Histological observations of age-related changes in the epiglottis associated with decreased deglutition function in older adults

  • Although the epiglottis plays a vital role in deglutition, histological studies of the epiglottis and surrounding ligaments associated with swallowing dysfunction are limited. Therefore, we performed histological observations to clarify age-related changes in the morphological characteristics of the epiglottis and surrounding structures. Tissue samples comprising the epiglottis and surrounding structures were collected from corpses that were both orally fed and tubefed during their lifetimes. Following hematoxylin and eosin, Elastica Van Gieson, and immunohistochemical staining procedures, the chondrocytes, connective tissue, and glandular tissue were observed under the epiglottis epithelium, and intervening adipose tissue was observed in the surrounding area. Fatty degeneration of acinar cells was also observed in the glandular tissue, possibly because of aging. Bundles of elastic fibers were present around the vascular wall in the peri-epiglottic ligament, but some were reduced. Furthermore, large amounts of collagen fibers ran toward and through the cartilage, whereas the mesh-like elastic fibers stopped in front of the cartilage. Microfibrils considered to be oxytalan fibers, which are thinner and shorter than elastic fibers, were observed around the vascular wall and in the fiber bundles. Age-related changes included connective tissue fibrosis shown by the large amount of collagen fibers, atrophy of salivary glands, and an accompanying increase in adipose tissue. Regarding stretchability and elasticity, the elastic fibers may have an auxiliary function for laryngeal elevation during deglutition. This suggests that disuse atrophy of the laryngeal organs with or without oral intake might reduce the amount of elastic fiber in older adults.

말초신경 손상 백서모델에서 초음파 치료의 신경 및 근육 재생효과 (The Effect of Ultrasound Treatment on Regeneration Nerve and Muscle on Injured Peripheral Nerve of Rats Model)

  • The purposes of this study is to discuss and analyze the effect on the recovery from cut in sciatic nerve. This study used 9 weeks male rats of Sprague-Dawley family. Rat in groups 4 were treated with pulsed therapeutic ultrasound for 3 minutes. 3 times weekly at 3MHz respectively (intensity; $0.2W/cm^2,\;0.5W/Cm^2,\;10W/cm^2$); rat in group 1 received placebo ultrasound. In addition, changes of serum aspartate amino-transferase(AST) and creatine phosphokinase(CPK) levels were also demonstrated with diameter of individual muscle fasciculate and number of muscle fiber in each of three types of muscles located in gastrocnemius, soles. The results of comparing the changes in groups are as follows; 1. We found out that hypertrophic epineurium was present in sciatic nerve injured ultrasound treatment of groups. 2. In the gastrocnemius morphological investigation of the group I (control group), severe muscle atrophy were observed at the 7th days of the sciatic nerve injury. however, muscle atrophy of the group IV ($1.0W/cm^2$) were slightly recovered at the 14th days after treatment ultrasound. At the 28th days, muscular fibers were formed in polygon and were significantly recovered. 3. C-fos immunoreactive of the group II ($0.2W/cm^2$), III ($0.5W/cm^2$) were remarkably increased at the 1th day after treatment of ultrasound. Group IV were markedly deceased. 4. Brain-Derived Neurotrophic Factor(BDNF) immunoreactive of the group II, III were increased after 7 days of the sciatic nerve injury. Group IV were markedly increased from 14th days to 28th days after treatment of ultrasound. 5. A significant increase of serum AST levels were demonstrated in control group. However, serum AST levels of massage groups were significantly decreased compared to that of control group in followed order ; ($0.2W/cm^2<0.5W/cm^2<1.0W/cm^2$). 6. A significant increase of serum CK levels were demonstrated in control of group. However, serum CK levels of massage groups were significantly decreased compared to that of control group in followed order ; ($0.2W/cm^2<0.5W/cm^2<1.0W/cm^2$). The above results suggest that ultrasound treatment after peripheral nerve injury might reduce noxious stimuli, facilitate nerve recovery and effective in the functional improvement delaying muscle atrophy or degeneration.

20대와 40대의 급성 및 만성요통환자의 척추주위 근육에 관한 횡단면의 비교 (Characteristic of Cross-sectional Area of Lumbar Paraspinal Muscle in Patients of Acute and Chronic LBP)

  • 이 연구는 20대와 40대의 급성과 만성 요통환자를 대상으로 척추 주위근육의 단면도를 비교하는 것이다. 20대(남자-9, 여자-10)와 40대(남자-8, 여자9)의 대상자들은 MRI로 요추(4번 하부 종단판) 단면도의 척추주위근육(장요근, 다열근, 척추기립근)들의 크기를 PACS(free-handling technique of the picture archiving and communication system(PACS)을 이용하여 측정하였다. 연구 결과 20대 급성과 만성 요통 환자들의 척추 주위근들의 차이는 통계학적으로 유의성이 없게 나타났으나, 40대 급성과 만성 요통환자들의 척추 주위근들의 차이는 만성 요통환자들이 근 위축이 통계학적으로 유의성 있게 나타났다. 또한 20대와 40대의 만성요통환자 척추 주위근들의 차이도 40대 만성요통 환자들에게 유의성 있게 나타났다. 연구에서 20대 만성환자들의 근 위축은 나타나지 않았으나 40대 만성환자들은 다양한 이유로 근 위축이 나타났다. 따라서 40대의 만성 요통환자들은 조기의 다양한 요부 근력 운동 및 기능적 활동 등으로 허리 주위근육의 위축을 감소시키는 것이 필요하다.

The postoperative trismus, nerve injury and secondary angle formation after partial masseter muscle resection combined with mandibular angle reduction: a case report

  • A patient, who underwent partial masseter muscle resection and mandibular angle reduction at a plastic surgery clinic, visited this hospital with major complaints of trismus and dysesthesia. A secondary angle formation due to a wrong surgical method was observed via clinical and radiological examinations, and the patient complained of trismus due to the postoperative scars and muscular atrophy caused by the masseter muscle resection. The need for a masseter muscle resection in square jaw patients must be approached with caution. In addition, surgical techniques must be carefully selected in order to prevent complications, and obtain effective and satisfactory surgery results.

운동 신경원 질환에서의 상부 운동 신경원 침범: 운동 유발 전위 연구 (Upper Motor Neuron Involvement in Motor Neuron Disease: Motor Evoked Potentials Study)

  • Background & Objectives : Motor evoked potentials(MEPs) to magnetic trans cranial stimulation were performed to evaluate upper motor neuron involvement and relationship to lower motor neuron involvement in motor neuron disease patients. Method : MEPs were obtained in the 17 consecutive patients with motor neuron disease. These patients were divided into three group based on clinical evidence of upper and lower motor neuron involvement, bulbar symptom; amyotrophic lateral sclerosis(ALS), progressive muscular atrophy(PMA), progressive bulbar palsy(PBP). MEPs were recorded from abductor pollicis brevis and abductor hallucis muscles. Abnormal MEPs were defined by delayed central motor conduction time or absent MEP. Results : MEPs were abnormal in 64%(11/17) of patients; 100%(7/7) in ALS, 64%(4/7) in PMA, 0%(0/3) in PBP respectively. In 68 total recording muscles, 34 muscles had evidence of motor weakness and showed abnormal responses in 59%(20/34). Whereas 34 muscles with normal strength, only 3%(1/34) of muscles showed abnormal response. Conclusion : MEPs are well correlated with upper motor neuron signs in ALS and may detect masking upper motor neuron signs in PMA. The muscles with lower motor neuron sign(weakness) usually relate with abnormal MEPs reponses.

Imaging Characteristics of Perosomus elumbis in a Puppy

  • A 4-day-old, male Poodle dog was presented with dull, depressed and exhausted activity after the birth. On physical examination, the puppy showed arthrogryposis, muscular atrophy and no movement of hindlimbs. Palpation on dorsum revealed an absence of lumbar and sacral vertebrae. On prenatal and postnatal radiography, lumbar vertebrae, sacrum and coccygeal vertebrae were not visualized. On ultrasonography, bilateral kidney and urinary bladder were observed. On computed tomography, there were no apparent abnormalities in the forelimbs, cervical vertebrae or head, while lumbar vertebrae, sacrum and coccygeal vertebrae were not observed. At necropsy examination, the liver, stomach, intestine, kidney and urinary bladder were normal. This congenital anomaly was consistent with Perosomus elumbis. Perosomus elumbis in dogs is a rare condition of unknown etiology. In this report, Perosomus elumbis was evaluated with radiography, ultrasound and computed tomography.

기능적 전기 자극에 대한 고찰 (A Study on the Functional Electrical Stimulation)

  • Functional Electrical Stimulation (FES) is used for muscle reeducation, reduction of spasticity, delay of atrophy, and muscle strengthening. FES stronger stimulation than other forms of electrical stimulation. The efficacy of FES in improving function has been substantiated in the literature. Treatment programs employing FES - activation of muscular tissue through the intact peripheral nervous system - can be broken into five major categories, according to the goal of treatment. These broad areas would include the use of FES to: (1) a direct excitation onto the alpha motoneuron, through peripheral stimulation of the Ia myotatic sensory system and ascending afferent information, which will be integrated at conscious and subconscious level of the CNS. (2) The quality of a stimulated muscle contraction is determined by combination of many parameters, including stimulus amplitude, pulse duration, stimulus frequency, and duty cycle. (3) A unit that has a pulse duration between 200 and $400{\mu}sec$ will be more than adequate for FES applications. (4) The neuromuscular plasticity is critically important to return of function using muscle re-education and facilitation applications. (5) Prior to using FES as an electrical orthosis, the patient should build up endurance in the muscles to be stimu1ated during the gait cycle.

중환자실 환자의 입원기간에 따른 사지 둘레 변화 및 관련요인 (Changes in Limb Circumferences among Intensive Care Unit Patients and Related Factors)

  • Purpose: This study was conducted to identify changes in limb circumferences among patients admitted to the intensive care unit (ICU) and related factors. Methods: We conducted a prospective observational study with 27 patients from 3 ICUs at a university hospital located in B city of Korea, from September 1 to October 30, 2015. The circumferences of the left and right upper arms, thighs, and lower legs were measured on the first, third, fifth, and seventh days of ICU admission. Information on the related factors was collected from the medical records. The data were analyzed using a linear mixed model method. Results: The limbs circumferences significantly reduced from day 3, and the changes continued till day 7. These changes were related to the gender of the subjects, restraints application, use of steroids, and continuous renal replacement therapy. Conclusions: Based on the above results, it can be concluded that ICU-acquired weakness begins before the third day of admission. Thus, early mobilization protocols for ICU patients need to be developed and implemented in order to improve long-term outcomes.

Dual effect of Low- frequency Electromagnetic Field on Muscle Histopathology of Caspian Sea Cyprinus carpio

  • The effect of electromagnetic field on aquatic organisms has received little attention. In the current study, the effect of 50Hz electromagnetic field on muscle histopathology of Caspian Sea Cyprinus carpio, a species of economic importance, was investigated. A total of 120 healthy fish were used in this study. They were classified randomly in one of two groups as follows: Control or unexposed EMF group and experimental group with 5 different magnetic field intensities (0.1, 1, 3, 5 and 7mT) at 2 different exposure times including 30 and 60 minutes. Fish in the experimental group were exposed only once. Two weeks after exposure, dorsal muscles sectioned transversely, stained and were examined using a light microscope. Histopathologic assessments showed significant difference between control and EMF exposed groups at both 30 min. (p<0.01) and 60 min. (p<0.001) exposure times. We report for the first time that electromagnetic field in interaction with muscular tissue of Cyprinus carpio exhibits a dual effect which depends on the field intensity, and exposure time. At short exposure time (30 min.), EMF stimulates muscle growth process. At longer exposure time (60 min.), EMF can damage muscle tissue and result in muscle necrosis. More research is required to elucidate precise mechanisms involved in muscle hypertrophy and pathologic changes.

운동생리학으로 본 노인과 운동 (Aged and Exercise in view of Exercise Physiology)

  • Making aged exercise by their age it unreasonable because there an differences of personal strength of their body. The three elements which control the aged's possibility of exercising effectivencess are the level of the physical strength in the present state, potential capacities of physical strength and age. Examined the physical strength of aged, the pliancy of body is considerably marked decline by and large and fast twitch muscle are withered. The reason why aged often do prime exercise like and infant. Aged are not fit for the speedy exercise and they can give full scope to their ability by continuing the long time exercise. The reason why aged should dwindle is mainly the retardation of recovering their fatigue which is related to the decline of oxygen activity. Anaerobic energy supply reduction makes aged be bad for severe physical exercise. Basal metabolic rate of rely on physical cellular quantity and people exercising has a high basal metabolic rate because of muscular amount. There were no decline of basal metabolic rate fellowing the age and physical non activiti influences on the atrophy of muscle.
