관상동맥 우회로 수술 환자에서 심근의 탄성도 변화 (Improvement in Regional Contractility of Myocardium after CABG)

  • 목적: 심근의 최대탄성도는 전부하 후부하에 독립적인 지표로서 SPECT를 이용하여 좌심실의 국소탄성도($rE_{max}$)를 비침습적으로 측정하였다. 게이트 심근 SPECT 영상에서 국소부피변화를 얻고, 요골동맥 긴장도를 측정하여 중심동맥의 압력 곡선을 얻어 측정한 최대탄성도를 관상동맥우회로 수술 전후 환자를 대상으로 수술전후 관류 및 기능지표와 비교하여보았다. 대상 및 방법: 관상동맥우회로 수술이 결정되어 시행한 환자 21명(남:여=17:4, $58{\pm}12$세)을 대상으로 $^{201}TI$ 휴식기 영상과 디피리다몰 부하 $^{99m}Tc$-sestamibi 게이트 SPECT를 수술전과 수술 후 3개월째 시행하였다. 동시에 요골동맥으로부터 압력곡선을 얻었다. 기능과의 관계를 보기 위해서 관류와 심벽두꺼워짐을 탄성도와 비교하였으며, 심벽두꺼워짐이 20%미만일 때 수술 후 10%이상 호전되지 않는 기능이상 분절에 대해 수술 전후의 탄성도를 비교하였다. 결과: 수술 전 탄성도가 $2.41{\pm}1.64$ mmHg/mL에서 $2.78{\pm}1.83$ mmHg/mL 으로 수술 후에 증가하였으나, 관류와 심벽두꺼워짐과는 낮은 상관성을 보였다. (r=0.35, p<0.001). 관류 60% 이상의 분절에서는 $2.65{\pm}1.67$ mmHg/mL 이었으나, 관류나 낮은 분절의 탄성도는 $1.30{\pm}1.24$ mmHg/mL 였다. 심벽두꺼워짐이 40% 이상 되는 분절의 수술 전 탄성도는 $3.01{\pm}1.92$ mmHg/mL 였고, 기능이 조금 약한 부분인 40%에서 20% 사이의 심벽두꺼워짐 값을 가진 분절에서는 $2.40{\pm}1.19$ mmHg/mL로 심각한 기능이상을 반영하는 20%미만 분절의 탄성도는 $1.13{\pm}0.89$ mmHg/mL의 분포를 보였다. 수술 전 심벽두꺼워짐이 20% 미만일 때 수술 후 10%이상 회복을 보인 생존심근와 그렇지 않은 비생존심근사이의 수술 전후 탄성도는 $1.27{\pm}1.07$ mmHg/mL에서 $1.79{\pm}1.48$ mmHg/mL, $0.97{\pm}0.59$ mmHg/mL에서 $1.22{\pm}0.71$ mmHg/mL로 생존심근의 수술 후 값의 향상이 조금 더 높았다. 그러나, 심벽두꺼워짐의 정도가 탄성도 높아짐의 정도 사이에는 상관성이 없었다(r=0.007). 결론: 수술 전 $rE_{max}$는 관류와 심벽두꺼워짐과 상관성이 약하게 있었다. 기능이상이면서 생존능이 있는 심근의 탄성도는 수술 후 증가하였지만 심벽두꺼워짐의 향상과는 상관성이 없었다. 심근기능의 전부하 후부하에 독립적인 지표인 탄성도는 실제 부피의 증가와 연관되지 않으면서도 생존능이 있는 심근의 기준과 일치하는 양상을 보였으므로 독립적인 매개변수로 사용될 수 있을 것으로 기대한다.

비소세포폐암의 림프절 병기 결정에서 Coincidence PET의 역할 (The Role of Camera-Based Coincidence Positron Emission Tomography in Nodal Staging of Non-Small Cell Lung Cancer)

  • 연구배경: 비소세포폐암의 림프절의 병기 결정은 치료 및 예후 결정에 매우 중요하나 정확한 병기 결정에는 많은 어려움이 있다. 야에 저자들은 최근에 개발된 CoDe PET의 림프절 병기 결정에 대한 유용성을 알아보고자 하였다. 방 법: 1998년 2월부터 1999년 6월까지 아주대학교병원에 내원하여 원발성 비소세포폐암으로 진단되어 치료를 시행하였던 환자 21예를 대상으로 하였다. 수술적 술전 폐암의 병기 결정을 위하여 흉부 전산화단층촬영, 복부 초음파검사, 전신 골주사 촬영, CoDe PET를 시행하였다. 대상 환자들의 수술후 병리학적 병기를 기준으로 하여서 CoDe PET로 결정된 림프절 병기의 민감도, 특이도, 양성 예측도, 음성 예측도, 정확도를 분석하였다. 결 과: 21예의 비소세포폐암중 20예에서 원발 종괴에 $^{18}FDG$가 섭취되었으며 위음성이 1예가 있었다. 림프절 병기에 있어 전체 21예중 13예에서는 병리학적 병기와 일치하였다. 병리학적 N0는 14예이었으며 CoDe PET의 N0에 대한 특이도는 64.3%이었다. N1 림프절에 대한 민감도는 83.3%, 특이도는 73.3%, 양성 예측도는 55.6%, 음성 예측도는 91.7%, 정확도는 76.2%이였다. N2 림프절에 대한 민감도는 60.0%, 특이도는 87.5%, 양성 예측도는 60.0%, 음성 예측도는 87.5%, 정확도는 90.0%이었다. 병리학적으로 그 크기가 측정된 림프절 29개중 암세포의 침윤이 있었던 림프절은 6개이었으며 이중 3개의 림프절은 CoDe PET에 국소 섭취로 나타났다. 위음성으로 나타난 3개의 림프절은 그 크기가 모두 1cm 이하였으며, 양성으로 나타난 림프절의 크기는 각각 1.1cm, 1.0cm, 0.5cm이었다. 크기가 1cm 이상이나 암세포의 침윤이 없었던 12개의 림프절중 1예에서만 위양성으로 나타났다. 5예에서는 위양성 소견을 보였는데 이중 1예는 폐결핵에 의한 육이종을 관찰할 수 있었으며, 1예는 pneumoconiosis, 1예는 anthracosis이었다. 결 론: CoDe PET는 크기가 1.5cm 이상의 림프절에 대한 음성 예측도가 비교적 높기 때문에 N3 림프절의 확대가 있는 비소세포폐암에서 CoDe PET를 시행하여 국소 섭취가 없는 경우에는 수술적 치료를 고려해 볼 수 있으리라 생각된다. 또한 CoDe PET는 아직 개발 단계에 있어서 향후 보다 더 발전된 영상을 얻을 수 있을 것으로 기대된다.

심질환 병력이 없었던 중환자에서 B-type Natriuretic Peptide 검사의 유용성 (The Usefulness of B-type Natriuretic Peptide test in Critically Ill, Noncardiac Patients)

  • 연구배경 : B-type natriuretic peptide(BNP)는 주로 심장의 심실에서 분비되는 호르몬으로서 심부전의 진단, 중증도 및 예후와도 연관이 있다고 알려져 있다. 중환자의 경우 급성병색시 심부전이 올수 있는 것으로 알려져있고 이는 환자의 예후와 연관이 있을 가능성이 있다. 그래서 저자들은 우선 이전 심질환의 병력이 없는 중환자들에서 BNP를 측정하여 증가여부를 살펴보고, BNP와 중환자의 중등도 및 예후와의 관련성 여부를 살펴보기로 하였다. 방 법 : 2002년 6월부터 10월까지 본원 중환자실에 입원하였던 환자 중 환자의 중증도와는 관련이 없이 BNP가 증가될 수 있는 울혈성 심부전, 심방 세동, 허혈성 심질환, 신부전 등을 제외한 32명을 대상으로 하였고, 대조군으로는 일반 병동 및 외래 환자 32명을 무작위로 추출하였다. BNP는 Triage B-Type Natriuretic Peptide test를 이용하여 fluorescence immunoassay를 통해 측정하였다. APACHE II score와 APACHE III score 및 중환자실 사망 여부를 추적하였다. 결 과 : 남녀비는 16 : 16이었고, 연령은 $59.2{\pm}20.6$세 였다. BNP는 $186.7{\pm}274.1$ pg/mL으로 정상 대조군의 $19.9{\pm}21.3$ pg/mL 보다 유의하게 높았다. 중환자군에서 BNP 100 pg/mL이상은 14명으로 43.8%였다. APACHE II score는 $16.5{\pm}7.6$이었고, 11명이 사망하였다. BNP값은 APACHE II score및 사망과 유의한 상관 관계를 보였고(r=0.443, p=0.011, r=0.530, p=0.002), 생존군과 사망군 BNP값은 유의한 차이를 보였으나($83.2{\pm}55.8$ pg/mL vs. $384.1{\pm}401.7$ pg/mL ; p=0.033), $PaO_2/FiO_2$와는 상관 관계를 보이지 않았다. 결 론 : BNP는 이전 심질환병력이 없었던 중환자에서 증가되어 있었고, 사망률 및 중증도 점수와도 유의한 상관 관계가 있었다. 따라서 중환자들의 예후를 예측하는데 비침습적이고 빠른 방법으로 도움이 될 가능성이 있다.

류마티스 관절염환자에서 폐침범의 진단에 있어서 면역학적 지표와 폐기능 검사의 유용성 (The Utility of Immunological Markers and Pulmonary Function Test in the Early Diagnosis of Pulmonary Involvement in the Patients with Rheumatoid Arthritis)

  • 연구배경: 류마티스 관절염 환자에서 폐침범의 빈도는 10~50%로 보고되어 있고 사망의 주요원인으로 알려져 있다. 따라서 최근에 면역학 및 방사선학의 발달과 더불어 폐침범의 조기진단에 대한 관심이 증가되고 있다. 방법: 1990년 3월부터 1995년 7월까지 본원에 입원한 환자중 임상소견과 흉부 고해상도 컴퓨터 단층 촬영상 폐침범 소견을 보였던 29명의 환자와 임상적 소견 및 검사소견상 폐침범이 없었던 연령 범위가 같은 18명의 환자에서 면역학적 지표와 폐기능 검사소견을 비교하여 비침습적 검사를 이용한 폐침범의 조기 진단에 유용한 지료플 찾고자 하였다. 결과: 1) 남녀의 비는 폐침범군의 경우 1 : 1.07이었고 대조군의 경우 18명 모두 여자이었으며 그중 흡연력은 폐침범군에서 31%(9/29)였으며 대조군은 없었다. 2) 류마티스 인자는 폐침범군의 경우 96.5%(28/29)에서 양성이였으며 대조군의 경우 100%(18/18)에서 양성이었다(p=0.12). 3) 항핵주변인자는 폐침범군의 경우 86.9%(20/23, 평균치: 2.0)에서 양성이었고 대조군의 경우 50%(8/16, 평균치: 1.1)에서 양성으로 유의한 차이가 있었다.(p=0.04). 4) 항핵항체는 폐침범군의 경우 60.7%(17/28)에서 양성이었고 대조군의 경우 72.2%(13/18)에서 양성이었다.(p=0.33). 5) Cryoglobulin은 폐침범군의 경우 72%(18/25)에서 양성이었고 대조군의 경우 56.2%(9/16)에서 양성으로 유의한 차이가 있었다(p=0.02). 6) 골 미란은 폐침범군의 경우 61.5%(16/26)에서 양성이었으며 대조군의 경우 77.7%(14/18)에서 양성이었다(p=0.8). 7) 폐기능 검사상 폐포량으로 교정된 폐확산능치와 페잔기량은 폐침범군의 경우평균값이 각각 1.07mmol/min/KPa(predicted value: 64.2%)와 1.32L(70%)이었고 대조군은 1.44mmol/min/KPa(78.1%)와 3.75L(86.6%)으로 유의한 차이가 있었다(p=0.003, p=0.004). 총폐용적과 최대자발성호흡량은 폐침범군의 평균값이 각각 3.98L(102.9%)와 70.63L/min(101.6%)이었고 대조군은 4.0L(87.4%)와 74.59L/min(100.9%)이었디(p=0.51, p=0.42). 결론: 결론적으로 류마티스 관절염 환자에서 폐침범을 진단하는데 있어서 혈청학적 검사중 항핵주변인자와 cryoglobulin, 폐기능 검사상 폐잔기량과 폐확산능의 측정은 유효한 지표로 사용될 수 있겠다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
