Internal Motivation, Perceived Health Competency, and Health Literacy in Primary and Secondary Cancer Prevention

  • Objective: The aim of this study was to identify associations of internal motivation, perceived health competency, and health literacy with primary and secondary cancer prevention. Methods: A telephone survey was conducted with a sample of 2,700, 30-69 year olds, proportionally extracted from Gangwon Province, South Korea. The dependent variables were actions in primary and secondary prevention and the explanatory variables were 13 questions in three areas: internal motivation (4 items), perceived health competency (4 items), and health literacy (5 items). Result: Multiple linear regression analysis showed that internal motivation, perceived health competency, and health literacy positively impacted primary prevention after controlling for gender and age. As internal motivation, perceived health competency, and perceived literacy increased by 1 point, primary prevention scores increased by 0.11, 0.11, and 0.07 points, respectively. In addition, logistic regression results for secondary prevention showed that health literacy had a positive impact on secondary behavior. As health literacy increased by 1 point, the odds ratio of the practice of secondary prevention was 1.4 times higher. Conclusion: This study suggests that primary and secondary prevention of cancer are significantly related to intrinsic motivation factors, perceived health competency, and actual health literacy. Health literacy concepts that cover the capacity of health management in comprehensive areas need to be applied to education and promotion for improvement of primary and secondary prevention of cancer.

Neurochemical Characterization of the TRPV1-Positive Nociceptive Primary Afferents Innervating Skeletal Muscles in the Rats

  • Objective: Transient receptor potential vanilloid subfamily type 1 (TRPV1), a most specific marker of the nociceptive primary afferent, is expressed in peptidergic and non-peptidergic primary afferents innervating skin and viscera. However, its expression in sensory fibers to skeletal muscle is not well known. In this study, we studied the neurochemical characteristics of TRPV1-positive primary afferents to skeletal muscles. Methods: Sprague-Dawley rats were injected with total $20{\mu}l$ of 1% fast blue (FB) into the gastrocnemius and erector spinae muscle and animals were perfused 4 days after injection. FB-positive cells were traced in the L4-L5 (for gastrocnemius muscle) and L2-L4 (for erector spinae muscle) dorsal root ganglia. The neurochemical characteristics of the muscle afferents were studied with multiple immunofluorescence with TRPV1, calcitonin gene-related peptide (CGRP) and $P2X_3$. To identify spinal neurons responding to noxious stimulus to the skeletal muscle, 10% acetic acids were injected into the gastrocnemius and erector spinae muscles and expression of phospho extracellular signal-regulated kinase (pERK) in spinal cords were identified with immunohistochemical method. Results: TRPVl was expressed in about 49% of muscle afferents traced from gastrocnemius and 40% of erector spinae. Sixty-five to 60% of TRPV1-positive muscles afferents also expressed CGRP. In contrast, expression of $P2X_3$ immnoreaction in TRPV1-positive muscle afferents were about 20%. TRPV1-positive primary afferents were contacted with spinal neurons expressing pERK after injection of acetic acid into the muscles. Conclusion: It is consequently suggested that nociception from skeletal muscles are mediated by TRPV1-positive primary afferents and majority of them are also peptidergic.

Analysis of Joint Multiband Sensing-Time M-QAM Signal Detection in Cognitive Radios

  • We analyze a wideband spectrum in a cognitive radio (CR) network by employing the optimal adaptive multiband sensing-time joint detection framework. This framework detects a wideband M-ary quadrature amplitude modulation (M-QAM) primary signal over multiple nonoverlapping narrowband Gaussian channels, using the energy detection technique so as to maximize the throughput in CR networks while limiting interference with the primary network. The signal detection problem is formulated as an optimization problem to maximize the aggregate achievable secondary throughput capacity by jointly optimizing the sensing duration and individual detection thresholds under the overall interference imposed on the primary network. It is shown that the detection problems can be solved as convex optimization problems if certain practical constraints are applied. Simulation results show that the framework under consideration achieves much better performance for M-QAM than for binary phase-shift keying or any real modulation scheme.

  • Spectrum sensing, as a fundamental functionality of Cognitive Radio (CR), enables Secondary Users (SUs) to monitor the spectrum and detect spectrum holes that could be used. Recently, the security issues of Cognitive Radio Networks (CRNs) have attracted increasing research attention. As one of the attacks against CRNs, a Primary User Emulation (PUE) attack compromises the spectrum sensing of CR, where an attacker monopolizes the spectrum holes by impersonating the Primary User (PU) to prevent SUs from accessing the idle frequency bands. Energy detection is often used to sense the spectrum in CRNs, but the presence of PUE attack has not been considered. This study examined the effect of PUE attack on the performance of energy detection-based spectrum sensing technique. In the proposed protocol, the stationary helper nodes (HNs) are deployed in multiple stages and distributed over the coverage area of the PUs to deliver spectrum status information to the next stage of HNs and to SUs. On the other hand, the first stage of HNs is also responsible for inferring the existence of the PU based on the energy detection technique. In addition, this system provides the detection threshold under the constraints imposed on the probabilities of a miss detection and false alarm.

Strong concordance between RNA structural and single nucleotide variants identified via next generation sequencing techniques in primary pediatric leukemia and patient-derived xenograft samples

  • Acute leukemia represents the most common pediatric malignancy comprising diverse subtypes with varying prognosis and treatment outcomes. New and targeted treatment options are warranted for this disease. Patient-derived xenograft (PDX) models are increasingly being used for preclinical testing of novel treatment modalities. A novel approach involving targeted error-corrected RNA sequencing using ArcherDX HemeV2 kit was employed to compare 25 primary pediatric acute leukemia samples and their corresponding PDX samples. A comparison of the primary samples and PDX samples revealed a high concordance between single nucleotide variants and gene fusions whereas other complex structural variants were not as consistent. The presence of gene fusions representing the major driver mutations at similar allelic frequencies in PDX samples compared to primary samples and over multiple passages confirms the utility of PDX models for preclinical drug testing. Characterization and tracking of these novel cryptic fusions and exonal variants in PDX models is critical in assessing response to potential new therapies.

Characterization of Embryonic Feather Follicle Development in the Chinese Indigenous Jilin White Goose

  • To investigate goose feather follicle development and difference among the dorsal, ventral, and thoracal tracts during embryonic stage, the present study was conducted on 180 embryos at different ages obtained from the Jilin White goose, a Chinese indigenous breed. The study indicated that the epidermis and dermis of goose embryo formed between embryonic day 10 (E10) and 12 (E12). The thickness of the epidermis remained unchanged until hatching; while the thickness of the dermis increased throughout embryonic development. The primary feather follicles formed around E13-E14 and there were no new primary feather follicles forming after E18. The secondary feather follicles formed coincidently at E18. The density of primary and secondary feather follicles on the ventral and thoracal tracts were significantly higher than those on the dorsal tract (p<0.05). For primary and secondary follicles, the diameter of the feather bulbs and the depth of the feather follicles on the dorsal tract were much greater than those on the thoracal and ventral tracts (p<0.01), respectively; while the difference between the ventral and thoracal tracts was not significant (p>0.05). It is concluded that the Jilin White goose is of a single-follicle group structure, differing from mammals which are of multiple-follicle group structure.

Second Primary Malignant Neoplasms: A Clinicopathological Analysis from a Cancer Centre in India

  • Context: Patients diagnosed with a cancer have a life time risk of developing another de novo malignancy depending on various inherited, environmental and iatrogenic risk factors. Of late the detection of new primary has increased mainly due to refinement in both diagnostic and treatment modalities. Cancer victims are surviving longer and thus are more likely to develop a new metachronous malignancy. Aims: To report our observed trend of increase in prevalence of both synchronous and metachronous second malignant neoplasms among cancer victims and to review the relevant literature. Settings and Design: A hospital based retrospective collection of prospective data of patients diagnosed with second denovo malignancy. Materials and Method: The study was conducted over a 5 year period from July 2008 to June 2012. All patients diagnosed with a histologically proven second malignancy as per Warren Gate's criteria were included. Various details regarding sex, age at presentation, synchronous or metachronous, treatment and outcome were recorded. Conclusions: The occurrence of multiple primary malignancies is not rare. Awareness of the possibility alerts the clinician in evaluation of patients with a known malignancy presenting with unusual sites of metastasis. Individualizing the treatment according to the stages of the primaries will result in durable cancer control particularly in synchronous double malignancy.

중심지이론을 이용한 의사의 지역적 분포에 관한 실증분석 (An Empirical Analysis on Geographic Distribution of Physicians using the Central Place Theory)

  • This study provides an empirical analysis of location competition for demand maximization by central place theory among physicians in nonmetropolitan areas of Korea. The results show that the primary care physicians distribute themselves evenly from urban communities to rural communities. The subspecialists, however, cluster together in major cities rather than decentralize themselves in rural counties. This study establishes the three statistical models : Primary care physicians, subspecialist physicians, and total physicians. Two models of primary care physicians and total physicians have a strong significance in multiple regression analysis (p=-.0001). The primary care model explains approximately 45% of the variation and the total physicians model explains approximately 70% of the variation in physician/1,000 population ratios across national counties. The subspecialist physicians model analysze the tobit regreassion because of the left consored and truncated values(57 cases = 0). In all three models, analysis of the coefficiencts for physician centralization degrees in the 0- to 5- and 5- to 10-km rings around the core county reveals each a positive and negative association betwee these degrees and the physician/1,000 population ratios in the core county. Also, the results provide moderate evidence that the relationship between clinic physicians and community hospitals is competitive, and the relationship between clinic physicians and pharmaceutists is synergistic. This suggests that public policy makers and local self-governing bodies must take an active role to ensure procider availability and the regional health planning in all nonmetropolitan areas of Korea.

  • 최근 주파수 지원의 효율적 사용에 대한 관심이 높아짐에 따라 Cognitive Radio가 활발히 연구되고 있다. 기존에는 Cognitive Radio의 이기적 공생방식과 비이기적 공생방식을 이용하여 시스템 용량을 향상하는 방안이 제안되었다. 이에 본 연구에서는 OFDMA 기반의 CR 네트워크에서 효율적인 부반송파 할당을 통한 시스템 용량 극대화를 제안한다. 또한 하나의 Primary 사용자가 존재하는 네트워크를 다수의 Primary 사용자가 존재하는 네트워크로 확장하여 극대화 방안을 적용하는 방안과 그 영향을 분석한다.

  • Objectives : The purpose of this study is to examine the relationship between the symptoms of primary dysmenorrhea and pulse energy of Chon, Gwan and Cheok. Methods The subjects of this study who had certain range of dysmenorrhea symptom. The degree of dysmenorrhea were assessed by Visual Analog Scale (VAS), Multidimensional Verbal Rating Scale (MVRS), Cox Menstrual Symptom Scale (CMSS) and measured by 3D Blood Pressure Pulse Analyzer(3D-MAC) tests at menstruation start date. Data analysis included descriptive statistics and multiple linear regression using the SPSS/WIN 18.0 program. Results : The induced results are as follows;The induced results are as follows 1. There were no significant relationship between the pulse energy and the sum values of VAS, MVRS and checking part of severity of CMSS. 2. The frequency domain of CMSS were significantly associated with pulse energy. 3. In linear regression models, the model of Left Chon had the especially highest value of explanatory power. (R 2 = 0.517) Conclusions : The pulse energy changes are related to the symptoms of primary dysmenorrhea.
