Background: Factors associated with the prognosis of patients with small cell lung cancer (SCLC) is relatively unknown, than of those with non-small cell lung cancer. This study was undertaken to identify the prognostic factors of SCLC. Methods: The medical records of 333 patients diagnosed with SCLC at tertiary hospital from January 1, 2008, to December 31, 2012 were retrospectively reviewed. Patients were categorized by age (${\leq}65$ years vs. >65 years) and by extent of disease (limited disease [LD] vs extensive disease [ED]). Overall survival and progression free survival rates were determined. Factors associated with prognosis were calculated using Cox's proportional hazard regression model. Results: Most baseline characteristics were similar in the LD and ED groups. Eastern Cooperative Oncology Group (ECOG) performance status (PS), first chemotherapy regimen, and prophylactic cranial irradiation (PCI) differed significantly in patients with LD and ED. Mean ECOG PS was significantly lower (p<0.001), first-line chemotherapy with etoposide-cisplatin was more frequent than with etoposide-carboplatin (p<0.001), and PCI was performed more frequently (p=0.019) in LD-SCLC than in ED-SCLC. Prognosis in the LD group was better in younger (${\leq}65$ years) than in older (>65 years) patients, but prognosis in the ED group was unrelated to age. Conclusion: This study showed that overall survival (OS) was significantly improved in younger than in older patients with LD-SCLC. Univariate and multivariate analyses showed that age, PCI and the sum of cycles were significant predictors of OS in patients with LD-SCLC. However, prognosis in the ED group was unrelated to age.

Power control is widely used to reduce co-channel interference in wireless networks and guarantee the signal-to-interference plus noise ratio (SINR) of ongoing connections. This technique is also effective for wireless body area networks (WBANs). Although achieving satisfactory SINR is important for WBAN users, they may not be willing to achieve it at arbitrarily high power levels since power is a scarce resource in WBANs. Besides, for WBANs with different purposes, the QoS requirements and concern about the power consumption may be different. This motivates us to formulate the power control problem using the concepts from microeconomics and game theory. In this paper, the QoS objective is viewed as a utility function, which represents the degree of user satisfaction, while the power consumption is viewed as a cost function. The power control problem consequently becomes a non-cooperative multiplayer game, in which each player tries to maximize its net utility, i.e., the utility minus the cost. Within this framework, we investigate the Nash equilibrium existence and uniqueness in the game and derive the best response solution to reach the Nash equilibrium. To obtain the optimal transmission power in a distributed way, we further propose a utility-based and QoS-aware power control algorithm (UQoS-PCA). Tunable cost coefficient in UQoS-PCA enables this scheme to be flexible to satisfy diverse service requirements. Simulation results show the convergence and effectiveness of the proposed scheme as well as improvements over existing algorithm.
This paper analyzes optimal watershed management focusing on reservoir-level sediment removal techniques. Although dams and reservoirs provide several benefits, sedimentation may reduce their storage capacity. As of today, the Aswan High Dam (AHD) in Egypt faces approximately 76% reduced life of the reservoir. Since the AHD is the major fresh water source in Egypt, sustainable use of this resource is extremely important. A model is developed to simultaneously determine optimal sediment removal strategies for upstream soil conservation efforts and reservoir-level sediment control. Two sediment removal techniques are considered: mechanical dredging and hydro-suction sediment removal system (HSRS). Moreover, different levels of upstream soil conservation efforts have introduced to control soil erosion, which is a major contributor of reservoir storage capacity reduction. We compare a baseline case, which implies no management alternative, to non-cooperative and social planners' solution. Our empirical results indicate that the socially optimal sediment removal technique is a mechanical dredging with unconstrained amount with providing a sustainable life of the reservoir. From the empirical results, we find that social welfare can be as high as $151.01 billion, and is sensitive to interest rates and agricultural soil loss.
Recently, many researches have been done to solve the challenging problem of Blind Source Separation (BSS) problems in the underdetermined cases, and the “Two-step” method is widely used, which estimates the mixing matrix first and then extracts the sources. To estimate the mixing matrix, conventional algorithms such as Single-Source-Points (SSPs) detection only exploits the sparsity of original signals. This paper proposes a new underdetermined mixing matrix estimation method for time-delayed mixtures based on the receiver prior exploitation. The prior information is extracted from the specific structure of the complex-valued mixing matrix, which is used to derive a special criterion to determine the SSPs. Moreover, after selecting the SSPs, Agglomerative Hierarchical Clustering (AHC) is used to automaticly cluster, suppress, and estimate all the elements of mixing matrix. Finally, a convex-model based subspace method is applied for signal separation. Simulation results show that the proposed algorithm can estimate the mixing matrix and extract the original source signals with higher accuracy especially in low SNR environments, and does not need the number of sources before hand, which is more reliable in the real non-cooperative environment.
Objectives: To observe the mitigating effects of a Traditional Korean Medicine(TKM) treatment program especially including pharmacopuncture, with Cyclophosphamide and doxorubicin chemotherapy on a both sides breast cancer patient. Methods: AA 74 year-old female patient diagnosed with both sides breast cancer (Right) pT1bpN0M0, (Left) pT1cpN1Mx was admitted to hospital of Dong-eui university in May of 2017. She received Cyclophosphamide and Doxorubicin from May $31^{st}$ to August $2^{nd}$, 2017 followed by TKM treatment consisting of herbal medicine, acupuncture, moxibustion and pharmacopuncture (Trionycis Carapax, Non-toxic Bee Venom, and Cultivated Wild Ginseng Extract) for a period of almost 4 months, from May $13^{th}$ to August $19^{th}$, 2017. Symptoms were evaluated by the grade of chief complaints refer to Common Terminology Criteria for Adverse Events (CTCAE) and Eastern Cooperative Oncology Group (ECOG). Results: TKM including pharmacopuncture alleviated chemotherapy-induced nausea, fatigue, joint pain, diarrhea, insomnia. Conclusions: This case study potentiates TKM with pharmacopuncture's significant efficacy in aiding breast cancer patients suffering from Cyclophosphamide plus Doxorubicin induced adverse effects. Further research should take place for clear understanding of the exact amount of dosage and safety. Moreover it must be accompanied by long-term follow up researches.
We consider the problem of optimizing the performance of a system shared by selfish non-cooperative users. In this problem, small jobs which the users request should be scheduled on a set of shared machines with their speed functions, each of which dependson the amount of jobs allocated on a machine. The performance of the system is measured by the maximum of the completion times when the machines complete the jobs allocated on them. The selfish users can choose a machine on which their jobs are executed, and they choose the fastest machine. But it typically results in suboptimal system performance. The Price of Anarchy(PoA) was introduced as a measure of the performance degradation due to the user's selfish behavior. The PoA is the worst-case ratio of the cost of a Nash equilibrium to the optimal cost. In this paper, we estimate the PoA for the above scheduling problem.
The present study purposed to establish a network for building an efficient disaster response system and for this purpose, we compared cases of disaster response in Japan and Korea. In Japan, disasters are responded jointly through the network of the central government, local self governing bodies and non governmental organizations. In Korea, however, the cooperative network among different areas is weak and this is aggravating damages caused by disasters. That is, in Korea, disaster response services have been mainly carried out by the public sector. The public sector can operate a powerful disaster management system using its extensive management resources, compelling power, and legal authorities. However, it was found that, due to the characteristics of contemporary society, efforts by the public sector including the central government and the local self government bodies have limitations in managing disasters effectively. Thus, for efficient disaster response in Korea, it is urgently required to establish a national disaster response network as well as a private sector cooperation system and to induce NGOs to participate in the disaster response system through institutional devices.
Purpose - After the transition, the development of defense industry in Eastern Europe has been regressed. Recently, they have internationally recognized that new products have been exported and contributed to the Innovation-Based Manufacturing of national economy such as unmanned reconnaissance aircraft, water purification technology, and mobile chemical laboratory, etc. The military forces in Eastern Europe are re-armed by the localization of self-produced munitions in their own defense industry, and then emphasize fostering their own defense industry. Thus, if they make a collaboration with other nations as a industrial cluster, it will gain a competitive edge on the defense industry. Research design, data, and methodology - The study was designed with the data of each national defense department. The research of the subject was reviewed before and after the transition. Thousands of workers have worked in defense industries before the transition, however, the defense industry and experts left after the transition. The Hungarian defense spending on GDP also dropped sharply from 1.72% in 2000 to 0.85% in 2013. But, due to the crisis in Ukraine and the crisis of Syrian refugees, the Viségrad Group (V4) member countries have also increased their interest in a defense and industries as well as the confidence in the EU and NATO. Results - On the whole, the joint of military training and purchase of defense materials were found in order to form the EU cooperative combat troops in CEE. There are the implementation of a joint manual plan for strengthening V4 security policy and the joint military exercises for V4 every year, and the others are electronic warfare and innovation of V4 national forces. Through such a performance analysis methodology, we found that the defense industry is developed through the national cluster cooperation among CEEs and spreading global distribution. Conclusions - Eastern Europe and Balkan countries have been looking forward to cooperating with the non-EU countries such as Korea and other defense industries. There are a lot of potential development into a new civilian cooperation defense industry for global-distribution. Thus, Korea should develop electronic commercial applications, not just as a weapon exporting region.
This paper is designed to make a national strategic concept for the ROK's maritime security and to explore cooperation directions for the ROK Navy and Coast Guard in order to implement the newly-made maritime security strategic concept. As strategy is composed of three main categories(goals, ways, means), the goal of the ROK's maritime security strategy is 'Safe and Affluent Sea' and the way to realize the goal is the principle of cooperative leverage, and the means as tasks to implement the strategic concept are maritime safety, maritime security, and maritime stewardship. The concept of national fleet as used in the US is applied to promoting the cooperation between the ROK Navy and Coast Guard. Thus, under the newly-established maritime security strategic concept along with the national fleet model, followings are suggested as policy proposals for facilitating mutual cooperation between the ROK Navy and Coast Guard in dealing with not only traditional threats but also non-traditional treats at sea and from the sea as well. First, the ROK Navy and Coast Guard has been making efforts to enhance interoperability between the two sea services. However, the mutual cooperations have been focused mainly on areas on operational level rather than policy level. Therefore, the two sea services are recommended to enlarge exchanges and cooperation in policy areas. Second, there are still demands for further cooperation areas between the two sea services in command and communications. The interoperability in C2 between the two needs to be upgraded even to the areas of anti-terrorist activities ar sea, ASUW, ASW, maritime interdiction, etc. Third, mutual comparability between the two needs to be reflected in the maritime forces development to ensure the comparability in UNREP and other logistics areas. Fourth, the standardization of logistical materials and equipments is needed as a way of sustaining operational capability and logistical capacity for the ROK Navy and Coast Guard as well. Fifth, the ROK Navy and Coast Guard are recommended to participate more actively in international maritime cooperation activities such as PASSEX. Sixth, Complementary laws and regulations need further to be revised and to be newly made for collectively managing swiftly maritime accidents and natural disasters at sea.
Coded cooperation is a promising user cooperation scheme. In this paper, we first propose a novel network-coding-based coded cooperation scheme. When a user decodes its partner's information correctly in the first frame, it transmits the combination of the partner's parity bits and its own parity bits through network coding in the second frame. This is distinct from the classical scheme, where the user only transmits the partner's parity bits during cooperation. We analyze the outage probability of the proposed scheme, and show that it achieves a full diversity order. Numerical evaluations reveal that the proposed scheme outperforms the classical scheme when the inter-user channel is poor, yet is worse when the inter-user channel is strong. Also, the results show that the proposed scheme always outperforms that of no cooperation in various channel conditions while the performance of classical scheme is worse than that of no cooperation with the poor inter-user channels. This means that the performance of the proposed scheme is more stable than the classical scheme and the proposed scheme is more tolerant to the poor inter-user channels. To combine the advantages of the proposed scheme and the classical scheme under different inter-user channel conditions, we propose an adaptive solution. This adaptive scheme enhances the system performance considerably in all channel conditions in spite of the inter-user channel quality, at the expense of only one acknowledgement or non-acknowledgement bit.
