구안와사(口眼喎斜)의 평가방법(評價方法)에 대한 고찰(考察) (A study of facial nerve grading system)

  • Background and Objetive : Lack of uniformity in reporting facial nerve recovery in patients with facial nerve paralysis has been a major disadvantage in comparing treatment modalities. The objective evaluation of facial nerve function is a complex procedure. The House and Brackmann grading system, the Yanagihara grading system has been recommend as a universal standard for assessing the degree of facial nerve palsy. However, clinical studies for treatment of facial palsy have rarely used this universal standard in oriental medicine. That is the reason for analysing this facial nerve grading system. Material and Method : We choose 10 scales reported from 1955 till 1995. These facial nerve grading systems may be classified as Gross system, Regional system and Specific system. Result and Conculsion : The scales of Botmann and Jonkees, May, Peitersen, and House and Brackmann are the gross facial nerve grading systems with which we grossly assess the facial motor dysfunction and the secondary defect. Among these scales, H-B scale is the most widespred The scales of Yanagihara(若杉文吉), Smith, Adour and Swanson, Jassen, FEMA are the regional facial nerve grading system in which we weight, or unweight the facial motor dysfunction and the secondary defect. For example, the scales of Yanagihara(若杉文吉) and Smith are the unweighted regional scale, the scale of Adour and Swanson, Jassen, FEMA are the weighted regional grading system. The scale of Stennert is the Specific facial nerve grading system in which we respectively assess the grade of facial dysfunction at rest, in motion and the secondary defect. For the objective evaluation of the oriental medicine treatment for facial palsy, we must use the universal standard scale, i.e. the H-B scale, the Yanagihara scale.

부교감신경계의 분포와 배유혈(背兪穴)의 상관성 연구 (A Study on the Correlation between Parasympathetic Nerve System and Back-shu Points)

  • Objectives : The purpose of this report is a study on the correlation between parasympathetic nerve system and Back-shu points. Methods : We have studied about the correlation between anatomical distribution of parasympathetic nerve system and that of Back-shu points. Back-shu points are located on the back area as the acupoints of Bladder Meridian(BL), doing its own organic physical and pathological working and directly connected with its own organ. Results and Conclusions : 1. It is suggested that Back-shu points has correlation with parasympathetic nerve system could be Pangguanshu(BL28), Zhonglushu(BL29), Baihuanshu(BL30), Ciliao(BL32), Zhongliao(BL33), Xialiao(BL34), Shangliao(BL31), Xiaochangshu(BL27), Baohuang,(BL53) Zhibian(BL54) etc. 2. We proved acupuncture treatment on the nearby spinal area could be one of the method that could stimulate parasympathetic nerve system. 3. The symptom of parasympathetic nerve system's disorder is similar to Back-shu points' chief efficacy a lot. And we proved the Back-shu points which has relation with parasympathetic nerve system as distribution of nerve system are only located at sacral area.

Dinamika를 이용한 중풍환자의 자율신경계 활성도에 대한 연구 (Study on Activity of Autonomic Nerve System Using Dinamika on Stroke)

  • This study was designed to research whether Dinamika can yield a suitable diagnosis for Activity of Autonomic Nene System on stroke. The testing of Dinamika was carried out at Oriental Medical Centar of Dong-Eui University with the participation of 27 patients. The results of Dinamika examinations shows as follows : Group A(the rising of a parasympathetic nerve system) is 0 persons. And Group B(the balancing autonomic none system) is 4 persons. Group C(the rising of a sympathetic nerve system) is 23 persons. There are 7 persons that it is difference of more than $20\%$ between B1 (the control ability of autonomic nerve system) and B2(the potential energy to control autonomic nerve system), 11 person that B1 and B2 are less than $10\%$, 1 person that B1 and B2 are balanced about $60\%$, 8 person that it is balanced domain from $10\%\;to\; 60\%$ between B1 and B2. According to this study on activity of autonomic nerve system using Dinamika on Stroke, these results suggest that 27 patients evidence the problem of autonomic none systems.

Peripheral Nerve Regeneration Through Nerve Conduit Composed of Alginate-Collagen-Chitosan

  • Although the peripheral nerve system has a relatively good regenerating capacity compared to the central nerve system, peripheral nerve repair remains a clinical challenge as restoration of normal nerve function is highly variable. Synthetic tubular nerve conduits were designed as an alternative repair method in order to replace the need for an isograft. These nerve conduits guide regenerating axons from the proximal toward the distal end, maintain within growth-promoting molecules released by the nerve stumps, and protect regenerating axons from infiltrating scar tissue. In this work, we prepared cinnamoylated alginate (CA)-collagen-chitosan nerve conduit using the lyophilization method to generate a controllable parallel channel in the center and then investigated its influence on peripheral nerve regeneration in an animal study. At 12 weeks after implantation, histological study showed that tissue cable was continuously bridging the gap of the sciatic nerve in all rats. Our newly developed nerve conduit is a promising tool for use in peripheral nerve regeneration and provides a suitable experimental model for future clinical application.

영상처리를 이용한 안면신경마비 평가시스템 개발 (Development of Facial Nerve Palsy Grading System with Image Processing)

  • Objectives The objective and universal grading system for the facial nerve palsy is needed to the objectification of treatment in Oriental medicine. In this study, the facial nerve palsy grading was developed with combination of image processing technique and Nottingham scale. Methods The developed system is composed of measurement part, image processing part, facial nerve palsy evaluation part, and display part. With the video data recorded by webcam at measurement part, the positions of marker were measured at image processing part. In evaluation part, Nottingham scales were calculated in four different facial expressions with measured marker position. The video of facial movement, time history of marker position, and Nottingham scale were displayed in display part. Results & Conclusion The developed system was applied to a normal subject and a abnormal subject with facial nerve palsy. The left-right difference of Nottingham scores was large in the abnormal compared with the normal. In normal case, the change of the length between supraorbital point and infraorbital point was larger than that of the length between lateral canthus and angle of mouth. The abnormal case showed an opposite result. The developed system showed the possibilities of the objective and universal grading system for the facial nerve palsy.

납 중독 랫드의 말초신경내 myo-inositol 수송 체계에 관한 연구 (A study on myo-inositol transport system in peripheral nerve isolated from lead-intoxicated rat.)

  • In our previous studies, we reported that lead intoxicated nerve cell by inhibition of the Na$^{+}$-K$^{+}$ ATPase activity and reduction of myo-inositol in nerve cell. As the second series of experiments, in order to understand toxic mechanism of lead for nerve cell, the characteristics of myo-inositol transport system and the effect of lead on its system have been studied in the sciatic nerves of control and lead-treated rats. A lead intoxicated animal model was induced by feeding diet containing lead to Sprague-Dawley rat for two weeks. Four weeks aged Sprague-Dawley rats were divided into three group : normal control group, 10ppm-lead treated group, 100ppm-lead treated group. All rats were sacrified at the end of two weeks. The rate o myo-inositol transport by sciatic nerve isolated from lead-treated rat was significantly decreased compared with that of control rat. This deficit results from that myo-inositol transport system which is carrier mediated and sodium-potassium dependent was inhibited by the lead treatment (both 10ppm and 100ppm) due to increase of the Km value without affecting Vmax value for myo-inositol carrier. These observations suggest that the toxic mechanism of lead on nerve myo-inositol transport system might be a change of affinity without change of maximum transport velocity for carrier.

초파리 신경계특이적인 단일클론항체의 제작과 그 항원의 국재 (Monoclonal Antibody Recognizing Nervous System Specific Protein of Drosophila melanogaster)

  • 초파리(Drosophila melanogaster)의 두부를 항원으로 사용하여 신경계 특이적인 단일클론항체를 제작하였다. 이 항체의 항원은 expression cDNA library screen을 한 결과 tyrosine kinase substrate의 일종인 disabled 분자를 인식하고, DNA sequencing결과 disabled 단백질의 C-말단부분인 7427에서 8761bp 사이를 특이적으로 인식한다는 것을 알 수 있었다. 이 항원의 국재를 조사한 결과 초기발생단계의 배에서는 중추신경계에 강하게 발현되었으며 성충에서는 시신경계와 뇌신경계 그리고 흉부신경계의 축색돌기가 밀집한 부분에 특이적으로 발현되었고, 또한 근육신경에서도 발현되는 것을 알 수 있었다. 따라서 disabled 분자는 배발달단계에서 중추신경계의 발생에 중요한 역할을 하는 것으로 예상되고, 성체에서는 신경계의 축색에서 뿐 아니라 근신경계에서도 어떤 기능을 수행하는 것으로 사료된다.

Enhancement of Optic Nerve in Leukemic Patients: Leukemic Infiltration of Optic Nerve versus Optic Neuritis

  • Purpose: To identify magnetic resonance imaging (MRI) findings of leukemic infiltration of optic nerve and optic neuritis in leukemic patients with emphasis of clinical findings as reference standard to differentiate them. Materials and Methods: MRI and clinical findings of 7 patients diagnosed as leukemic infiltration of optic nerve (n = 5) and optic neuritis (n = 2) in our institution between July 2006 and August 2015were reviewed retrospectively. In particular, MR imaging findings involved perineural enhancement and thickening of optic nerve and its degree, signal intensity, laterality (unilateral/bilateral), intraconal fat infiltration and its degree, and associated central nervous system abnormalities. Results: Of 5 cases of leukemic infiltration of optic nerve, 4 cases showed positive cerebrospinal fluid (CSF) study for leukemia relapse and 1 case was positive on bone marrow (BM) biopsy only. Moreover, of 5 leukemic infiltration of optic nerve, 2 cases showed the most specific MR findings for leukemic central nervous system involvement including 1 prominent leptomeningeal enhancement and 1 chloroma. However, other MR imaging findings of the patients with leukemic infiltration or optic neuritis such as thickening and perineural enhancement of optic nerves are overlapped. Conclusion: Enhancement and thickening of optic nerve were overlapped MR findings in leukemic infiltration of optic nerve and optic neuritis. Our findings suggest that enhancing optic nerve thickening with associated central nervous system MR abnormality favors the diagnosis of leukemic infiltration of optic nerve, especially in patients with history of acute lymphoblastic leukemia. However, CSF and BM study were required for differentiation between leukemic infiltration of optic nerve and optic neuritis.

Transcutaneous Electrical Nerve Stimulation System for Treating Tinnitus based on the Cortex-M4 Microcontroller

  • Among the methods of treating tinnitus, the transcutaneous clectrical nerve stimulation (TENS) method of treating by electrical stimulation is common. However, there is a problem that surgical operation is required to stimulate the vagus nerve (VN) main trunk near most of the bronchus. Alternatively, we found that the same effect could be achieved by electrically stimulating the vagus nerve VN branch (Arnold's nerve) distributed in the outer ear. The TENS system for stimulation of vagus nerve has been developed, but it has not been able to implement to stimulate as a parameter optimized for the patient by simultaneously playing the sound of eliminating the tinnitus frequency. Therefore, in this paper, it is important to develop a safe and practical TENS device for tinnitus treatment based on a 32-bit microprocessor that simultaneously applies non-invasive and notched sounds and to develop optimal treatment methods for treating tinnitus.

카페인이 신경전도속도에 미치는 영향 (Effects of Caffeine on Nerve Conduction Velocity)

  • 본 연구는 신경계를 자극하는 카페인이 신경전도속도(nerve conduction velocity. NCV)에 미치는 영향에 대해 알아보고자 하였다. 카페인의 종류는 커피를 사용하였다. 커피는 가장 쉽게 접할 수 있고, 많은 현대인들이 좋아하는 기호 식품으로 자리잡고 있다. 커피안에 있는 특수한 성분인 카페인은 카페인은 알칼로이드계 메틸화 화합물의 하나로서 이뇨 촉진, 혈관 확장, 중추신경흥분, 골격근 강화, 기관지 확장 등의 작용이 있다. 카페인 섭취가 신경속도를 향상시켜 반응시간의 감소를 실험한 연구는 미흡한 실정이다. 이에 본 연구는 신경계를 자극하는 카페인이 신경전도속도(nerve conduction velocity. NCV)에 미치는 영향에 대해 알아보고자 하였다. 대조군 및 실험군 개별 집단 내에서의 사전, 사후 신경전도속도(nerve conduction velocity. NCV) 수치를 비교한 결과, 대조군의 경우 두 곳의 측정 구간 모두 통계적으로 유의미한 차이가 나타나지 않았으나, 실험군은 팔꿈치 위 액와 구간에서의 신경전도속도(nerve conduction velocity. NCV)가 카페인 섭취 후 유의미하게 증가한 것으로 나타났다. 카페인을 섭취함으로 인해 신경전도속도(nerve conduction velocity. NCV)가 증가하였고, 이것은 일시적으로 신경전도속도(nerve conduction velocity. NCV)를 증가시킴으로써 운동능력향상, 근력향상, 신경기능향상 등에 대해 도움이 될 것이라고 판단하였다. 본 연구를 통하여 적정량의 카페인섭취는 신경전도속도신경전도속도(nerve conduction velocity. NCV)향상에 대해 도움을 줌으로써 카페인이 말초신경계에 영향력이 있다는 것을 알게 되었다. 이러한 결과를 통해 근기능장애, 신경기능장애 환자의 치료와 진단법 개발에 도움을 줄 것이라 기대하는 바이다.
