Oxygen matters: hypoxia as a pathogenic mechanism in rhinosinusitis

  • The airway epithelium is the first place, where a defense mechanism is initiated against environmental stimuli. Mucociliary transport (MCT), which is the defense mechanism of the airway and the role of airway epithelium as mechanical barriers are essential in innate immunity. To maintain normal physiologic function, normal oxygenation is critical for the production of energy for optimal cellular functions. Several pathologic conditions are associated with a decrease in oxygen tension in airway epithelium and chronic sinusitis is one of the airway diseases, which is associated with the hypoxic condition, a potent inflammatory stimulant. We have observed the overexpression of the hypoxia-inducible factor 1 (HIF-1), an essential factor for oxygen homeostasis, in the epithelium of sinus mucosa in sinusitis patients. In a series of previous reports, we have found hypoxia-induced mucus hyperproduction, especially by MUC5AC hyperproduction, disruption of epithelial barrier function by the production of VEGF, and down-regulation of junctional proteins such as ZO-1 and E-cadherin. Furthermore, hypoxia-induced inflammation by HMGB1 translocation into the cytoplasm results in the release of IL-8 through a ROS-dependent mechanism in upper airway epithelium. In this mini-review, we briefly introduce and summarize current progress in the pathogenesis of sinusitis related to hypoxia. The investigation of hypoxia-related pathophysiology in airway epithelium will suggest new insights on airway inflammatory diseases, such as rhinosinusitis for clinical application and drug development.

Correlation between Magnifying Narrow-band Imaging Endoscopy Results and Organoid Differentiation Indicated by Cancer Cell Differentiation and its Distribution in Depressed-Type Early Gastric Carcinoma

  • Background: A close association between patterns identified by magnifying narrow-band imaging (M-NBI) and histological type has been described. M-NBI patterns were also recently reported to be related to the mucin phenotype; however, detials remain unclear. Materials and Methods: We investigated the cellular differentiation of gastric cancer lesions, along with their mucosal distribution observed by M-NBI. Ninety-seven depressed-type early gastric cancer lesions (74 differentiated and 23 undifferentiated adenocarcinomas) were visualized by M-NBI. Findings were divided into 4 patterns based on abnormal microvascular architecture: a chain loop pattern (CLP), a fine network pattern (FNP), a corkscrew pattern (CSP), and an unclassified pattern. Mucin phenotypes were judged as gastric (G-type), intestinal (I-type), mixed gastric and intestinal (M-type), and null (N-type) based on 4 markers (MAC5AC, MUC6, MUC2, and CD10). The relationship of each pattern of microvascular architecture with organoid differentiation indicated by cancer cell differentiation and its distribution in each histological type of early gastric cancer was investigated. Results: All CLP and FNP lesions were differentiated. The cancer cell distribution showed organoid differentiation in 84.2% (16/19) and 61.1% (22/36) of the two types of lesions, respectively, and there was a significant difference from the unclassified pattern with organoid differentiation (p<0.001). Almost all (94.7%; 18/19) CSP lesions were undifferentiated, and organoid differentiation was observed in 72.2% (13/18). There was a significant difference from the unclassified pattern with organoid differentiation (p<0.05). Conclusions: Cellular differentiation and distribution are associated with microvascular architecture observed by M-NBI.

Evaluation of Anti-Asthmatic Activity of Essential Oils from the Lauraceae Family in Lipopolysaccharide (LPS)-Stimulated NCI-H292 Cells

  • The Lauraceae family has commercial uses, such as in the food, pharmaceutical, and perfume industries. This study was conducted to investigate anti-asthmatic activity of essential oils from the seven species in the Lauraceae family. The essential oils were extracted from the leaves of seven species, and the chemical composition was investigated by gas chromatography-mass spectrometry. The major constituents of essential oils differed depending on the species, even if they belonged to the same family. The main constituents were camphor (89.09%) in Cinnamomum camphora, linalool (26.91%) in Cinnamomum cassia, 1,8-cineole (23.90%) in Cinnamomum japonicum, d-limonene (10.27%) and β-eudesmol (10.03%) in Lindera obtusiloba, δ-cadinene (13.85%) and α-phellandrene (11.57%) in Machilus japonica, cis-,trans-β-ocimene (13.80% and 12.06%) and elemol (11.46%) in Neolitsea aciculata, and cis-β-ocimene (37.94%) and sabinene (24.91%) in Neolitsea sericea. The anti-asthmatic activity of essential oils was investigated using the lipopolysaccharide-induced NCI-H292 cells. The relative expression levels of the pro-inflammatory cytokines [interleukin (IL)-1β and IL-6] and mucus gene (MUC5AC and MUC5B) were significantly reduced by essential oils from seven species in the Lauraceae family. Among the seven essential oils, the essential oil from L. obtusiloba had the most superior anti-asthmatic activity. These results suggest that the essential oil of L. obtusiloba leaves could be used as an agent to suppress mucus hypersecretion.

Pistacia weinmannifolia ameliorates cigarette smoke and lipopolysaccharide-induced pulmonary inflammation by inhibiting interleukin-8 production and NF-κB activation

  • Pistacia weinmannifolia (PW) has been used in traditional Chinese medicine to treat headaches, dysentery, enteritis and influenza. However, PW has not been known for treating respiratory inflammatory diseases, including chronic obstructive pulmonary disease (COPD). The present in vitro analysis confirmed that PW root extract (PWRE) exerts anti-inflammatory effects in phorbol myristate acetate- or tumor necrosis factor α (TNF-α)-stimulated human lung epithelial NCI-H292 cells by attenuating the expression of interleukin (IL)-8, IL-6 and Mucin A5 (MUC5AC), which are closely associated with the pulmonary inflammatory response in the pathogenesis of COPD. Thus, the aim of the present study was to evaluate the protective effect of PWRE on pulmonary inflammation induced by cigarette smoke (CS) and lipopolysaccharide (LPS). Treatment with PWRE significantly reduced the quantity of neutrophils and the levels of inflammatory molecules and toxic molecules, including tumor TNF-α, IL-6, IL-8, monocyte chemoattractant protein-1, neutrophil elastase and reactive oxygen species, in the bronchoalveolar lavage fluid of mice with CS- and LPS-induced pulmonary inflammation. PWRE also attenuated the influx of inflammatory cells in the lung tissues. Furthermore, PWRE downregulated the activation of nuclear factor-κB and the expression of phosphodiesterase 4 in the lung tissues. Therefore, these findings suggest that PWRE may be a valuable adjuvant treatment for COPD.

위릉채 추출물 및 생물전환 분획물의 만성호흡기 질환 효과 검증 (Verification of the effect of Potentillae Chinensis Chinensis Herba extract and bioconversion fraction on chronic respiratory diseases)

  • Potentillae Chinensis Herba 추출물의 항산화 효능 평가를 위해 DPPH 자유라디칼 소거 활성과 ABTS 양이온 라디칼 활성 평가를 시행하였다. 세포 실험에서 항염증 평가를 위해 호흡기 점막 상피세포 NCI,H292와 RAW264.7 세포 대상으로 항염증 효능을 평가하였다. 그 결과, DPPH 자유라디칼 소거 활성과 ABTS 양이온 라디칼 활성 모두 농도 의존적으로 항산화 활성이 증가하는 것으로 나타났다. 세포 MTT 분석 결과, 각 농도 100 ㎍/ml로 처리한 경우 NCI-H292 세포의 생존율이 70% 미만으로 감소하였고, 이후 실험은 50 ㎍/ml로 진행하였다. 항염증 효능 평가에서는 NO생성, TNF-𝛼, IL-1𝛽, PGE2가 감소하였고, COX-2도 50 ㎍/ml에서 유의하게 감소하였다. Potentillae Chinensis Herba 추출물과 생물전환 추출물의 뮤신 단백질 발현은 MUC5AC 발현이 유의하게 감소하는 것으로 관찰되었다. 이상의 결과는 Potentillae Chinensis Herba의 호흡기 점액 단백질 발현을 도와 염증을 억제하는 기능성 소재로 활용하는데 가치가 있음을 알 수 있다.

  • 배경 : 본 연구에서는 세균성 리포다당질(lipopolysac-charide, LPS)로 인한 호중구성 염증이 EGFR 시스템을 통해서 배상 세포의 이형성 및 점액의 과다 분비를 유발할 것이라는 가설 하에, LPS와 MMPs 억제제(matrix metalloproteinase inhibitor, MMPI)를 투여한 후 EGFR 및 MMP-9의 발현을 연구하고자 하였다. 방법 : Pathogen-free Sprague-Dawley 를, 다양한 농도의 LPS를 투여한 군과 투여하지 않은 대조군으로 나누어 기도의 조직학적인 변화를 날짜 별로 관찰하였고, MMPI(CMT-3)를 LPS 투여 3일 전부터 매일 구강을 통해 섭식시켰다. 호중구의 침윤은 다섯 개의 고배율 시야에서 관찰된 호중구의 수로 정량화하여 비교하였고, mucus glycoconjugate에 대한 AB/PAS 염색 및 MUC5AC, EGFR, MMP-9에 대한 면역조직화학 염색 (immunohistochemical stain)을 시행하였다 결과 : LPS를 투여한 경우 기도 상피의 AB/PAS 및 MUC5AC의 염색 정도는 시간 및 용량 의존적으로 증가하였고, MMPI를 치료할 경우에 LPS로 인한 배상세포의 과형성이 유의하게 감소하였다. LPS를 주입할 경우 호중구의 침윤이 증가하였고 기도 상피에서 EGFR의 발현을 증가시켰다. MMPI로 치료할 경우 LPS로 인한 호중구의 침윤 및 EGFR의 발현 그리고 배상세포의 과형성이 현저하게 감소되었다. 결론 : Matrix metalloproteinase는 호중구성 염증 및 EGFR에 의해 발생하는 LPS에 의한 배상 세포의 과형성 및 점액 과다분비의 기전에 있어서 밀접하게 관련되고, 따라서 점액 과다분비를 특징으로 하는 세균 감염으로 인한 기도 질환의 치료에 있어서 MMPI가 잠재적인 임상적 효과가 있을 것으로 사료된다.
