Effects of Acupuncture at the Acupoint Yongok$(KI_2)$, Umgok$(KI_10)$, Sobu$(HT_8)$. Sohae$(HT_3)$ on Aquaporin-2 expression, RSNA, MAP, HR, serum ANP, renin and norepinephrine in Rats (족소음신경(足少陰腎經) 연곡(然谷).음곡(陰谷)과 수소음심경(手少陰心經) 소부(少府).소해(少海) 침자(鍼刺)가 신장(腎臟) Aquaporin-2 발현(發現)과 신교감신경활성(腎交感神經活性), 혈압(血壓) 및 혈청(血淸) ANP, renin, norepinephrine에 미치는 영향(影響))

  • Objectives : This research was performed to investigate the effect of acupuncture at the $KI_2\;KI_{10}\;HT_8\;HT_3$, on Aquaporin-2(AQP2) expression related with the renal functions in rats. Methods : Acupuncture was performed during 100-seconds, 6-times, at 150-seconds intervals under anesthesia in rats. We observed rats' mean arterial pressure(MAP), heart rate(HR), renal sympathetic nerve activity(RSNA) during acupuncture and AQP2 expression by western blot method and atrial natriuretic peptide(ANP), renin, norepinephrine of plasma after decapitation. Results : The AQP2 expression was significantly increased in $HT_8$ group, but decreased in $KI_{10}$ group. Average MAP during 6-times acupuncture was significantly increased in $HT_8$ group. Average HR was significantly increased in $HT_8$ group, Average RSNA was increased in $KI_{10}$ group, but that was marginally increased in $KI_{10}$ group. Plasma renin concentration was increased in $KI_2,\;HT_3$, groups. Plasma ANP show a tendency to decrease in $KI_10\;HT_3$ groups, increased in $KI_2,\;HT_3$ but not significant. Plasma norepinephrine concentration was significantly decreased in $KI_{10},\;HT_3$ groups. Conclusions : These results suggested that acupuncture at $HT_8$ activate renal function to reuptake, but $KI_{10}$ show a decline on effect of AQP2 expression, blood pressure, nerve activity and renin.

A Case Report on a Patient with Cervical Vertigo Who Improved with a Combination of Korean Medicine Treatments (한방 복합치료로 호전된 경추성 현훈 환자의 증례보고 1례)

  • Objective: To report the case of a patient suffering from cervical vertigo who was treated with a combination of Korean medicine treatments (including acupuncture, pharmaco-acupuncture, and Chuna manual therapy). Method: An outpatient with cervical vertigo was given a combination of Korean medicine treatments five times, from December 5 to 16, 2016. All of the treatments, acupuncture with electric stimulation and pharmaco-acupuncture, were applied on the trigger points (TP) of the cervical muscles and the upper trapezoid muscles. Chuna manual therapy was then applied on the cervical area. The Numeric Rating Scale (NRS) - according to outpatient's statement on vertigo, neck pain and headache - and the Korean Dizziness Handicap Inventory (DHI), which is a self-evaluating scale of dizziness in daily life, were used to evaluate the effects of this treatment. Results: At the end of the treatment, the vertigo and the headache disappeared. The NRS decreased from 9 to 0 for vertigo and from 5 to 0 for headache. Although the neck pain also decreased, it did not disappear; this NRS decreased from 7 to 1. The DHI score also decreased significantly, from 64 to 4. Conclusion: Since the symptoms of the patient with cervical vertigo improved after the very first treatment and the improvement of the symptoms was observed as the treatment progressed, this case report is worth supporting the efficacy of combined Korean medical treatments. However, because there was only one subject, it is hard to say that this efficacy can be generalized. Moreover, this report cannot prove the individual efficacy since various treatments were combined. While there are many researches related to cervical vertigo in Western medicine, the studies in Korean medicine are comparably insufficient; therefore, follow-up research on more subjects is needed.

Comparative Study of Therapeutic Effect of Needle-free Bee Venom Aqua-acupuncture (BVA) into Zusanli (ST36) in the Rat Formalin Test (포르말린으로 유도된 통증 유발 쥐에서 무침주입기를 이용한 봉독약침의 진통효과)

  • Bee Venom aqua-acupuncture (BVA) simultaneously exerts pharmacological effects of biologically active compounds, existed in the whole bee venom, and medicinal effect of the stimulation of acupuncture points. BVA has been considered as a promising therapeutic method for treating various chronic diseases, mainly accompanying severe pain and inflammation. As a painless injection device, jet injectors have been commercially marketed for various clinical applications including insulin injection and vaccination. Among them, a pressure-driven jet injector system could be used for intradermal delivery of a variety of drugs. The aim of this study was to investigate the analgesic effects of the BVA using a needle-free injector (Biojector $2000^{\circledR}$, Bioject Inc., OR, USA), compared to the conventional BV aqua-acupuncture using a typical syringe. Adult Sprague-Dawley rats were injected with bee venom $(0.08mg/kg,\;50{\mu}l)$ using Biojector $2000^{\circledR}$ (BVA-B) or a syringe (BVA-5) into the Zusanli (ST36) acupoint, 30 minutes before plantar injection of 2% formalin. It was found that BVA-B-, or BVA-5-treated rats, compared to controls, exhibited significantly less licking behavior during the late phase in the rat formalin test, when compared to controls. During early phase, however, those effects were not significant but substantial. The analgesic effect of BVA-B was also compatible with that of the conventional BVA-5. In the immunohistochemical studies, BVA-B significantly suppressed the expression of formalin-evoked c-fos, a biomarker of neuronal activity, in the lumbar dorsal horn of the spinal cord. These results indicated that BVA-B waseffective in the modulation of pain in the rat formalin test, compared to BVA-5. Taken together, the needle-free jet injector system could be substituted for the conventional aqua- acupuncture with the advantage of little pain.

Comparative study of 202 herbs on interferon-gamma secretion (202종(種) 한약재(韓藥材)의 $IFN-{\gamma}$ 분비조절에 관한 비교연구)

  • Objects : This study has been carried out to assess the effects of the variable herbs on $IFN-{\gamma}$ secretion in the mouse spleen cell. Methods : 202 kinds of herb extracts were used to evaluate the $IFN-{\gamma}$ secretory distinction by each $1{\mu}g/ml$ and $10{\mu}g/ml$ density of water. All experimental herbs were grouped by oriental herbalogical method. But each herb had its independent variables. Results : The secretions increased in 20% of all herbal water. The density differences also make different effects on the secretion of $IFN-{\gamma}$. The secretion of IFN-${\gamma}$ inclosed in some kinds of herbs of IFN-${\gamma}$. It has representatively increased in Imperaetae Rhizoma(白茅根) of $1{\mu}g/ml$ and Notopterygii Rhizoma(羌活)of $10{\mu}g/ml$. $IFN-{\gamma}$ incresed in 12 kinds of heybs of both densities. The secretion of $IFN-{\gamma}$ decreased in some kinds of herbs of $IFN-{\gamma}$. It has representatively decreased in Moutan Radicis Cortex(蘇丹皮) of $1{\mu}g/ml$ and Angelicae Radix(富歸尾) of $10{\mu}g/ml$. $IFN-{\gamma}$ decreased in 18 kinds of herbs of both densities. In t oriental herb group, The secretion of $IFN-{\gamma}$ increased in Bang-Hyang-Hwa-Sup group(芳香化濕藥), He-Pyo group(解表藥), I-Su-Sam-SuP group(利水渗) The secretion of $IFN-{\gamma}$ decreased in Gu-Chung group(驅蟲藥), An-Sin group(安神藥). Su-Sap group(收澁) On-Li group(溫裏藥), I-Gi group(利氣藥). Conclusions : The result of this study will not only broaden applications of oriental medicine to biological therapy, but also form the basis of oriental medical therapy to find out the meaning of oriental classification.

The Effect of Electroacupuncture on Upper-Extremity Spasticity of Stroke Patients. (뇌졸중 환자의 상지 경직에 대한 전침의 치료 효과)

  • Objectives : This study was executed in order to evaluate the effects of electroacupuncture on upper-extremity spasticity control in stroke patients. Methods : Eighteen patients with stroke were enrolled and classified into two groups, the study and control group. The control group did not receive any oriental medical treatment. In the study group, the electroacupuncture points were applied to Kokt'aek (PC3) and $Ch'{\u{o}}ch'{\u{o}}n$ (PC2), Naegwan (PC6) and $Ch'{\u{o}}kt'aek$(LU5) of the affected limb. H-reflexl M-response ratio (HIM ratio), modified Ashworth scale (MAS) and Fugl-Meyer motor function assessment (FMA) were used for evaluation of spasticity control before electroacupuncture, within two hours after electroacupuncture, and at two weeks. Results : In MAS, the study group declined more than the control group. but there was no statisticallysignificant consideration. In H/M ratio. the study group was more efficient than the control group. and spasticity decreased successively during the series in the study group. In FMA, motor function in the study group improved more than in the control group and motor function in the study group increased successively during the series. Conclusions : These results showed that electro acupuncture was a useful method to decrease upper-extremity spasticity in patients with stroke. Further studies are needed to explore more cases and the long-lasting carryover effects on upper-extremity spasticity in electroacupuncture.

Current Status of Systems Biology in Traditional Chinese medicine - in regards to influences to Korean Medicine (최근 중의학에서 시스템생물학의 발전 현황 - 한의학에 미치는 영향 및 시사점을 중심으로 -)

  • Objectives : This paper serves to explore current trends of systems biology in Traditional Chinese Medicine (TCM) and examine how it may influence the Traditional Korean medicine. Methods : Literature review method was collectively used to classify Introduction to systems biology, diagnosis and syndrome classification of systems biology in TCM perspective, physiotherapy including acupuncture, herbs and formula functions, TCM systems biology, and directions of academic development. Results : The term 'Systems biology' is coined as a combination of systems science and biology. It is a field of study that tries to understand living organism by establishing a theory based on an ideal model that analyzes and predicts the desired output with understanding of interrelationships and dynamics between variables. Systems biology has an integrated and multi-dimensional nature that observes the interaction among the elements constructing the network. The current state of systems biology in TCM is categorized into 4 parts: diagnosis and syndrome, physical therapy, herbs and formulas and academic development of TCM systems biology and its technology. Diagnosis and syndrome field is focusing on developing TCM into personalized medicine by clarifying Kidney yin deficiency patterns and metabolic differences among five patterns of diabetes and analyzing plasma metabolism and biomarkers of coronary heart disease patients. In the field of physical therapy such as acupuncture and moxibustion, researchers discovered the effect of stimulating acupoint ST40 on gene expression and the effects of acupuncture on treating functional dyspepsia and acute ischemic stroke. Herbs and formulas were analyzed with TCM network pharmacology. The therapeutic mechanisms of Si Wu Tang and its series formulas are explained by identifying potential active substances, targets and mechanism of action, including metabolic pathways of amino acid and fatty acid. For the academic development of TCM systems biology and its technology, it is necessary to integrate massive database, integrate pharmacokinetics and pharmacodynamics, as well as systems biology. It is also essential to establish a platform to maximize herbal treatment through accumulation of research data and diseases-specific, or drug-specific network combined with clinical experiences, and identify functions and roles of molecules in herbs and conduct animal-based studies within TCM frame. So far, few literature reviews exist for systems biology in traditional Korean medicine and they merely re-examine known efficacies of simple substances, herbs and formulas. For the future, it is necessary to identify specific mechanisms of working agents and targets to maximize the effects of traditional medicine modalities. Conclusions : Systems biology is widely accepted and studied in TCM and already advanced into a field known as 'TCM systems biology', which calls for the study of incorporating TCM and systems biology. It is time for traditional Korean medicine to acknowledge the importance of systems biology and present scientific basis of traditional medicine and establish the principles of diagnosis, prevention and treatment of diseases. By doing so, traditional Korean medicine would be innovated and further developed into a personalized medicine.

  • About Oui-Ga-Sil(胃家實) in order to considerate the contents recorded in Nai-Gyung Sang-Han-Lon and latter literature, definition, etiopathology, syndrome, differential diagnosis, therapy, Prognosis and prevention were classified. And the results were as follows: 1. Oui-Ga(胃家)was a term which indicated the whole digestive system such as stomach, small intestine, large intestine, rectum and anus. Sil(實)could be defined as the peculiar concept pertaining to the acute and last stage which was invaded to inside bowels because of abundance with evil influence. 2. Eliology of Oui-Ga-Sil was abunt gastric fever originally, injured mucus because of mistreatment, the invasion to inside of outside evil influence through meridian. Pathology was the opening and shutting appearance of gastric abundance with intestinal emptiness, and intestinal abundance with gastric emptiness, Oui-Ga-Sil could be occurred because of gastric abundant dryness and splenic humidifying capacity decrease. 3. Symptom of Oui-Ga-Sil was classified as for the sunlight outside syndrome and the inside abundant syndrome. The sunlight outside syndrome was body fever, sweating, no chilling, on the contrary hatred of fever. The chief complaint of inside abundant syndrome was daily fever, talking in delirium, hand and foot sweating, abdominal distention, difficult defection and those could be pertained to sunlight bowel syndrome. 4. Diagnostic views of Oui-Ga-Sil were that pulse was descending abundant large strong and smooth quick, a coated tongue was yellow, deep yellow, old yellow, thick, scorching dry rough or gray black. On abdominal diagnosis, pressing by hand, patient was conscious of pain, excessive pain, rejection against press, impossible press or intermittent abdominal pain and bowel cutting pain without press. 5. Differential diagnosis was that the sunlight of Nai-Gyung-Fever-Theory was outside desease making meridian the prime object, Baik-Ho-Tang syndrome was making figureless abundant fever the pivotal point. And important differential standard of splenic shrink syndrome was that a daily fever, an irritation with fever were not occurred. 6. Theory of Oui-Ga-Sil was that Seng-Gi-Tang classes had been used in attacking downward or making balance, and moxibustion on Jung-Wan, honey boiling induction theory had been also used. Attacking downward therapy was invigorating method to preserve mucus, and if mucus had been exausted with complicating emptiness prognosis had been appeared badly. 7. Preventing from Oui-Ga-Sil diet by rule, fitness to cold and warmth may be needed to prevent outside evil influence attack and inside evil influence occurrence. Prudence with being very busy, fatigue, wine and woman may be also needed not to be an injury to splanic and gastric spirit.

Study on the Heat Transfer Characteristics of Warm Needle Materials in Korean Medicine (온침소재의 열전달특성에 관한 연구)

  • Objectives : Warm needling is the method combining the effects of acupuncture and moxibustion. In modern clinics, warm needling is only affected by treatment time or frequency. We need to study the physical characteristics of warm needles currently used in clinics in order to develop more efficient and economical treatments. Methods : We collected various warm needle specimens and analyzed chemical constitutions, measured heat transfer velocity, micro Vicker's hardness and specific resistance. We studied the relationship between heat transfer velocity and micro Vicker's hardness as well as that between heat transfer velocity and specific resistance. Results : The heat transfer velocity of the Silver_HL was 3.3 mm/sec, of the Au alloy group was 1.3~2.6 mm/sec, and Silver_IN, Silver_ZK and SS groups was less than 0.3 mm/sec. We therefore concluded that the needle composed of Ag has the best heat transfer velocity. In the Micro Vicker's hardness test, Vicker's hardness of the Au alloy group was 159~170 Hv, of Silver_HL was 181 Hv, and of the Silver_IN, Silver_ZK, SS group was 450.8~519 Hv. In the Silver_IN, Silver_ZK and SS groups, hardness was inversely proportial to thermal conductivity. In the specific resistance test, the specific resistance of Silver_HL was the lowest, that of the Au alloy group was the second lowest, and that of the Silver_IN, Silver_ZK, SS groups were the highest. Conclusions : We concluded that the needle composed of Ag has the best heat transfer velocity, highest electric conductivity and thermal conductivity, therefore the needle composed of Ag is suitable for warm needling.

A Literature Study on the External Treatment of a Burn (火傷의 外治法에 對한 文獻的 考察 (外用藥을 중심으로))

  • The burn is acute skin injury caused by fire, hot water. steam. hot oil, sour and salty. It is occurred frequently in the daily life as well as oriental therapy like moxibustion therapy, physical therapy. Nevertheless, medical treatment of the burn is almost dependent on western cure. So we chose the oriental medicine textbooks and the oriental medicine journals that were dealing with the drugs, processing the drugs. peculiar treatment put first external cure. The results were as follows; 1. The burn is acute skin injury caused by fire, hot water, steam, hot oil, sour and salty. 2. The burn cause blisters, irritability and restlessness, nausea, dryness of mouth, constipation, in case of serious, coma, dyspnea and death. The early stage of the burn, blisters form by skin damage and they burst into skin ulceration from which pus issues, the latter term, the wound form scab and healed up. 3. In a light case, medical treatment of the burn was used external treatment by medicine for externalism use, in a serious case, it was used both as an internal remedy and medicine for outward application. Also in the early stage, it was careful of using the cold and cool medicine, as the process of healing, it was used alleviating pain, detoxicating, moistening the skin, growing muscle and skin, convergence, evacuating pus, regeneration of the tissue, strengthen the spleen and nourishing the stomach. 4. The external treatment medication is Herba Ephedrae Oil(麻油), Radix ET Rhizoma Rhei(大黃), Glauberitum(寒水石), Water(水), Pig OiI(猪油), Pig Fat(猪脂), Radix Angelicae Gigantis(當歸), Rhizoma Coptidis(黃連), Cortex Phellodindri(黃栢). The White of an Egg(鷄子淸), Raw Honey(生蜜), Honey(蜜), Wine(酒), Etc. It is mostly the cold and cool medications. 5. Soft extracted and powered dosage form in external treatment is much used. The soft extracted form(32times used) are mostly Chung Ryang paste(淸凉膏) and Fructus Papaveris paste(罌粟膏). The powered form(30times used) are mostly Bingsang Powder(氷霜散), Bosaenggugo Powder(保生救苦散), Sahwang Powder(四黃散). The others is much a various powder adding solvent. 6. If varicella stage, erosion after varicella stage, oozing stage and extreme pain stage, the powder adding solvent is much used. If little oozing stage. ulcering stage, scabing stage and a chronic stage, Soft extracted dosage form is much used. 7. The most many(26.65%) used method is that apply each medication power mixed water(水), wine(酒), honey(蜜) in a wounded part.

A Trends of Studies in Journal of Korean Academy of Medical Gi-Gong (대한의료기공학회지 연구 동향)

  • Objective : The purpose of this review is to overview and evaluate the trends of the studies in J. of The Korean Academy of Medical Gi-Gong. Methods : All 186 articles' headlines and abstracts from voI.1(1996) to Vol.17(20l7) were evaluated and classified. The dataset was searched from the Hompage of the Korean Academy of Medical Gi-Gong. Results : 1. In the type of study, the literature studies consisted of 67%(126studies), clinical studies 21%(39), experimental studies 12%(22) in order. 2. Gigong classification was 60.96%(114studies) while Non-Gigong classification was 37.04%(73studies). 3. In Gigong classification, General Gigong took the largest part and there were no Taoist Sexual Practices studies. 4. In Non-Gigong classification, Obstetrics & Gynecology & Pediatrics took the largest part by 25 studies(12.37%). An-Kyo-Hak was the second largest by 24 studies(11.88%). 5. Analysis Research(112 studies, 59.89%) took the largest part in study method. Conclusions : 1. The Korean Academy of Medical Gi-Gong published average 8.9 studies per year(187 studies per 21 years). 2. Following the object of The Korean Academy of Medical Gi-Gong, 114 studies(60.96%) of total 187 studies, published by J. of The Korean Academy of Medical Gi-Gong, were associated to Gigong. 3. In Non-Gigong classification, 25 studies about Obstetrics & Gynecology & Pediatrics, 24 studies about An-Kyo-Hak, 16 studies about Acupuncture & Moxibustion Medicine and Meridian & Acupoint Study were published by J. of The Korean Academy of Medical Gi-Gong. 4. In Gigong classification, studies about Meditation and External Gigong Therapy were insufficient. There are even no studies about Taoist Sexual Practices. We need more studies about those categories to come.
