The Relationship Between Neck Pain and Physical Factors in Female Office Workers (사무직 여성 근로자의 경부 통증과 물리적 요인의 상관성 연구)

  • Objectives : The purpose of this study was to investigate the relation between neck pain and physical factors in female office workers. Methods : Neck pain group of 31 female subjects complained of neck and arm discomfort. Normal group of 20 female subjects had no complaints or minimal discomfort. Cervical curvature and muscle tone were assessed by whole spine x-ray, meridian-electromyography(MEMG), craniovertebral angle. Neck pain was evaluated by Neck Disability Index(NDI) and Visual Analog Scale(VAS). Results : The NDI score and contraction power of upper trapezius by MEMG had a relationship significantly. However, there was no relationship between NDI and cervical curvature. Conclusions : The results suggest that neck pain is related to muscle tone rather than physical stress and cervical curvature.

The Correlations between a Forward Head Posture and the Endurance and Maximal Voluntary Contraction of the Deep Neck Flexor, Neck Pain, and the Changed Position of the Mandible (전방머리자세와 깊은목굽힘근의 지구력과 최대근수축력 및 통증, 아래턱위치변화간의 상관관계)

  • Purpose: This study examined the correlations between a forward head posture and the endurance and maximal voluntary contraction of the deep neck flexor, neck pain, and the changed position of the mandible. Methods: The subjects of this study were 50 male and female adults who work at a desk for at least four hours a day. The head-spine angle was photographed with a camera, and the endurance and maximal voluntary contraction of the deep neck flexor and the changed position of the mandible were measured using pressure biofeedback. The Neck Disability Index was used to measure neck pain. To examine the correlations between a forward head posture and the endurance and maximal voluntary contraction of the deep neck flexor as well as the changed position of the mandible, a Spearman's correlation analysis was conducted. The statistical significance was set at 0.05. Results: A forward head posture and the endurance of the deep neck flexor showed a statistically significant positive correlation, and a forward head posture and neck pain showed a statistically significant negative correlation. In addition, the endurance of the deep neck flexor and neck pain showed a statistically significant negative correlation. Conclusion: The results of this study show that a forward head posture and the endurance of the deep neck flexor were correlated; in addition, a forward head posture and neck pain were correlated. Therefore, enhancing the endurance of the deep neck flexor can assist in correcting an imbalanced forward head posture, which can reduce neck pain.

A Case Report of Warming Acupunture Therapy at Buyang, Golyun and Its Cotreatment with Traditional Korean Medicine in a Traffic Accident Patient with Neck Pain (교통사고 후 발생한 경항통 환자에 대한 부양, 곤륜혈의 온침 병행치료 증례보고)

  • Objectives : The aim of this study was to observe the clinical effect of warming acupunture therapy at Buyang(BL59), Golyun(BL60) in a traffic accident patient with neck pain. Methods : A 32-years-old, female patient with neck pain was treated by warming acupunture therapy at Buyang(BL59), Golyun(BL60) from April 28th to May 5th. The improvement of the patient's neck pain was evaluated by Visual Analog Scale(VAS), Pain Rating Score(PRS) and Neck Disability Index(NDI). Result : After treatment, flexion & extension VAS was decreased from 7.0 to 3.0 and lateral bending VAS was decreased from 7.5 to 4.0. PRS and NDI was improved 70 to 54, 41 to 35, respectively. Conclusions : The result suggests that warming acupunture therapy at Buyang(BL59), Golyun(BL60) may have therapeutic effect on neck pain patients.

Two Patients Who Were Hospitalized for Clavicle Fracture Caused by a Traffic Accident and Improved with Korean Medicine Complex Treatment (교통사고로 발생한 쇄골 골절로 입원하여 한방복합치료로 호전된 환자 2예)

  • To report improvement of 2 hospitalized patients who had clavicle fractures due to traffic accident with Korean medicine complex treatment. We collected a data from February 2020 to April 2022. A total of 2 patients were studied and we compared numeric rating scale (NRS), neck disability index (NDI), range of motion (ROM) at admission and discharge date of hospitalization. After receiving an average of 28.5 days of Korean medicine inpatient treatment, both patients had reduced NRS, NDI scores and increased ROM. After Korean medicine complex treatment, 2 patients with clavicle fracture showed to have been effective. But further studies need to be done.

The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain (8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향)

  • This study examined how the eight-week spinal stabilization exercise program affects the cervical disability index, postural balance ability, and body shape change. The exercise program performed 60 minutes of spinal stabilization exercise three times a week for eight weeks. Sixteen patients with chronic neck pain, who complained of neck pain for six months, were classified into exercise group (n=8 patients) and control group (n=8 patients). The results before and after the eight-week exercise program were observed. Significant differences were observed in the time, group, and interaction of the neck disability index (p<.05). The balance ability showed significant interaction effects between the groups and periods (p<.05). Significant differences were noted in the timing and interaction in the pelvic inclination angle in posture change (p<.05), and there were significant differences in the group, timing, group, and interaction in the cervical and shoulder position angles (p<.05). The above results showed that the spinal stabilization exercise significantly improved the cervical disability index, balance ability change, and body shape change in patients with chronic neck pain. Future studies will analyze the specific changes in spinal structure through radiographic imaging to increase the validity of spinal stabilization exercise.

Comparison of effectiveness for fluoroscopic cervical interlaminar epidural injections with or without steroid in cervical post-surgery syndrome

  • Background: Neck and back pain are leading sources of disability placing substantial burden on health care systems. Surgical interventions in managing chronic neck pain secondary to various disorders continue to increase. Even though surgical interventions are effective, a significant proportion of patients continue to have symptomatology and develop cervical post-surgery syndrome. This study was performed to know the effectiveness of cervical interlaminar epidural injections with or without steroids. Methods: The effectiveness of fluoroscopic cervical interlaminar epidural injections in post-surgery syndrome was evaluated in a randomized, active controlled trial. The study population included 116 patients assigned to 2 groups. Group 1 received cervical interlaminar epidural injections with local anesthetic alone and Group 2 received injection with local anesthetic and steroids. The main outcomes were defined as significant improvement (greater than 50%) of pain relief using the numeric rating scale and/or functional status improvement using the Neck Disability Index (NDI). Results: Both groups had similar results with significant improvement (${\geq}50%$ pain relief and functional status improvement) in 69% of the patients in Group I, whereas, in Group II, 71% of the patients showed significant improvement at the end of 2 years. During a 2-year period, the average number of procedures was 5 to 6, with an average of approximately 12 weeks of significant improvement per procedure. Conclusions: Fluoroscopic cervical interlaminar epidural injections administered in cervical post-surgery syndrome using local anesthetic, regardless of the use of steroids, may be effective in approximately 70% of the patients at 2-year follow-up.

A Study on the Effect of Cryotherapy and Joint Mobilization Leading in Patients With Acute Whiplash-Associated Disorder by Traffic Accident (교통 사고에 의한 급성 편타성 관련 손상 환자에서 냉각치료와 관절가동술의 효과 연구)

  • Purpose: Whiplash-associated disorder is often caused by external impact, such as a car accident, and it involves acceleration and deceleration of the flexion and extension of the neck. The purpose of this study is to determine whether the combination of cyrotherapy and a joint mobilization intervention in patients with acute whiplash-associated disorder is effective for pain, cervical range of motion (ROM), and muscle tone in the neck muscles. Methods: In this study, 20 patients with acute whiplash-associated disorder were randomly assigned to two groups. Group A was treated with cyrotherapy for 5 minutes, while group B was treated with cyrotherapy and a joint mobilization intervention. In both groups, visual analogue scale (VAS), neck disability index (NDI), cervical ROM, and muscle tone values were measured pre- and post-intervention. Results: According to the VAS values, both groups showed significant differences pre- and post-intervention (p<0.05), and in both groups, NDI was significantly decreased pre- and post-intervention (p<0.05). Finally, in terms of muscle tone, both groups showed significant changes before and after the intervention (p<0.05). Conclusion: These results showed that cryotherapy and joint mobilization can be effective interventions in patients with whiplash-associated disorder. In addition, to improve the ROM of the cervical joint and reduce NDI, the combination of cryotherapy and joint mobilization is considered more effective than the single intervention of cryotherapy.

The Effect of Intensive Integrative Korean Medicine Treatment of Patients with Adrenoleukodystrophy: A Case Report (부신백질이영양증 환자에 대한 한의 복합 치료의 효과: 증례보고)

  • The aim of this case report was to present the effects of intensive integrative Korean medicine treatment in a patient with lower back pain, both under extremity numbness, neck pain induced by adrenoleukodytrophy. A 49-year-old male patient was treated with Cheongpajeon-H, acupuncture, pharmacoacupuncture (Shinbaro2), chuna therapy for total 50 days of hospitalization. Lower back pain, both under extremity numbness, neck pain were assessed with a Oswestry disability index (ODI), Vernon-Mior neck disability index (NDI), numeric rating scale (NRS) and EuroQoL-5 dimension (EQ-5D). With 50 days of treatment, the pain was relieved from NRS 6 to 2. EQ-5D score increased from 0.75 to 0.779, ODI decreased from 34 to 15.56, NDI decreased from 30 to 15.56. These results show that lower back pain, both under extremity numbness, neck pain can be relieved with intensive integrative Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

The relationship between neck pain and psychological state in female office workers. (경항부 통증과 심리적 상태와의 상관성에 관한 연구)

  • Objectives : The purpose of this study was to investigate the relationship between pain and psychological state in female office workers with neck pain. Methods : Experimental group of 31 healthy subjects complained of neck and arm discomfort related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Outcomes were assessed by meridian-electromyography(MEMG), Neck Disability Index(NDI), Visual Analog Scale(VAS), Beck Depression Inventory(BDI), Stress Reaction Inventory(SRI), Holmes & Rahe Social Readjustment Rating Scale(SRRS). Results : The NDI score and BDI score had a correlationship(r=0.48), and the NDI score and SRI score also had a correlationship(r=0.48), significantly. The NDI score and contraction power of upper trapezius by MEMG had a correlationship significantly, but with the other muscles the NDI score didn't have correlationships. The NDI score and the SRI score were significantly higher in depression group(BDI score ${\circ}{\surd}$14). The contraction power of trapezius by MEMG was significantly lower in depression group. Conclusions : The results suggest that mental stress can be a major risk factor of neck pain in female office workers.

  • Background: The purpose of this study was to effect of action observation with deep neck flexors strengthening exercise on pain and function in patients with chronic neck pain. Methods: Thirty patients with chronic neck pain were randomly action observation with deep neck flexors strengthening exercise group (AOT group; n=15) and deep neck flexors strengthening exercise group (FSE group; n=15). The intervention was applied for 50 minutes a day, 5 times a week for a total of 4 weeks. The intervention effect were evaluated before and after the intervention. visual analogue scale (VAS), pressure pain threshold (PPT), and Korea neck disability index (KNDI). Results: As a result of comparing within group, AOT group showed pre and post test, VAS (p<.01), PPT (p<.01), KNDI (p<.01) and FSE group VAS (p<.01), PPT (p<.05), KNDI (p<.01) significantly. In the comparison between the two groups, AOT group than FSE group in VAS, PPT and KNDI significantly (p<.05). Conclusion: Action observation with deep neck flexors strengthening exercise were founded to effective in reducing pain and improving function in patients with chronic neck pain. It is suggested as basic data of clinical intervention.
