중환자실 간호사의 수면장애가 피로, 졸음과 근접오류에 미치는 영향 (Effect of Sleep Disturbance on Fatigue, Sleepiness, and Near-Miss among Nurses in Intensive Care Units)

  • Purpose : This study aims to investigate the differences in fatigue, sleepiness, and near-miss according to sleep disturbance among shift nurses in intensive care units (ICUs). Methods : A cross-sectional study in a tertiary hospital was performed. A total of 122 shift nurses working in the six ICUs were recruited. They completed self-reported questionnaires about sleep disturbance, fatigue, sleepiness, and near-miss in the past two weeks. Results : The prevalence of reported sleep disturbance was 30.3% (37 out of 122 subjects). Compared to the non-sleep disturbance group, the sleep disturbance group reported significantly more sleepiness (11.46 vs. 8.86) and higher fatigue (82.62 vs. 69.39). The sleep disturbance group showed higher rates of near-miss (78.4 vs. 57.6%) and a higher frequency of them (4.49 vs. 2.11/2weeks) compared to the non-sleep disturbance group. Medication error was the most common type of near-miss. Conclusions : This study suggests that sleep disturbances could increase fatigue, sleepiness, and near-miss among ICU shift nurses. Personal and organizational programs should be developed to support the sleep of ICU nurses.

  • Near / Miss, Charles Bernstein's poetry collection, is replete with poems of distinctive styles and pluralistic forms in his idiosyncratic and artistic cosmos. With poetic antics, queerness, sarcasm, irony, and humor, the book showcases the motif of loss, chaos and trauma in postmodern America and the world. The multiplicity and multi-dimensional $M{\ddot{o}}bius$ effect in Near / Miss echo earlier Bernstein's poems, as well as poems by ancient and contemporary poets, with visual artists and musicians, and rabbis and Jewish philosophers. I argue that Near / Miss offers an apotheosis of echopoetics, which has been launched in his previous book Pitch of Poetry. Poems in the book reveal the dark and thick "pitch," namely the queer, the uncanny, the invisible, the disabled, the dispossessed, and the silenced poetic Other and make it explicit. The estrangement and alienation of $clich{\acute{e}}$ through diverse malaprops, mondegreens, non-sequiturs and fragmentations in Near / Miss aim at deconstructing the fixation of language so as to display the poetic Other. The motif of "nothingness" in echopoetics significantly multiplies its meanings. Nothingness mainly refers to the loss of origin, the defiance of tyranny, and the sublimity of the universe and the poetic Other. Melding his personal loss and misfortune, the current political discontent and the postmodern chaos in America and the world, nothingness in echopoetics resonates with American literary tradition and Zen with a healing and transforming power.

베이지안 네트워크를 이용한 아차사고 평가 모델 개발 및 주요 원인 도출 (Development of Near miss Assessment Model Using Bayesian Network and Derivation of Major Causes)

  • The relationship between near misses and major accidents can be confirmed using the ratios proposed by Heinrich and Bird. Systematic reviews of previous national and international studies did not reveal the assessment process used in near-miss management systems. In this study, a model was developed for assessing near misses and major factors were derived through case application. By reviewing national and international literature, 14 factors were selected for each dimension of the P2T (people, procedure, technology) model. To identify the causal relationship between accidents and these factors, a near-miss assessment model was developed using a Bayesian network. In addition, a sensitivity analysis was conducted to derive the major factors. To verify the validity of the model, near-miss data obtained from the ethylene production process were applied. As a result, "PE2 (education)," "PR1 (procedure)," and "TE1 (equipment and facility not installed)" were derived as the major factors causing near misses in this process. If actual workplace data are applied to the near-miss assessment model developed in this study, results that are unique to the workplace can be confirmed. In addition, scientific safety management is possible only when priority is given through sensitivity analysis.

  • Near miss reporting is essential for improving safety performance in construction organizations. Traditional methods, however, often fail to sufficiently motivate worker participation due to a lack of incentives and the absence of secure, reliable, and transparent reporting mechanisms. This paper introduces a conceptual framework that leverages blockchain technology and token incentives to increase employee involvement in near miss reporting. The integration of blockchain ensures transparency and decentralization, while token incentives effectively monetize employees' efforts in reporting near misses. The study begins by identifying key modules essential for developing this framework and proceeds to detail their integration, illustrating a comprehensive process flow of activities within the system. This research significantly contributes to the enhancement of near miss reporting practices in construction, ultimately fostering safer and more proactive workplace environments.

  • In this study, a new technique for detecting near miss using 4M risk assessment method is suggested. Until now, the safety education with instances of near miss has just been progressed in most industrial settings, without any systematic guideline. By menas of appling 4M risk assessment method, the organized technique, which could effectively manage the fundamental prevention of industrial accident in advance, is developed. The organized technique of near miss-management suggested in this study will take an effective role in basically expanding the application of risk assessment method, as well as in contributing the activity of zero-accident as a safety guideline in hazardous workshops.

건설현장의 아차사고 연결가능성에 대한 패턴분석 (A Pattern Analysis on the Possibility of Near Miss Connection in Construction Sites)

  • 연구목적: 아차사고의 분석을 통하여 재해를 예측하여 사고를 예방하는 목적이 있다. 연구방법: 본 연구에 서는 건설 현장의 아차사고 문헌조사 및 데이터를 수집하고, 설문조사를 실시하여 아차사고 연결 가능성 분류를 위해 로지스틱 회귀분석 및 의사결정나무 분석을 이용하였다. 연구결과: 아차사고 types이 정신적, 신체적, 안전습관 행동에 미치는 영향을 분석한 결과, 신체에 영향력이 높은 요인은 아차사고 관리의 필요성, 직종은 전기·정보통신, 건강 상태 순으로, 정신적 요인에서 공사 규모가 영향력이 높았으며, 경험 공종, 중상자 수, 직종 순으로 습관 행동 요인에 영향력이 높은 요인은 착각, 부적절한 작업지시, 신체 부위 순으로 분석되었다. 의사결정나무 분석을 통해 아차사고가 놀랄 정도의 사고로 연결가능성에 영향을 미치는 요인과 패턴을 확인하였다. 결론: 건설현장관계자는 아차사고 관찰을 고려하여 정신적. 신체적 측면의 아차사고와의 연관성에 대한 구체적 관리가 실행되어야 하며, 부적절한 작업지시가 아차사고로 연결되지 않도록 인원 배치, 작업계획, 작업절차 및 방법, 피드백을 통해 중대재해가 저감하는 작업환경을 기대한다.

A Study on the Relationship between Marine accidents and Near-miss using PARK model

  • Marine accidents often stem from near-miss incidents, which are critical indicators of potential collisions. Understanding the causal factors associated with these incidents is essential for enhancing maritime safety. This study employs the PARK model to identify causal factors related to maritime accidents, with a focus on near-miss incidents in Busan Port. By analyzing AIS data from 2017 to 2021, the research identifies 36 collision incidents. The PARK model assesses risk levels, setting a threshold of 5.0 for near-miss scenarios and examines the relationship between these incidents and actual collisions. The analysis indicates an average of 0.8 collisions per year, with the PARK model estimating a probability of approximately 2.34 collisions annually per million vessel encounters at a risk score of 5.0. This research highlights the effectiveness of the PARK model in identifying causal factors related to marine accidents. By correlating near-miss incidents with actual collision probabilities, the study offers insights that can improve reporting practices and proactive safety measures among vessel operators, ultimately contributing to enhanced maritime safety in South Korea.

Relationship Between Companies' Responses to Near-Miss Reports and Turnover Intentions of Workers: A Nationwide Cross-Sectional Study

  • Background: Effective near-miss management is important in preventing workplace accidents. A company's inadequate response to near-miss reports can lead workers to feel insecure and dissatisfied with the company. We investigated the relationship between companies' responses to near-miss reports and turnover intentions of workers. Methods: We conducted a cross-sectional study using online self-administered questionnaire survey to workers aged ≥20 years in Japan in March 2022. The analysis included 5,071 participants who had nearmiss experiences and reported them to their companies. The independent variable was companies' responses to near-miss reports, classified into three categories: adequate response group, inadequate response group, and no response group. The dependent variable was turnover intentions. We calculated the odds ratio and 95% confidential interval (CI) using multilevel logistic regression analyses nested for industries and adjusted for covariates. Results: Of the 5,071 participants, 3,058 (60.3%) were adequate response group, 1,484 (29.3%) were inadequate response group, and 529 (10.4%) were no response group. In multivariable adjusted model, compared with adequate response group, the odds ratio of inadequate response group and no response group were 1.80 (95% CI: 1.56-2.08) and 2.63 (95% CI: 2.15-3.22), respectively. Conclusion: Our results suggested that there was a relationship between companies' responses to the near-miss reports and turnover intentions of workers. It is important not only to collect near-misses but also to respond appropriately to the reports and provide feedback to workers.

중소병원 간호사의 투약 근접오류경험 영향요인 (Influencing Factors of Near Miss Experience on Medication in Small and Medium-Sized Hospital Nurses)

  • 본 연구는 중소병원 간호사의 투약 근접오류경험의 영향요인을 확인하여 근접오류 예방과 안전한 투약간호를 위한 교육프로그램 개발의 기초자료를 제공함으로써 중소병원의 환자안전문화 구축에 기여하고자 시도된 서술적 조사연구이다. 수집된 자료의 분석은 SPSS/WIN 20.0을 이용하여 𝑥2-test, Independent t-test, one-way ANOVA, Logistic regression analysis를 실시하였다. 연구결과, 투약 근접오류경험에 영향을 미치는 요인은 일반적 특성 중 근무부서와 환자안전문화였다. 모수 추정치 값의 승산비(odds ratio)는 특수부서 근무자보다 일반병동 근무자가 근접오류를 경험하지 않을 교차비가 2.23(95% 신뢰구간: 1.07~4.67, p=.032)이었으며, 환자안전문화 점수가 1점 증가할 때 근접오류를 경험하지 않을 교차비가 2.24(95% 신뢰구간: 1.02~4.95, p=.045)인 것으로 나타났다. 즉, 특수부서 근무자가 일반병동 근무자보다 근접오류를 경험할 확률이 높고, 환자안전문화 인식정도가 높을수록 근접오류를 경험할 확률이 낮은 것을 알 수 있었다. 따라서 간호사들의 환자안전문화 인식 개선을 위한 병원차원의 오류감시시스템의 개발과 간호조직 차원에서 경력 및 부서별 특성에 따른 맞춤형 투약교육 프로그램을 개발하여 이론 교육과 함께 시뮬레이션 훈련이 필요할 것으로 사료된다.

국내 제조업의 아차사고 발굴활동 현황 및 인식에 관한 연구 (A Study on the Status and the Perception of Near Miss Reporting Activities in Domestic Manufacturing Industry)

  • 아차사고란 재해가 일어날 수 있는 상황이 발생하였으나 인원의 부상이나 자산의 손상 등 직접적인 손실로 이어지지 않은 사건을 의미한다. 재해 방지에 있어 아차사고의 중요성은 충분히 강조되어 왔지만, 아차사고를 발굴하기 위한 활동과 그에 대한 인식에 대해서는 알려진 것이 많지 않다. 본 연구는 국내 제조업 사업장들의 아차사고 관련 활동 현황을 조사하고, 이러한 활동이 산업재해의 발생 및 안전문화와 어떠한 관련이 있는지 밝히는 것을 목표로 한다. 이를 위해 제조업 사업장을 대상으로 설문조사를 실시하여 그 결과를 분석하였다. 분석 결과, 아차사고 발굴활동을 수행하는 사업장은 조사대상의 56%로 절반을 약간 넘는 것으로 나타났으며, 발굴활동을 수행하는 사업장과 그렇지 않은 사업장의 아차사고에 대한 인식 수준에는 상당한 차이가 있었다. 또한, 아차사고의 건수는 아차사고 발굴에 대한 포상 여부에 따라 차이가 있는 것으로 나타났다. 아차사고 발굴활동의 실시 여부와 산업재해 발생 이력 간에는 관계를 발견할 수 없었지만, 안전문화 수준은 발굴활동을 수행하는 사업장이 수행하지 않는 사업장에 비해 더 높은 것으로 나타났다. 본 연구는 국내 제조업 사업장의 아차사고에 대한 인식과 발굴활동의 개략적인 현황을 파악했다는 데 의의가 있으며, 이를 기초로 추후 아차사고 발굴활동과 재해 발생 간의 연관관계검증 및 아차사고 발굴활동의 효과 제고를 위한 후속 연구가 필요할 것으로 보인다.
