본초 중량비를 이용한 부산대학교 한방병원의 첩약 사용 빈도 분석(1) - 보험처방을 중심으로 - (Frequency Analysis of Clinical Prescriptions in the Korean Medicine Hospital Pusan National University based on Herb Weight Ratio(1) - Focusing on Insured Herbal Mixture Extracts -)

  • Objectives : This Study aims to search for the actual prescriptions worth being Insured Herbal Mixture Extracts(IHME), which frequently used frequently in the clinical settings by comparing clinical prescriptions with the list of prescriptions covered under the national health insurance system. Methods : By making comparisons of the herb weight ratios of IHME with those recorded in EMR, the frequency is measured on the basis of the IHME and the frequency indication is computed for the clinical prescriptions with lower level of differences. Results & Conclusions : On the basis of the details of the clinical prescriptions used at the EMR, we have found out that many clinical prescriptions of EMR are similar for banhasasim-tang, banhabaekchulcheonmatang, bojungikgi-tang and jaeumganghwa-tang in the national health insurance system. And we could analyze indications of those prescriptions. So, if we can make a similarity criteria of prescriptions and this methods are used at nationwide research, we will be able to obtain a satisfactory result in study, medical industry and clinics.

알레르기 비염의 한의 진료 현황 설문조사 (A Survey on Treatment Trend for Allergic Rhinitis in Korean Medicine Clinic)

  • Objectives : The purpose of this study was to investigate the treatment trend for allergic rhinitis in Korean Medicine. Methods : We conducted an online survey for Korean Medicine Doctors who were registered in the association of Korean Medicine. The questionnaire was consisted of patient characteristics, diagnosis status, treatment status, and future research needs. Results : Data from total of 396 respondents were analyzed. More than 70% of the patient came to the Korean Medicine Clinic after visiting the Western Medicine Clinic in 43.9% of the respondents. 55.6% of the respondents performed combination therapy. History taking, nasal examination, x-ray, and Korean Medicine diagnostic test were used for diagnosis. The mean duration of treatment ranged from $4.9{\pm}2.91$ to $15.2{\pm}8.45$ for pediatric and early childhood patients and from $17.5{\pm}16.15$ to $5.3{\pm}3.85$ weeks for adolescents and adult patients. The mean number of treatment times was from $9.8{\pm}7.00$ to $33.5{\pm}24.45$ for pediatric and early childhood patients and from $10.8{\pm}11.55$ to $40.4{\pm}48.18$ times for adolescents and adult patients. 64.5%, 48.0%, and 91.2% of the respondents used herbal medication in national health insurance coverage, herbal medication uninsured in health insurance and herbal prescription filled at each medical institution, respectively for pediatric and early childhood patients and 67.6%, 42.8% and 86.1% for adolescents and adult patients. 36.9% and 36.4% of respondents answered that the study of acupuncture and herbal medicine are needed preferentially. Conclusions : The results of this survey will be used to develop clinical practice guideline that reflect actual clinical practice.

한의사의 한약제제 인식도와 만족도 (The Awareness and Satisfaction of Herbal Medicine Preparations of Korean Medicine Doctor)

  • Objectives : This study was conducted to identify awareness and satisfaction of herbal medicine preparations in Korean medicine doctor(KMD). Methods : The questionnaire consisted of three professors of Korean medicine based on previous researches and reports. The pilot survey was conducted to five KMDs who work in local clinic to get face validity. Based on the results of the pilot survey, three professors of Korean medicine completed the final version. Online surveys was conducted to member of the association of Korean medicine from march 25 to april 6 in 2016. Results : The proportion of patients who prescribe herbal medicine was not significantly correlated to gender (p=0.346), but significantly correlated to age(p<0.01), specialty(p=0.017), monthly income(0.022), and clinical experience(p<0.001). The most common reason for using the herbal preparations which is covered by NHI (National Health Insurance) was due to patient's payment(39.3%). Likewise, the most common reason for using the herbal preparations which is not covered by NHI was various kind of prescription(34.1%). Conclusions : The utilization of herbal preparations could be enhanced by expansion of NHI cover range of herbal preparations.

통풍의 한의임상진료현황 - 온라인(웹기반) 설문조사 (Current Status of Clinical Practice for Gout in Korean Medicine - On-line (Web-Based) Survey)

  • Objectives : The purpose of this study was to investigate the current status of clinical practice for gout in Korean Medicine (KM). Methods : On-line survey was adopted for this study, targeted at KM Doctors who were registered in the Association of Korean Medicine. The questionnaire included the general status of treatment, pattern identification and treatment method according to three stages given: acute, chronic and asymptomatic hyperuricemia stage. Results : Data from a total of 384 respondents was analyzed. Participants who responded most were in their 40s, working in KM clinics located in Seoul. Acupuncture and herbal medicines were the top priority treatments in preparing recommendations in clinical practice guidelines for gout, and where clinical trials were considered essential. As of the past year, the average number of first visits per month was 86.7%, and the average treatment duration was less than 1 month, accounting for 72.9%. As for the diagnostic method used for gout patients, the "diagnosed by clinical pattern" response was the highest, and the "blood and urine test" response was the highest as the diagnostic equipment used. As for the evaluation scale being used, the "VAS/NRS" response was the highest. Regardless of the gout stage, acupuncture was the most commonly used treatment method for gout patients, and bee venom pharmacopuncture was the highest for the pharmacopuncture used. In the case of herbal medicine, it was found that a wider variety of herbal medication uninsured in health insurance and herbal prescription were used for the staged treatment of gout than herbal medication in national health insurance coverage. In clinical practice, "acupuncture treatment three times a week" was the most common, and the "acupoints in the limb" were the most frequently used; LR3 and SP3 were the most frequently used acupoints. Conclusions : It is suggested that this survey should be helpful to develop clinical practice guideline for gout that reflects actual clinical practice.

Effects of three different formulae of Gamisoyosan on lipid accumulation induced by oleic acid in HepG2 cells

  • Background: Gamisoyosan (GSS) is an herbal formula which has been used to treat women's diseases for several hundred years in Korea. GSS is one of the three most common prescriptions among women and is used to treat menopausal symptoms. Fatty liver disease is also common in postmenopausal women and can precede more severe diseases, such as steatohepatitis. The present study compared the effects of GSS on fatty liver using three different formulae, Dongui-Bogam (KIOM A), Korean Pharmacopeia (KIOM B) and Korean National Health Insurance (KIOM C). Methods: In oleic acid-induced HepG2 fatty liver cells, cellular lipid accumulation, triglycerides and total cholesterol were measured after treatment with three GSS formulae and simvastatin as a positive control. To investigate the phytoestrogen activity of GSS, MCF-7 cells were treated with GSS, and hormone levels were quantified. Also, qualitative analysis was performed with UPLC. Results: All types of GSS decreased cellular lipid accumulation. KIOM A was slightly less effective than the other two GSS formulae. KIOM B and KIOM C decreased cellular triglycerides more effective than simvastatin, but KIOM A did not affect cellular triglycerides. Cellular total cholesterol was decreased by all GSS and simvastatin. GSS showed phytoestrogen activity in MCF-7 cells. From the UPLC analysis data, geniposide, paeoniflorin and glycyrrhizin were detected form three GSS formulae. Conclusion: These results suggest that all GSS formulae have a beneficial effect on fatty liver disease during menopause and that differences of formula have no effect on the efficacy of the prescription.
