지역사회 당뇨병 유병자와 비유병자의 심근경색증 및 뇌졸중 조기증상과 대처방안 인지도 비교: 성향점수매칭 분석 (Awareness of Early Symptoms and Emergency Responses to Myocardial Infarction and Stroke in People with Diabetes Mellitus Compared to Non-diabetic Population in the Community: A Propensity Score-Matched Analysis)

  • Background: We determined the differences in awareness of myocardial infarction and stroke according to the presence or absence of diabetes mellitus in the community. Methods: The 2018 Community Health Survey identified 20,812 people with diabetes mellitus aged 40-79 years. Using 1:1 matching by propensity score, 20,812 people without diabetes mellitus but with similar sociodemographic characteristics were selected as a comparison. Outcome variables were awareness of early symptoms of myocardial infarction and stroke and awareness of coping strategies in case of occurrence. Results: There was no significant difference between nondiabetic and diabetic people in terms of recognizing all early symptoms of myocardial infarction (nondiabetic, 42.7%; diabetic, 43.0%; p=0.43) and stroke (nondiabetic, 49.4%; diabetic, 49.4%; p=0.91). In addition, no significant difference was found between nondiabetic and diabetic people in the proportion of knowing correct emergency response to myocardial infarction (nondiabetic, 84.6%; diabetic, 84.4%; p=0.56) and stroke (nondiabetic, 81.3%; diabetic, 81.4%; p=0.77). Conclusion: Since people with diabetes are at greater risk of cardiovascular disease than the general public, it is important to lower the risk of disability and death by improving their awareness of early symptoms and correct emergency response to myocardial infarction and stroke.

고당질식이(高糖質食餌)가 정상(正常)쥐와 Alloxan 당뇨(糖尿)쥐의 혈당량(血糖量)과 혈청지질성분(血淸脂質成分)에 미치는 영향(影響) (Effects of High Carbohydrate Diets on Serum Glucose and lipid Values of Normal and Alloxan-Diabetic Rats)

  • The possible effects of 10% carbohydrate containting diets on the serum glucose and lipids values were studied with normal and alloxan - diabetic male rats for a period of 28 days. The diets were supplemented with either glucose, cornstarch or sucrose, fructose or the basal diet containing no added carbohy drates. After this period, 3 to 4 rats among the each group were killed and samples of serum, liver, spleen and kidney were collected. The others were rendered diabetic by the intraperitoneal injection of alloxan ($190mg/\frac{wt}{kg}$). Then original feeding scedule was continued for 3 days in all five diabetic groups, before the collection of tissue samples and serum. Feeding the nondiabetic rats with glucose and fructose delayed the bodyweight development relatively compared with nonsugar group. The weight deductions after alloxan injection were similar in all feeding groups. All diabetic animals exhibited increased blood glucose triglceride levels but almost unchanged total cholesterol values. Blood glucose values for nondiabetic rats were normal ranges, and then glucose feeding group was the highest. Total - cholesterol values were the highest in nondiabetic rats fed glucose, fructose and in diabetics fed sucrose, glucose. Triglyceride values were the highest in nondiabetic rats fed sucrose, and no difference in diabetic rats. Some cornstarch group exhibited fatty-livers in diabetic and nondiabetic, and more studies need.

대두와 Magnesium 급여가 당뇨쥐의 혈당 및 지질대사에 미치는 영향 (Effects of Raw Soy Flour and Magnesium on Serum Glucose and Lipid Metabolism in Streptozotocin-Diabetic Rats)

  • 20% 생콩가루 및 생콩가루에 마그네슘 첨가 식이로 streptozotoxin 유발 당뇨쥐를 4주간 사육하여 체중, 간과 췌장무게, 혈당, 혈중 지질 농도 및 간의 지질 농도에 미치는 영향을 조사하였다. 모든 당뇨군은 당뇨병으로 인한 대사장애로 정상군에 비하여 체중이 감소되었다. 간의 총 무게는 대조군과 모든 당뇨군이 비슷한 무게를 보였으나, 체중에 대한 간 무게의 %비율은 모든 당뇨군이 대조군 보다 유의하게 증가하였다. 췌장의 무게는 대조군 및 당뇨대조군에 비하여 당뇨생콩군 및 당뇨생콩+마그네슘군이 유의하게 증가되었다. 혈당량은 당뇨대조군에 비하여 당뇨생콩군 및 당뇨생콩+마그네슘군이 각각 73% 및 44% 유의하게 감소되었으나, 정상대조군에 비하여는 당뇨생콩군 및 당뇨생콩+마그네슘군이 각각 77및 111%증가되어 정상수준으로 혈당을 낮추지는 못하였다. 혈청의 중성지질, 인지질 및 총 콜레스테를 농도는 당뇨생콩군과 당뇨생콩+마그네슘군이 당뇨대조군에 비하연 유의하게 감소되어 정상대조군과 비슷한 경향으로 혈청의 지질 농도는 정상 수준으로 개선되었다. 간의 총 지질과 중성지질 농도는 대조군에 비하여 각 당뇨군이 유의하게 감소 되었으며, 특히 당뇨생콩+마그네슘군이 가장 낮은 농도를 보였다. 인지질 농도는 당뇨생콩+마그네슘군이 유의하게 감소되었다. 그리고 간의 총 콜레스테를 농도는 각 실험군이 변화를 보이지 않았다. 혈청의 HDL-콜레스데를 농도는 대조군과 당뇨실철군이 비슷한 수준을 보였다. 이상의 결과로 보아 생콩이 당뇨쥐의 혈당을 낮추어 주는 작용이 있고, 혈청 지질대사의 개선효과 등 부분적인 개선 효과가 있는 것으로 나타났으나 앞으로 많은 연구가 이루어져야 할 것으로 사료된다.

Maternal and neonatal outcomes in Korean women with type 2 diabetes

  • Background/Aims: The purpose of this study was to compare maternal and neonatal outcomes in Korean women with type 2 diabetes and nondiabetic controls. Methods: We performed a retrospective survey of 200 pregnancies in women with type 2 diabetes (n = 100) and nondiabetic controls (n = 100) who delivered from 2003 to 2010 at Cheil General Hospital & Women's Healthcare Center, Korea. We compared maternal characteristics as well as maternal and neonatal outcomes between groups matched by age, pre-pregnancy weight, body mass index, parity, and gestational age at delivery. Results: The number of infants that were small for gestational age and the rate of major congenital malformations were not significantly different. However, women with type 2 diabetes showed a slightly higher risk for primary caesarean section (35.0% vs. 18.0%, p = 0.006) as well as pre-eclampsia (10.0% vs. 2.0%, p = 0.017), infections during pregnancy (26.0% vs. 2.0%, p < 0.001), neonatal weight ($3,370{\pm}552.0$ vs. $3,196{\pm}543.3$, p = 0.025), large for gestational age (22.0% vs. 9.0%, p = 0.011), and macrosomia (15.0% vs. 5.0%, p = 0.018) compared to nondiabetic controls. Conclusions: Maternal and neonatal outcomes for women with type 2 diabetes were worse than those for nondiabetic controls. Diabetic women have a higher risk for primary caesarean section, pre-eclampsia, infections during pregnancy, large neonatal birth weight, large for gestational age, and macrosomia.

커피가루 첨가식이가 당뇨 쥐의 혈당 및 간 조직 항산화효소 활성에 미치는 영향 (Effects of coffee powder supplementation on the blood glucose and antioxidative enzyme activity of liver tissue in STZ-induced diabetic rats)

  • 커피첨가식이를 섭취한 당뇨 쥐에서 혈당, 혈중 지질농도, 간지질함량 및 간조직의 항산화효소인 SOD, GPx, CAT에 미치는 영향을 분석한 결과를 요약하면 다음과 같다. 실험시작 시 체중은 실험군 간에 유의적인 차이가 없었고 실험종료 시 체중은 당뇨군내에서 커피군이 대조군보다 유의적으로 높았으며, 실험기간동안 체중증가량은 당뇨군이 비당뇨군보다 유의적으로 낮았으나 커피첨가식이 섭취에 따른 유의적인 차이는 없었다. 평균 식이섭취량은 당뇨군이 비당뇨군보다 유의적으로 높았고, 당뇨군내에서는 커피군이 대조군보다 유의적으로 높았으며, 식이효율은 당뇨군이 비당뇨군보다 유의적으로 낮았으나 커피섭취에 따른 유의적인 차이는 없었다. 당뇨군과 비당뇨군 모두 혈중 HDL-콜레스테롤 농도는 커피군이 대조군보다 유의적으로 높았고, LDL-콜레스테롤 농도와 atherogenic index는 커피군이 대조군보다 유의적으로 낮았다. 간의 중성지방 함량은 당뇨군과 비당뇨군 모두 커피군이 유의적으로 낮았고, 총콜레스테롤 함량은 커피섭취에 따른 유의적인 차이가 없었다. 간 조직의 MDA 함량은 당뇨군과 비당뇨군 모두 커피군이 대조군보다 유의적으로 낮았다. 간 조직의 항산화효소인 SOD, GPx, CAT 활성은 당뇨군과 비당뇨군 모두 커피섭취에 따른 유의적인 차이는 없었다. 혈당은 당뇨군과 비당뇨군 모두 커피군이 대조군보다 유의적으로 낮았다. 결론적으로 당뇨 쥐에서 0.5% 커피첨가식이가 간의 항산화효소 활성에는 유의적인 영향이 없었으나, 당뇨 쥐의 혈당을 낮추었고, 혈중 HDL-콜레스테롤 농도는 높였으며, 혈중 LDL-콜레스테롤 농도, atherogenic index, 간 중성지방 함량 및 과산화지질 MDA는 낮추어 혈당과 혈중 및 간 지질 개선에 도움이 되는 것으로 나타났다.

당뇨병환자의 체지방량 및 체지방분포에 관한 연구 (Body Fat Content and Its distribution in Diabetics)

  • We designed this study to compare the total body fat content and its distribution of diabetics with those of normal subjects. Skinfold thicknesses at eight sites(subscapular, subcostal, abdomen, suprailiac, triceps, forearm, thigh and calf) and body circumferences at five sites(waist, hip, arm, thigh and calf) were measured on 220 diabetics(82 male, 138 female) and on 160 nondiabetic subjects(male 57, female 103). We matched 92 pairs with diabetics and nondiabetic control subjects by sex, age, body weight and height, and made comparisons between two groups(case-control study). The results were as follows: 1) There was no significant difference in total body fat content of diabetics and control (male ; 20.40$\pm$2.12%, 19.20$\pm$3.52%, female ; 26.46$\pm$2.53%, 27.01$\pm$2.92%, respectively). However, body muscle mass(%) in diabetic men(33.37$\pm$4.19%) was significantly lower than in nondiabetic men(38.16$\pm$7.11%). 2) Diabetics, especially women, were characterized by more central body fat than control. That is, indices of centrality of body fat distribution(subscapular/triceps skinfold : STR, central/peripheral fat : CPR) of diabetics were higher than those of control. 3) Body weight, body mass index and %IBW(current body weight$\times$100/ideal body weight) had negative correlations with duration of diabetes(r=-0.23~-0.33), but total body fat content(%) and indices of body fat distribution, such as STR, CPR, waist/hip girth ratio(WHR), and waist/thigh girth ratio(WTR), were not related to duration of diabetes.

당뇨병 유발 쥐에서 신경 세포 증식에 대한 저선량 방사선의 효과 (The Effect of Low Dose Radiation on the Neuronal Cell Proliferation in Diabetic Rats)

  • 목적: 저선량 방사선이 당뇨쥐의 해마에서 신경 세포의 증식에 미치는 영향을 확인하기 위해서이다. 대상 및 방법: 정상 쥐를 대상으로 방사선을 조사하지 않은 군, 0.1 Gy 방사선 조사군, 10 Gy 방사선 조사군으로 나눠 방사선이 정상 신경 세포의 증식에 미치는 영향을 확인하였다. 당뇨쥐를 대상으로 한 이차 실험은 방사선을 조사하지 않은 정상 쥐, 방사선을 조사하지 않은 당뇨쥐, 0.01 Gy 방사선 조사한 당뇨쥐, 0.1 Gy를 조사한 당뇨쥐, 1 Gy를 조사한 당뇨쥐, 10 Gy를 조사한 당뇨쥐로 나눠 시행하였다. 5-bromo-2'-deoxyuridine (BrdU)를 이용한 면역 염색을 시행하여 쥐 해마의 치아이랑 세포의 수를 세어 비교하였다. 결과: BrdU 양성 세포의 수는 대상으로 방사선을 조사하지 않은 정상쥐, 0.1 Gy 방사선 조사한 정상쥐, 10 Gy 방사선 조사한 정상쥐에서 각각 $45.95{\pm}3.42/mm^2,\;59.34{\pm}5.20/mm^2,\;19.26{\pm}2.98/mm^2$로 나타났다. 당뇨쥐를 대상으로 한 이차 실험에서 BrdU 양성 세포의 수는 방사선을 조사하지 않은 정상 쥐, 방사선을 조사하지 않은 당뇨쥐, 0.01 Gy 방사선 조사한 당뇨쥐, 0.1 Gy를 조사한 당뇨쥐, 1 Gy를 조사한 당뇨쥐, 10 Gy를 조사한 당뇨쥐에서 각각 $55.44{\pm}8.57/mm^2,\;33.33{\pm}6.46/mm^2,\;67.75{\pm}10.54/mm^2,\;66.63{\pm}10.05/mm^2,\;23.59{\pm}6.37/mm^2,\;14.34{\pm}7.22/mm^2$로 나타났다. 결론: 0.1 Gy 이하의 저선량 방사선은 당뇨쥐의 해마 치아이랑세포의 수를 증가시켜 세포 증식효과를 보이는 것을 확인하였다.

A comparative analysis of odontogenic maxillofacial infections in diabetic and nondiabetic patients: an institutional study

  • Objectives: The increased prevalence of antibiotic resistance is an outcome of evolution. Most patients presenting with odontogenic space infections also have associated systemic co-morbidities such as diabetes mellitus resulting in impaired host defense. The present study aims to compare the odontogenic spaces involved, antibiotic susceptibility of microorganisms, length of hospital stay, and the influence of systemic comorbidities on treatment outcome in diabetic patients. Materials and Methods: A 2-year prospective study from January 2012 to January 2014 was conducted on patients with odontogenic maxillofacial space infections. The patients were divided into two groups based on their glycemic levels. The data were compiled and statistically analyzed. Results: A total of 188 patients were included in the study that underwent surgical incision and drainage, removal of infection source, specimen collection for culture-sensitivity, and evaluation of diabetic status. Sixty-one out of 188 patients were found to be diabetic. The submandibular space was the most commonly involved space, and the most prevalent microorganism was Klebsiella pneumoniae in diabetics and group D Streptococcus in the nondiabetic group. Conclusion: The submandibular space was found to be the most commonly involved space, irrespective of glycemic control. Empiric antibiotic therapy with amoxicillin plus clavulanic acid combined with metronidazole with optimal glycemic control and surgical drainage of infection led to resolution of infection in diabetic as well as nondiabetic patients. The average length of hospital stay was found to be relatively longer in diabetic individuals.

식이 단백질의 급원에 따른 당뇨쥐의 혈당 및 혈청지질에 미치는 영향 (Effects of the Source of the Dietary Protein on Serum Lipids and Glucose in Streptozotocin-diabetic Rats)

  • This study was designed to find out the effects of soybean protein and milk protein between nondiabetic and diabetic rats. The experimental results are summarized as follows. 1. Total food intake was higher in diabetic soybean protein group than other groups but it was not significant. 2. The change of body weight was lower in diabetic soybean protein group than other groups and the soybean protein was effective to maintain the ideal body weight. 3. The effects of lowering total cholesterol and glucose in serum was higher in soybean protein groups than the milk protein groups.
