Efficacy of Permanent Iodine-125 Seed Implants and Gemcitabine Chemotherapy in Patients with Platinum-Resistant Recurrent Ovarian Carcinoma

  • Background: The aim of this study was to explore the efficacy and adverse reactions of CT-guided radioactive 125I-seed implantation treatment combined with chemotherapy for platinum-resistant recurrent ovarian carcinoma. Materials and Methods: From September 2010 to December 2012, 23 patients with platinum-resistant recurrent ovarian carcinoma were enrolled. All the patients refused, could not bear, or were not suitable for surgery. They all had no more than 3 lesions, which were detected and could also be measured by CT. All were clarified as single-lesion or multiple-lesion groups. A total of 41 lesions underwent implantation of from 8 to 106 125I seeds (median=43). Multi-plane implanting was adopted and 125I-seeds of (0.4-0.7)mCi were placed at intervals of (0.5-1.0) cm. After implantation treatment, all patients underwent 4 cycles of chemotherapy with gemcitabine $800mg/m^2$ (days 1, 8 and 15). Results: The outcome was evaluated with CT 3 weeks and every 3 months after implantation treatment. After 6 months, the volume of 32 out of 41 lesions (78.0%) was reduced at least 30%, within which 9 lesions completely disappeared(22.0%). Complete response was observed in 7 cases (30.4%), with a partial response in 4 cases (17.4%), 4 cases stable(17.4%)and 8 cases showing progression (34.8%). The total clinical remission rate was 47.8% (11/23). The clinical remission rate was 77.8% (7/9) in the single-lesion group and 28.6% (4/14) in the multiple-lesion group with a significant difference between the two(P=0.036). The common side effects observed were mild gastrointestinal reactions. Conclusions: 125I-seed implantation combined with chemotherapy applies an effective way in the treatment of platinum-resistant recurrent ovarian epithelial carcinoma with the advantages of high local control rates, good short-term effects, little trauma and less side effects.

Serotype and antimicrobial susceptibility of Salmonella spp. isolated from pigs and cattle (소와 돼지유래 Salmonella속 균의 혈청형 및 약제감수성)

  • At the present study, it was aimed to explore the states of antimicrobial resistant Salmonella spp. isolates from 3,850 pigs (2,732 ileocecocolic lymphnodes and 1,118 cecal contents) and 1,764 cattle (965 cecal lymphnodes and 799 cecal contents) slaughtered in Busan province from December 2000 to November 2001. Among 5,614 samples, 457 of Salmonella spp. were isolated from pig lymphnodes (13.5%), pig cecal contents (4.4%), cattle lymphnodes (3.5%) and cattle cecal contents (0.5%). Salmonella spp. were showed different isolation ratio, that was 10.8% in summer, 9.0% in autumn, 8.4% in spring and 5.0% in winter. As a result of serotyping, B group (65.4%) were identified as the most common in pigs and cattle, in order of $C_1$ (14.0%), $D_1$ (5.5%), $C_2$ (4.2%), $E_1$ (4.2%) and L (3.5%). 34 serotypes were found, among them, Salmonella Typhimurium (S. Typhimurium) (21.0%) was the most common serotype from pigs and cattle. The major serotypes were in order of S. Derby (15.3%), S. Schwarzengrund (14.7%), S. Typhimurium var Copenhagen (9.2%), S. Mbandaka (5.7%), S. Enteritidis (5.5%) and S. Ruiru (3.5%). The most common serotype was S. Typhimurium in pigs, and S. Ruiru in cattle. S. Ruiru was firstly isolated from pigs and cattle in Korea. In antimicrobial susceptibility test, all the isolates were demonstrated susceptibility to norfloxacin and ofloxacin. But the isolates were showed resistance other antibiotics in order of doxycycline (68.3%), tetracycline (67.8%), penicillin (54.5%) and streptomycin (52.5%). S. Typhimurium were exhibited resistance to ampicillin (34.8%), chloramphenicol (36.2%), streptomycin (94.9%), sulfamethoxazole/trimethoprim (34.8%) and tetracycline (97.8%). There were 53 strains (38.4%) which had multi drug resistant (MDR) isolates, resistant to more than 6 antimicrobial agents. The most common resistance patterns of MDR isolates were ampicillin, chloramphenicol, carbenicillin, doxycycline, nalidixic acid, penicillin, streptomycin, sulfamethoxazole/trimethoprim and tetracycline (ACCbDNaPSSuT).

Drug Resistance of Bacterial Flora Isolated from Kunsan Bay (군산 내만에서 분리된 세균총에 대한 약제 내성 조사)

  • One hundred and twenty three strains of bacterial flora collected from Kunsan bay and examined for drug resistance to 9 antibiotics. The isolated and examined bacteria were Vibrio spp.(44 strains), Pseudomonas spp.(42 strains), Aeromonas spp.(26 strains), Moraxella spp.(9 strains), Enterobacteria spp.(6 strains), Bordetella spp.(3 strains), Alkaligenesis spp.(3 strains), Staphylococcus spp.(3 strains), and Flavobacterium spp.(2 strains). The drugs used were Ampicillin(AM), Penicillin-G(PM), Rifampicin(RF), Streptomycin(SM), Oxolinic acid (OA), Nalidixic acid(NA), Oxytetracycline(OT), Amikacin(AK), and Enorfloxacin(EF). Forty two strains were found to be sensitive to all drugs. The remaining strains showed resistance to various combinations of drugs. Among the resistant strains were mostly restricted to AM(54 strains/43.9%), PM(47 strains/38.2%), RF(35 strains/28.4%), SM(9 strains), OA(5 strains/ 4.06%), and NA(1 strains/0.8%), in combination at high degree showing 15 different drug resistant patterns. The most frequently showed resistant patterns were AM-PM-RF(16 strains/13.4%), AM-PM(8 strains/6.5%), and PM-RF(7 strains/5.6%). These results suggested that Kunsan bay were contaminated with various strains of highly resistant strains to drugs(AM, PM and RF). These results suggest that high levels of various antibiotics have already been introduced to Kunsan bay. Furthermore it seems that chemotherapy of fish disease has become extremely difficult because of the acquirement of multi-drug resistance to wide range of antibiotics.

The resistance rate of anti-tuberculosis drug isolated from initial tuberculosis patients at a general hospital in Daejeon area (대전지역 일개 종합병원 초진결핵 환자들로부터 분리(分離)된 결핵균(結核菌)의 약제(藥劑) 내성률(耐性率))

  • According to the nationwide survey of tuberculosis from 1965 to 1995, the incidence and drug resistance rate of tuberculosis have been decreased in Korea, but the prevalence of multidrug resistance of Mycobacterium tuberculosis is still a serious problem. The purpose of this study is to investigate the drug resistance rate and pattern of tuberculosis in Daejeon from 2001 to 2008. Of the total 581cases where the drug susceptibility test was performed, resistance to at least one anti-TB drug was found in 104 cases(17.9%) of these, 68(11.7%) were resistant to at least INH and 41(7.1%) were resistant to at least RFP. Single-drug resistance was found for isolates from 37(6.4%) ; 18(3.1%) of these were resistant to INH and 5(0.9%) to RFP. Multidrug resistance, where TB was resistant to at least isoniazid and refampin, was found in 35 cases(6.0%). and Factors associated with MDR-TB included age under 40-60.The drug-resistance rate of pulmonary TB, especially MDR-TB, is higher in the initial treated patients at a private referral hospital than in those in the pubulic sector. Initial drug resistance is common and the drug susceptibility test is informative for pulmonary TB patients who have not received previous TB treatment. The need for an improved control program, coupled with early diagnosis of MDR-TB, to reduce the spread and development of resistance. Multidrug resistance rate is still problem in korea. Efforts to decrease multidrug resistance rate either independently or in cooperation with the pubulic sector will be needed.

Marker Assisted Selection of Brown Planthopper Resistance and Development of Multi-Resistance to Insect and Diseases in Rice (Oryza sativa L.) (DNA 마커를 이용한 벼멸구 저항성 선발 및 복합내병충성 벼 계통 육성)

  • The main objective of this study was to develop the multi-resistance lines to insects(brown planthopper; BPH, rice green leafhopper; GRH) and disease(blast; BL, bacterial blight; BB and rice stripe virus disease;RSV) with good grain quality and plant type by combining conventional breeding and marker assisted selection(MAS) and to eliminate the linkage drag effects between Bph1 gene and culm length, we conducted MAS of Bph1 gene in advanced backcross and double cross progenies. 'Nampyeong', 'Junam' and 'Milyang220' were used as the parent in this study. 'Milyang220' was used as the donor of brown planthopper resistance gene Bph1 with tall culm length. Two backcross progenies were developed using two recipients 'Nampyeong' carrying GRH resistance gene Grh3(t) with good grain appearance and 'Junam' harboring bacterial blight resistance gene Xa3 with short culm length. Two $BC_1$ generations were resulted from the backcrossing of the $F_1$ plants with recurrent parents 'Nampyeong' and 'Junam'. The second rounds of backcrossing($BC_2$) were derived from the cross of selected resistant $BC_1F_1$ plants based on heterozygous genotype of RM28493 linked to Bph1 gene. The double crossed population was constructed from the cross of between each heterozygous $BC_2F_1$ plants at RM28493 locus of '$Nampyeong^*3$ / Milyang220' and '$Junam^*3$ / Milyang220'., The homozygous alleles in Bph1 gene were selected using co-dominant DNA marker RM28493 in double crossed population. Eighty-five lines with multi-resistance to BL, BB, RSV, GRH and BPH were selected by bio-assay and MAS in generation of double crossing. The culm length, head rice ratio and yield of the selected multi resistance lines was ranged from 71 to 88 cm, from 51 to 93%, from 449 to 629 kg/10a. respectively. We can select a promising multi resistance line similar with 'Nampyeong' of major agronomic traits such as culm legnth, head rice ratio and yield. It was designated as Milyang265. Finally this study was developed the multi resistant varieties against to insects and diseases with the good grain quality 'Milyang265' by the advanced backcross and double cross combining MAS and it can be used as genetic resources of multi-resistance to insect and diseases in rice breeding programs.

Prevalence of Extended Spectrum $\beta-Lactamase-Producing$ Clinical Isolates of Escher­ichia coli in a University Hospital, Korea (국내 대학병원에서 분리된 Eschepichia coli의 Extended-spectrum $\beta-Lactamase$ (ESBL) 현황)

  • Recently, the rapid increase and global spread of extended-spectrum $\beta-lactamase$ producing clinical isolates has become a serious problem. The incidence of extended-spectrum $\beta-lactamase$ producing clinical isolates of Escherichia coli in Korea and susceptibility to antimicrobial agents were investigated. Total 233 isolates of E. coli were obtained from urine from hospitalized patients in Guro hospital, Korea University in 2001. One hun­dred and eighty four isolates $(78.9\%)$ were resistant to ampicillin, 80 isolates $(34.3\%)$ were resistant to ceph­alothin, 93 isolates $(39.9\%)$ were resistant to gentamicin, and 64 isolates $(27.5\%)$ were resistant to norfloxacin. Among 233 isolates, 17 isolates $(7.3\%)$ were positive as determined by the double disk synergy test. When min­imal inhibitory concentrations were assayed with additional 6 antimicrobial agents, 13 isolates $(76.5\%)$ were multi-drug resistant to at least four different class antimicrobial agents. Extended-spectrum $\beta-lactamase$ were characterized with isoelectric focusing gel electrophoresis and DNA sequencing. They were TEM-1 in 5 iso­lates, TEM-15 in 1 isolate, TEM-20 in 1 isolate, TEM-52 in 4 isolates, TEM-1 and AmpC in 2 isolates, TEM-1 and OXA-30 in 1 isolate, TEM-1 and OXA-33 in 1 isolate, TEM-1, CTX-M-3, and AmpC in 1 isolate, but SHV was not detected. Antimicrobial resistance genes were transferred to animal isolate of E. coli (CCARM No. 1203) by the filter mating method. Extended spectrum $\beta-lactamase$ producers studied in the current study have low correlation to each other as determined by random amplified polymorphic DNA and pulsed field gel elec­trophoresis. This is a contradictory result from the general hypothesis that extended-spectrum $\beta-lactamase$ pro­ducers in one hospital is a result from a clonal spread.

Studies on the Resistance of Leading Rice Varieties to Leaf-and Planthoppers (멸구, 매미 충류에 대한 한국주요수도품종의 저항성에 관한 연구)

  • The object of this study is to clarify the varietal resistance to green leaf-and planthoppers and to evaluate the nature of the resistance to the insects in connection with the antibiosis. This study investigated the reaction of the 46 rice varieties to insects including recommended varieties of Korea. At seedling stage they were infested with second or third instar nymphs. The results were as follows. 1. The body weight of brown planthopper was increased more than 3 times on susceptible rice variety Mankyung at 15 days after infestation but only 2 times on resistant mudgo. 2. The mortaliy of the brown planthopper was significantly different between resistant and susceptible rice plant, showing 74 percents in resistant Mudgo while 31.2 percents in susceptible Mankyung at 15 days after caging. 3. Most of the rice varieties tested showed highly susceptible reaction to brown planthopper except Mudgo, KR 108-243-1 and HR 529-41-3-2 as resistant varieties, and Akamochi, Satominori, IR 24 and IR 8 as moderately susceptible ones. 4. In the test of varietal resistance to white-backed planthhopper, Mudgo, KR 108-243-1, KR 109-154-2 and HR 529-45-2 and HR 529-45-3-2 were resistant, and Suweon #82, Tongil, IR 8, Palkeum, Iri #309, Hokwang and Chukoku #31 were moderatly susceptible. 5. Most of the varieties tested were observed as highly resistant to green rice leafhopper, but Tongil, Suweon 82, IR 24, Milsung and Chukoku #31 were moderatly susceptible. 6. The new bred lines HR 108-243-1 are multi-resistant to brown planthopper, green rice leafhopper and White-backed planthopper, and some others showed moderate susceptibility.

Parametric Study on the Finite Element Idealization Method for Multi-Spar WIng (다중스파 날개의 유한요소 이상화 방법에 관한 인자연구)

  • A parametric study has been conducted to evaluate the effects of finite element modeling methods on the internal loads, sizing and the weight of the multi-spar aircraft wing structures. The wing is idealized into total 18finite element models and subjected to 4typical external load conditions. An automatic sizing algorithm based on MSC/NASTRAN and MSC/PATRAN is developed. The results show that the critical part affection the internal loads and weight of the structure is wing skin. Effect of modeling of the spar and rib on the structural behavior is not manifest. On the contrast to the general expectation, the models using the bending-resistant elements show the heavier weight than ones by the elements without bending stiffness. From this results, designers of multi-spar wing are recommended to construct the finite element model considering the bending stiffness, or to check the characteristics of the structure before modeling.

Efficacy of Disinfectants against Health-Associated Multi-drug Resistant Clinical Isolates

  • The purpose of this study was to evaluate the disinfecting efficacy of peracetic acid (PAA), sodium hypochlorite (NaOCl) and phenol, which are representative disinfectants in medical environments using four types of multi-drug resistance (MDR) clinical isolates with healthcare-associated infections (HAI). 26 antibiotic susceptibility tests were conducted for the four types of MDR clinical isolates in the same way as for clinical specimens. The minimum inhibitory concentration (MIC) and the minimum bactericidal concentration (MBC) of the disinfectants were determined by using in vitro liquid medium dilution method and inoculation of the plate medium. Both the MIC and MBC of phenol against MRSA and VRE were 3.1%, while those against KPC and MRPA were 6.2%. The MIC and MBC of peracetic acid (PAA) against MRSA, VRE, KPC, and MRPA were 0.18%. The MIC and MBC of sodium hypochlorite (NaOCl) against MRSA were 0.39% and 0.78%, respectively. Both values of MIC and MBC were 0.78% for VRE. In addition, KPC and MRPA showed 0.39% for MIC and 0.78% for MBC. For all MDR strains used in this study, sodium hypochlorite and peracetic acid showed significant sterilizing efficiency, while no clear correlation was identified between antibiotic resistance clinical isolated and ability of disinfection.

Difference analysis of the collapse behaviors of the single-story beam-column assembly and multi-story planar frame

  • The collapse behavior observed in single-story beam-column assembly (SSBCA) do not accurately represent the actual overall stress characteristic of multi-story frame structure (MSFS) under column loss scenario owing to ignoring the interaction action among different stories, leading to a disconnection between the anti-collapse behaviors of "components" and "overall structures", that is, the anti-collapse performance of frame structures with two different structural scales has not yet formed a combined force. This paper conducts a numerical and theoretical study to explore the difference of the collapse behaviors of the SSBCA and MSFS, and further to reveal the internal force relationships and boundary constraints at beam ends of models SSBCA and MSFS. Based on the previous experimental tests, the corresponding refined numerical simulation models were established and verified, and comparative analysis on the resistant-collapse performance was carried out, based on the validated modeling methods with considering the actual boundary constraints, and the results illustrates that the collapse behaviors of the SSBCA and MSFS is not a simple multiple relationship. Through numerical simulation and theoretical analysis, the development laws of internal force in each story beam under different boundary constraints was clarified, and the coupling relationship between the bending moment at the most unfavorable section and axial force in the composite beam of different stories of multi story frames with weld cover-plated flange connections was obtained. In addition, considering the effect of the yield performance of adjacent columns on the anti-collapse bearing capacities of the SSBCA and MSFS during the large deformation stages, the calculation formula for the equivalent axial stiffness at the beam ends of each story were provided.
