  • In patients with newly established ideal occlusion through full-mouth rehabilitation using fixed prostheses, complications and occlusal changes over time can arise because of various factors. This case report describes a 10-year follow-up of a patient with masticatory dysfunction and aesthetic problems who underwent full-mouth rehabilitation with an increased vertical dimension. During the follow-up, complications such as tooth fracture, occlusal changes, infraocclusion of few implant-supported prostheses, and loss of interproximal contacts were observed. Detecting these issues early through periodic follow-up is important. This case report aims to review the causes of complications after full-mouth rehabilitation using fixed prostheses and the strategies for their management.

  • BACKGROUND. Generalized aggressive periodontitis (GAP) is a destructive periodontal disease that can develop in young age. Only a few cases of full mouth rehabilitation, using dental implants, have been reported in a patient with aggressive periodontitis. CASE DESCRIPTION. This clinical report describes the treatment procedures and results of full mouth rehabilitation in a patient with aggressive periodontitis. After all teeth were extracted, 6 implants were placed in the maxilla and mandible, respectively. Fixed detachable implant prostheses were made. The patient was satisfied with the final results. She was followed for 10 months postloading. CLINICAL IMPLICATION. For a long-term success, continuous maintenance care is critical, as the contributing factors of the disease (such as immune factors or periodontal pathogens) may not be controlled adequately.

  • Clinical therapy that combines full-mouth rehabilitation with immediate implantation and orthognathic surgery poses a challenge to prosthodontists. This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient presenting with skeletal discrepancy and rampant caries. The results thus achieved indicate that full-mouth rehabilitation by fixed immediate and early loading implantation accompanied by orthognathic surgery can be a predictable and effective treatment procedure.

  • The diagnosis and treatment plan for forming ideal occlusal plane in full mouth rehabilitation are difficult because each process is complicated and information exchange between dentist and technician is subjective. The OP finder® system simplifies this process and helps to deliver more objective and accurate information. In this case, full mouth rehabilitation was performed using OP finder® system for patients with old bad fixed prosthesis and severely worn mandibular teeth, and reported that the result of proper occlusal plane setting and masticatory function recovery was obtained.

  • Purpose : This study was performed to understand the relationship between hand and mouth shapes using functional magnetic resonance imaging(fMRI). Methods : Two healthy volunteers without any previous history of physical or neurological illness were recruited. fMRI was done that volunteers was 6 repeated of natural mouth, close mouth and open mouth while power grip and pinch grip movement. Results : Cerebral cortex activation was not well observed for the natural mouth during the power grip exercise. For the closed mouth, the temporal lobe, Broca's area, the prefrontal area related to thinking and judgment, the supplementary motor area, the auditory area and Wernicke's area were activated. For the open mouth, cortical activation was also observed in the temporal lobe, Wernicke's area, the prefrontal area related to thinking and the orbital frontal area related to visual sense. During the pinch grip exercise, cortical activation was observed for the natural mouth in the primary sensory area, Wernicke's area, the primary and supplementary motor area, and the prefrontal area. For the closed mouth, cortical activation was observed in the temporal lobe, Wernicke's area, the prefrontal area related to thinking, the secondary visual area, the primary sensory area and the supplementary motor area. In the case of the open mouth, cortical activation was observed in a few parts in the temporal lobe as well as Wernicke's area, the prefrontal area related to thinking, and other areas related to visual sense such as the primary visual area, the secondary visual area and the visual association area. Conclusion : Brain was more activation for close mouth and open mouth more than natural mouth movement.

  • Severe tooth wear may cause the pathologic change of the TMJ and masticatory muscles, unesthetic facial appearance, pathogenic pulp and occlusal disharmony. Treating patients with severely worn dentition often requires full mouth rehabilitation with increasing vertical dimension. Proper diagnosis and treatment planning are important for esthetic and functional definitive restorations and the long term stability of the neuromuscular system and the TMJ. In this case, 66 year-old female presented with generalized worn dentition. Based on assessment, pathologic destruction of teeth structure on entire dentition was caused by masticatory force and diet habit without loss of vertical dimension. Subsequently, 3 mm increase of vertical dimension that based on incisor for tooth restoration and esthetic improvement was determined. After 8 weeks stabilization period with temporary fixed prostheses, definitive prostheses were fabricated. After 6 months follow up period, satisfactory outcomes were attained both functional and esthetic aspects through this procedure.

  • The case report describes the occlusal rehabilitation of a male patient with Angle Class III malocclusion and its effect on maximum bite force. The main complaints of patient were masticatory difficulty and poor esthetic. The patient's expectations from the treatment were a good esthetic and function with a less invasive and relatively promptly way. Therefore, increasing of the occlusal vertical dimension (OVD) and then restoring the maxillary and mandibular teeth was chosen by the patient among the treatment options. At the beginning of treatment maximum bite force of patient was measured. Then an occlusal splint was provided to evaluate the adaptation of the patient to the altered OVD. Full mouth rehabilitation with metal ceramic restorations was made. After the completion of full mouth restoration, bite force measurement was repeated and patient exhibited increased maximum bite force. Full mouth restorative treatment in a patient with Class III malocclusion could be an effective treatment approach to resolve esthetic concern and to improve masticatory function related to maximum bite force.
