Noninvasive markers for esophageal varices in children with cirrhosis

  • Background: The diagnosis of esophageal varices (EV) is based on the findings of esophagogastroduodenoscopy (EGD), biopsy, and serum markers. Thus, noninvasive cost-effective tests through which high-risk EV children can be diagnosed are needed. Purpose: This cross-sectional study aimed to identify the noninvasive markers for EV in children with liver cirrhosis. Methods: A total of 98 children with liver cirrhosis were evaluated in this study. The spleen size, platelet count, serum albumin, liver function test results, and risk scores were evaluated prior to endoscopy. The endoscopic investigations aimed to identify the presence of EV and red signs, and determine varices sizes. Results: Endoscopy revealed varices in 43 subjects (43.9%). The spleen size, platelet count, international normalized ratio, aspartate aminotransferase to platelet ratio index (APRI), platelet count to spleen size ratio, and risk score differed significantly between patients with and without EV on univariate analysis; however, the logistic regression analysis showed no differences, indicating that none of these parameters were independently associated with the presence of EV. Conclusion: Platelet count, risk score, platelet count to spleen size, and APRI can be useful tools for the identification of high-risk patients with EV and might reduce the need for invasive methods like EGD.

  • Gastric cancer (GC) is associated with high morbidity and mortality rates. Thus, early diagnosis is important to improve disease prognosis. Endoscopic assessment represents the most reliable imaging method for GC diagnosis; however, it is semi-invasive and costly and heavily depends on the skills of the endoscopist, which limit its clinical applicability. Therefore, the search for new sensitive biomarkers for the early detection of GC using noninvasive sampling collection methods has attracted much attention among scientists. Urine is considered an ideal biofluid, as it is readily accessible, less complex, and relatively stable than plasma and serum. Over the years, substantial progress has been made in screening for potential urinary biomarkers for GC. This review explores the possible applications and limitations of urinary biomarkers in GC detection and diagnosis.

Emerging Role of Hepatobiliary Magnetic Resonance Contrast Media and Contrast-Enhanced Ultrasound for Noninvasive Diagnosis of Hepatocellular Carcinoma: Emphasis on Recent Updates in Major Guidelines

  • Hepatocellular carcinoma (HCC) can be noninvasively diagnosed on the basis of its characteristic imaging findings of arterial phase enhancement and portal/delayed "washout" on computed tomography (CT) and magnetic resonance imaging (MRI) in cirrhotic patients. However, different specific diagnostic criteria have been proposed by several countries and major academic societies. In 2018, major guideline updates were proposed by the Association for the Study of Liver Diseases, European Association for the Study of the Liver (EASL), Korean Liver Cancer Association and National Cancer Center (KLCA-NCC) of Korea. In addition to dynamic CT and MRI using extracellular contrast media, these new guidelines now include magnetic resonance imaging (MRI) using hepatobiliary contrast media as the first-line diagnostic test, while the KLCA-NCC and EASL guidelines also include contrast-enhanced ultrasound (CEUS) as the second-line diagnostic test. Therefore, hepatobiliary MR contrast media and CEUS will be increasingly used for the noninvasive diagnosis and staging of HCC. In this review, we discuss the emerging role of hepatobiliary phase MRI and CEUS for the diagnosis of HCC and also review the changes in the HCC diagnostic criteria in major guidelines, including the KLCA-NCC practice guidelines version 2018. In addition, we aimed to pay particular attention to some remaining issues in the noninvasive diagnosis of HCC.

Noninvasive Depthwise Temperature Measurement in Skin Tissue Using Laser Speckle Imaging Technique (레이저 스펙클 이미징 기법을 이용한 피부 조직의 깊이 방향 비침습적 온도 측정)

  • Accurate tissue temperature monitoring during clinical procedures, such as laser therapy or surgery, is crucial for ensuring patient safety and treatment efficacy. Noninvasive techniques are essential to prevent tissue disturbance while providing real-time temperature data. However, current methods often struggle to accurately measure temperature at various depths within the skin, which is essential to avoid damage to surrounding healthy tissues due to excessive heat. In response to this challenge, we developed a confocal imaging system that utilizes the laser speckle imaging (LSI) technique for precise depthwise temperature monitoring. LSI uses laser light scattering to capture subtle changes in speckle patterns on the skin's surface due to temperature fluctuations within the tissue. By analyzing these changes, LSI enables accurate depth-resolved temperature measurements. This technique enhances the precision and safety of medical procedures, offering significant potential for broader clinical applications, improved patient outcomes, and better thermal management during interventions.

A Study of Urinary Ascorbic Acid as a Noninvasive Biomarker for Cadmium-Intoxication in Rats (카드뮴 중독 흰쥐에 대한 Noninvasive Biomarker로서의 요중 Ascorbic Acid에 관한 연구)

  • This study was planned to evaluate the urinary ascorbic acid as a new biological marker for the intoxication of cadmium, which could possibly be driven by its increased utilization and environmental pollution. In order to meet this goal, we have peformed measurement of urinary ascorbic acid concentration, histopathological examination of the kidney, and biochemical test for the liver function using cadmium-intoxicated rats by oral administration. The average concentrations of urinary ascorbic acid in the $CdCl_2$-treated rats were 214.0 mg/dl for 100 ppm group and 254.3 mg/dl for 200 ppm group during experimental period of 50 days. These levels are 24 and 28 times higher than one in the control group (9.0 mg/dl), respectively. Ultrastructural study showed the eosinophilic hyaline cast and focal effacement, fusion in the renal tubules, as well as loss of foot processes on the glomerular epithelial cells. These results suggested that cadmium may be responsible for renal glomerular injury. The blood levels of AST, ALT and LDH in the treated groups (199 IU/I, 88 IU/I, 1190 U/I for the 100 ppm group and 270 IU/I, 226 IU/I, 760 U/I for the 200 ppm group) were higher than ones in the control group(143 IU/I, 50 IU/I, 334 U/I). These results indicated the cadmium induced the damage of liver function. In conclusion, the administration of cadmium showed a remarkable increase of urinary ascorbic acid with renal and hepatic damage. Therefore, it is expected that measurement of urinary ascorbic acid would be an powerful method as a noninvasive biomarker for cadmium intoxication.

Effects of noninvasive electrical stimulation on osseointegration of endosseous implants;A histomorphometric evaluation in the rabbit tibia (비침습적인 전기자극이 임프란트의 골융합에 미치는 영향;토끼경골에서의 조직계측학적 평가)

  • The procedure that enhances osteogenesis and shortens the healing period is required for successful implant therapy. It has been introduced that osteogenesis is enhanced by the generation of electric field. Many researchers have demonstrated that application of electric and electromagnetic field promote bone formation. It also has been shown that electrical stimulation enhances peri-implant bone formation. Recently, several investigators have reported that noninvasive electrical stimulation using negatively charged electret such as polytetrafluoroethylene(PTFE) promotes osteogenesis. Therefore, we were interested in the effect of noninvasive electrical stimulation using negatively charged electret on the periimplant bone healing. After titanium implant were installed in the proximal tibial metaphysis of New Zealand white rabbit, negatively charged PTFE membrane fabricated by corana dischage was inserted into the inner hole of the experimental implant and noncharged membrane was applied into control implant. After 4 weeks of healing, histomorphometric analysis was performed to evaluate peri-implant bone response. The histomorphometric evaluations demonstrated experimental implant tended to have higher values in the total bone-to-implant contact ratio(experimental ; $49.9{\pm}13.52%$ vs control ; $37.5{\pm}19.44%$) , the marrow bone contact ratio(experimental ; $34.94{\pm}13.32%$ vs control ; $24.15{\pm}13.69%$), amount of newly formed bone in the endosteal region(experimental ; $1.00{\pm}0.30mm$ vs control ; $0.61{\pm}0.24mm$) and bone area in the medullary canal(experimental ; $13.55{\pm}4.98%$ vs control ; $9.03{\pm}3.05%$). The mean values of the amount of newly formed bone(endosteal region) and bone area(medullary canal) of the experimental implant demonstrated a statistically significant difference as compared to the control implant(p<0.05). In conclusion, noninvasive electrical stimulation using negatively charged electret effectively promoted peri-implant new bone formation in this study. This method is expected to be used as one of the useful electrical stimulation for enhancing bone healing response in the implant therapy

Scintigraphic Evaluation of Gastrointestinal Motility Disorders (기능성 위장관 질환에서 핵의학 검사의 역할)

  • Current scintigraphic tests of gastrointestinal motor function provides relevant pathophysiologic information, but their clinical utility is controversial. Many scintigraphic methods are developed to investigate gastrointestinal motility from oral cavity to colon. These are esophageal transit scintigraphy, oropharyngeal transit study, gastric emptying test, small bowel transit time measurement, colon transit study and gastroesopahgeal reflux scintigraphy. Scintigraphy of gastrointestinal tract is the most physiologic and noninvasive method to evaluate gastrointestinal motility disorders. Stomach emptying test is regarded as a gold standard in motility study. Gastrointestinal transit scintigraphy also has a certain role in assessment of drug effect to GI motility and changes alter therapy of motility disorders. Scintigraphy provides noninvasive and quantitative assessment of physiological transit throughout the gastrointestinal tract, and it is extremely useful for diagnosing gastrointestinal motor dysfunction. This article reviews the current procedures, indications, significance and guidelines for gastrointestinal motility measurements by scintigraphy.

A Basic Study on the Estimation of Noninvasive Temperature Distribution in Carcinoma Cell for the Hyperthermia (가온요법을 위한 암조직 세포의 무침습 온도분포 추정에 관한 기초연구)

  • In the clincal application of Hyperthermia, tissue temperature measurements are made at only a few selecte4 locations because of patient tolerance and practical clinical limitation. Since it is necessary to know the complete carcinoma cell temperature field in order to treat effectively, The difficulty of making such estimates from only a few point compounded because of the lack of knowledge of the carcinomoma cell blood perfusion characteristics. To solve the temperature on carcinoma cell, A noninvasive method (the finite element method) is used. The simulation results show that the finite element method is promising for estimating the complete corcinoma cell temerature distribution, if some knowledge of the blood perfusion pattern is available.
