Effects of sleep quality and occupational stress on health-related quality of life among a university's security workers (일개 대학 경비근로자의 수면의 질과 직무스트레스가 건강관련 삶의 질에 미치는 영향)

  • Objectives: The purpose of this study was to identify the effects of sleep quality and occupational stress on health-related quality of life among a university's security workers. Methods: 128 security workers voluntarily participated in the study. The survey asked demographic, health related, job related factors, the Korean Occupational Stress Scale Short Form(KOSS-SF), sleep quality questionnaire, and 12-Short Form Health Survey version 2.0(SF-12v2). The data were analyzed by using descriptive analysis, t-test, ANOVA, multiple linear regression, and multiple logistic regression. Results: The interrupted rest time was significantly negatively related to occupational stress. The occupational stress significantly affected sleep quality. The significant variable affecting Physical Component Score(PCS) and Mental Component Score(MCS) was sleep quality and occupational stress, respectively. Conclusions: HRQOL was founded to be affected by occupational stress and sleep disorder among the security workers. Based on this study, we suggest that the management of both occupational stress and sleep quality is crucial to improve the HRQOL.

Infection with Opisthorchis viverrini and Use of Praziquantel among a Working-age Population in Northeast Thailand

  • Infection with Opisthorchis viverrini (OV) due to eating certain traditional freshwater fish dishes is the principal risk factor for cholangiocarcinoma in Northeast Thailand where the infection is endemic and the incidence of this form of primary liver cancer has been the highest in the world. This paper is the second report of a prospective research project to monitor the impacts of a national liver fluke control programme in a rural community of Northeast Thailand. A sample of 684 villagers aged 20-65 years completed an interview questionnaire and were tested for infection using the Kato thick smear technique. The questionnaire was designed for the exploration of associations between OV infection, previous treatment with praziquantel, and knowledge and beliefs about the drug. The data were analysed using descriptive statistics and multiple logistic regression. The overall prevalence of OV infection was 37.2% and was highest in the 20-35 year age group, in those with a university degree and in those employed in the government sector. As many as 91.8% reported eating fish dishes known to place them at risk of infection. In the multiple regression analysis, previous use of praziquantel and lack of knowledge about whether or not the drug has a protective effect against re-infection were the only factors related to OV infection ($OR_{adj}$= 2.31, 95%CI =1.40-3.79 and $OR_{adj}$= 1.95, 95%CI= 1.24-3.05). The findings were discussed in terms of the possibly unwise dependency on praziquantel as a primary element in a control programme.

The Convergent Influence of the Incidence of Delirium in Patients after Arthroplasty (인공관절치환 수술 후 발생하는 섬망에 따른 융복합적 영향)

  • This study examined artificial joint replacement surgery for early intervention and prevention of the occurrence of delirium surgery. Data of study were analysed using the sample through the EMR (Electronic Medical Record) and after surgery to provide basic data. The subjects were elderly aged 60 years or more and the number of the sample was 821. Data were analysed by using SPSS 20.0 with t-test, $x^2$-test and multiple logistic regression analysis. The study results showed patients with artificial joint replacement surgery incidence of delirium was 13.5%, findings of these variables insisted that the main influencing factors of delirium were caused by age, fall history, physical activity, emotional status, body mass index (BMI) before surgery. The study suggested that the above findings are required for early intervention, early detection and prevention of delirium.

Testing of the Theory of Planned Behavior in the Prediction of Smoking Cessation Intention and Smoking Cessation Behavior among Adolescent Smokers (청소년 흡연자의 금연의도 및 금연행위 예측을 위한 계획적 행위이론(Theory of Planned Behavior)의 검증)

  • Objectives: The purpose of this study was to test the Theory of Planned Behavior (TPB) in the prediction of smoking cessation intention and smoking cessation behavior among adolescent smokers, in order to provide basic data to develop a future smoking cessation program as a nursing intervention. Method: The study subjects were 80 adolescent smokers who had smoked one cigarette and attended a five-day school smoking cessation program. The data were collected from October 24 to December 21, 1999. The instruments used in this study were the tools developed by Jee (1994) to measure TPB variables such as attitude toward smoking cessation behavior, subjective norm, perceived behavioral control, smoking cessation intention, and smoking cessation behavior. The data were analyzed with the SAS/PC program using descriptive statistics, hierarchical multiple regression, and logistic multiple regression. Results: 1. Attitude toward smoking cessation behavior, subjective norm, and perceived behavioral control were partially significant in predicting smoking cessation intention. 2. Smoking cessation intention and perceived behavioral control toward smoking cessation behavior did not significantly predict smoking cessation behavior. 3. There were partial interaction effects among the attitude toward smoking cessation behavior, subjective norm, and perceived behavioral control in the prediction of smoking cessation intention. 4. There were partial interaction effects between smoking cessation intention and perceiver behavioral control toward smoking cessation behavior in the prediction of smoking cessation behavior. Conclusion: This study partially demonstrated support for the TPB model that was partially useful in predicting smoking cessation intention and smoking cessation behavior among adolescent smokers. Therefore, it is recommended that attitude toward smoking cessation behavior and perceived behavioral control should be considered in developing smoking cessation programs and implementing nursing interventions to change the smoking behavior of adolescent smokers.

Disease Prevention Knowledge, Anxiety, and Professional Identity during COVID-19 Pandemic in Nursing Students in Zhengzhou, China

  • Purpose: This study aimed to evaluate nursing students' understanding of the prevention of COVID-19, as well as their anxiety towards the disease and their perception of their professional identity in the wake of the pandemic, in Zhengzhou, China. Methods: A cross-sectional study was designed to investigate 474 nursing students by cluster sampling using a stratified questionnaire from February 15 to March 31, 2020. Multiple linear regression was used to identify the factors affecting professional identity. Binary and multiple logistic regression were used to identify the factors affecting anxiety. Results: Responders with a high level of understanding of COVID-19 and frequent use of behavioral strategies for its prevention comprised 93.2% and 30.0% of the cohort, respectively. Professional identity was significantly associated with gender and anxiety (p < .050). The prevalence of anxiety among nursing students was 12.4%. Male (odds ratio [OR] = 2.39; 95% confidence interval [CI] = 1.26~4.52), sophomores (OR = 5.30; 95% CI = 1.61~7.45), and infrequent use of prevention measures (OR = 3.49; 95% CI = 1.16~5.19) had a significant effect on anxiety. Conclusion: Anxiety during the COVID-19 epidemic gives an adverse effect on the professional identity of nursing in students. Nursing education institutions need to provide psychological counseling services for nursing students, in addition to improving their teaching of COVID-19 prevention strategies.

Affecting Factors on Financial Performance of Medical Institutions in Taegu Metropolitan and Kyungpook Province (의료기관의 재무성과에 영향을 미치는 요인)

  • This study was performed to assess the financial performance of medical institutions and examine the affecting factors in Taegu metropolitan and Kyungpook province. The major results are as follows ; The liability to total assets was significantly different according to the ownership type of medical institution. It was the highest in the case of juridical person having a special status. The total assets turnover and value added to total assets were significantly different according to the type of medical institution, period of establishment, and ownership type of medical institution. They were higher in the tertiary medical institution, in private hospital and university hospital, and they were increased with establishment period of medical institution. The growth rate of patient revenues were significantly different according to the type of medical institution, period of establishment, and the growth rate of adjusted inpatient days were significantly different by period of establishment. The return on assets and net profit to gross revenues were the highest in private hospital. Private hospital went into the black, but other hospitals went into the red figures. According to the multiple regression analysis for the net profit to gross revenues, ownership type, period of establishment, and management strategy of medical institution were significant It was higher in private hospital and medical institution with prospector management strategy, and it was increased with establishment period of medical institution. In multiple logistic regression analysis for the status of financial performance in medical institution, period of establishment, management strategy of medical institution were significant It was better in medical institution with prospector management strategy and longer period of establishment.

Influence of achievement motivation and self concept on clinical practice satisfaction in the dental hygiene students (치위생과 학생의 성취동기 및 자아개념이 임상실습 만족도에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the influence of achievement motivation and self concept on clinical practice satisfaction in the dental hygiene students. Methods: A self-reported questionnaire was filled out by 206 dental hygiene students in Jeonnam from December to October, 2013. The questionnaire consisted of 6questions of general characteristics, 6 questions of achievement motivation, 18 questions of self concept, and 23 questions of clinical practice satisfaction. Cronbach's alpha was 0.881, 0.889, and 0.935 in achievement motivation, self concept, and clinical practice satisfaction, respectively. The data were analyzed by t-test, one way ANOVA, Pearson correlation coefficient and multiple regression analysis. Results: Clinical practice satisfaction showed a significant difference by grade(p<0.05), personality(p<0.05) and satisfaction of major(p<0.01). Clinical practice satisfaction had a positive correlation with achievement motivation(r=0.508, p<0.01) and self concept(r=0.324, p<0.01). Multiple logistic regression analysis revealed that clinical practice satisfaction was related with achievement motivation(p<0.05) and self concept(p<0.01). Conclusions: The clinical practice satisfaction depends on achievement motivation and self concept. It is necessary to develop the program to improve satisfaction level of clinical practice by achievement motivation and self concept.

Association between fish and shellfish, and omega-3 PUFAs intake and CVD risk factors in middle-aged female patients with type 2 diabetes

  • BACKGROUND/OBJECTIVES: This study was performed to investigate the association between the dietary intake of fish and shellfish, and omega-3 polyunsaturated fatty acids (PUFAs) and cardiovascular disease (CVD) risk factors in the middle-aged Korean female patients with Type 2 diabetes (T2D). SUBJECTS/METHODS: A cross-sectional analysis was performed with 356 female patients (means age: 55.5 years), who were recruited from the Huh's Diabetes Clinic in Seoul, Korea between 2005 and 2011. The dietary intake was assessed by a validated semi-quantitative food frequency questionnaire and analyzed using the Computer Aided Nutritional Analysis program (CAN-Pro) version 4.0 software. RESULTS: In a multiple regression analysis after the adjustment for confounding factors such as age, BMI, duration of diagnosed T2D, alcohol consumption, fiber intake, sodium intake, and total energy intake, fish and shellfish intake of the subjects was negatively associated with triglyceride and pulse wave velocity (PWV). Omega-3 PUFAs intake was negatively associated with triglyceride, systolic blood pressures, diastolic blood pressures, and PWV. The multiple logistic regression analysis with the covariates showed a significant inverse relationship between the omega-3 PUFAs consumption and prevalence of hypertriglyceridemia [OR (95% CI) for greater than the median compared to less than the median: 0.395 (0.207-0.753)]. CONCLUSIONS: These results suggest that the consumption of fish and shellfish, good sources of omega-3 PUFAs, may reduce the risk factors for CVD in the middle-aged female patients with T2D.

Association between dietary flavanones intake and lipid profiles according to the presence of metabolic syndrome in Korean women with type 2 diabetes mellitus

  • BACKGROUND/OBJECTIVES: This study was aimed at examining the association between dietary flavanones intake and lipid profiles according to the presence of metabolic syndrome (MetS) in Korean women with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A cross-sectional analysis was performed among 502 female T2DM patients (non-MetS group; n = 129, MetS group; n = 373) who were recruited from the Huh's Diabetes Clinic in Seoul, Korea between 2005 and 2011. The dietary intake was assessed by a validated semi-quantitative food frequency questionnaire (FFQ) and the data was analyzed using the Computer Aided Nutritional Analysis program (CAN-Pro) version 4.0 software. The intake of flavanones was estimated on the basis of the flavonoid database. RESULTS: In the multiple linear regression analysis after adjustment for confounding factors, daily flavanones intake was negatively associated with CVD risk factors such as total cholesterol, LDL-cholesterol, and apoB and apoB/apoA1 ratio only in the MetS group but not in the non-MetS group. Multiple logistic regression analysis revealed that the odds ratio for a higher apoB/apoA1 ratio above the median (${\geq}0.74$) was significantly low in the $4^{th}$ quartile compared to that in the $1^{st}$ quartile of dietary flavanones intake [OR: 0.477, 95% CI: 0.255-0.894, P for trend = 0.0377] in the MetS group. CONCLUSIONS: Dietary flavanones intake was inversely associated with the apoB/apoA1 ratio, suggesting a potential protective effect of flavanones against CVD in T2DM women with MetS.

Associations between dietary patterns and screen time among Korean adolescents

  • Data are limited on the association between dietary patterns and screen time among Korean adolescents. The present study identified dietary patterns of 691 adolescents, aged 13-18 years, who had participated in the Third Korean National Health and Nutrition Examination Survey (KNHANES III) and analyzed their associations with screen time. Screen time was defined as the time spent watching TV, using a computer, or playing video games was calculated as a sum of all these times. Dietary patterns and their factor scores were derived from a food frequency questionnaire using the factor analysis method. To analyze the association between dietary patterns and screen time, we conducted multiple linear regression analysis. We also performed multiple logistic regression analysis to estimate odds ratios (OR) of excessive screen time (2 hours or longer per day) and 95% confidence intervals (CI). We identified 2 dietary patterns labeled "the Korean healthy dietary pattern" and "the Western diet and fast foods pattern". The former included mixed grains, legumes, potatoes, red meat, eggs, fish, dairy products, fruits, vegetables, seaweeds, and mushrooms, whereas the latter included noodles, bread, red meat, poultry, fast foods, snack, and soft drinks. After controlling for potential confounding factors, factor scores for the Korean healthy dietary pattern were inversely associated (P-value for trend < 0.01) and those for the Western diet and fast foods pattern were positively associated with the screen time (P-value for trend < 0.01). Adolescents in the top tertile of the scores for the Korean healthy dietary pattern had a multivariable-adjusted OR [95% CI] of 0.44 (0.25-0.75) for excessive screen time compared with those in the lowest tertile. On the basis of these findings, adolescents who have excessive screen time may need to be encouraged to consume a more healthy diet.
