일 지역사회 간호사들의 금연지도활동에 대한 조사연구 (Smoking Cessation Counselling Activity among Nurses in a Community)

  • Purposes: The purposes of this descriptive correlational study were to assess knowledge and attitude toward smoking and smoking cessation counseling activity among nurses in a community, and to identify predictors of their smoking cessation counseling activity. Method: Nurses employed by institutions such as university hospital, hospital, clinic, and health department in a community were surveyed using a structured questionnaire. Of the nurses invited to participate in this study, 760 (88.5%) responded with completed questionnaires. Results: Nurses had relatively positive attitude toward their roles and responsibilities about smoking cessation counseling activity. However, smoking cessation counseling activity was not a routine part of their nursing practice. Moreover, the level of engagement with smoking cessation counseling activity was significantly different by working place. Nurses who were working at the university hospital, hospital, and health department were more actively engaged with smoking cessation counseling activity than nurses who were working at the clinic. Smoking cessation counseling activity was significantly correlated with knowledge of smoking. attitudes toward smoking-related issues, and self-efficacy for smoking cessation counseling activity. In the final stepwise multiple regression, smoking cessation activity was predicted by the nurses' working place, attitudes toward smoking-related issues, and self-efficacy for counseling knowledge and skills. Conclusion: In conclusion, nurses need to participate routinely and actively in smoking cessation counseling activity. To help nurses counsel and intervene patients regarding smoking cessation more effectively, it is essential to integrate educational information on smoking cessation intervention into curriculums of nursing schools as well as to offer smoking cessation intervention as a continuing education program available for currently practicing nurses.

Oral Cancer Awareness of the General Public in Gorakhpur City, India

  • Objectives: Global cancer statistical data show that India has one of the highest incidence rates of oral cancer worldwide. Early detection is extremely important as it results in lower morbidity and death rates. The present study was undertaken to assess awareness of oral cancer and knowledge of its early signs and risk factors in the general public of the semi-urban Gorakhpur area of Uttar Pradesh (India). It was also intended to educate the same population for early detection by increasing their ability to recognize signs and risk factors. Method: A questionnaire-based household survey was conducted over a period of one month in different parts of Gorakhpur district, a region where tobacco use is apparently very high. A total of 2,093 persons participated in the survey. The collected data were analyzed using SPSS software to assess and associate oral cancer awareness with the prevalence, and abstract risk factors, as well as other confounding variables. Results: The general awareness, knowledge of signs and risk factors of oral cancer were found to be proportionate to the literacy level with the highest rate of awareness being among high school and graduates and lowest among illiterates. It was also observed that on most of these dimensions the younger age groups (<30 years) were significantly more knowledgeable. Conclusion: Overall, the awareness of oral cancer in the high-risk population of Gorakhpur was not satisfactory, pointing to a need for further dissemination of information on this issue and its associated risks. This is especially important for the youngsters, as this may possibly help them keep away from the deleterious habit of tobacco indulgence in any form. If necessary risk factor cessation counselling should be provided.

Impact of Admission Diagnosis on the Smoking Cessation Rate: A Brief Report From a Multi-centre Inpatient Smoking Cessation Programme in Singapore

  • Objectives: Few studies have been published regarding the relevance of the admission diagnosis to the smoking cessation rate. We studied smoking cessation rates in relation to admission diagnoses in our inpatient smoking cessation programmes. Methods: This retrospective study included all patients recruited into our inpatient smoking cessation programmes at 2 institutions in Singapore between June 2008 and December 2016. Patients were given individualized intensive counselling and were followed up via phone interviews for up to 6-month to assess their smoking status. Multivariable logistic regression was used to analyse potential associations between admission diagnoses and 6-month abstinence. Results: A total of 7194 patients were included in this study. The mean age was 54.1 years, and 93.2% were male. In total, 1778 patients (24.7%) were abstinent at the 6-month follow-up call. Patients who quit smoking tended to be of Chinese ethnicity, have initiated smoking at a later age, be better educated, and have lower Fagerström Test of Nicotine Dependence scores. After adjusting for these factors, patients with a cardiovascular admission diagnosis had a significantly higher probability of quitting tobacco use than patients with a respiratory or other diagnosis. Conclusions: In patients acutely admitted to the hospital, a diagnosis of cardiovascular disease was associated with the highest quit rate. Smoking cessation interventions need to be incorporated into all cardiovascular disease treatment pathways to leverage the patient's motivation and to improve the quit rate. In addition, patients in groups with lower quit rates may benefit from more intensive programmes to increase the rate of successful cessation.

영재학생과 일반학생의 성취목표지향성과 학업소진이 학교행복감에 미치는 영향 (The Effects of Achievement Goal Orientation of Gifted and General Students on their Academic Burnout and School Happiness)

  • The main purpose of this study is to see the differences of the achievement goal orientation, academic burnout, and school happiness between gifted and general students, and also to explore the effects on school happiness of their achievement goal orientation and academic burnout. To resolve the above research questions, this study used questionnaire method to 256 1st grade middle school students and 84 gifted students enrolled in Daegu metropolitan city. All data were analyzed by SPSS WIN 19.0 program. The results of this study are as follows. First, mastery goal, performance-approach goal and performance-avoidance goal of the gifted students were higher than the general students'. Second, all sub-variables of academic burnout except hostility and total score of academic burnout of the gifted students were lower than the general students. On the other hand, all sub-variables of school happiness except relationship with friends and emotional stability and total score school happiness of the gifted students were higher than the general students. Third, this study revealed that the achievement goal and academic burnout explain considerable amount of students' school happiness. In other words, the disfavor and skeptical attitude of general students for the study hinder school happiness. Lastly, this study suggest that some program or consulting need to be administerd to change the affective attitude of students into positively. And also variety of counselling program or some instructions dealing with stress for gifted students since the exhaustion of gifted students affects considerably their school happiness.

정신건강복지센터 간호사의 자살예방 지식 및 기술, 자살에 대한 태도와 자살예방업무 소진의 관계: 혼합연구방법의 적용 (Relationships among Knowledge and Skills about Suicide Prevention, Attitudes toward Suicide, and Burnout of Suicide Prevention Work of Nurses at Mental Health Welfare Centers: A Mixed Methods Study)

  • Purpose: This study aimed to identify relationships among knowledge and skills about suicide prevention, attitudes toward suicide, and burnout of suicide prevention work of nurses at mental health welfare centers. Methods: An explanatory sequential mixed-method research was conducted. For the quantitative study, the subjects (nurses) were 133 nurses executing suicide prevention work. The data were analyzed using the SPSS 25.0 program. For the qualitative research, 13 nurses with high burnout scores were interviewed. The data were analyzed using theme analysis method. Results: Quantitative results showed the average burnout of suicide prevention work was 54.62 ± 12.51. The burnout of suicide prevention work had significant correlations with attitudes toward suicide (r = .30, p < .001) and suicide prevention skills (r = - .18, p = .037). Qualitative results showed six themes related to burnout of suicide prevention work. They were 'feeling a lack of confidence in one's suicide counselling skills', 'feeling of the limits of one's ability to cope with a suicide crisis', 'feeling regret for not being able to help the clients', 'being over-empathetic to the clients', 'Not being able to understand the clients because the subjects (nurses) opposes committing suicide', and 'thinking that the suicidal thoughts of suicide attempters do not improve'. Conclusion: To reduce burnout of nurses' suicide prevention work at mental health welfare centers, there is a need to develop an educational program considering nurses' attitudes toward suicide and one to enhance their confidence in suicide prevention skills.

일개 시지역의 가정간호 수혜자들의 만족도 조사 (A Survey on Clients' Home Care Satisfaction in Taejon City in Korea)

  • This study enrolled one thousand five hundred twenty one clients that were receiving home care services at five health centers and a home care services center in the city of Taejon from November 17 through December 7, 1997. The purpose of this study was to analyze satisfaction levels, general characteristics and efficacies, of the client served home care. The main results were as follows; The age group of most of the subjects was 70-79 years: 76.9% of them were female and 87.3% were over 65 years. In education variables, over 90% of the clients were below primary school. In marital status variables, 63.4% of them had no spouse, 57.5% of them were widows and widowers. In living arrangement variables, alone(35.8%) was the most. In insurance status variables, medical aid(69.9%) was the most. In household income variables, below 000 won(72.6%) was the most. In hospitalization variables, 53% of the clients had no hospitalization. The satisfaction level of home care service of male and female was similar. The older the age, the higher the satisfaction level. The satisfaction level of the group having religion was higher than the group having no religion and the Christianity group had the highest satisfaction level(p<0.001). In education variables, the over high school group was the highest satisfaction level(p<0.01). In living arrangement variables, the other (sisters or neighbors etc.) group indicated the highest satisfaction level(p<0.001). In insurance status variables, the other group(except for medical aid) was the highest satisfaction level(p<0.001). In household income variables, below 390,000 won was the highest satisfaction level(p<0.001). In hospitalization variables, 'over 4 times' group indicated the highest satisfaction level (p<0.01). Home care took place more in health centers than in home care services center. In frequency of home care per month three times was the most. In opinion of home care frequency per month 82.8% of the people answered 'proper'. A lot of present illness was neuralgia, arthritis, digestive disease, hypertension & CVA. In contents of home care services variables, education & counselling was the most and medication was second. In duration of illness variables, over 10 years was the most. In place of treatment before home care service variables, hospital(57%) was the most. In illness condition after home care variables, 'moderate' was the most and 'much better'(85.5%) was second. In help of home care variables, 'much help'(71.5%) was the most and 'moderate'(28.1%) was second. In contents of counselling variables, treatment method of illness was the most. Home care services center indicated higher level of satisfaction than health center(p<0.001). In opinion of home care frequency per month variables, 'less' was the highest satisfaction level (p<0.001). In duration of illness variables, below one year was the highest and over 10 years was second(p<0.001). In place of treatment before home care service variables, health center was the highest (p<0.001). In illness condition after home care variables 'much better' was the highest and 'worse' was second (p<0.001). I n help of home care variables, 'much help' was the highest (p<0.001). In contents of counselling variables, cause of illness was the highest(p<0.001). According to the 14 items which consisted of 3 point scales the total level of satisfaction of home care service was very high, with total mean score 36. According to the above results, most clients taking home care services are satisfied. However, organization and a fund are required to support high quality home care services to those who need them. Furthermore, a follow-up survey should be accomplished to evaluate the status of clients.

노인장기요양기관 입소노인의 가족관계가 시설적응에 미치는 영향 (The Effect of Family Relationship on Facility Adaptation of Long-Term Care Facility Residents)

  • 본 연구에서는 노인장기요양기관 입소노인의 가족관계가 시설적응에 미치는 영향을 파악하는데 연구의 목적이 있다. 장기요양기관을 평가등급과 지역에 따른 할당표본을 통한 표집틀을 구성하여, 2018년 1월과 2월에 한 기관 당 입소노인 5~6명을 대상으로 설문조사를 하였으며, 최종적으로 381명의 데이터를 분석에 사용하였다. 주된 연구결과는 첫째, 노인의 시설적응수준은 높은 편이었다. '집단행동부적응'과 '이동쇼크'와 같이 부적응차원은 2점대로 낮고, '새로운 거주지 인정'과 '친구 만들기'와 같은 적응차원은 중간수준 이상을 보이고 있다. 둘째, 노인의 시설적응에는 노인의 사회인구학적 특성이나 시설관련변수의 영향력보다는 가족관계변수의 영향력이 더 컸다. 그 중에서 가족과의 친밀감 변화 변수의 영향력이 가장 컸다. 따라서 노인이 시설에 입소하기 전부터 입소 후까지 전 과정에 걸쳐 가족과의 친밀감이 유지되도록 적극적인 지원과 노력이 필요하다는 점을 제안한다.

대학교의 인성교육진흥을 위한 교양교과 개발 및 효과성 연구 (Efficacy Analysis of Developed Course for Promoting Character Education with Women's University Students)

  • 본 연구는 대학의 인성교육을 목표로 개발된 교양교과목의 효과성을 검증하고 결과를 논의함으로써, 향후 개선된 인성교육 교과를 개발하기 위함에 목적이 있다. 연구는 지방의 K대학교의 마음나눔교육부에서 대학의 교육 목표에 따라 개발된 '마음교양' 교양교과를 대상으로 효과성을 검증하였다. 교과는 15주로 진행했으며 구체적인 인성교육의 내용은 '감사'의 가치관 증진으로 구성되었다. 효과성을 검증하기 위해 켄터키 마음챙김기술 척도, 게슈탈트접촉경계진단검사, 나의식우리의식 척도, 자존감 척도를 사용하였다. 연구대상자는 '마음교양' 교양교과를 수강한 학생(실험군)과 수강하지 않은 학생(통제군) 229명의 자료이며, 연구방법은 집단간 차이분석을 하였다. 분석결과, 게슈탈트접촉경계진단검사의 하위요인인 연결성 척도에서만 유의미한 집단간 차이를 보였다(t=2.323, p=.021). 이는 대인관계에서의 연결성 회복을 시사하며, 인성교육의 일부 효과성이 검증되었음을 의미한다. 추후 연구에서는 감사의 마음이외에 K대학교의 교육이념을 반영한 교과 내용을 구체화하고, 교육방법에서도 K여자대학의 교육 목표를 구체적으로 반영한 교과개발이 필요함이 논의되었다.

청소년의 성지식 요구 (Needs for Sex Knowledge in Adolescents)

  • The purpose of this article was to get hold of adolescent's needs for sex knowledge and to reveal their understanding of sex. These results may be applied to the sex education and counselling for adolescent. The participants of study were 267 students (females, 144 ; males, 123) who were the first and second grade form 1 middle and 2 high schools. They were asked to describe 3 questions that they wanted to know or to learn about sex. Their questions about sex were total 779(girls, subtotal 456, mean 3.2, boys, subtotal 323, mean, 2.6). These questions categorized to 9 items by content analysis. The items were ‘sexual drive, behavior and tendency(229, 29.4%)’ , ‘anatomy and physiology of reproductive system(140, 18.0%)’, ‘reproduction(131, 16.8%)’, ‘concept of sex(31, 4.0%)’, ‘acquaintance between the other sexes(17, 2.2%)’, and ‘the others(9, 1.2%)’ in order of frequency. The most frequent item that the student want to know or team about sex was ‘sexual drive, behavior and tendency’ except girls of sophomore in middle school. There were several features in participants' needs for sex knowledge as respects of the understanding of sex- biological sex, gender, and sexuality. The prominent feature in knowledge need of bio logical sex was that the participants had the interests according to their biological sex. They had the negative attitude to the phenomenons (ex, menstruation and pregnancy, phimosiectomy, etc) that they experienced or would experience due to their biological sex. A part of them asked the questions based the misconceptions and used the slangs or ‘××’, ect. to name the male or female genitalia. The male students wanted to know the female genitalia. The participants' understanding of gender reflected the sexism of our society, but they didn't accept and had doubts about the dual, hierarchical structure of that. The students, especially female seemed to be powerless to the harms of the sexual violence. Girls had much interest in their appearances and complained to our sexual culture that women comforted and served men. The participants had the dual perspective that the sexuality as respects of physiologic phenomenon was considered as natural but that as related to heterosexuals was as negative. And they deemed that men's sexual drive was stronger than women's and was difficult or was not able to be inhibited. They had much interests in homosexuality but reflected the negative attitude to that in our society. Adolescent felt wonder why the expressions of sexuality of adult were permitted but theirs were not. Lastly, a part of boys substituted querying the sexuality of animals for asking that of human. Maybe it was because of the embarrassment to talk about human's sexuality directly. The teenaged participants understood that the sex had not only the biological meanings but also the sociocultural meanings. They regarded the sex as natural and wanted to know it, but they had conceptions that it was difficult and embarrass to talk about it openly and directly.

의과대학생의 자기조절학습이 진로적응에 미치는 영향 (The Effects of Medical Students' Self-regulated Learning on Career Adaptability)

  • 본 연구는 의과대학생들의 진로적응에 대한 자기조절학습의 영향을 알아보기 위한 것이다. 이를 위하여 대구지역 K 의과대학에 재학 중인 252명의 학생들을 대상으로 자기조절학습과 진로적응 검사를 실시하였으며, 진로적응 수준에 따라 고 저 집단으로 나눈 후 이들 집단 간 자기조절학습 정도에 대하여 집단 간 차이검증(독립표본 t검증)을 실시하였다. 그 결과 시험불안을 제외한 모든 자기조절학습 영역에서 진로적응이 낮은 집단에 비하여 높은 집단의 점수가 높은 것으로 나타났다. 또한 진로적응의 하위요인인 책임감, 대인관계, 주도성, 긍정적 태도에 대한 자기조절학습의 영향력을 알아보기 위하여 회귀분석을 실시해본 결과, 전체 진로적응점수에 대하여 인지조절과 행동조절이 통계적으로 유의한 영향을 미치는 반면, 동기조절은 유의한 영향을 미치지 않는 것으로 나타났다. 그러나 진로적응의 하위요인인 책임감과 대인관계에서는 통계적으로 유의한 영향을 미치지 않았지만 주도성과 긍정적 태도에 대해서는 동기조절의 유의한 영향력이 검증되었다. 이러한 결과에 따라 본 연구자들은 개인적인 메타인지, 동기, 행동 전략의 구사와 조절능력이 개인의 진로적응성에 미치는 영향에 대하여 논하였으며, 의과대학에서의 학생적응을 돕기 위한 자기조절과 관련된 중재의 필요성과 진로적응을 돕기 위한 상담 및 교육의 필요성을 제안하였다.
