ALT and all showed mild histological changes. Of all patients with elevated ALT, 49% achieved normal ALT levels after lifestyle intervention. Those with more severe histological changes tended to have continuously increasing ALT levels. There was no correlation between the normalization of posttreatment ALT level and BMI, as well as ultrasonographic findings at diagnosis. Conclusion: ALT elevation in NAFLD is highly associated with higher BMI scores and more severe degree of fatty changes on ultrasonographic examination. Lifestyle intervention can significantly improve ALT in children with NAFLD. The degree of histologic changes appears to be a predictor of the treatment response to NAFLD.

Hashimoto's encephalopathy (HE) is a rare and underdiagnosed neuropsychiatric illness. We present the case of a 17-year-old girl who was admitted to a tertiary-care psychiatric center with acute onset psychosis and fever. Her psychotic symptoms were characterized by persecutory and referential delusions, as well as tactile and visual hallucinations. Her acute behavioral disturbance warranted admission and treatment in a psychiatric setting (risperidone tablets, 3 mg/day). She had experienced an episode of fever with a unilateral visual acuity defect approximately 3 years before admission, which was resolved with treatment. Focused clinical examination revealed an enlarged thyroid, and baseline blood investigations, including thyroid function test results were normal. Abnormal laboratory investigations revealed elevated anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-TG) levels (anti-TPO of 480 IU/mL; anti-TG of 287 IU/mL). Results of other investigations for infection, including cerebrospinal fluid examination, electroencephalography, and brain magnetic resonance imaging were normal. She was diagnosed with HE and was treated with intravenous corticosteroids (methylprednisolone up to 1 g/day; tapered and discontinued after a month). The patient achieved complete remission of psychotic symptoms and normalization of the anti-thyroid antibody titers. Currently, at the seventh month of follow-up, the patient is doing well. This case highlights the fact that in the absence of well-defined clinical diagnostic criteria, a high index of suspicion is required for early diagnosis of HE. Psychiatrists need to explore for organic etiologies when dealing with acute psychiatric symptoms in a younger age group.

Accidents caused by black ice occur frequently every winter, and the fatality rate is very high compared to other traffic accidents. Therefore, a systematic method is needed to predict the black ice formation before accidents. In this paper, we proposed a black ice prediction model based on heterogenous and multi-type data. To this end, 12,574,630 cases of 46 types of land, infrastructure, transport public data and meteorological public data were collected. Subsequently, the data cleansing process including missing value detection and normalization was followed by the establishment of approximately 600,000 refined datasets. We analyzed the correlation of 42 factors collected to predict the occurrence of black ice by selecting only 21 factors that have a valid effect on black ice prediction. The prediction model developed through this will eventually be used to derive the route-specific black ice risk index, which will be utilized as a preliminary study for black ice warning alart services.
A number of different methods have been used for modeling the sorption of volatile organic chlorinated compounds such as tetrachloroethylene/perchloroethylene (PCE). In this study, PCE was adsorbed in several natural sorbents, i.e., Pahokee peat, vermicompost, BionSoil®, and natural soil, in the batch experiments. Several sorption models such as linear, Freundlich, solubility-normalized Freundlich model, and Polanyi-Manes model (PMM) were used to analyze sorption isotherms. The relationship between sorption model parameters, organic carbon content (foc), and elemental C/N ratio was studied. The organic carbon normalized partition coefficient values (log Koc = 1.50-3.13) in four different sorbents were less than the logarithm of the octanol-water partition coefficient (log Kow = 3.40) of PCE due to high organic carbon contents. The log Koc decreased linearly with log foc and log C/N ratio, but increased linearly with log O/C, log H/C, and log (N+O)/C ratio. Both log KF,oc or log KF,oc decreased linearly with log foc (R2 = 0.88-0.92) and log C/N ratio (R2 = 0.57-0.76), but increased linearly with log (N+O)/C (R2 = 0.93-0.95). The log qmax,oc decreased linearly as log foc and log C/N increased, whereas it increased with log O/C, log H/C and log (N+O)/C ratios. The log qmax,oc increased linearly with (N+O)/C indicating a strong dependence of qmax,oc on the polarity index. The results showed that PCE sorption behaviors were strongly correlated with the physicochemical properties of soil organic matter (SOM).

Purpose : The dose distribution of organ at risk (OAR) and normal tissue is affected by treatment technique in postoperative radiation therapy for prostate cancer. The aim of this study was to compare dose distribution characteristic and to evaluate treatment efficiency by devising VMAT plans according to applying differed number of arc and IMRT plan for postoperative patient of prostate cancer radiation therapy using a rectal balloon. Materials and Methods : Ten patients who received postoperative prostate radiation therapy in our hospital were compared. CT images of patients who inserted rectal balloon were acquired with 3 mm thickness and 10 MV energy of HD120MLC equipped Truebeam STx (Varian, Palo Alto, USA) was applied by using Eclipse (Version 11.0, Varian, Palo Alto, USA). 1 Arc, 2 Arc VMAT plans and 7-field IMRT plan were devised for each patient and same values were applied for dose volume constraint and plan normalization. To evaluate these plans, PTV coverage, conformity index (CI) and homogeneity index (HI) were compared and $R_{50%}$ was calculated to assess low dose spillage as per treatment plan. $D_{25%}$ of rectum and bladder Dmean were compared on OAR. And to evaluate the treatment efficiency, total monitor units(MU) and delivery time were considered. Each assessed result was analyzed by average value of 10 patients. Additionally, portal dosimetry was carried out for accuracy verification of beam delivery. Results : There was no significant difference on PTV coverage and HI among 3 plans. Especially CI and $R_{50%}$ on 7F-IMRT were the highest as 1.230, 3.991 respectively(p=0.00). Rectum $D_{25%}$ was similar between 1A-VMAT and 2A-VMAT. But approximately 7% higher value was observed on 7F-IMRT compare to the others(p=0.02) and bladder Dmean were similar among the all plan(P>0.05). Total MU were 494.7, 479.7, 757.9 respectively(P=0.00) for 1A-VMAT, 2A-VMAT, 7F-IMRT and at the most on 7F-IMRT. The delivery time were 65.2sec, 133.1sec, 145.5sec respectively(p=0.00). The obvious shortest time was observed on 1A-VMAT. All plans indicated over 99.5%(p=0.00) of gamma pass rate (2 mm, 2%) in portal dosimetry quality assurance. Conclusion : As a result of study, postoperative prostate cancer radiation therapy for patient using a rectal balloon, there was no significant difference of PTV coverage but 1A-VMAT and 2A-VMAT were more efficient for dose reduction of normal tissue and OARs. Between VMAT plans. $R_{50%}$ and MU were little lower in 2A-VMAT but 1A-VMAT has the shortest delivery time. So it is regarded to be an effective plan and it can reduce intra-fractional motion of patient also.

An information retrieval system has to retrieve all and only documents which are relevant to a user query, even if index terms and query terms are not matched exactly. However, term mismatches between index terms and qucry terms have been a serious obstacle to the enhancement of retrieval performance. In this paper, we discuss automatic term normalization between words in text corpora and their application to a Korean information retrieval system. We perform two types of term normalizations to alleviate semantic term mismatches: equivalence class and co-occurrence cluster. First, transliterations, spelling errors, and synonyms are normalized into equivalence classes bv using contextual similarity. Second, context-based terms are normalized by using a combination of mutual information and word context to establish word similarities. Next, unsupervised clustering is done by using K-means algorithm and co-occurrence clusters are identified. In this paper, these normalized term products are used in the query expansion to alleviate semantic tem1 mismatches. In other words, we utilize two kinds of tcrm normalizations, equivalence class and co-occurrence cluster, to expand user's queries with new tcrms, in an attempt to make user's queries more comprehensive (adding transliterations) or more specific (adding spc'Cializationsl. For query expansion, we employ two complementary methods: term suggestion and term relevance feedback. The experimental results show that our proposed system can alleviatl' semantic term mismatches and can also provide the appropriate similarity measurements. As a result, we know that our system can improve the rctrieval efficiency of the information retrieval system.
Network reliability means network ability which perform normal operation of network components within defined time period, on the other hand, network survivability is the index of normal operation in nerwork failure. In occurrence of network failure, network operator is interested in the measurement of network survivability, but, in a view of subscribers, it is more interested in how many subscribers are out of service and how long the out of service state lasts. In this paper, a service outage evaluation model fitted for telecommunication network in Korea is proposed, which measures the impacts of subscribers in case of network failure which induces a service outage. The outage evaluation result of "Mobile to Land" service outage which has 30.000 subscribers affected for 30 minutes is 2.5. This value is 5 times larger than that of the same service outage which has 10,000 subscribers affected for 30 minutes, i.e, the impact of subscribers in the former case is 5 times larger than that of the latter case. If 30,000 subscribers have isolated in "Mobile to Land," "Local Mobile to Mobile" service for 30 minutes by the exchange out of order in PCS network, then the evaluation result of this service outage will be 3.75.
