Antibiotic Resistance Patterns of Acinetobacter baumannii and Pseudomonas aeruginosa from a Hospital in Daegu City Area (대구지역병원에서 분리된 Acinetobacter baumannii와 Pseudomonas aeruginosa의 항생제 내성현황)

  • 86 clinical isolates of Acinetobacter baumannii and 116 clinical isolates of Pseudomonas aeruginosa strains isolated from clinical specimens were collected from a hospital in Daegu city area. We investigated the Antimicrobial susceptibility patterns of A. baumannii and P. aeruginosa isolated from sputum, urine, wound, blood, nasal swab, body fluid. The antimicrobial resistance of A. baumannii were shown 96% for piperacillin, carbenicillin 82%. cefotaxime 78%, ciprofloxacin 77%, sulfamethoxazole/trimethoprime 76%, ceftazidime 75%, tobramycin 72%. For P. aeruginosa, the resistance of cefotaxime and sulfamethoxazole/trimethoprime were 100%, carbenicillin 49%, piperacillin 47%, ticarcillin 45%, ticarcillin/ clavulanic acid 40%.

Ki-67 Can Predict the Response to the Gemcitabine, Oxaliplatin And L-asparaginase Regimen (GELOX) and Prognosis in Patients with Nasal Natural Killer/T-cell Lymphoma

  • GELOX (gemcitabine, oxaliplatin and L-asparaginase) regimen showed an impressive result in our previous study, but the effect of this new regimen is still dissatisfying for some patients, so it is necessary to identify which patients will benefit from this regimen. A total of fifty-one cases with nasal natural killer/T-cell lymphoma receiving initial GELOX chemotherapy were enrolled in this study. The ki-67 expression detected by immunohistochemistry (IHC) in the specimens ranged from 10% to 90%, with a median value of 70%, so cases higher than the median value (${\geq}70%$) were defined as high ki-67 expression, and the others were designated as low ki-67 expression. The response rate had no statistical difference between low ki-67 expression group and high ki-67 expression group (P=0.291) though the value in the former group was relatively high. After a median follow-up of 18.03 months, the 3-year progression-free survival (PFS) for patients with low ki-67 expression was significantly higher than those with high ki-67 expression (83.8% vs. 47.9%, P=0.038). In the stage I/II subgroup, 3-year PFS and overall survival (OS) were statistically higher in the patients with low ki-67 expression than those with high ki-67 expression. Multivariate analysis revealed high ki-67 expression was an independent prognostic factor for PFS. These results suggest that low ki-67 expression can predict a good response of GELOX in these patients, and the combination of ki-67 expression and early stage is helpful to identify an excellent prognosis subgroup from patients receiving GELOX in this disease.

First Record of a Poacher, Aspidophoroides monopterygius (Scorpaeniformes: Agonidae) from the East Sea, Korea (한국산 날개줄고기과 (Agonidae) 어류 1 미기록종, Aspidophoroides monopterygius)

  • Six specimens (151.4~171.2 mm in standard length) of the poacher Aspidophoroides monopterygius were collected by Danish seine net in the coastal waters off Samcheok and Gangneung, the East Sea, Korea. This species is characterized by a slender body, one dorsal fin, terminal mouth, no barbel on the head, and a pair of nasal spines on the snout. It differs from Anoplagonus occidentalis Lindberg in having a nasal spine. We describe it as the first record within the Korean fish fauna, and propose the new Korean name "Ak-eo-jul-go-gi" for this species.

  • The blenniid fish, Istiblennius dussumieri (Valenciennes) was reported on the basis of six specimens collected from rocky shores on the Yokonami Peninsula, Kochi Prefecture, southern Japan. This species was characterized by wavy, dusky stripes on its dorsal fin, 14 pectoral fin rays, 38 to 42 vertebrae, branched supraorbital cirrus, finger-like nasal cirrus, seven to ten vertical, dark bands on the body and canines absent on both jaws. Although the occurrence of this species may be due to larval transport by the Kuroshio Current, this is the first record of I. dussumieri from Japan represents a remarkable northern range extension of the species in the western Pacific Ocean.

Risk Factors for Acquisition of Methicillin-Resistant Staphylococcus aureus in a Neurosurgical Intensive Care Unit(NSICU): Case-Control Study (신경외과 중환자실에서의 MRSA 획득 위험요인 분석)

  • Purpose: Methicillin-resistant Staphylococcus aureus (MRSA) is a major nosocomial pathogen in the intensive care units (ICUS). The purpose of this case-control study is to identify risk factors for acquisition of MRSA during ICU stays in patients with and without MRSA. Method: The study was conducted in a 16 beds-neurosurgical intensive care unit of a 2200-bed tertiary care university hospital in Seoul, Korea. Medical record and Critical Classification Scoring System were reviewed retrospectively in patients who were admitted more than 3 days from August 1, 2003 to May 30, 2004. Cases and controls were matched for age and gender. The obtained specimens were nasal swab and sputum. Result: There were 950 patients' admissions during the period. Among them, MRSA was isolated from twenty-three patients who were considered as hospital acquired. Artificial airway (p=.045), frequency of suction (p=.002), nasogastric tube (p=.004), wound drain (p=.045), and vancomycin (p=.019) were risk factors for MRSA acquisition in univariate analysis. Frequency of suction (p=.012, OR 3.5) was revealed as the only risk factor in multivariate conditional logistic regression. Conclusion: Our findings give support to recent studies that suggest that frequent physical contact maγ increase the nosocomial acquisition of MRSA in a neurosurgical ICU.

Biochemical characteristics and antimicrobials susceptibility of pathogenic Streptoccus spp. isolated from respiratory tract of Thoroughbred horses in Jeju, Korea (제주지역에서 사육중인 더러브렛 말 호흡기로부터 분리된 병원성 Streptoccus spp.의 생화학성상 및 약제감수성 양상)

  • This study carried out to investigate the pathogenic Streptococcus spp. isolated from respiratory tract of Thoroughbred in Jeju province. The specimens were collected from nasal mucosa using a culture swab from 113 Thoroughbred horses. Suspected colonies were selected onto blood and MacConkey agar plate, and identified by standard biochemical properties using Vitek 2 system and PCR method. In this study, we isolated S. equi (n=6), S. zooepidemicus (n=31), S. equisimilis (n=5), S. dysgalactiae (n=2), S. agalctiae (n=1), non identified ${\beta}$-hemolytic Streptococcus spp. (n=1) from Thoroughbred horses. In antimicrobial susceptibility test, it showed a high sensibility in the most of antimicrobial except for neomycin, streptomycin, spectinomycin, erythromycin and clindamycin. These results will provide the basic information to establish control measures for the treatment and prevention of respiratory disease by pathogenic Streptococcus spp. in Thoroughbred horses in Korea.

Carriage Rates of Methicillin-resistant Staphylococcus aureus in Neonates with Neonatal Jaundice (신생아황달 환아에서의 메티실린내성 황색포도알균 보균율에 관한 연구)

  • Purpose : It is known that carriage rates of Staphylococcus aureus (S. aureus) are highest in newborns and that the asymptomatic carriage of methicillin-resistant Staphylococcus aureus (MRSA) is associated with invasive MRSA infection with the colonizing strain. This study was carried out to investigate the carriage rates of MRSA in neonates with neonatal jaundice. Methods : We reviewed the medical records of 545 neonates admitted with neonatal jaundice to neonatal intensive care units between January 2006 and December 2010. Nasal and inguinal swab specimens had been taken from them and cultured for the isolation of S. aureus. Antimicrobial susceptibility tests had been done for such isolates to determine methicillinresistance. Results : Out of 545 neonates, 318 (58.3%) were colonized with S. aureus and 214 (39.3%) were colonized with MRSA. Results of the antibiogram analysis showed that 65.7% of MRSA isolates were likely to be community-associated (CA) MRSA. Conclusion : Based on the MRSA carriage rate of 39.3%, a surveillance program for MRSA colonization is considered necessary in neonates transferred from other clinics or hospitals. Out of MRSA isolates, 65.7% were likely to be CA-MRSA. This suggests that CA-MRSA strains were already present in obstetric clinic environments where the neonates were born. It is thought that MRSA surveillance programs in these environments are also necessary.

Comparison of the Genetic Relationships and Osteological Aspects in Six Branchiostegid Fish Species (Perciformes)

  • We analyzed partial sequences of cytochrome b (cyt-b), a mitochondrial DNA (mtDNA) gene, to determine the genetic relationships between six horsehead fish species: Branchiostegus japonicus, Branchiostegus albus, Branchiostegus auratus, Branchiostegus argentatus, Branchiostegus wardi, and an unidentified Branchiostegus species. The specimens were collected in Korea, China, Japan, and Vietnam. We compared their molecular phylogenetic relationships inferred from mtDNA cyt-b sequences with an osteological analysis. The unidentified species, B. sp., was similar to B. albus in terms of the lack of triangular silver-white dot at the posterior region of eyes (vs. large one present in B. japonicus), but was also similar to B. japonicus in terms of the presence of a straight-shaped first hemal spine (vs. a curve-shaped hemal spine in B. albus). Analysis of the mtDNA cyt-b sequences indicated that the smallest estimated sequence divergence was between the B. japonicus and B. sp. (0.70-0.94%), whereas the largest difference was between B. auratus and B. argentatus (23.06-23.36%). Both the maximum parsimony and maximum likelihood trees showed that the B. sp. was closely clustered with B. japonicus, and that B. auratus was most distant from the other species. When comparing the osteological characters, UPGMA tree showed that the B. japonicus and B. sp. were the most closely clustered species, and B. auratus was the most distantly clustered fish relative to the other species. The shape of the nasal, otolith and first hemal spine was informative for distinguishing B. auratus from the other species. These osteological differences were consistent with the differences in mtDNA.

Antibiotic Resistance Patterns of Staphylococcus aureus and Methicillin Resistant S. aureus Isolated from the Specimen of Elementary School Students

  • Staphylococcus aureus is a major cause of nosocomial infections and is one of the most commonly isolated bacterial species in the hospital and continues to be an important pathogen in both community and hospital-acquired infection. Methicillin resistant S. aureus (MRSA), which is associated with hospitals is now being isolated in the community. The purpose of this study is to investigate the carrier rate of S. aureus in the community, antibiotic resistance patterns of the organism, detection of MRSA and mecA gene in MRSA. Ninety strains $(46.4\%)$ of S. aureus were isolated from the nasal specimens of 194 elementary school students. Eighty-nine strains $(98.9\%)$ of 90 S. aureus were resistant to penicilin, 36 strains $(40.0\%)$ to erythromycin, 14 strains $(15.6\%)$ to fusidic acid, 11 strains $(12.2\%)$ to gentamycin, 9 strains $(10.0\%)$ to tobramycin, 5 strains $(5.6\%)$ to oxacillin, 4 strains $(4.4\%)$ to clindamycin, 2 strains $(2.2\%)$ to tetracycline, 1 strains $(1.1\%)$ to fosfomycin. None of $90(0\%)$ S. aureus isolates was resistant to ciprpfloxacin, trimethoprim/sulfamethoxazole, levofloxacin, linezolid, moxifloxacin, nitrofurantoin, norfloxacin, rifampicin, quinupristin/dalfopristin, teicoplanin, and vancomycin. Five strains $(5.6\%)$ of 90 S. aureus isolates were MRSA. The mecA gene was detected from five MRSA strains by PCR.

Pasteurella anatipestifer infection in ducklings in Korea (오리의 Pasteurella anatipestifer 감염증 발생)

  • This study was conducted to investigate the cause of a new duck disease occured in southern part of Korea. A meat type duck farm located in Kangjin, Chonnam Province had experienced outbreaks of septicemic disease at around 20 days of age in nearly every batch of ducklings from early spring to early summer in 1989. Main symptoms of the birds were eye and nasal discharge, depression, inappetence, diarrhea and nervous signs such as tremor and ataxia. Some birds died suddenly without any signs. Mortality reached from 20% to 80% in severe cases. The autopsy findings of the affected ducklings revealed consistantly severe airsacculitis, fibropericarditis, perihepatitis and occasionaly enteritis and distended ureter with urate deposit. A rod shaped gram-negative bacterium was isolated purely from brain and liver of the diseased ducks by culturing the specimens on blood agar for 48 hours in candle jar. The isolate neither produced hemolysis nor grew on MacConKey Agar. It formed colony relatively slowly being recognizable at least 36 hours after culturing, reaching colony size of about 1mm in diameter at 48 hours culture. The colony looked iridescent under oblique light and had muddy odor. The isolate did not ferment carbohydrates tested but produced gelatinase, hippuricase and oxidase which were considered as characteristics of P anatipestifer. The isolate induced similar signs and lesions when infected experimentally into ducks of 3 to 38 days age via intraperitoneum or intratrachea. However it did not produce any clinical signs wen inoculated via intranasal route. It produced only mild signs in chicken just injected with a very large dose. The bacteria did not produce any signas or lesions in mice. It was concluded through biochemical and physiological tests and animal inoculation tests that the new disease was caused by P anatipestifer.
