개별 치아 트레이의 재료와 형태에 따른 인상체의 정밀성에 관한 연구 (A STUDY ON THE ACCURACY OF THE IMPRESSION BODY ACCORDING TO MATERIALS AND FORMS OF THE INDIVIDUAL TOOTH TRAY)

  • Accurate impression is very important to achieve desirable prosthesis and there are many factors in taking a good impression. For example, types of impression material, types of impression tray, impression taking methods and so on. Recently individual tooth tray technique is accepted as obtaining good impression that can be applied to multiple abutment impression, heavy salivated patient, to minimize the effect of natural teeth s undercuts and to reduce pain during cord packing procedures. The purpose of this study was to compare the accuracy according to materials and forms of the individual tooth tray which is clinically applied nowadays. Used materials in experiment were divided into 3 types (acrylic resin. $Futar^{(R)}$ occlusion. $Blu-mousse^{(R)}$) and forms were divided into 2 types (forming occlusal vent hole or not and forming marginal vent space or not). Stone master model from impression body and metal master model were measured by $X-PLAN360d^{(R)}$ to compare occlusal surface discrepancy and marginal discrepancy. The results were as follows: 1. In comparison of occlusal surface discrepancy and marginal discrepancy according to materials, groups with three materials showed no statistical difference 2. In comparison of occlusal surface discrepancy and marginal discrepancy according to occlusal vent hole, groups with occlusal vent hole showed significantly less marginal discrepancy than groups with no occlusal vent hole(p<0.05). 3. In comparison of occlusal surface discrepancy and marginal discrepancy according to 0.5mm-marginal-vent-space, groups with no 0.5mm-marginal-vent-space showed significantly less marginal discrepancy than groups with 0.5mm-marginal-vent-space (p<0.05). In summary these results suggest that individual tooth tray made of 3 types of materials with occlusal vent hole and individual tooth tray made of acrylic resin with no marginal vent space showed good accuracy of impression. In addition, individual tooth tray which is made of bite registration materials may be more useful because of advantage in facility and timesaving aspect of fabrication.

치조제 결손부 증대를 위한 새로운 결합조직 이식술 (Novel Connective tissue graft technique for Ridge Augmentation in case of Conventional fixed partial denture : Case reports)

  • 치아의 상실로 인한 국소적 치조제 결손(localized alveolar defect)은 고정성 국소의치(fixed partial denture) 제작시에 많은 기능적, 심미적 문제점들을 야기한다. 특히 전치부에서의 치조제 결손 부위는 그 형태와 양상에 따라서 심미적 결과에 많은 영향을 미치게 되는데, 이 결손 부위의 회복 여부가 최종적인 심미적 결과의 성패를 좌우하게 된다. 치조제 결손의 양상은 발치 전 치조골의 상황과 염증 정도에 따라 다양하게 나타날 수 있는데, 그 양상은 크게 보면 수평적, 수직적, 혹은 복합적 결손 등으로 분류할 수 있다. 이러한 결손부의 형태와 양에 따라서 우리가 회복시켜 주어야 할 정도도 다르고, 그 난이도 또한 다르다고 할 수 있다. 여기에 추가적으로 고려되어야 할 요소가 결손부와 자연치가 만나는 proximal gingival height이다. 인접 치아와 접하는 gingival height는 그 부위의 치조골 높이에 많은 영향을 받게 되는데, 만약 이 부위에 결손이 존재한다면 역시 심미적 결과에 부정적인 영향을 줄 것이다. 하지만 결손부위 내에서도 치아와 인접한 부위의 gingival height를 동시에 증대시키기 위해 소개된 technique은 많지 않으며 시술의 예지성도 높지 않은 것이 사실이다. 이 case report에서 필자는 수평적 혹은 수직적 결손 및 복합적 결손에 다양하게 적용 가능하면서도 인접치아와 인접한 부위의 gingival height에도 도움이 될 수 있는 술식을 소개하려고 한다.

치은섬유아세포와 치은인대세포의 형태와 화학주성에 미치는 대조추출물의 효과에 관한 연구 (A STUDY OF THE EFFECT OF ZIZYPHUS FRUCTUS EXTRACTS ON MORPHOLOGY & CHEMOTAXIS OF GINGIVAL FIBROBLAST & PERIODONTAL LIGAMENT CELLS)

  • The most important object of periodontal treatment is the perfect regeneration of destructed periodontal tissue. The healing of periodontal lesion is affected by several cells & factors, which result in formation of long juntional epithelium, root resorption, bony ankylosis or connective tissue attachment. And ideal healing is enhanced by epithilial exclusion or periodontal ligament cell activation. In this investigation, I studied the effect of Zizyphus Fructus extract which enhances biologic activity& collagen synthesis, on the chemotaxis & cell nature. The cells were obtained from interdental area & middle third area of the freshly extracted teeth for the orthodontic purpose. And they were fully incubated in${\alpha}-MEM$ solution containing $100{\mu]g/ml$ penicillin & $100{\mu]g/ml$ streptomycin followed by 6 generation incubation. The test cells were collected by trypsin-EDTA & centrifuge in the fully incubated cells, counted by Hernacyotmeter, incbated $5{\times}10^5/ml$ cells for 24 hours, re-incubated 24 hours in media containing natural extract and photographed. The cells were incubated for 4 hours in 48 well microchemotaxis chamber bisecting upper & lower chamber by 8ug/m pore polycarbonate membrane coating 5mg/ml gelatin solution. The migrated cells in microscope were counted, which meaned cell chemotaxis activity. The study had shown that the morphology of cell was spindle-shaped as the control group, and the subextract test groups were not significantly different. In gingival fibroblasts, the chemotaxis effect of PDGF was statistically significant compared to control group. The Zizyphus Fructus extract was more or less enhanced chemotaxis effect and in $1{\mu}g/ml$ concentration the chemotaxis effect was slightly elevated compared with $10{\mu}g/ml$ concentration. But, among the subextracts, it was not significantly defferent. In PDL cells, the chemotaxis effect of PDGF in statistically significant, and the zizyphus Fructus extract had shown the enhanced effect. The effect was slightly higher in $1{\mu}g/ml$ concentration than 10g/ml concentration,and no significance among the subextracts.

완전 무치악 환자에서 자유 착탈가능 임플란트 지대주를 이용한 임플란트 융합 국소의치 수복증례 (Implant-assisted removable partial denture using freely removable abutment in a fully edentulous patient: A case report)

  • 완전무치악환자의 보철 치료를 위한 방법 중에서 임플란트 지지 상부보철물을 서베이드 금관의 형태로 제작하여 국소의치의 지대치로 활용하는 임플란트 융합 국소의치(Implant-Assisted Removable Partial Denture, IARPD) 치료가 많은 관심을 받고 있다. 이 치료 방법에서 지대치로 사용되는 임플란트 상부보철물은 유지 형태에 따라 전통적으로 나사 유지형과 시멘트 유지형으로 분류되며 각각 장, 단점이 존재한다. 시멘트 제거, 수리의 어려움, 나사구멍(screw hole)으로 인한 비심미성 등을 극복하기 위해 최근에 새로운 유지 형태의 임플란트 보철 시스템인 EZ crown system (Samwon DMP, Yangsan, Korea)이 소개되었다. 이 시스템은 fixture에 체결되는 나사 일체형의 특수 abutment와 니켈-티타늄 스프링과 지르코니아 볼이 포함된 cylinder 구조물을 microlocking 방식으로 연결하여 유지력을 얻는다. 이번 증례의 환자는 하악 피개의치의 지대치가 발거되어 새로운 의치를 제작하고 싶다는 주소로 내원하였다. 환자 맞춤형의 진단 과정을 통해 전방부의 임플란트 서베이드 보철물을 이용한 임플란트 융합 국소의치를 계획하였다. 특히, 전통적인 방식의 임플란트 상부 보철물들의 단점을 보완하고자 EZ crown system을 적용한 임플란트 서베이드 보철물을 제작하여 기능적, 심미적으로 양호한 결과를 얻을 수 있었기에 이를 보고하는 바이다.

법랑모세포 섬유종에 의한 하악 제1대구치의 맹출지연 (DELAYED ERUPTION OF LOWER FIRST MOLAR ASSOCIATED WITH AMELOBLASTIC FIBROMA)

  • 법랑모세포 섬유종은 드문 진성의 혼합성 양성종양으로 20세 이하의 비교적 어린 연령층의 하악 후방부에 호발한다. 흔히 하악 구치부의 미맹출치와 관련하여 발생하며 치조골 표면에서 서서히 성장하여 이환치를 치근단 방향으로 변위시켜 정상적인 치아 맹출을 방해한다. 약간의 피질골 팽창 외에 다른 증상은 거의 없어서 정기적인 방사선학적 평가에서 우연히 발견되는 경우가 많다. 임상적 및 방사선학적으로 법랑모세포 섬유치아종 및 치아종과 비슷하나 조직학적으로 치아 경조직의 형성이 관찰되지 않는 것이 다르다. 치료방법은 적출술과 주위 골의 소파술을 포함한 보존적 외과적 처치가 일반적이나 재발가능성이 있고 악성종양으로의 변이를 보인다는 일부 보고가 있어 절제술과 같은 보다 공격적인 치료방법이 추천되기도 한다. 하지만 대부분의 병소는 피막으로 잘 둘러싸여 있으며 주변의 골와에서 쉽게 분리되므로 재발률은 낮은 편이다. 본 증례는 하악 좌측 제1대구치의 맹출지연과 관련된 법랑모세포 섬유종을 주소로 본과에 내원한 환아들을 대상으로 보존적인 적출술 및 소파술을 시행한 것으로 정기적인 관찰 결과 비교적 양호한 제1대구치의 자연적 맹출양상을 보여 보고하는 바이다.

치과도재의 Glazing 여부에 따른 수분침투 정도와 굽힘강도에 미치는 영향 (Influence of Water Infiltration and Flexural Strength Change with Glazing Treatment of Dental Porcelain)

  • 본 연구에서는 소성한 치과도재 시편에 대해 glazing 처리 여부에 따른 수분 침투와 기계적 물성의 비교분석을 통해 glazing 처리의 중요한 의미를 도출하고자 하였다. 또한, 제한된 가속 시효처리 방법으로 구강 내 환경에서 치과용 수복물의 내구수명 예측이 가능하였으며 수분 내에서 가속 시효처리를 통하여 치과도재는 수분 노출로 인한 열화가 일어나고 굽힘강도 저하 현상을 보였다. 치과도재의 glazing 처리를 통해 연마 시 발생하는 시편 표면의 흠집 또는 소성시 발생하는 미세균열이나 기공의 노출을 억제함으로써 굽힘강도 향상 결과를 보였다. 또한 glazing 처리한 시편이 가속 시효처리의 시간이 증가할수록 강도 저하 현상을 보이긴 하지만 glazing 처리를 하지 않은 시편과 비교하여 볼 때, 열화되는 속도가 작은 것을 알 수 있었다. 따라서 구강 내에서 치과도재 보철의 내구성을 확보하기 위해서 영구장착 전 glazing 처리를 반드시 시행할 것을 권하며, 수분 침투 억제 방법에 대해서도 향후 체계적인 연구가 되어야 할 것으로 생각한다.

김천시 초등학교 구강보건실 운영을 통한 계속구강건강관리사업 효과 (Effect of continuing dental care program by elementary school dental clinic in Gimcheon)

  • Objectives: The purpose of the study is to evaluate the effect of continuing dental care program by elementary school dental clinic in Gimcheon from 2008 to 2013. Methods: The subjects were experimental group of 152 students from three elementary schools having dental clinics and control group of 136 students from three elementary schools of no dental clinics. The six dentists in Gimcheon community health center carried out the regular dental checkups in the elementary schools and four dental hygienists recorded the dental examination results. After receiving WHO dental survey guideline education, the examiners used dental mirror and dental probe under the natural light. The contents of the survey included dental caries in the deciduous and permanent teeth, malocclusion, remaining ratio of sealant, treatment, and follow up. Results: DFT index by grade showed that 3.71 in experimental group and 4.56 in control group and it was remarkable in the 3rd grade. Thus. DFT index in the experimental group was lower than that of control group (p<0.05). DT index by grade was 0.63 in experimental group and 1.28 in control group marked in the 6th grade. DT index in the control group was higher than that of the experimental group (p<0,001). DFT index and DMFT Index by grade showed no significant difference between the control group and the experimental group. DFT and DMFT Index of the control group in the 3rd and 6th grade were lower score than those of experimental group. DFT and DMFT index in the third and 6th grade showed no significant difference between the control group and the experimental group. The remaining ratio of the sealant was 4.89 experimental group in the 6th grade. The remaining ratio of the sealant in the experimental group was higher than that in the control group. Conclusions: The continuing dental care program in the elementary school is very effective method in the treatment of oral disease in the elementary school children. The oral health care and prevention for the elementary school children should be implemented and carried out continuously.

디지털 인상법과 전통적 인상법의 정확도 비교: 체계적 고찰 (Comparison of the accuracy of digital impressions and traditional impressions: Systematic review)

  • 목적: 본 연구는 보철물의 적합 및 인상의 정확성에 관한 문헌들을 체계적으로 고찰하여 구강 내 스캐너를 이용한 디지털 인상법이 전통적인 인상법을 대체할 만한 정확도를 보이는지 알아보고자 한다. 재료 및 방법: 1997년 8월 1일부터 2017년 7월 31일까지 출판된 영어로 쓰여진 연구를 대상으로 디지털 인상법의 정확성에 대한 문헌을 MEDLINE/PubMed 전자 검색 및 수기탐색을 시행하였다. 조건에 따라 선택된 연구의 데이터를 스프레드 시트에 2명의 검토자가 독립적으로 정리하였다. 결과: 총 35개의 연구가 선택되었고, 26개의 연구는 보철물의 적합성을 비교하였고 9개의 연구는 보철물 없이 스캔 데이터를 통해 인상의 정확성을 비교하였다. 대부분의 연구가 보철물을 이용해 인상법의 정확성을 비교하였다. 결론: 본 고찰에 따르면 자연치열에서 디지털 인상법으로 단일관이나 근원심으로 길이가 짧은 보철물을 제작하는 것은 임상적으로 충분히 신뢰할만한 하다. 그러나 디지털 인상법으로 길이가 긴 보철물을 제작하는 것은 연구가 적어 아직 한계가 있다. 임플란트 보철물 제작에 있어서 구내스캐너를 이용한 디지털 인상법으로 전통적인 인상법을 완전히 대체하기는 아직 어렵다.

측두악관절 잡음자의 수평면상 하악 편위 운동에 관한 연구 (A STUDY ON THE MANDIBULAR ECCENTRIC MOVEMENT OF THE SUBJECTS WITH TMJ CLICK IN HORIZONTAL PLANE)

  • Although pantograph has been used to investigate whether the determinents of the mandibular movement were possible contributing factors of TMJ click, there was the problems to understand the role of tooth morphology upon the occurrence of click because of using appliance without tooth contacts. There Were advantages to evaluate the effects of tooth morphology upon the mandibular movements, because intraoral tracing device(Functiograph$^{(R)}$) had been obtained maintaining occlusal contact between the upper and lower natural teeth during mandibular movement. The purpose of this study was to record the mandibular eccentric movement quantitatively performed in 20 adult control subjects and 20 adult subjects with TMJ click and to investigate the effects of occlusion upon the occurrence of TMJ click. The obtained results were as follows : 1. The average ICP-P distance was $3.07{\pm}0.73mm$ in subjects with TMJ click, $2.14{\pm}0.85mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 2. The average ICP-P distance was $3.07{\pm}1.14mm$ in subjects with TMJ click, $2.61{\pm}0.96mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.05). 3. The average distance of right and left lateral movement was not statistically significant between subjects with TMJ click and control subjects. 4. The average lateral displacement from midline during RCP was $0.75{\pm}0.54mm$ subjects with TMJ click, $0.16{\pm}0.17mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 5. The average lateral displacement from midline during protrusive movement was $0.88{\pm}0.54mm$ in subjects with TMJ click, $0.20{\pm}0.23mm$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.001). 6. The average angle of right and left lateral movement was $144.2{\pm}20.20^{\circ}$ in subjects with TMJ click, $138.15{\pm}20.09^{\circ}$ in control subjects. There was a statistical significance between subjects with TMJ click and control subjects(P<0.05).

레이저 처리후 임프란트 표면 변화에 관한 연구 (The Evaluation of the atomic composition and the surface roughness of Titanium Implants following Various Laser treatment with air-powder abrasive)

  • Various long-term studies have shown that titanium implants as abutments for different types of prostheses have become a predictable adjunct in the treatment of partially or fully edentulous patients. The continuous exposure of dental implants to the oral cavity with all its possible contaminants creates a problem. A lack of attachment, together with or caused by bacterial insult, may lead to peri-implantitis and eventual implant failure. Removal of plaque and calculus deposits from dental titanium implants with procedures and instruments originally made for cleaning natural teeth or roots may cause major alterations of the delicate titanium oxide layer. Therefore, the ultimate goal of a cleaning procedure should be to remove the contaminants and restore the elemental composition of the surface oxide without changing the surface topography and harming the surrounding tissues. Among many chemical and mechanical procedure, air-powder abrasive have been known to be most effective for cleaning and detoxification of implant surface. Most of published studies show that the dental laser may be useful in the treatment of pen-implantitis. $CO_2$ laser and Soft Diode laser were reported to kill bacteria of implant surface. The purpose of this study was to obtain clinical guide by application these laser to implant surface by means of Non-contact Surface profilometer and X-ray photoelectron spectroscopy(XPS) with respect to surface roughness and atomic composition. Experimental rough pure titanium cylinder models were fabricated. All of them was air-powder abraded for 1 minute and they were named control group. And then, the $CO_2$ laser treatment under dry, hydrogen peroxide and wet condition or the Soft Diode laser treatment under Toluidine blue O solution condition was performed on the each of the control models. The results were as follows: 1. Mean Surface roughness(Ra) of all experimental group was decreased than that of control group. But it wasn't statistically significant. 2. XPS analysis showed that in the all experimental group, titanium level were decreased, when compared with control group. 3. XPS analysis showed that the level of oxygen in the experimental group 1, 3($CO_2$ laser treatment under dry and wet condition) and 4(Soft Diode laser was used under toluidine blue O solution) were decreased, when compared with control group. 4. XPS analysis showed that the atomic composition of experimental group 2($CO_2$ laser treatment under hydrogen peroxide) was to be closest to that of control group than the other experimental group. From the result of this study, this may be concluded. Following air-powder abrasive treatment, the $CO_2$ laser in safe d-pulse mode and the Soft Diode laser used with photosensitizer would not change rough titanium surface roughness. Especially, $CO_2$ laser treatment under hydrogen peroxide gave the best results from elemental points of view, and can be used safely to treat peri-implantitis.
