Objectives : We wanted to evaluate the medical underutilization for benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS) among Korean elderly men and we wanted to determine their associated factors. Methods : This study was conducted on 239 men with LUTS and 116 men with BPH who were compatible with the diagnostic criteria from a total of 641 participants. These participants were over 50 years old and they were randomly chosen in a community-based study for estimating the prevalence of BPH. Using a self-reported questionnaire, we surveyed the sociodemographics, health status, quality of life, lower urinary tract symptoms, medical utilization and reasons for not seeking treatment. Results : Only 27.6% of the men with LUTS and 31.0% of the men with BPH reported having visited a doctor for urinary symptoms. The reasons for not visiting a doctor were, in order of responses from the group with LUTS: 'considered the symptoms as a part of the normal ageing process', 'not enough time to visit a doctor', 'financial difficulty' and 'the symptoms were not severe or bothersome'. Regarding BPH, the responses were the same as those of the group with LUTS however, 'financial difficulty' placed second. Among the men with experience of visiting a doctor for urinary symptoms, 33.3% of those with LUTS and 28.1% of those with BPH were not treated. The most common reason in both groups was 'the symptoms were not severe to be treated'. On a multiple logistic regression analysis, the larger size household (odds ratio (OR) 3.03, 95% confidence interval (CI)=1.40-6.54) and an unsatisfactory quality of life related with urinary symptoms (OR 2.98, 95% CI=1.23-7.21) were associated with medical utilization in the group of LUTS. For BPH, the current employment status was related with the medical utilization (OR 2.80, 95% CI=1.10-7.11), in addition to the larger size household (OR 3.24, 95% CI=1.14-9.21). Conclusions : Many men with urinary symptoms do not visit a doctor. This medical underutilization for people with LUTS and BPH may be associated with economic status in Korea.
Objectives : To assess the prevalence of knee osteoarthritis through proper diagnosis in a rural community and to elucidate the relevant factors of this health problem in order to develop educational methods for the prevention of female knee osteoarthritis as well to provide basic data for prospective research. Methods : Over a period of three months starting from August, 2000, 432 women over the age of 40 and living in a rural area were selected by a multistage cluster sampling method in order to investigate the prevalence of knee osteoarthritis and the related factors. The study utilized interviews and radiological examination. The criteria of knee osteoarthritis used was the American College of Rheumatology classification criteria for osteoarthritis of the knee(1995). Results : The adjusted prevalence of knee osteoarthritis among the subjects was 55.0%, and multiple logistic regression analysis showed that the odds ratio(OR) of knee osteoarthritis among subjects 50-59 years of age and subjects older than 60 years of age as compared to subjects 40-49 years of age were 2.43(95% CI: 1.30-4.70) and 4.40(95% CI: 2.47-7.83), respectively. The OR of knee osteoarthritis among factory workers or farmers as compared to others was 1.79(95% CI: 1.03-3.12). The OR of knee osteoarthritis among subjects who had a family history and subjects who had knee injury or surgical history against those subjects had neither were 2.56(95% CI: 1.42-4.03) and 4.70(95% CI:1.45-15.19), respectively. The OR of knee osteoarthritis among smokers against non smokers was 0.47(95% CI: 0.22-0.97). Conclusions : Related factors of knee osteoarthritis included age, occupation, family history, smoking, knee injury and history of surgery In order to prevent knee osteoarthritis in high risked rural woman, education concerning self-care methods and safety guideline must be provided at the work place by the public and private health sectors. Additionally, these women should be continually encouraged to exercise, including jogging and swimming regularly.
Objectives : To investigate the current status of hospital-based health promotion programs in Korea and to elucidate the factors which affect to the process of implementation. Methods : We conducted a mail survey of all 875 hospitals in Korea from March to May 2001. In reference to 12 specific kinds of health promotion programs, hospital CEOs were asked whether their hospital have such programs, whether they are fully staffed and whether the program is paid for by the patients. Contextual factors(location, hospital type, number of beds, length of operation, public/private status, economic level of the community, the level of competition) and organizational factors (the extent of market, compatibility with vision, formalization), strategic types of the CEOs (defender/analyser/prospector) were also surveyed. The relationships between each variable and the implementation of health promotion services, activation of services, and the target groups(patient/community resident) were analyzed by univariate analysis and the independent effects of these variables were examined with multiple logistic regression. Results : 100 of 125 hospitals responding (84.8%) had mere than one health promotion program. However, they showed fluctuations in the adoption rate of each programs, meaning that comprehensive health promotion services were not provided. Many programs were not fully staffed and few hospitals had paid programs. In factors affecting health promotion service implementation, private hospitals showed a higher rate in implementation than public hospitals. In contrast, when the competition among nearby hospitals was intense, the level of implementation of service lowered. In the strategic type of the CEOs, the prospectors were shown to have instituted more health promotion programs in their hospitals and the analysers had a greater tendency to have programs for community residents than the defenders. Conclusion . Considering the above results, contextual factors may contribute greatly to the introduction of health promotion services in Korean hospital, although the CEO's personal preference and organizational factors play a larger role in the activation of services. Additionally, the CEO's personal preference may be the major influencing factor in the introduction of programs for community residents.

This study was performed to evaluate relationship between health behaviors of local residents and stomach cancer incidence. Case-control study was carried out on 91 cases matched for age and sex with 91 controls in a general hospital in Seoul. It was surveyed using questionnaires and interview from December 20, 2008 to January 30, 2009. Major findings are as follows. First, significant negative association was observed in recreational activity ; OR for active group was 0.23, 0.41 relative to the inactive group. Secondly alcohol consumption was a significant risk factor; relative to non-drinker for drinker of more than 30 years(OR=5.17 95% Cl=1.78-19.95). Thirdly, Eating broiled food showed a positive significant association with risk for stomach cancer(OR=3.79 95% Cl=1.61-9.84) for consumption of three times more per week. As a result of multiple logistic regression analysis, income, BMI 10 years ago, recreational activity, alcohol consumption, eating broiled and salt food were associated with stomach cancer. In conclusion, the correlation of stomach cancer incidence can be found in recreational activity, alcohol consumption, eating broiled and salt food, income, and BMI 10 years ago.

The purpose of this study was to investigate eating habits and patterns in accordance with the mental health status in Korea. Data from the fifth Korean National Health and Nutrition Examination Survey in 2010-2012 by the CDCP were used to target 19-year-olds. A total of 15,484 subjects (6,348 men and 9,500 women) were enrolled in this study. The collected data were analyzed using frequency analysis, cross-analysis, and multiple logistic regression analysis. The results revealed that men showed, with linear tendency, a decrease in stress and suicidal ideation with an increase in the number of meals. (p<.05). Women also showed, with linear tendency, that an increase in the number of meals resulted in a reduction of stress, depression, and suicidal ideation (p<.05). The stress level of women decreased by 0.667 times when they ate two meals rather than one time meal per day. If women ate three meals compared with one meal per day, stress, depression, and suicidal ideation were reduced by 0.522, 0.605, and 0.508 times, respectively (p<.05). It was found that Aa regular eating habit may have an impact on stress, depression, and suicidal ideation. Moreover, irregular meals, brought on by modern influences, could have a negative impact on mental health, leading to social and economic losses. These results suggest that we need to educate and promote awareness on proper eating habits.
Purpose: The number of patients with dyslipidemia have been increasing steadily over the past few decades in South Korea. The association between the chromium level and chronic disease has attracted considerable interest, but few studies have been conducted on the Korean population. The aim of this study was to identify the dietary and non-dietary correlates of the toenail chromium level, and evaluate the association between the toenail chromium level and dyslipidemia. Methods: The baseline data of an ongoing prospective cohort study in Yeungnam area in South Korea were analyzed. A total of 500 participants aged 35 years or older who completed questionnaires on their demographics, lifestyle characteristics, and medical information were included. The toenail chromium level was analyzed by neutron activation analysis. The dietary intake was assessed using a validated 146-item semi-quantitative food frequency questionnaire. The blood lipid profiles were obtained from medical examinations conducted by the Korean National Health Insurance Service or medical institutions. Results: Higher chromium levels were associated with the residential area (urban), higher education level, higher intakes of noodles and vegetables, and lower intake of fruits. Multiple logistic regression analysis showed that the toenail chromium levels were not associated significantly with the prevalence of dyslipidemia (odds ratio: 0.99, 95% confidence interval: 0.61 ~ 1.60). Conclusion: This study is the first study in Korea to determine the independent correlates of the toenail chromium levels and the association between chromium exposure and dyslipidemia. These findings provide useful scientific evidence for the development of chromium intake guidelines for the Korean population.

Objective : Local Community and Nursing-home of the elderly of the general characteristics and time use compared with the Occupational balance of the two groups is to look at what is being done. Method : The study conducted by the Time Use Survey 2009 of National Statistical office, the results were statistically analyzed 246 by based on more than 65 years of age and older living in Daejeon. The Comparison subjects of this study compared the Daejeon nursing homes in two locations were selected and 146 people of use on the general characteristics and time use data were collected. Analysis using SPSS 12.0 for correlation analysis and multiple logistic regression analysis for the study were analyzed. Result : Local Community and Nursing-home of the elderly are compared. Each classified behavior correlation showed significant results in a difference in a time-use, respectively and also Nursing-home of the elderly, non-activity time in the form of the activity time was higher than the results. Conclusion : Through this research, according to the living environment for elderly people with a time use to find out that there was significant, the elderly living in Nursing-home more than the elderly living in the community and show the imbalance of the Occupational could be.
Purpose: The present study intended to reveal the association between the physique of adolescent women and dysmenorrhea, and the factors related to the frequency of dysmenorrhea. Methods: The study subjects included 511 girls of a women's high school in Daejeon city and they were, during the period of March 1st through April 30th, 2005, given self-administered questionnaires about frequency of menstruation, age, physique, exercise, and such menstruation-related items as age of menarche, menstrual period and days, and amount. Results: As with the frequency of dysmenorrhea, 38.4% experienced it 「always」, 44.6% 「occasionally」, and 17.0% 「almost scarcely」, and those with dysmenorrhea accounted for 83.0% of the subjects. The frequency of dysmenorrhea was not significant difference according to the BMI and HPI. However, the lower the age of menarche and the greater the amount of menstrual flow, the more frequent dysmenorrhea. The symptoms of dysmenorrhea included the highest frequency of low back pain(68.5%) and it was followed by abdominal pain(65.9%), sensibility(54.0%), fatigue(51.7%), and nervousness(49.5%). These accompanied symptoms tended to be worse as the frequency of dysmenorrhea increased. The multiple logistic regression analysis revealed that the relative risk of frequency of dysmenorrhea is 2.2 times in age of 14, by a standard age of 15 as menarchial age, 2.2 in 13, and 3.1 in below 12. The menstrual days was 3.0 times in both below 2 days and over 8 days with the standard days of 3-7 days. The menstrual amount was 1.9 times in the group with a greater amount of menstrual bleeding and 1.5 times in the group with a less amount than the normal group, respectively. Conclusion: The above results suggest that the rate of experiencing dysmenorrhea was up to 83.0% and 38.4% from these had dysmenorrhea upon every menstruation. It is thought that dysmenorrhea could be a great disadvantage sufficient to impair optimal health to a larger body of school girls. Further, for quality of life, it is required that more fundamental strategies instead of pain killers or others for palliation of dysmenorrhea would be established among adolescents.
Objectives : To investigate changes in, and predictors of, metabolic syndrome(MetS) status over a 5-year period in chronic schizophrenic patients and to identify factors associated with the prevention of or recovery from MetS. Methods : In total, 107 patients, all of whom provided written informed consent, were followed from 2011 to 2016 at Naju National Hospital for this study. MetS was defined according to the revised National Cholesterol Education Program-Adult Treatment Panel III guidelines. Results : During follow-up period, 22(20.5%) patients were newly diagnosed to MetS, 14(13.1%) were disappeared, 77(66.4%) were not changed[MetS : 34(31.8%), No MetS 37(34.6%)]. Common significant factors in the two changed groups were triglyceride and waist circumference, not dose and type of antipsychotic medication. Multiple logistic regression analysis revealed that female gender(odds ratio[OR]=2.846, 95% confidence interval[CI] : 1.020-7.942), attending two or more outpatient visits per month(OR=3.155, 95% CI : 1.188-8.379) and taking antidepressant medication(OR=3.991, 95% CI : 1.048-15.205) were significantly associated with MetS after controlling for other confounding variables. Type and dose of antipsychotic medication were not significantly associated with MetS. Conclusions : Triglyceride and waist circumference were important manageable indicator of MetS. Adoption of a healthy lifestyle is more important than adjusting the dose or type of antipsychotic medication in the treatment of chronic schizophrenia patients with MetS.

This research was conducted to investigate the prevalence of depression and convergent factors related to depression in male and female wage workers. Using data from the 2014 and 2016 Korea National Health and Nutrition Examination Survey, 3,763 adults (1,888 males, 1,875 females) between the ages of 19 and under 65 were analyzed. Depression was measured by the Patient Health Questionnaire-9 (PHQ-9), and the depression (PHQ-9≥10) of all workers was 4.1% (3.2% for men, 5.0% for women). Multiple logistic regression analysis was performed to evaluate the depressive factors of male and female wage workers. In male workers, work-related characteristics such as employment type and working hours per week were found to be significantly related to depression even after adjusting for socio-demographic characteristics, health-related characteristics, and life style characteristics. However, in female workers, work-related characteristics did not expose a significant relationship, showing a difference from male workers. Therefore it is necessary to develop and implement workplace counselling or mental health promotion programs that take into account the gender characteristics of depression risk factors. Also, as health-related characteristics (diagnosis of depression, suicide plan, perceived stress level, unmet medical services, self-rated health status) were found to be related to depression for both male and female workers, continuous management of these factors is required.
