재난 생존자 경험의 내러티브 분석 -재난 간호를 위한 제언- (Narrative Analysis on Survivor's Experience of Daegu Subway Fire Disaster - The Hypothetical Suggestions for Disaster Nursing Practice -)

  • Some fifty survivors participated in the narrative therapy sessions from March 2003 to December 2004, and thirty cases were analyzed using the transcripts. Each participant's surviving story was summarized, and then interpreted and reinterpreted by the survivors themselves in collaboration with the researcher. The two main principles in narrative analysis were hermeneutical distanciation and hermeneutical circulation. Result and Conclusion: First, nursing involvement should play active roles from the early stages of disaster incidents. Specifically, emergency nursing and flexible coping plans are to be available according to the specific properties of each case. Secondly, it is necessary to try to understand the extreme emotional experiences of disaster survival. The horror and pain people feel at an incident cannot simply be generalized, and it requires that each case be approached individually to help stop social alienation. Thirdly, more constant and long-term studies are required to set up nursing strategies for disaster survivors. Forming a trusting relationship with survivors is basic, and formally registering as participants is necessary for continuous interventions. Fourthly, we should deeply appreciate the danger and complexity of modern society and understand the complex nature of disaster. Fifthly, interdisciplinary activities and studies are necessary in combination with various other fields to establish a framework of total nursing care for disaster incidents. Lastly, it is urgently necessary to educate families and friends of the survivors and the society as a whole about life after a disaster.

  • 본 연구의 목적은 회복탄력성 증진을 위한 '가족 이야기' 집단 프로그램에서 중요하게 다루어야 할 내용의 우선순위를 조사하고, 회복탄력성의 7개 하위영역별로 유의미한 영향을 미치는대학생들의 개인적 특성 및 가족 이야기 관련 특성을 파악하여, 대학생들의 회복탄력성 향상을 위한 프로그램 방안을 제시하는 것이다. 본 연구에서는 회복탄력성의 7개 하위영역별로 대학생들의 수준을 '높음' 집단과 '낮음' 집단으로 나눠 로지스틱 분석을 시행하였으며, 회복탄력성의 각 하위영역별로 유의미한 영향을 미치는 대학생의 개인적 특성 및 가족 이야기 관련 특성을 조사하였다. 연구결과, 사회복지 대학생들이 프로그램의 우선순위로서 선택한 내용은 (1) 우리 가족의 주요 가족사건 이해하고 의미 찾기; (2) 우리 가족의 위기극복 방법 알고 의미 찾기; 그리고 (3) 우리 가족의 성취와 관련된 이야기를 알고 그 의미 찾기 등이었으며, 이러한 내용을 바탕으로 회복탄력성을 향상시키기 위한 프로그램 방안을 제안하였다.

  • Loin pain haematuria syndrome (LPHS) is an uncommon clinical entity that has divided renal physicians, pain practitioners, and even psychiatrists since its initial description. A relative paucity of data exists regarding the condition, with best practice guidelines lacking amid the existing threads of anecdotal experiences and variable follow-up observations. The aim of this article was to review the cumulative published experience of pain relief strategies for LPHS.

영화 <아바타>에 나타난 육체성 연구 (A study on the corporality in the film Avatar)

  • This paper aims to look into various symbolic meanings of the body shown in the film, Avatar (2009, James Cameron), and to ponder over a variety of media strategies carried by the pursuit of the corporality shown in this film. In a nutshell, the body in Avatar is a symbol of primitiveness rather than civilization, and the body language is a fundamental and effective means of communication much better than verbal language. A variety of physical contacts that appeared in many scenes of the movie emphasizes the role of the body as a means of real communication. Also, in the composition of the film dominated by the confrontation of civilization, numerous creatures in the planet Pandora with a variety of colors as well as a number of agencies and large body sizes express primitive richness. The narrative of the film telling the story of a 'moving' body ultimately emphasizes the superiority of the body with respect to consciousness, unlike the narrative of conventional movies dealing with the problems of the body. In addition, the corporality pursued by this film implies several important media strategies. It may reveal a self-reflection of the material civilization and the imperialism, or, on the contrary, an attempt to conceal or dilute them. It may also represent a self-reflection of the overdeveloped media technology, or simply the dilution of it. Finally, it may be an attempt to recover the feeling of "presence", not fully supported by the 3D technology, by the identification of the spectator's body and the character's body.

Preoperative risk evaluation and perioperative management of patients with obstructive sleep apnea: a narrative review

  • Obstructive sleep apnea (OSA) is a common sleep-breathing disorder associated with significant comorbidities and perioperative complications. This narrative review is aimed at comprehensively overviewing preoperative risk evaluation and perioperative management strategies for patients with OSA. OSA is characterized by recurrent episodes of upper airway obstruction during sleep leading to hypoxemia and arousal. Anatomical features, such as upper airway narrowing and obesity, contribute to the development of OSA. OSA can be diagnosed based on polysomnography findings, and positive airway pressure therapy is the mainstay of treatment. However, alternative therapies, such as oral appliances or upper airway surgery, can be considered for patients with intolerance. Patients with OSA face perioperative challenges due to difficult airway management, comorbidities, and effects of sedatives and analgesics. Anatomical changes, reduced upper airway muscle tone, and obesity increase the risks of airway obstruction, and difficulties in intubation and mask ventilation. OSA-related comorbidities, such as cardiovascular and respiratory disorders, further increase perioperative risks. Sedatives and opioids can exacerbate respiratory depression and compromise airway patency. Therefore, careful consideration of alternative pain management options is necessary. Although the association between OSA and postoperative mortality remains controversial, concerns exist regarding adverse outcomes in patients with OSA. Understanding the pathophysiology of OSA, implementing appropriate preoperative evaluations, and tailoring perioperative management strategies are vital to ensure patient safety and optimize surgical outcomes.

서술 전략의 전환-「진보의 전초기지」에서 『어둠의 핵심』으로 (The Conversion of Narrative Strategy: from "An Outpost of Progress" to Heart of Darkness)

  • Even though "An Outpost of Progress" and Heart of Darkness were based upon Joseph Conrad the sailor's same experience in Congo Free State, their narrative strategies are quite different. The realistic representation of "An Outpost of Progress," with which Conrad was not satisfied at all, was converted into the modernistic narrative strategy of Heart of Darkness so that the sympathetic power of the story should be improved. The conservative value system of realism is expressed by the omniscient author in "An Outpost of Progress," whereas the frame narrator of Heart of Darkness is proved to be an unreliable one whose norms and behavior are not in accordance with the implied author. The glorious history of the British Empire, which was proudly presented by the frame narrator at the beginning of Heart of Darkness, was strongly opposed by Marlow, another narrator, who said that the British Empire had been "one of the dark places of the earth" when ruled by the Roman Empire. The feeling of the frame narrator was uneasily changed into the gloomy mood when he described the Thames as the flow which "seemed to lead into the heart of an immense darkness" at the end of Heart of Darkness. Similar to the straightforward narrative strategy of representation in "An Outpost of Progress," the realistic approach of Part I in Heart of Darkness is considered by Conrad as insufficient to reveal the darkest truth of imperialism, which was declared by Kurtz as "The Horror! The Horror!" Thus Conrad uses the Chinese-box structure, in which Kurtz' episode is enveloped by Marlow's tale which is enclosed by the frame narrator's story, in order to penetrate into the mind of ordinary readers in the novelist's age of New Colonialism, while attacking the ideology itself of imperialism instead of critisizing its inefficiency and individualism.

  • 본 연구는 사회·문화적 변화에 능동적으로 대처하는 장수브랜드인 박카스의 TV광고 내러티브를 분석하였다. 특히, 본 연구는 TV광고에서 박카스의 내러티브의 유형과 그에 따른 의미작용의 중요성에 천착하였다. 이러한 논의는 TV광고에서의 박카스 내용과 형식에 대한 흐름과 변화 그리고 그에 따른 스타일의 유형을 파악할 수 있다. 또한 TV 광고를 통해 박카스 브랜드 전략의 변천과정과 시대별 비주얼 내러티브의 특수성과 콘텐츠를 이해할 수 있다. 따라서 연구는 로만 야콥슨(Roman Jakobson)의 6가지 커뮤니케이션 기능모델에 근거하여, 박카스 브랜드의 TV광고 내러티브에 대한 메시지의 기능을 제시한다. 결론적으로, 박카스의 TV광고 내러티브를 통한 장수브랜드의 차별적 의미와 가치를 정리해 보면 다음과 같다. 첫째, 사회·문화적 상황과 밀접하게 연관된 소비자의 소구변화에 따른 내러티브를 구성한다. 둘째, 지속적인 브랜드의 이미지 관리와 누적효과에 따른 아이덴티티를 형성한다. 셋째, 사회·문화적 상황과 소비자 간의 함의적이며 유대적인 관계를 형성하여, 커뮤니케이션 메시지로서 기능한다.

NIC과 연계된 산부인과 환자 간호중재에 대한 한국 건강보험 수가체계와 미국 ABC 코드체계와의 수가 비교 분석 (Korean and United States: Comparison of Costs of Nursing Interventions)

  • Purpose: This study was performed to compare the costs of nursing interventions implemented for the obstetrical and gynecological patients using Korean Reimbursement System and ABC codes system developed in the US for costing out interventions performed by health care professional. Methods: First, the narrative data on nursing interventions were extracted from electronic medical record system of a tertiary university and mapped with Nursing Intervention Classification (NIC) by two researchers until 100% consent was reached. Narrative nursing interventions mapped with NIC were then remapped with ABC codes system using the electronic program developed in the research. The mapping data were analyzed with real numbers, frequency, percentage, mean, and standard deviation. Results: More nursing interventions were mapped with ABC codes than Korean reimbursement system. Total of 97 different types of narrative interventions could be mapped with NIC, 43 NIC interventions could be reimbursed by ABC code but only 16 NIC interventions were reimbursed by Korea Reimbursement System. Conclusion: Korean medical insurance fee system needs amendment to include more comprehensively interventions performed by nurses which are very important to patient outcomes. Further study is needed to develop strategies to costing out nursing interventions.

중도시각장애인 부인의 위기 직면 및 극복 경험에 관한 내러티브 연구 (A Narrative Study on the Experiences of Facing and Coping the Crisis in the Wives of Men Having Visual Impairment)

  • Purpose: This study examined the life experiences of the wives of men who were born with normal vision but gradually developed a visual impairment, with particular focus on their way of facing and coping with their life crisis. Method: A narrative approach was used to not only understand women's experiences within sociocultural contexts, but also reveal the subjective meaning of their personal life stories through their own voices. A purposive sample of three participants having husbands unexpectedly diagnosed with visual impairment during their marriage was recruited. The data were analyzed using the narrative analysis methods suggested by Clandinin and Connelly. Results: The wives took the crisis of their husbands' visual impairment as an economic crisis of the family. They faced a variety of difficulties, including extreme despair, role changes, and economic burdens. On the other hand, they tried to keep the family secure through a range of coping strategies that were motivated by compassion toward the husbands, maternal affection for the children, and a sense of responsibility for their families. Conclusion: This study revealed the need for financial support for temporarily disadvantaged families. In addition, various nursing and counseling programs for the family members will also be needed to help recover the family functions.

A Constructivist Approach to Understanding Russian's Public Diplomacy through Humanitarian Aid during COVID-19

  • Applying discourse analysis of Russia's narrative on humanitarian aid and its perception by the Western collective identity at the beginning of the COVID-19 pandemic, this study identifies several challenges that constructivism poses to the current understanding of public diplomacy (PD). In contrast to the mainstream positivist tendency to evaluate the effectiveness of PD through models, this article expands the PD narrative by inquiring about the role of power, intersubjective knowledge, and collective identities in public diplomacy. In particular, it examines the PD questions often ignored by researchers regarding how collective identities can exercise discursive power to interpret incoming narratives, which challenge domestic intersubjective knowledge. It also argues that, because the Russian political elite failed to ensure a coherent story and provide informational support for its humanitarian aid, the Western intersubjective knowledge on Russia negatively contributed to the perception of PD narratives. Thus, the article underscores the importance for PD practitioners to understand how the socially constructed nature of knowledge can improve or harm PD strategies.
