정신분열증에서 감각운동피질의 기능적 비대칭성의 장애: 기능적 자기공명영상을 이용한 연구 (Disturbed Functional Asymmetry of Sensorimotor Cortex in Schizophrenia: A Study with Functional Magnetic Resonance Imaging)

  • 목적: 기능적 자기공명영상을 이용하여 정신분열증 환자에서 운동자극에 의한 뇌 피질의 반응양상을 정상인과 비교하여 알아보고자 하였다. 대상 및 방법: 9 명의 오른손잡이 정신분열증 환자와 정신과학적으로 이상이 없는 6 명의 건강한 오른손잡이 자원자를 대조군으로 하였다. EPI 기법을 이용하여 한 절편 당 120 개의 연속영상 중 3 번의 휴식기와 활성화 시기를 반복하여 전체 4개의 절편에서 총 480개의 영상들을 얻었으며 오른손 운동을 운동자극으로 사용하였다. 영상 후 처리는 교차상관(cross correlation)을 이용하였고 운동자극 시에 우세 대뇌반구를 결정하기 위해 편측화 지수(lateralization index)를 측정하여 환자와 정상인 사이에서 비교하였다. 결과: 정상군에서는 오른손 운동자극시 좌측 감각운동중추에서 우측에 비해 월등히 많은 활성화를 나타내어 높은 펀측화 지수를 보인 반면 정신분열증 환자군의 경우는 좌측 감각운동중추의 활성화는 상대적으로 감소하고 동측인 우측의 활성화가 증가하여 정상군에 비해 낮은 편측화 지수를 나타냈다. 한 명의 환자에서는 우측 감각운동중추가 좌측에 비해 높은 활성화를 보이는 경우도 있었다(역편측화 reversed lateralization) 결론: 정상인에서 관찰되는 좌-우 감각운동피질의 기능적 비대칭성이 정신분열증환자에서는 다른 양상으로 관찰되었다. 이는 이 질환에서 뇌 피질내 운동회로의 기능적 이상이 있음을 반영하는 소견이며, 앞으로 기능적 자기공명영상이 이 질환의 진단과 연구에 많은 도움을 줄 것으로 생각한다.

Auto-parametric resonance of framed structures under periodic excitations

  • A framed structure may be composed of two sub-structures, which are linked by a hinged joint. One sub-structure is the primary system and the other is the secondary system. The primary system, which is subjected to the periodic external load, can give rise to an auto-parametric resonance of the second system. Considering the geometric-stiffness effect produced by the axially internal force, the element equation of motion is derived by the extended Hamilton's principle. The element equations are then assembled into the global non-homogeneous Mathieu-Hill equations. The Newmark's method is introduced to solve the time-history responses of the non-homogeneous Mathieu-Hill equations. The energy-growth exponent/coefficient (EGE/EGC) and a finite-time Lyapunov exponent (FLE) are proposed for determining the auto-parametric instability boundaries of the structural system. The auto-parametric instabilities are numerically analyzed for the two frames. The influence of relative stiffness between the primary and secondary systems on the auto-parametric instability boundaries is investigated. A phenomenon of the "auto-parametric internal resonance" (the auto-parametric resonance of the second system induced by a normal resonance of the primary system) is predicted through the two numerical examples. The risk of auto-parametric internal resonance is emphasized. An auto-parametric resonance experiment of a ${\Gamma}$-shaped frame is conducted for verifying the theoretical predictions and present calculation method.

자기 공명 영상을 이용한 악관절 기능 장애에 관한 연구 (A STUDY ON TEMPOROMANDIBULAR JOINT DYSFUNCTION USING MAGNETIC RESONANCE IMAGING)

  • The temporomandibular joint was evaluated using magnetic resonance imaging using a urface coil in 11 patients having reciprocal clicking or locking and compared with the normal joint in five subjects. Serial multisection 3㎜-thick parasagittal, paracoronal, and axial image on both closing and opening mouth were obtained with a 1.5 Tesla MR system and surface coil using CSMEMP, GRASS, MPGR, powerful extensions of fast imaging that is currently under clinical evaluation. MR images obtained were analized correlating with the theory of internal derangement. The obtained results were as follows: 1. The serial findings of structures in joint were determined on the serially sectioned images of joint with reciprocal clicking or locking by CSMEMP and MPGR on closing mouth. 2. The delta shaped white images of synovial fluid in the glenoid fossa and on the posterior surface of condyle were revealed on the parasagittal images by MPGR on opening mouth as in the normal joints. 3. The white image of joint fluid surrounding meniscus was recognized on the paracoronal image by GRASS on opening mouth as in the normal joints. 4. In joints having temporomandibular dysfunction the smooth image of displaced meniscus was recognized, but otherwise in the normal joints the image of muscle was noted on the paracoronal image sectioned at the anterior portion of condyle by GRASS. 5. The more thickened fascial plane between superior and inferior belly of lateral pterygoid muscle was not recognizable in joints having temporomandibular dysfunction than in the normal joints.

Comparison of Metabolic Profiles of Normal and Cancer Cells in Response to Cytotoxic Agents

  • Together with radiotherapy, chemotherapy using cytotoxic agents is one of the most common therapies in cancer. Metabolic changes in cancer cells are drawing much attention recently, but the metabolic alterations by anticancer agents have not been much studied. Here, we investigated the effects of commonly used cytotoxic agents on lung normal cell MRC5 and lung cancer cell A549. We employed cis-plastin, doxorubicin, and 5-Fluorouracil and compared their effects on the viability and metabolism of the normal and cancer cell lines. We first established the concentration of the cytotoxic reagents that give differences in the viabilities of normal and cancer cell lines. In those conditions, the viability of A549 decreased significantly, whereas that of MRC5 remained unchanged. To study the metabolic alterations implicated in the viability differences, we obtained the metabolic profiles using $^1H$-NMR spectrometry. The $^1H$-NMR data showed that the metabolic changes of A549 cells are more remarkable than that of MRC5 cells and the effect of 5-FU on the A549 cells is the most distinct compared to other treatments. Heat map analysis showed that metabolic alterations under treatment of cytotoxic agents are totally different between normal and cancer cells. Multivariate analysis and weighted correlation network analysis (WGCNA) revealed a distinctive metabolite signature and hub metabolites. Two different analysis tools revealed that the changes of cell metabolism in response to cytotoxic agents were highly correlated with the Warburg effect and Reductive lipogenesis, two pathways having important effects on the cell survival. Taken together, our study addressed the correlation between the viability and metabolic profiles of MRC5 and A549 cells upon the treatment of cytotoxic anticancer agents.

Tumor-like Presentation of Tubercular Brain Abscess: Case Report

  • A 17-year-old girl presented with complaints of headache and decreasing vision of one month's duration, without any history of fever, weight loss, or any evidence of an immuno-compromised state. Her neurological examination was normal, except for papilledema. Laboratory investigations were within normal limits, except for a slightly increased Erythrocyte Sedimentation Rate (ESR). Non-contrast computerized tomography of her head revealed complex mass in left frontal lobe with a concentric, slightly hyperdense, thickened wall, and moderate perilesional edema with mass effect. Differential diagnoses considered in this case were pilocytic astrocytoma, metastasis and abscess. Magnetic resonance imaging (MRI) obtained in 3.0 Tesla (3.0T) scanner revealed a lobulated outline cystic mass in the left frontal lobe with two concentric layers of T2 hypointense wall, with T2 hyperintensity between the concentric ring. Moderate perilesional edema and mass effect were seen. Post gadolinium study showed a markedly enhancing irregular wall with some enhancing nodular solid component. No restricted diffusion was seen in this mass in diffusion weighted imaging (DWI). Magnetic resonance spectroscopy (MRS) showed increased lactate and lipid peaks in the central part of this mass, although some areas at the wall and perilesional T2 hyperintensity showed an increased choline peak without significant decrease in N-acetylaspartate (NAA) level. Arterial spin labelling (ASL) and dynamic susceptibility contrast (DSC) enhanced perfusion study showed decrease in relative cerebral blood volume at this region. These features in MRI were suggestive of brain abscess. The patient underwent craniotomy with excision of a grayish nodular lesion. Abundant acid fast bacilli (AFB) in acid fast staining, and epithelioid cell granulomas, caseation necrosis and Langhans giant cells in histopathology, were conclusive of tubercular abscess. Tubercular brain abscess is a rare manifestation that simulates malignancy and cause diagnostic dilemma. MRI along with MRS and magnetic resonance perfusion studies, are powerful tools to differentiate lesions in such equivocal cases.

  • A helmholtz resonator has been widely used for the purpose of suppressing low frequency noises propagated from various heat and fluid machineries. However, the resonator has demerits that the absorption bandwidth at resonance frequency is very small and a large cavity is necessary. In order to overcome these problems, in this paper, a resonator with perforated panels at the neck and/or in the cavity is proposed. The absorption performances of resonators are measured by two-microphone method and are estimated by transfer matrix method. The experimentally measured values of normal absorption coefficients are agreed well with the corresponding values from the transfer matrix method. By introducing perforated panels at the neck of a resonator, it is shown that the absorption performances and bandwidth have a significant improvement.

Scaling methods for wind tunnel modelling of building internal pressures induced through openings

  • Appropriate scaling methods for wind tunnel modelling of building internal pressures induced through a dominant opening were investigated. In particular, model cavity volume distortion and geometric scaling of the opening details were studied. It was found that while model volume distortion may be used to scale down buildings for wind tunnel studies on internal pressure, the implementation of the added volume must be done with care so as not to create two cavity resonance systems. Incorrect scaling of opening details was also found to generate incorrect internal pressure characteristics. Furthermore, the effective air slug or jet was found to be longer when the opening was near a floor or sidewall as evidenced by somewhat lower Helmholtz frequencies. It is also shown that tangential flow excitation of Helmholtz resonance for off-centre openings in normal flow is also possible.

  • The helmholtz resonator with the perforated neck has demerits that the absorption performance is not so outstanding in an anti-resonance frequency and high frequency bandwidth. In order to overcome these problems, in the paper, a resonator combined with porous material is proposed. The absorption performances of resonators are measured by two-microphone method and estimated by transfer matrix method. The experimentally measured values of normal absorption coefficients agree well with the corresponding values from the transfer matrix method. Because of the porous material, it is shown that the absorption performance have been significantly improved in the anti-resonance frequency and high frequency bandwidth.

  • Test results of friction-factor for the flow of air in a narrow channel lined with various honeycomb geometries show that, generally, the friction-factor is nearly constant or slightly decreases as the Reynolds number(or Mach number) increases, a characteristic common to turbulent flow in pipes. However, in some test geometries this trend is remarkably different. The friction factor dramatically drops and then rises as the Mach number increases. This phenomenon can be characterized as a "friction-factor jump." Further investigations of the acoustic spectrum indicate that the "friction-factor jump" phenomenon is accompanied by an onset of a normal mode resonance excited coherent flow fluctuation structure, which occurs at Reynolds number of the order of $10^4$. New empirical friction-factor model for "friction-factor jump" cases is developed as a function of Mach number and local pressure.ach number and local pressure.

  • The purpose of this paper is to show the radiologic features of various lesions appearing as skin thickening or enhancement under the breast MRI. And histopathologic results of the skin lesions were correlated. Radiologist must be familiar with normal appearance of the breast skin under the MRI and a wide variety of conditions may affect the skin of the breast.
