  • Superior mesenteric artery (SMA) aneurysms are rare and often fatal. A 72-year-old man had previously been admitted to the emergency room with epigastric pain and heart murmur. The echocardiographic diagnosis was vegetation on the aortic and mitral valves, with moderate regurgitation from both valves due to infective endocarditis. No aneurysm was detected on abdominal computed tomography, and emergency double-valve replacement was performed. On postoperative day 25, the patient experienced abrupt abdominal pain, and computed tomography revealed a mycotic SMA aneurysm. Open surgical repair of the SMA aneurysm was performed using the femoral vein, and the patient's postoperative course was uneventful.

  • A two-year-old male Laika weighing 19 kg and a two-year-old female Rottweiler dog weighing 33 kg were presented to the Veterinary Medical Center, Chungbuk National University. Chest radiographs showed miliary interstitial lung pattern in the overall lung fields and the ill-defined mass in the perihilar area, which is consistent with lymphadenopathy in both cases. In the tracheal wash fluid smears in two cases, 90% of cells were eosinophils. Also, Aspergillus fumigatus was isolated from the tracheal wash fluid in Czapek's agar in one case. Ketoconazol (10 mg/kg, PO, ql2hr) alleviated the clinical signs in two dogs. Follow up radiographs showed marked reduction of lung lesions in one case.

  • A very rare case of mycotic aneurysm in the descending thoracic aorta due to salmonellosis was treated in our hospital The patient was a 62 year-old male who 48 days before the operation was admitted to the depart, cent of internal medicine complaining of fever. nausla. vomiting, and loose stool. He was treated for 35 days and discharged. Three days after discharge, however, the patient was readmitted to the hospital comE긴ainin프 of $\ulcorner$i포ht uppe$\ulcorner$ 킥uadrant abdominal pain. Fever developed on the third hospiti린 day, and on the eighth hospital day, the patient complAined of back pain and epigastric pale. A simple chest x-ray showed evidence of hemothorax in the left plueral space, and therefore, computed tomography of the chest was done. The patient was diagnosed as a ruptured mycotic aneurysm of the descending thoracic aorta, and was transferred to our Department of Thoracic and CArdiovascular Surgery. The aneurysm and infected tissues was widely debrided, and the site was then patched with a Dacron graft. Salmonella choleraesuis was identified in the blood and aneurysm cultures, and antibiotics were administered for weeks according to the sensitivity of the organism. The patient experienced no complication thereafter and for the last three months since the operation, he has been leading a healthy and normal social life.

  • Invasive candidiasis is associated with high morbidity and mortality. Clinical diagnosis is complicated by a lack of specific clinical signs and symptoms of disease. Laboratory diagnosis is also complex because circulating antibodies to Candida species may occur in normal individuals as the result of commensal colonization of mucosal surfaces thereby reducing the usefulness of antibody detection for the diagnosis of this disease. In addition, Candida species antigens are often rapidly cleared from the circulation so that antigen detection tests often lack the desired level of sensitivity. Microbiological confirmation is difficult because blood cultures can be negative in up to 50% of autopsy-proven cases of deep-seated candidiasis or may only become positive late in the infection. Positive cultures from urine or mucosal surfaces do not necessarily indicate invasive disease although can occur during systemic infection. Furthermore, differences in the virulence and in the susceptibility of the various Candida species to antifungal drugs make identification to the species level important for clinical management. Newer molecular biological tests have generated interest but are not yet standardized or readily available in most clinical laboratory settings nor have they been validated in large clinical trials. Laboratory surveillance of at-risk patients could result in earlier initiation of antifungal therapy if sensitive and specific diagnostic tests, which are also cost effective, become available. This review will compare diagnostic tests currently in use as well as those under development by describing their assets and limitations for the diagnosis of invasive candidiasis.

  • Mycotic infection seems to be increasing in importance as the causal agents of disease in man, especially invaders in already debilitated persons. This paper presents the identification of 39 stock strains of yeast like cells which were isolated from patients at National Medical Center. It reveals 21 strains of C. albicans, 5 strains of T. glabrata, 4 strains of C. tropicalis, one strain of T. mogii & 8 strains of unidentified.

  • Superior mesenteric artery aneurysm is the third most common lesion and comprises approximately 5.5 % of all visceral artery aneurysms. The first successful repair was performed by DeBakey and Cooley in 1949. Since then, more than 100 cases have been reported. Fifty to sixty percent of these aneurysms are mycotic in origin. Other less frequent causes include arteriosclerosis, trauma, and medial degeneration. The operations are bypass with autologous tissue or with artificial vascular graft and aneurysmorrhaphy. We have experienced a case of superior mesenteric artery aneurysm which had undergone aneurysmectomy and artificial graft interposition. This is the first domestic case which was successful surgical repaired.

  • Aspergillus fumigatus (A. fumigatus) is one of the most important mycotic infection in wild animals stressed by captivity and concurrent disease. A 14-year-old Carribean flamingo controlled bumble foot with antibiotics died after having dyspnea, depression, and anorexia. Necropsy findings revealed pyogranulomatous inflammations of lung and greenish gray-colored mold on air sacs. A carribean flamingo was diagnosed as pyogranulomatous pneumoniae due to A.fumigatus infection by results of lung tissue culture and microscopic examination.
