  • Clq nephropathy is an immune complex glomerulonephritis defined by the presence of mesangial Clq deposits in immunofluorescence microscopy and electron dense deposits on electron microscopy. It was described as a distinct disease entity in 1985 by Jennette and Hipp. Thirty four cases were reported in the literature but there has been no pediatric case reported in Korea yet. It commonly presents with steroid- resistent nephrotic syndrome in older children and young adults, and occasionally nephritic-nephrotic syndrome or rapidly progressive glomerulonephritis We report a case of Clq nephropathy in a 23-month-old girl with steroid-dependent nephrotic syndrome.   • Type 1 diabetes mellitus (T1DM) commonly occurs in childhood and adolescence and diabetic nephropathy is a serious metabolic complication of T1DM that leads to serious morbidity. With poor glycemic control prepubertal diabetes duration contributes to the risk of long-term microvascular complications, however, the younger age at onset or longer prepubertal diabetes duration seems to prolong the time to development of microalbuminuria or later end-stage renal disease (ESRD). Therefore, there have been a few cases of diabetic nephropathy in prepubertal patients and therefore the ESRD cases developed during adolescence in T1DM children were very rare. Here we report an adolescent with T1DM who had poor glycemic control and was diagnosed as diabetic nephropathy in a prepubertal period and leading to end-stage renal disease during adolescence.

  • Since Combes' first description of hepatitis B virus associated membranous glomerulonephritis, many reports have shown a strong association of hepatitis B virus infection with various types of nephropathies, especially membranous glomerulonephritis and membranoproliferative glomerulonephritis. Recently, the authors experienced a case of membraoproliferative glomerulonephritis, type I in a 16-year-old male patient with persistent HBs and HBe antigenemia. One year prior to admission the patient was told of hepatitis at routine check, but he remained asymptomatic throughout. He was admitted to our hospital with chief complaints of proteinuria and microscopic hematuria found on routine urinalysis. Liver and kidney biopsy revealed chronic persistent hepatitis and membranoproliferative glomerulonephritis type I, respectively. We report a case of hepatitis B virus associated nephropathy with review of the literatures. Considering the endemic nature of hepatitis B virus infection in Korea, the incidence of hepatitis B viurs associated nephropathy would be proportionally high in comparison with those of other countries. So, the importance of this entity merits special consideration in our country. Further study concerning pathogenesis, epidemiology, and treatment may be needed.

Beneficial Effect of the Combination of Oral Administration and Herbal-acupuncture Stimulation with Anti-diabetic Prescriptiom 1(AD-1) on Streptozotocin-induced Diabetic Nephropathy Rats (당뇨1호방(糖尿1號方)의 약침(藥鍼) 및 구강투여(口腔投與)가 Streptozotocin에 의한 흰쥐의 당뇨병성(糖尿病性) 신증(腎症)에 미치는 영향)

  • Objectives : The Present study was carried out to investigate the preventive effect of Anti-diabetic prescription 1(AD-1) on streptozotocin(STZ)-induced diabetic nephropathy. Methods : AD-1 consists of eleven herbs that have an effect on diabetes mellitus. AD-1 was given to rats with the combination of oral administration and herbal-acupuncture stimulation. The experimental animals were divided into 3 groups : normal group of rats, control group of STZ-induced diabetic rats, sample group with AD-1 treatment. Experimental diabetes was induced by the injection of STZ(60mg/kg) to e rat via the peritoneum. The effect of AD-1 on STZ-induced diabetic nephropathy was observed by measuring the serum level of creatinine and BUN. Urine secretion of albumin for 24 hours and urine level of glucose measures too. Anti-oxidative stress of AD-1 administration in living body was estimated by measuring lipid peroxide and GSH content in cortex of kidneys. Results : STZ induced increase of serum creatinine, BUN and albumin secretion were lowered by AD-1 treatment. Conclusion : The AD-1 treatment showed protective effect on rat diabetic nephropathy model, and action mechanism of the effect was thought to be concerned with anti-oxidative stress.

Network Pharmacology: Prediction of Astragalus Membranaceus' and Cornus Officinalis' Active Ingredients and Potential Targets to Diabetic Nephropathy (네트워크 약리학을 통한 당뇨병성 신병증에서의 황기와 산수유의 활성 성분 및 잠재 타겟 예측)

  • The purpose of this study is to predict the effects of macroscopic and integrative therapies by finding active ingredients, potential targets of Astragalus membranaceus (Am) and Cornus officinalis (Co) for diabetic nephropathy. We have constructed network pharmacology-based systematic and network methodology by system biology, chemical structure, chemogenomics. We found several active ingredients of Astragalus membranaceus (Am) and Cornus officinalis (Co) that were speculated to bind to specific receptors which had been known to have a role in the progression of diabetic nephropathy. Four components of Am and eleven components of Co could bind to iNOS; two ingredients of Am and six ingredients of Co could docking to cGB-PDE; one component of Am and nine components of Co could bind to ACE; three ingredients of Co with neprilysin; three components of Co with ET-1 receptor; four ingredients of Am and fourteen ingredients of Co with mineralocorticoid receptor; one component of Am and seven components of Co with interstitial collagenase; one ingredient of Am and ten ingredients of Co with membrane primary amine oxidase; one component of Am and four components of Co with JAK2; two ingredients of Am and one ingredient of Co with MAPK 12; one component of Am and five components of Co could docking to TGF-beta receptor type-1. From this work we could speculate that the possible mechanisms of Am and Co for diabetic nephropathy are anti-inflammatory, antioxidant and antihypertensive effects.

Therapeutic Potential of Chinese Prescription Hachimi-Jio-Gan and Its Crude Drug Corni Fructus against Diabetic Nephropathy (중국처방전 팔미지황환과 구성생약인 산수유의 당뇨병성 신증에 대한 보호 효과)

  • Background: Traditional plant drugs, are less toxic and free from side effects compared to general synthetic drugs. They have been used for the treatment of diabetes and associated renal damage. In this study, we evaluated effect of Hachimi-jio-gan against diabetic renal damage in a rat model of type 1 diabetic nephropathy induced by subtotal nephrectomy plus streptozotocin (STZ) injection, and in Otsuka Long-Evans Tokushima Fatty (OLETF) rats and db/db mice as a model of human type 2 diabetes, and its associated complications. To explore the active components of Hachimi-jio-gan, the antidiabetic effect of corni fructus, a consituent of Hachimi-jio-gan, and 7-O-galloyl-${{\small}D}$-sedoheptulose, a phenolic compound isolated from corni fructus, were investigated. Methods and Results: We conducted an extensive literature search, and all required data were collected and systematically organized. The findings were reviewed and categorized based on relevance to the topic. A summary of all the therapeutic effects were reported as figures and tables. Conclusions: Hachimi-jio-gan serves as a potential therapeutic agent to against the development of type 1 and type 2 diabetic nephropathy. From the results of characterization active components of corni fructus, 7-O-galloyl-${\small}D$-sedoheptulose is considered to play an important role in preventing and/or delaying the onset of diabetic renal damage. 7-O-Galloyl-${\small}D$-sedoheptulose is expected to serve as a novel therapeutic agent against the development of diabetic nephropathy.

A Case Report on Acupuncture and Moxibustion Treatment of a Patient with Immunoglobulin A Nephropathy (IgA 신병증 환자의 침구 치료 치험 1례)

  • Objective: This study was conducted to investigate whether acupuncture and moxibustion treatment could improve proteinuria in immunoglobulin A nephropathy. Methods: A 25-year-old man diagnosed with immunoglobulin A nephropathy had severely increased proteinuria. We conducted acupuncture treatment and moxibustion treatment on the abdominal area between the abdominal aorta and kidney at level L2. Results: After five months of acupuncture and moxibustion treatment, the patient's urine albumin/creatinine ratio and urine protein/creatinine ratio had improved from 0.571 to 0.28 and 0.79 to 0.47, respectively. Conclusions: This case report suggests that acupuncture and moxibustion treatment may ameliorate proteinuria in immunoglobulin A nephropathy.

  • There are a considerable number of reports suggesting a common pathogenesis of IgA nephritis(IgANn) Henoch-$Sch{\ddot{o}}nlein$ Purpura(HSP). In previous reports, a patient develops IgAN after kidney transplantation for HSP nephritis, one of Identical twin boys, developed IgAN and the other HSP, and a boy with IgAN later developed HSP. We report two cases, one with IgAN who later developed HSP and the other with HSP who later developed IgAN, suggesting that IgAN and HSP have a common pathogenesis. (J. End Stage Renal Disease caused by IgA Nephropathy : One Case Report (IgA 신증에 의한 말기 신질환에 대한 증례보고)

  • This case is to report the effect of renal function of chronic kidney disease(CKD) caused by IgA nephropathy. A 37-year-old man visited a Korean medicine hospital, who has been diagnosed with end stage renal disease(ESRD), 5 stage of CKD, caused by IgA nephropathy, has had no improvement of western medical treatment, and wanted to be treated using Korean medicine before renal transplantation. The decrease of creatinine value, the increase of glomerular filtration rate(GFR), and the decrease of CKD stage (5 to 4) was observed after combination treatment of Ikkigeonbiisuhwalhyeoltang and saam acupuncture was applied. This case report is suggested that combination treatment of acupuncture and herbal medicine could be effective to renal function of CKD in spite of a single case.

  • Type 2 diabetes mellitus (T2DM) leads to many health problems like diabetic nephropathy (DN). One of the key factors for chronic kidney disease and end-stage renal disease (ESRD) is T2DM. Extensive work is being done to delineate the pathogenesis of DN and to extend possible remedies. This review is intended to understand the nature of DN risk factors, progression, effects of glycemic levels, and stages of DN. We also explored the novel diagnostic and therapeutic approaches for DN such as gene therapy and stem cell treatments.
