Pain and Weakness on Unilateral Upper Extremity Diagnosed as Brachial Plexopathy after Herpes Zoster Infection (대상포진후 상완신경총병증으로 진단된 편측 상지의 통증과 위약)

  • Motor paralysis is a less common neurologic complication of herpes zoster. Until now, a few cases have been reported, and most of these cases showed brachial plexopathy involving one or two segments. We report a patient with pain and weakness on upper extremity diagnosed as brachial plexopathy after herpes zoster infection. An 88-year-old female patient complained not only tingling sense, pain, and swelling on right whole arm, but also weakness on this right upper extremity. On physical examination, weakness is seen in right shoulder abduction·shoulder flexion·elbow flexion·elbow extension· wrist extension (grade 4), finger flexion·finger abduction·finger extension·finger DIP flexion (grade 3). In electrodiagnostic study and magnetic resonance imaging study, she was diagnosed as the brachial plexopathy, whole branch involved. This is the only case of post-herpetic brachial plexopathy involving whole branch in domestic.

Ipsilateral Motor Deficit in Patients with Unilateral Brain Damage (편측 뇌손상 환자의 동측 운동 결함에 대한 고찰)

  • Recently, several investigations revealed that after unilateral brain damage, movement abnormalities were exposed on the ipsilateral side as well as the upper extremity contralateral to the damaged hemisphere. Even the motor abilities had significantly recovered from ipsilateral motor deficits on not only simple sensoriomotor function, also clinical assessments since subacute stage, although could not completely returned. Such motor deficits were detected in a diversity of motor tasks depending on the interhemispheric specialization, further in clinical evaluation and a daily of activities. In the clinical features, muscular weakness, sensory loss and impaired manual dexterity were observed. In a laboratory experiment, there were increasing evidences that the kinematic processing deficits was founded in various-specific motor tasks, which ranged from simple basic element to complex tasks, such as tapping task, step-tracking, goal directional aiming task, and iso(and non-)directional interlimb coordination. In the point of view, the manifest understanding in related to ipsilateral deficits provide the clinicians with an important information for scientific management about brain injured patient's prognosis and therapeutic guidelines.

A Study on Insulation Property of White Powder Found on High Voltage Motor Stator Winding (백화현상이 나타난 고압전동기 고정자권선의 절연특성 연구)

  • During a routine inspection of a large pump motor at power plant, white powder was found on the surface of the stator end windings. Visual inspection and high voltage insulation diagnosis was performed to determine whether this motor is available. The purpose of this paper is to understand the insulation properties of white powder found on high voltage stator and to know prevention of insulation weakness.

Spatial and Temporal Features of Motor Modules in an individual with Hemiparesis During the Curvilinear Gait: A Pilot Single-Case Study

  • Purpose: This study aimed to investigate spatial and temporal features of motor control in an individual with hemiparesis during the curvilinear gait (CG) and proposed an exercise guideline. Research design, data and methodology: An individual aged 63 with hemiparesis by stroke disease was participated in the study. Autoencoder (AE) was used to extract four motor modules from eight muscle activities of the paretic leg during CG. After extraction, each module of four modules was operationally defined by numbering from M1 to M4 according to spatial and temporal features and compared with results reported in a previous study. Results: As a result, an individual with hemiparesis had motor module problems related to difficulty of weight acceptance (module 1), compensation for the weakness of ankle plantar flexor (module 2), a spastic synergistic pattern (module 3) and difficulty with transition from the swing to stance phase (module 4) in terms of spatial features. Also, a delayed activation timing of temporal motor module (module 2) related to the forward propulsion during CG was observed. Conclusions: Gait rehabilitation for the stroke will need to consider clinical significances in respect of the deterioration of motor module and provide the tailored approaches for each gait phase.

  • Lipoma is the most common benign soft tissue tumor. But the parosteal lipoma which occurs in deep tissue is very rare. The authors experienced a case of parosteal lipoma causing posterior interosseous nerve palsy around the proximal radius. A 53-year old male patient, who has motor weakness on right wrist and finger extension for 3 weeks visited. He was diagnosed as a parosteal lipoma causing postrior interosseous nerve palsy of the proximal radius. 6 months after the marginal excision, he was recovered from motor weakness.

  • Hemangioblastoma of the central nervous system rarely occurs in cervicomedullary junction. The unique pathologic features of the tumor involving midline structures are grossly solid in consistency and accompanying extensive spinal cord enlargement. A 63-year-old women presented with progressive right motor weakness and tingling sensation. The MR image showed a well enhancing mass having a cyst and diffuse cord enlargement in the cervicomedullary junction. A total surgical resection was performed and hemangioblastoma was histologicaly verified. Postoperative MR image showed the disappearance of cord enlargement. The right motor weakness was also improved. The authors report a rare case of hemangioblastoma in cervicomedullary junction and the pathophysiology of the spinal cord enlargement are discussed.

  • The carvenous angioma is the secondary common disease which occupies about 8-15% of most central nervous systemic disorders following to the arterio-venous malformation. According to the damaged functional area, the patients experience some sort of motor weakness, seizure or hemorrhage. Nevertheless, general operations and radiologic methods are not considerable in those clinical cases mostly. Typicaliy, the conservative treatments are mainly recommendable choice as the other clinical options have variable side effects like post operative complications. in this case, a 73 year-old male diagnosed as the carvenous angioma, his motor weakness resulted from carvenous angioma has improved with oriental medical treatments. So, the authors report this case with a brief review of related lieratures.

  • Charcot-Marie-Tooth (CMT) disease can be divided mainly into demyelination and axonopathy based on the results of the electrophysiological study. Mitofusin 2, encoded by MFN2 gene, has a crucial role in the fusion of mitochondria, which is known to associate with CMT type 2A as one of the axonal forms. We describe a 44-year-old man with progressive weakness on bilateral legs after noticing foot drop in his early teen. When we examined him at 45 years of age, he presented atrophy on entire legs and with distal muscle weakness on limbs. The nerve conduction study revealed severely decreased amplitude on motor nerve ranging from 0.2 to 4.5 mV, while conduction velocity remained more than 30.4 m/s. The whole-exome sequencing revealed a novel variant c.2228G>T in MFN2 by efficient genetic analysis tool, MutationDistiller. This report will not only expand the mutation spectrum of CMT2A but also introduce a time-saving genetic analysis tool.

  • Motor paralysis by herpes zoster is rare. The incidence of herpetic motor involvement was reported to be between 0.5-31%. This case was reported to evaluate the effects of oriental medicine therapy on vocal cord paralysis and upper limb motor paralysis caused by herpes zoster. The subject was a 48-year-old man who suffering from hoarseness, right upper limb paralysis, right upper limb weakness, right post ear pain, headache. After our treatment, clinical symptoms were improved and VAS score significantly decreased. This study suggests that herbal medicine and acupuncture complex therapy is probably effective in the treatment of vocal cord paralysis and upper limb motor paralysis.
