Clinical Apply of Dual Energy CT (kVp switching) : A Novel Approach for MAR(Metal Artifact Reduction) Method (듀얼에너지 CT(kvp switching)의 임상 적용: MAR(Metal Artifact Reduction) 알고리즘의 적용)

  • OThe purpose of this article was to measure and compare the value of the metal artifact reduction (MAR) algorithm by Dual energy(kVp switching) CT (Computed Tomography) for non using MAR and we introduced new variable Dual energy CT applications through a clinical scan. The used equipment was GE Discovery 750HD with Dual-Energy system(kVp switching). CT scan was performed on the neck and abdomen area subject for patients. Studies were from Dec 20 2010 to Feb 10 2011 and included 25 subject patients with prosthesis. We were measured the HU (Hounsfield Unit) and noise value at metal artifact appear(focal loss of signal and white streak artifact area) according to the using MAR algorithm. Statistical analyses were performed using the paired sample t-test. In patient subject case, the statistical difference of showing HU was p=0.01 and p=0.04 respectively. At maximum black hole artifact area and white streak artifact area according to the using MAR algorithm. However noise was p=0.05 and p=0.04 respectively; and not the affected black hole and white streak artifact area. Dual Energy CT with the MAR algorithm technique is useful reduce metal artifacts and could improve the diagnostic value in the diagnostic image evaluation of metallic implants area.

Genetic diversity and population structure of indigenous chicken of Bangladesh using microsatellite markers

  • Objective: The objectives of this study were to investigate the genetic diversity, population structure and relatedness among the five chicken populations of Bangladesh using microsatellite markers. Methods: A total of 161 individuals representing 5 chicken populations (non-descript Deshi [ND], naked neck [NN], hilly [HI], Aseel [AS], and red jungle fowl [JF]) were included in this study to investigate genetic diversity measures, population structure, genetic distance and phylogenetic relationships. Genotyping was performed using 16 selected polymorphic microsatellite markers distributed across 10 chromosomes. Results: The average observed and expected heterozygosity, mean number of alleles and polymorphic information content were found to be 0.67±0.01, 0.70±0.01, 10.7 and 0.748, respectively in the studied populations. The estimated overall fixation index across the loci (F), heterozygote deficiency within (FIS) and among (FIT) chicken populations were 0.04±0.02, 0.05 and 0.16, respectively. Analysis of molecular variance analysis revealed 88.07% of the total genetic diversity was accounted for within population variation and the rest 11.93% was incurred with population differentiation (FST). The highest pairwise genetic distance (0.154) was found between ND and AS while the lowest distance was between JF and AS (0.084). Structure analysis depicted that the studied samples can be categorized into four distinct types or varieties (ΔK = 3.74) such as ND, NN, and HI where AS and JF clustered together as an admixed population. The Neighbor-Joining phylogenetic tree and discriminant analysis of principal component also showed close relatedness among three chicken varieties namely AS, HI, and JF. Conclusion: The results reflected that indigenous chicken of Bangladesh still possess rich genetic diversity but weak differentiation among the studied populations. This finding provides some important insight on genetic diversity measures that could support the designing and implementing of future breeding plans for indigenous chickens of Bangladesh.

Long Term Outcomes and Prognostic Factors of N0 Stage Nasopharyngeal Carcinoma: a Single Institutional Experience with 610 Patients

  • Treatment responses of $N_0$ stage nasopharyngeal carcinoma were firstly analyzed comprehensively to evaluate long term outcomes of patients and identify prognostic factors. A total of 610 patients with $N_0$ NPC, undergoing definitive radiotherapy to their primary lesion and prophylactic radiation to upper neck, were reviewed retrospectively. Concomitant chemotherapy was administrated to 65 out of the 610. Survival rates of the patients were calculated using the Kaplan-Meier method and compared by log-rank test. Prognostic factors were identified by the Cox regression model. The study revealed the 5-year and 10-year overall, disease-free, disease-specific, local failure-free, regional failure-free, locoregional failure-free and distant metastasis-free survival rates to be 78.7% and 66.8%, 68.8% and 55.8%, 79.9% and 70.4%, 81.2% and 72.5%, 95.8% and 91.8%, 78.3% and 68.5%, 88.5% and 85.5%, respectively. There were 192 patients experiencing failure (31.5%) after radiotherapy or chemoradiotherapy. Of these, local recurrence, regional relapse and distant metastases as the first event of failure occurred in 100 (100/610, 16.4%), 15(15/610, 2.5%) and 52 (52/610, 8.5%), respectively. Multivariate analysis showed that T stage was the only independent prognostic factor for patients with $N_0$ NPC (P=0.000). Late T stage (P=0.000), male (P=0.039) and anemia (P=0.007) were independently unfavorable factors predicting disease-free survival. After treatment, satisfactory outcome wasgenerally achieved in patients with $N_0$ NPC. Local recurrence represented the predominant mode of treatment failure, while T stage was the only independent prognostic factor for overall survival. Late T stage, male gender, and anemia independently predicted lower possibility of the disease-free survival.

  • Xerostomia is subjective feeling of dry mouth, a symptom that may or may not be accompanied by hyposalivation, an objective decrease in salivary flow. There are many causes induced xerostomia like drugs, salivary gland diseases, radiation therapy to the head and neck region, Sjogren syndrome, emotional stress etc. Insufficient salivary flow creates complications with oral candidiasis, dental caries, periodontitis, halitosis, dysgeusia. So finally, these complications lead to an overall decline in quality of life. Managements of xerostomia are eliminating or alterating the etiologic factors, relieving symptoms, preventing or correcting the consequences of salivary dysfunction, treating underlying disease and stimulating salivation. One of the salivation stimulation agents studied to treat xerostomia was the pilocarpine muscarinic agonist. Pilocarpine is one of salivation stimulants, a parasympathomimetic drug and non-selective muscarinic receptor agonist. Systemic pilocarpine has been used to stimulate salivary secretion. But systemic administration of pilocarpine has limitations such as increased risk of side effects and contraindications. Side effects of systemic pilocarpine administration are sweating, urinary and gastrointestinal disturbance, risk of cardiovascular and pulmonary disorders. This drug must be used carefully by patients with controlled asthma, chronic bronchitis, pulmonary or cardiac disease. Patient with acute asthma, narrow angle glaucoma, iritis should not use pilocarpine. Like this, systemic pilocarpine has many limitations. So, many investigators also have looked at the effectiveness of topical pilocarpine. Here we present patients with xerostomia which was relieved by pilocarpine mouthwash.

Associations between Job Stress and Musculoskeletal Symptoms in Child Care Teachers (보육교사의 직무스트레스와 근골격계 자각증상의 관련성)

  • This study was to conducted to examine the associations between job stress and musculoskeletal symptoms of child care teachers. Questionnaires were administered to 132 child care teachers and the responses were analyzed using the SPSS/WIN 18.0 program. The average total job stress was 2.80. Based on NOISH criteria, musculoskeletal symptoms were prevalent in 46.2% of the participants. The musculoskeletal symptoms by body part appeared mostly on the shoulder (25.8%), followed by the lower back (18.9%), hands/wrist/fingers (17.4%), neck (11.4%), legs/feet (9.1%), and arms/elbows (7.6%). The positive symptom group showed higher levels of total job stress, work overload, and relationship with school parents stress scores than then the symptom free group. Musculoskeletal symptoms were significantly associated with job stress, work overload, and relationship with school parents stress. Overall, the results of this study indicate that it is necessary to develop strategies for reducing job stress to prevent musculoskeletal symptoms.

Characteristics of Classified Congenital Muscular Torticollis Applied Therapeutic Massage and Manual Stretch Therapy During 6 Months (6개월간의 치료적 마사지와 도수신장 치료에 대한 선천성 근성 사경 환아의 유형별 특성)

  • The main purpose of this prospective longitudinal study was to evaluate, according to a standard clinical classification and assessment method, the effect of a standardized manual stretching and therapeutic massage program on patients with clearly defined congenital muscular torticollis. A total of 52 cases of congenital muscular torticollis who were first seen when they were less than one year old and were treated with conservative physical therapy. Before treatment, the patients were divided into one of three clinical groups: (1) sternomastoid tumor group, (2) muscular torticollis group, and (3) postural torticollis group. Of the 52 patients, 32 (61.5%) made up the sternomastoid tumor group; 15 (28.9%) the muscular torticollis group; and 5 (9.6%) the postural torticollis group. Multivariate analysis of the outcomes showed that the duration of treatment was significantly differentiated according to the clinical group (p<0.01), age at presentation (p<0.01), and a passive rotation deficit of the neck (p<0.01). The current study showed that well controlled conservative physical therapy is safe and effective in the treatment of congenital torticollis in about 84.6% of patients who are first seen before the age of one year.

A Successful Therapy of Mite Infection with Topical and Spray Application of Ivermectin in Four Pruritic Pigeons (네 마리의 소양감을 보이는 비둘기에서 발생한 진드기 감염(Mite Infection)의 성공적인 Ivermectin 치료 증례)

  • Four pigeons which lived together of same cage were referred due to excessive feather loss, severe itching, and erythema on the head, neck and flank regions. They had behavioral problem with feather bloating and plucking. Although they were mildly depressed, appetite was normal. On microscopic examination of feather picking, prominent external parasite infection was found. Mite infection was diagnosed with morphological confirmation. On analysis of complete blood count (CBC), eosinophilia was evident. The patients were treated with ivermectin (apply 200 mcg/kg topically two times per every other week and spray 200 mcg/ml solution every week). Clinical signs of four, pigeons were improved 45 days following first therapy. This case report indicates that mite infection is accompanied with severe feather loss, itching, and generalized erythema on the skin and behavioral problem with feather bloating and plucking. And this infection can be managed with topical and spray application of Ivermectin without injection.

The Effect of Smooth Pursuit Eye Movement Using Portable Device to Chronic Hemispatial Neglect: A Pilot Study (휴대용 디바이스를 이용한 완곡추적 안구운동이 만성 편측무시에 미치는 영향: 예비연구)

  • Objective : This study aim is to identify whether smooth pursuit eye movement(SPEM) using portable device can alleviate chronic hemispatial neglect. Methods : We applied smooth pursuit eye movement to one chronic hemispatial neglect patient. Experimental duration was total 4weeks - 1week baseline, 2weeks SPEM intervention, 1week 2nd. baseline. The intervention was 10 SPEM sessions (30min each, 1session daily, from Monday to Friday) over a period of 2weeks. The neglect test carried out 5 times a week. The SPEM was provided on the screen when patient sat in front of the screen(8.4 inch tablet pc, distance 40cm). The SPEM video that the 24 yellow squares moving coherently from the right to the left side. Patients were instructed to perform smooth pursuit eye movement without head and neck movement. Results : As a result of the SPEM for 2weeks, the degree of neglect tended to decrease compared to baseline A. The degree of the LBT tended to increase the at the baseline A'. In contrast, the SCT showed that tendency which the degree of the neglect maintained at the baseline A'. Conclusion : We identified that SPEM using portable device is effective intervention method for chronic hemispatial neglect.

Three-dimensional finite element analysis for influence of marginal bone resorption on stress distribution in internal conical joint type implant fixture (변연골 흡수가 내측연결 임플란트 매식체의 응력분포에 미치는 영향)

  • Purpose: The change of the marginal bone around dental implants have significance not only for the functional maintenance but also for the esthetic success of the implant. The purpose of this study was to investigate the load transfer of internal conical joint type implant according to marginal bone resorption by using the three-dimensional finite element analysis model. Materials and methods: The internal conical joint type system was selected as an experimental model. Finite element models of bone/implant/prosthesis complex were constructed. A load of 300 N was applied vertically beside 3 mm of implant axis. Results: The pattern of stress distribution according to marginal bone resorption was similar. The maximum equivalent stress of implant was increase according to marginal bone resorption and the largest maximum equivalent stress was shown at model of 1 mm marginal bone resorption. Although marginal bone loss more than 1mm was occurred increasing of stress, the width of the stress increase was decreasing. Conclusion: According to these results, the exposure of thin neck portion of internal conical joint type implant is most important factor in stress increasing.

A Case of Intradiscal Oxygen-ozone Injection Therapy for Cervical Herniated Intervertebral Disc in a Dog (개에서 탈출된 경추디스크에 대한 디스크내 오존가스 주입 치료)

  • A 6-year-old castrated male Shih-tzu was presented because of a 1-month history of progressive tetraplegia. On the basis of the findings of neurological examination, radiographs and magnetic resonance imaging, herniated intervertebral disc of C4-C5, C5-C6 and C6-C7 compressed spinal cord. The oxygen-ozone was intra-operatively injected into the C4-C5 and C6-C7 intervertebral disc and a ventral decompression performed at C5-C6 intervertebral space. Immediately after the surgery, neck pain was disappeared and three weeks after surgery, there was complete resolution of the neurological deficits with normal gaits. On magnetic resonance images at 2-month after surgery, herniated disc materials of C5-C6 and C6-C7 were completely removed and that of C4-C5 remarkably reduced. Consequently decompression which was performed at all of the three lesions resulted in good clinical outcome without additional internal fixation for minimizing postoperative instability.
