Analysis on the National R&D Portfolio of Food Safety in Korea from 2008 to 2010 (최근 3년(2008-2010)간 식품안전 분야 국가연구개발사업 운영 현황 분석)

  • Food safety management should be based on scientific evidences. FAO and WHO presented risk analysis as one of four principles in food safety management. WTO also admits the self safety regulation only when it is made on the basis of risk assessment. Without scientific analysis, tracing and eliminating the cause of food poisoning is impossible. Research and development plays a key role to produce scientific evidences. The Korean government ran over 40 programs in 11 agencies from 2008 to 2010. However, there is no statistics on food safety R&D at present. In this research, food safety projects conducted from 2008 to 2010 are listed up by means of analysing National Science and Technology Information Service (NTIS). The analytical criteria are the name of programs, national standard classification of science and technology, and keywords. As result, Korea Food and Drug Administration, Ministry for Food, Agriculture, Forestry and Fisheries, and Rural Development Administration play major role in the food safety R&D. The portion of more than one year projects should rise up in order to achieve the data for risk assessment, which is strongly required to improve. Besides, the research should be deeper so as to publish more SCI papers. The R&D portfolio should be changed in direction to raise up the portion of biological hazards such as norovirus. In order to do so, a large number of food safety programs should be emerged. The categories of food safety management and the hygiene/quality management of the agricultural and livestock products in the national standard classification of science and technology should be emerged because they are set up reflecting agencies' interests in spite of few differences between them.

A Study on the Health Effects of Pesticide Exposure among Farmers (농약살포 농민의 농약노출로 인한 건강피해에 관한 연구)

  • This study was conducted to provide the basic data about the health effects of pesticide exposure among farmers for agricultural health study. We analyzed 412 self-administered questionnaires collected from the male farmers who spray pesticides in Kyoung-ju area, Korea. Survey questions were about chronic symptoms, acute symptoms while pesticide spraying, pesticide intoxication accident, safety rules, protective equipments etc. The correlations among the variables related to pesticide exposure and the factors in acute pesticide poisoning and chronic symptoms of farmers were also analyzed. For chronic symptoms, the prevalence of 'tiredness and languor', 'lumbago', 'nocturia', 'shoulder pain', 'numbness', and, for acute symptoms while pesticide spraying, the experience rate of 'itching sense of skin', 'dizziness/headache', 'fatigue', 'eye glaring' were high compared with other symptoms. For 'experience of intoxication accident by pesticide in family', 7.1% of the subjects experienced the accident and the causes were pesticide spraying, food contamination by pesticide, suicide etc. Among safety rules, 'take a bath after spraying', 'change clothes after spraying' were kept relatively well, and, for protective equipments, hat, boots, mask, gloves, protective clothes(lower) were put on relatively well. The factors associated with acute pesticide poisoning were the extent of keeping safety rules, spraying time, orchard cultivation, agricultural area and spraying days per year etc. And the factors associated with chronic symptoms were acute symptoms while pesticide spraying, agricultural area, farming career, extent of keeping safety rules, extent of agricultural work and the pesticide exposure index etc. From these results, it is suggested that to reduce the health effects by pesticide exposure among farmers, the education to promote to keep safety rules and wear protective equipments, and information services should be recommended. And further studies on the long term health effects of pesticide exposure among farmers are required.

A Study on the Occurrences and Preventive Strategies of Accident in Children (초등학생의 안전사고 실태 및 예방전략에 관한 조사 연구)

  • Accidents are important causes of death and disability in children. They also have enormous financial implications. Young children become an victim of accidents easily because of their physical fragileness and their coping behavior being vulnerable to any actions taken by accidents. Once they have a accident, the children whose not fully developed, suffer from devastating long-term after-effects. Lee, Lee, Kang and Han(1995) reported that ninety percent of accidents can be prevented. But there is no national system to manage, evaluate and analyse the information about child accidents, even though it is necessary for accident prevention policies and health promotion of the general public. The purpose of the study was to determine how often children have accidents and define the accident prevention strategies in children. The investigator conducted a descriptive study by performing the surveys, interviews, and workshops for the 2,458 young children, 10 teachers, and 1,494 parents. The data collection for the study began on September 2000 and completed on April 20, 2001. The analysis of the data was done with Window SPSS 10.0 for descriptive statistics. Among those children, 1,298 children(52.8%) injured from accidents. The children who had accident answered that they injured from traffic accident(27.3%), inside the home(26.3%), on the playground(17.0%), during playtime(13.6%), in the school(5.9%) and food poisoning(7.1%). To define accident prevention strategies for the school children, the parents and the teachers who had a special interest in this topic formed a special task force under the guidance of the investigator. The team was charged to prepare the basis of content materials by identifying the problems, setting standards for the program, prioritizing the process, and selecting the methods of implementation and evaluation. Eight issues and concerns identified by the team were: 1)allowing young children to learn undesirable habits and behaviors that would bother others without knowing; 2) not guarding young children from car accident; 3)unattended accident at playground; 4) considering home places safe; 5)unattended accident at school. These issues were found to be coinciding with the actual child accident cases occurred recent years in Korea. Greater efforts are required to reduce unnecessary deaths and disability from childhood accidents. This study gave a very useful and important data to prepare accident prevention educational program and to prepare accident prevention strategies.

Comparison of International Guidelines of Dermal Absorption Tests Used in Pesticides Exposure Assessment for Operators

  • The number of farmers who have suffered from non-fatal acute pesticide poisoning has been reported to vary from 5.7% to 86.7% in South Korea since 1975. Absorption through the skin is the main route of exposure to pesticides for farmers who operate with them. Several in vitro tests using the skins of humans or animal and in vivo tests using laboratory animals are introduced for the assessment of human dermal absorption level of pesticides. The objective of this study is to evaluate and compare international guidelines and strategies of dermal absorption assessments and to propose unique approaches for applications into pesticide registration process in our situation. Until present in our situation, pesticide exposure level to operator is determined just using default value of 10 as for skin absorption ratio because of data shortage. Dermal absorption tests are requested to get exposure level of pesticides and to ultimately know the safety of pesticides for operators through the comparison with the value of AOEL. When the exposure level is higher than AOEL, the pesticide cannot be approved. We reviewed the skin absorption test guidelines recommended by OECD, EFSA and EPA. The EPA recommends assessment of skin absorption of pesticides for humans through the TPA which includes all the results of in vitro human and animal and animal in vivo skin absorption studies. OECD and EFSA, employ a tiered approach, which the requirement of further study depends on the results of the former stage study. OECD guidelines accept the analysis of pesticide level absorbed through skin without radioisotope when the recovery using the non-labeled method is within 80~120%. Various factors are reviewed in this study, including the origin of skin (gender, animal species and sites of skin), thickness, temperature and, etc., which can influence the integrity of results.

Characterization of Water Quality and the Aerobic Bacterial Population in Leachate Derived from Animal Carcass Disposal (가축 매몰지 침출수에 대한 수질 특성 및 호기성 미생물 분포에 관한 연구)

  • Leakage of leachate from animal carcass disposal is a significant issue because disease can easily spread to humans and other livestock. In this study, we analyzed the physicochemical properties of leachate and tested for the presence of aerobic bacteria in leachate using molecular biology methods, for 16 animal carcass disposals in the first stage (after burial for 5 months). Leachate physicochemical analysis revealed higher total coliforms, TOC, $NH^{4+}$, and $NO^{3-}$ concentrations compared with previously published data. In most leachate samples, the concentrations of $NH^{4+}$ and $NO^{3-}$ exceeded the Korean guideline values for drinking water. In 16S rRNA sequence analysis of the distribution of leachate under aerobic conditions, Bacillus pumilus, Lysinibacillus sphaericus, and B. sphaericus were observed with high frequency, whereas no food-poisoning-related bacteria such as B. cereus or Salmonella were detected. The present findings improve our knowledge of the transport of leachate from animal carcass disposal sites through geologic media, and are useful in risk analysis and for subsequent studies.

Research for Intestinal Mucosal Immunity Induced by Salmonella enteritidis Infection (Salmonella enteritidis 감염에 의해 장내 점막에서 유도되는 면역반응에 관한 연구)

  • Mucosal immunity is a well-designed defense system that builds precise and dynamic relationships against pathogens, and the gastrointestinal tract is the most important organ with this system, acting as a guardian at the forefront of its activity. Salmonella spp. cause food poisoning, entering the body orally and mainly invading the Peyer's patches of the small intestine. Although Salmonella strains share similar mechanisms for inducing innate immunity, different serotypes may have different effects on the intestinal mucosa due to host specificities and pathogenicity. In this study, we evaluated the effects of Salmonella enteritidis infections in mouse intestine and observed significantly reduced dose-dependent survival rates in a challenge test. Flow cytometry data showed no significant differences in intestinal immune cell populations, although histology indicated increased mucin production and decreased goblet cell counts in the Salmonella-treated groups. Furthermore, Claudin expression was significantly decreased in the samples with Salmonella. To investigate the relationship between S. enteritidis infection and inflammatory response, dextran sodium sulfate (DSS) was administered after infection and the results indicate lower survival rate after DSS treatment. In conclusion, we were able to identify the optimal concentration of S. enteritidis to modulate the intestinal mucosal immunity of mice and inflammatory response.

A Study on the Occurrences and Policy Development for Accident Prevention (한국의 사고발생 실태와 사고예방을 위한 정책 연구)

  • There is no national system to manage, evaluate, and analyse the information about accidents, even though it is necessary for accident prevention policies and health promotion of the general public. The existing studies are, also limited as they focus only on particular group(for example : inpatients, emergency room patients) or on particular geographic areas. Thus the results of the studies cannot be ap-plied to the general public. In order to overcome these limitations, this research focuses on data collection and analysis from accident information for the general population. By providing the analysis on types and causes af accidents, this research aims to produce the basic data necessary for accident prevention policy development. The specific aims of this research are to : 1. Analyse the actual occurrences and characteristics of accidents. 2. Suggest for the accident prevention policies and safety education. Accident report form three major newspaper printed in Korea between January 1, 1989 and December 31, 1993 were collected, and the cause, place, time, and personal injury related to the accidents were classified and then analyzed by de-scriptive statistics. The results of this research conclude : 1. The number of accidents reported by the three newspapers were 2155. 2. The highest proportion of accidents were as follows ; occurred during the June-August(31.2%), Sunday(24.8%), and 5 p.m. (7.6%) of the day. 3. The highest proportion of the accident occurred in Seoul(33. l%), Kyunggi province(14.5%), Kangwon province (7.8%), and Kyungnam prov-ince(6.7%) were next highest. 4. The main causes include car accidents(32.4%), drowning(10.9%), falls(8.8%), explosion(7.1%), and poisoning(5.8%). 5. Slightly more than half of injuries(50.5%) and about two fifths of deaths(40.3%) were caused by car accidents. Therefore, the most serious type of accidents were car accidents. 6. The number of males in accidents were almost three times higher than that of females(males 72.3%, females : 27.7%). 7. The age group from 10 to 19 years old represents the highest proportion(21%) of accidents. 8. The number of the accidents in Korea, based on the information collected from the newspapers, is estimated to be 14, 367 per year. The number of in-juries is estimated to be 88, 480 persons, and the number of deaths 29, 007 respectively. It is said that ninety percent of accidents can be prevented. Several accidents prevention policies are suggested here. 1. Safety education should be done more actively throughtout life, with special emphasis on safety education for children. 2. Safety measures for children(halmets for cycling, children car seats, seats belts, and so on) should be emphasized. 3. An injury surveillance system should be initiated. The initiation of injury reporting system in each factory and school could contribute considerably to the reduction of accidents.

The Knowledge and Attitude of Prehospital Care among Emergency Medical Technicians Working at 119 Fire Safety Centers for Patients with Acute Drug Intoxication (약물중독 환자의 병원 전 단계 처치에 관한 119 응급구조요원의 지식과 수행 태도)

  • Objectives: The purpose of this research is to provide foundational data for reeducation concerning prehospital emergency care in order to improve the performance of emergency medical technicians (EMTs) working at 119 fire safety centers. Methods: Data were collected using a mail-in questionnaire developed by researchers from 288 subjects, who were EMTs working at 119 fire safety centers and local units in the Gwangju Metropolitan City and South Jeolla Province, from March 1, 2009 to April 31, 2009. Results: The mean score for EMTs' knowledge on drug intoxication was7.04 out of a total of 10 points. The mean score for EMT's attitude of emergency treatment performance on drug intoxication was 2.96 out of a total of 4 points. The level of EMT's knowledge is relatively high regarding prehospital care for drug intoxication. The EMT's attitude of performances demonstrated an understanding of the patients' conditions. However EMTs did not actively explain the prognosis and medical conditions for their patients. Conclusions: This research will contribute to making plans for reeducating emergency crew working at 119 fire centers so that can they have a more active attitude towards prehospital emergency medical care.

Effects on the Mortality Patterns by Religious and Related Factors in Korean Population (종교 및 융복합적 특성이 사망양상에 미치는 영향)

  • Objectives : Seventh-day Adventists is well known to place a strong doctrinal emphasis on healthy life style that do not consume tobacco, alcohol or pork, and many adhere to a lacto-ovo-vegetarian lifestyle. This study aimed to investigate the difference of mortality between Korean Seventh-day Adventists and the general Korean population. Methods : We got 592 mortality data of Adventist cemetery in Seoul city and Kyunggi provincce of Korea, which contains information on causes of death for 2000-2004. Also we used mortality data of the general Korean population in Korea National Statistics Office for 2000-2004. Results : The mean age at death was 70.45 in Korean Adventists, 65.63 in the general Korean population. Higher neoplasm cause of death, such as of stomach, liver and breast, were observed in Korean Adventists than the general population. The death cause of injury, poisoning and external causes, and cerebrovascular disease were higher in the general population than Korean Adventists. The death cause of stomach cancer was lower in Korean Adventists who had above 10 years religious period than general population above age 50 years. In male, the death cause of lung cancer was lower in Korean Adventists than the general population. Conclusions : The results point to the importance of healthy life-style in Korean peoples, and indicate that lifestyle changes in the population might change the causes of death.

The Survey on the Drug use status, Knowledge and Attitude of College Students (Seoul and Northern Part of the Kyunggi-do) (대학생의 약물 사용 실태.지식 및 태도(서울 및 경기 북부지역))

  • The Purpose of this study was to grasp the status of drug uses in college students, and to investigate the knowledge and attitude when they use it. We collected through questionnaires from Sep. 1 to Oct. 30 in 1999. The subjects were 490 college students in Seoul and northern of the Kyunggi-do. The data analysis was performed using SPSS (version 7.0) and ANOVA. The results of this study were as follows. 1. There was no correlation for all factors in the relationship between general characteristics of subjects and knowledge for drugs. On the contrary, in the relation of the general characteristics and attitude for drug of subjects, some factors are statistically significant e.g., department (F=3.049, p<.010), satisfaction for school life (F=6.167,p<.002), body shape(F=8.092, p<.000), and the relationship with ones parents (F=3.728, p<.005). 2. In the analysis of drug use status and knowledge, only in the factor of administration control was statistically significant(F=4.304, p<.014) and in the relation of attitude for drug uses, drug administration was statistically significant (F=4.521, p<.004). 3. In the mean scores for the drug knowledge analysis, the highest factor was 'A narcotic make deformed persons through poisoning of physical and mental' (M=4.14), the lowest factor was 'If catch the flu during the pregnancy, should be take drug as possible as quickly to reduce negative effect for fetal' (M=1.94). 4. In the analysis for drug attitude, the highest factor was 'A drug is alike a poison' (M=3.48), 'Should be keep the usage and dosage of drug' was the lowest (M=1.48). 5. From the investigation for status of drug use, it was revealed that the most subjects (73.6%) were purchase drugs after explain their symptoms to pharmacist. And they take drugs only when they felt painful in 43.1%. The most students (70.4%) were experienced control of drug administration. It was inquired that subjects were mainly obtained information about drugs from pharmacist and television (or radio) advertisement, 33.5% and 33.1%, respectively. In the examination for existence of long-term administrators in their family, 'none' and 'only parents' are 49.6% and 37.3%, respectively. When their parents have illness, the persons go to drugstore and hospital for heath-care, 47.8% and 44.3%, respectively. On the basis of results of this study, we suggest as follows. 1. This study was analyzed data from questionnaires for college students in a part of local areas, so we suggest that the next research should be perform for national-wide students as subjects to generalize the results. 2. It is need more intensive research methodologies such as interview and observation. 3. Additional research is required for knowledge and behaviors of drug uses that will how impact on ones health behavior.
