수술후 화학요법받는 위암환자의 부작용 증상, 피로 및 삶의 질과의 관계 (The Relationship of Symptoms of Side Effects, Fatigue and Quality of Life in Stomach Cancer Patients receiving Chemotherapy)

  • Purpose: The purpose of this study was to investigate the level of nausea, vomiting, anorexia, fatigue and quality of life as well as to determine the relationship between those variables and identify the influencing factors on fatigue and quality of life in stomach cancer patients receiving adjuvant chemotherapy. Method: Subjects were 94 stomach cancer patients undergoing postoperational chemotherapy in a general hospital in Seoul. Nausea and vomiting were measured with Rhodes et al(1984) and anorexia with one 5-point item. Fatigue was measured using Lee's tool(1999) except open questions. The tool for quality of life was modified based on the Quality of Life Index by Padilla et al(1983). Result: Subjects reported low level of fatigue(mean=3.86, range=0-10) compared with the results of previous researches and moderate quality of life(mean=2.64, range=1-4). Fatigue was positively correlated with nausea, vomiting and anorexia(r=.21 ~ .55, p<.05). Quality of life was negatively correlated with nausea, vomiting, anorexia(r= -.24 ~ -.45, p<.05) and fatigue (r=-.61, p<.01). Multiple regression analysis revealed that activity level, vomiting before admission, anorexia during chemotherapy and age explained 52.8% of the variance in fatigue. Fatigue, anorexia before admission, age and sex explained 50.5% of the variance in quality of life. Fatigue and quality of life were not influenced by the stage of disease, nor weight change. Conclusion: These results may contribute to a better understanding of how much the side effects of anticancer drugs can affect fatigue and quality of life in cancer patients undergoing chemotherapy. Also it is the remarkable fact that symptoms remaining after discharge such as vomiting or anorexia continued until re-admission, contributing to patients fatigue and lowered their quality of life.

말기(末期) 위암(胃癌)환자 11례에 대한 증례보고 (The Clinical Observation on 11 Cases of patient with terminal stage of Gastric cancer)

  • Objective : Gastic cancer has become major cancers which cause nausea, vomiting. Especially patients with terminal stage of gastric cancer may suffer from nausea, vomiting and other symptoms that can keep patients from taking medicine or food. In those cases, there may be no use of taking herbal medicine to treat or palliate symptoms. So we wanted to know the potential efficiency of Acupuncture and Moxibustion whether they could control the symptoms of terminal stage of Gastric cancer without herbal medicine. Methods : Under the assumption that Acupuncture and Moxibustion may be effective for palliating nausea, vomiting on terminal stage of Gastric cancer, the following points were administrated SaGwan(Hapkok($LI_4$), Taechung($LR_3$)), Chok-Samli($ST_{36}$), Kongson($SP_4$), Naegwan($PC_6$) for Acupuncture, Chungwan($CV_{12}$) for Moxibustion. This observation was carried out on 11 patients with terminal stage of Gastric cancer. We reviewed medical records, specifically intake/output check with vomiting, nausea. Results : After therapy of Acupunture and Moxibustion, there were 22% of complete responses, 46% of major responses and 32% of failures. Therapy resulted in 2 cases of goodness, 4 cases of fairness, 5 cases of badness as satisfaction degree. Unfortunately 2 cases of badness expired. Conclusion : We have concluded that Acupunture and Moxibusiton therapy were effective to palliate the nasea, vomiting of terminal Gastric cancer. So if Gastric cancer develop difficulties of taking medicine with patients, to consider using the methods of Acupunture and Moxibution is worthy to palliate the nausea, vomiting and so on.

  • Purpose: The purpose of this study was to evaluate the effects of progressive muscle relaxation on nausea, vomiting, fatigue, anxiety, and depression in cancer patients undergoing chemotherapy. Methods: This study was a quasi-experimental research using non-equivalent control group pretest-posttest design. Data were collected from outpatients in a university hospital from April to August, 2009. There were 74 participants, 39 in the experimental group and 35 in the control group. The experimental group was given daily the progressive muscle relaxation for 20 min during three weeks. The structured questionnaire was used to measure nausea, vomiting, fatigue, anxiety, and depression. Data were analyzed using the SPSS/WIN 12.0 program, $x^2$-test, Fisher's exact test, t-test and ANCOVA were conducted to examine the homogeneity and the research hypotheses. Results: There were statistically significant decreases in anxiety and depression in the experimental group compared to the control group. However, there were no significant differences in nausea, vomiting, and fatigue between the groups. Conclusion: In this study, progressive muscle relaxation was effective in alleviating anxiety and depression of cancer patients undergoing chemotherapy. Therefore, progressive muscle relaxation can be usefully utilized as a nursing intervention that enhances psychological function of cancer patients.

마취 중 내관 침 시술이 수술 후 오심구토 예방에 미치는 효과 (Acupuncture on PC6 in Anesthesia also Shows Effect for Preventing Post Operative Nausea and Vomiting)

  • This study aims to investigate the effect of acupuncture on PC6 for preventing opioid-induced nausea and vomiting under circumstance whether the participants are conscious or non-conscious. 143 patients receiving intravenous patient controlled analgesia(PCA) with fentanyl were assigned to three groups. Two groups were given acupuncture on PC6 before anesthesia(PreAN) or after anesthesia(PoAN) respectively. One group was control group without acupuncture treatment(NC). Assessment of nausea and vomiting was obtained from all patients for 48hours after surgery. The incidence of nausea and vomiting was significantly lower in the PreAN group and PoAN group than in the NC group. The severity of nausea was the lowest in PreAN group, second to in PoAN group, and the highest in NC group. This results show that acupuncture on PC6 even without patients' consciousness has effect on preventing PONV.

오심, 구토, 복통으로 인해 급격한 체중감소를 보인 환자 치험 1례 (A Case Report of a Patient with Rapid Weight Loss due to Nausea, Vomiting and Stomachache)

  • This patient was 18 year-old man who had severe nausea, vomiting, stomachache but medical examination revealed no abnormality. The symptom lasted for 2 months and he lost 15kilograms. The patient had no underlying diseases. He was treated by korean medicine, containing acupuncture, cupping therapy and herb medicines during 9 days. The severity of nausea was scored by the Numerous Rating Scale and the frequency of vomitng was evaluated by count. After that period, patient's symptom was improved and his other conditions were also better, compared with his admission. This study suggests that Korean medicine could have a therapeutic effect for nausea, vomiting, stomachache and rapid weight loss that the cause has not been diagnosed by Western medicine. It could help to improve patient's symptoms and make general conditions better.

부인과 복강경 수술 대상자에서 Desflurane 마취와 Desflurane-remifentanil 병용마취 시 수술 후 통증과 오심.구토의 비교 (Comparison of Postoperative Pain and Nausea and Vomiting between Desflurane and Desflurane-remifentanil Anesthesia for Gynecologic Laparoscopic Surgery)

  • Purpose: The purpose of this study was to compare desflurane anesthesia with desflurane-remifentanil anesthesia by measuring postoperative pain, nausea and vomiting after gynecologic laparoscopic surgery. Methods: Data were collected using a random double blind method. The experimental group treated with desflurane-remifentanil anesthesia had a total of 25 patients. The control group had 20 patients who were treated with desflurane anesthesia. Pain (VAS), pushing PCA button, nausea and vomiting (VAS) were assessed at PACU (Post Anesthesia Care Unit) arrival, 10 min, 20 min, 30 min in PACU and 2 hrs, 6 hrs, 24 hrs, 48 hrs in ward. INVR was assessed at 24 hrs. Extra analgesics and extra antiemetics were measured in PACU and the ward. Data were analyzed using t-test and repeated measure using ANOVA. Results: The experimental group presented with more postoperative pain (F=7.55, p<.001) than the control group. The experimental group took more extra analgesics in PACU and pressed the PCA button more often for 6 hours. The experimental group complained more postoperative nausea (F=2.11, p=.043) than the control group and took extra antiemetics during postoperative period of 24 hours. Conclusion: We conclude that desflurane-remifentanil anesthesia needs better management than desflurane anesthesia due to increased pain, nausea & vomiting after gynecologic laparoscopic surgery.

Prevention of Nausea and Vomiting: Methods and Utility after Surgery in Cancer Patients?

  • Most cancer patients experience nausea and vomiting after surgery. Today, many methods of treatment have been developed and used for the control of such symptoms. The most important are drug therapy, relaxation, oxygen therapy and gas therapy. In addition, dexamethasone, massage therapy and using a Venturi mask have also proven effective. Due to the nature of gas consumption which leads to nausea it is recommended that use of N2O in the operating room be avoided or applied in combination with oxygen or other gases with fewer complications.

  • The incidence of chemothrapy related among pediatric cancer patient was 90.1%. Adequate oral intake and nutrition have been shown to be important. These consideration prompted the decision to survey by means of a questionaire. The questionaire were included nausea-vomiting peak time, causing factor, coping method, education need, diet pattern change and food preference. Results are fellow 1. Almost(90.1%) pediatric cancer patient experienced nausea-vomiting during chemotherapy and required coping method or reducing method. 2 . The food preference form were Identified. Those were fluid form, cold and small amout and frequentry eating form. The patients preferred noodles, chickens, soap, juice. The results of the survey indicate that nasea-vomiting relief nursing intervention are required pediatric cancer patient received chemotherapy. Health care personnels recognize the pediatric cancer patient's diet pattern and encourage the nutritional counselling. The care of patient should be multidisciplinary team approach and the nurse occupies a key position with in this team, which includes the pediatrician, nutrionist.

임신오조 2례에 대한 임상적 고찰 (Clinical study of the two cases of the Hyperemesis)

  • Background: We had treatment for two cases of Hyperemesis Gravidarum . they had difficulty in taking herbal medcine(Gamibosengtang), because of bitter taste and increased nausea and vomiting. So We used acupuncture treatment for increasing of taking herbal medicine and reduction of nausea and vomiting. Methods: The patients estimated their dgree of chief complain on a visual analogue scale and change of oral intake and change of frequency of vomiting. Result: Herbal treatment andacupuncture treatment was efficacious for Hyperemesis Gravidarum. Conclusion: More study fo herbal medicine is needed because of bitter taste.

Acupuncture effect on Chemotherapy-induced Vomiting and Nausea: A case series

  • Objectives: We carried out an observational study of the effects of Nei Guan (P6) and Gong Sun (SP4) acupuncture for the treatment of CINV, preparing for further randomized controlled trial study. This is a case series to explore the changes in the incidence of CINV by acupuncture. Methods: Patients reported a Rhodex index indicating the severity of nausea and vomiting and loss of appetite, before acupuncture, after acupuncture and 1-week follow-up examination. Twelve patients with CINV participated in this study. We included patients receiving chemotherapeutic agents that might induce moderate or severe nausea and vomiting. We conducted 5 treatment sessions with P6 and SP4 acupuncture over the course of 5 days. Results: The median Rhodex score were decreased in 5 patients over time, while we observed loss of appetite of 6 patients were improved over time. During the study, the mean of Rhodex of total patients revealed a decrease of Day 1, Day 7, Day 14; 0.74, 0.68, 0.38, respectively, while the mean of loss of appetite showed a decrease of Day 1, Day 7, Day 14; 5.25, 4.08, 4.00, respectively. Conclusions: The study gives a preliminary data that manual acupuncture of P6 and SP4 may reduce the severity of CINV and loss of appetite, and justifies further study.
