뇌성마비 아동의 가족중심중재 프로그램이 대동작 기능과 수행력 및 균형에 미치는 영향 (The Effects of Family-Centered Intervention on Gross Motor Function and Performance, Balance in Children with Cerebral Palsy)

  • Purpose : The purpose of this study was conducted to examine the effects of application family-centered intervention, by acknowledging the families of disabled children as experts and maintaining mutually cooperative relation throughout the entire processes of treatment and rehabilitation, on gross motor and balance in children with cerebral palsy. Methods : This study was executed with two group, pre-post test quasi-experimental study design. Among the 24 children with cerebral palsy as the subjects were allocated to the experimental and control group. Both groups were subjected to neuro-developmental treatment, with the experimental group performing family-centered intervention program for 40 minutes, 3 times a week for 12 weeks. Assessments were conducted before intervention, after 6 weeks and 12 weeks of intervention on gross motor function and performance, static and dynamic balance. Results : Although there was significant difference in the gross motor function and performance after 12 weeks of intervention, the mean scores of the experimental group increased more after 12 weeks of intervention than those of the control group with significant difference between the groups. Although there was significant difference in the static and dynamic balance after 12 weeks of intervention, the mean values of the experimental group decreased more after 12 weeks of intervention than those of the control group with significant difference between the groups. Conclusion : Therefore, these results suggest that on family-centered intervention on children with cerebral palsy can be provided as an beneficial and reliable clinical intervention program in development on gross motor and balance.

Development of Computer-based Visual Perception Test Program for Korean Patients with Brain Injury

  • Up to now, several tools to evaluate visual perception tests have been introduced for different purposes. However, they were mostly manual tools, paper books in which the therapists would turn the pages while working together with patients. This paper discusses a potential plan to develop motor-free visual perception test software for Korean patients with brain injury, and to construct a centralized database for their evaluated data in a client/server environment. Through its development, we eventually hope to achieve effective management of the data for better understanding of patients' visual perceptual skills and the standardization of the evaluation for Korean patients. With the help of the computerized environment, we also expect some advantages such as acquisition of reliable results from patients with brain injury, automation for storing and accessing patients' data, construction of the patients' database and the management of a vast amount of the data within it and the provision of a foundation to promote further development of various perceptual-cognitive rehabilitation programs.

Effect of Stem Cell Transplantation on Pain Behavior and Locomotor Function in Spinal Cord Contusion Model

  • Purpose: Many trials for new therapeutic approaches such as stem cell-based transplantation have been conducted to improve the repair and regeneration of injured cord tissue and to restore functions following spinal cord injury (SCI) in animals and humans. Adipose tissue-derived stromal cells (ATSCs) have multi-lineage potential to differentiate into cells with neuron-like morphology. Most studies of stem cell transplantation therapy after SCI are focused on cellular regeneration and restoration of motor function, but not on unwanted effects after transplantation such as neuropathic pain. This study was focused on whether transplantation of ATSCs could facilitate or attenuate hindpaw pain responses to heat, cold and mechanical stimulation, as well as on improvement of locomotor function in a rat with SCI. Methods: A spinal cord injury rat model was produced using an NYU impactor by dropping a 10 g rod from a height of 25 mm on to the T9 segment. Human ATSCs (hATSCs; approximately $5{\times}10^5$ cells) or DMEM were injected into the perilesional area 9 days after the SCI. After transplantation, hindpaw withdrawal responses to heat, cold and mechanical allodynia were measured over 7 weeks. Motor recovery on the Basso, Beattie, and Bresnahan (BBB) locomotor rating scale and on the inclined plane test were also evaluated. Results: The present study demonstrated that increased hindpaw withdrawal responses to cold allodynia was observed in both groups after transplantation, but the development of cold-induced allodynia in the hATSC transplantation group was significantly larger than in the control group. The difference between the two groups in locomotor functional improvement after SCI was also significant. Conclusion: Careful consideration not only of optimal functional benefits but also of unintended side effects such as neuropathic pain is necessary before stem cell transplantation therapy after SCI.

구강운동촉진기술: 1 부-이론적 배경과 기초 요소 (Oral-Motor Facilitation Technique (OMFT): Part I-Theoretical Base and Basic Concept)

  • 서론 : 구강 운동 기능은 태아기에서 유년기 발달로 이어지는 감각 탐색, 음식 섭취, 의사소통 표현의 기초가 되는 기능이다. 구강 운동 기능에 문제가 발행하면, 구강 내 음식 처리 어려움, 삼킴 및 섭식기능 저하, 의사소통 기능 감소, 구강 위생 능력 저하 등이 나타난다. 이러한 증상 치료를 위한 구강운동 치료는 구강 내 감각 정상화, 자세 조절, 구강 움직임 증진, 구강 운동 기능 향상을 목표로 제공되는 치료법이다. 본론 : 구강운동촉진기술(OMFT)은 1) 구강 및 안면 근육계의 해부생리학적 이해; 2) 감각·적응·행동·인지의 통합적 접근; 3) 도수적 접근을 통한 감각 및 운동 자극; 4) 운동 조절과 운동 학습 이론에 기초한 구강운동의 4가지 주요 특징을 바탕으로 전반적이고 통합적인 구강 기능 증진을 목적으로 개발되었다. OMFT는 근거 중심의 새로운 치료 프로토콜로써 아동 및 성인, 신경계 및 근골격계의 구강 기능에 문제를 보이는 모든 이들에게 적용이 가능한 치료 기법이다. 결론 : 본 논문은 OMFT의 개발에 따른 이론적 배경 및 OMFT의 실제적 적용에 따른 기초 요소를 소개하는 것이 목표로, 본 논문을 통해 임상 전문가들이 정확한 이론적 배경과 치료법을 바탕으로 대상자에 맞는 전문적인 치료 제공에 도움이 되기를 바란다.

구강운동촉진기술: 2 부-개념적 위계 및 핵심 기법 (Oral-Motor Facilitation Technique (OMFT): Part II-Conceptual Hierarchy and Key Point Technique)

  • 서론 : 구강운동촉진기술(OMFT)은 감각-운동, 운동 조절, 운동 학습의 이론적 배경을 바탕으로 한 치료기법으로 주요 목표는 구강 운동 기능 증진이다. OMFT에서는 구강 운동 기능의 개념적 위계 및 발달 단계는 5단계이며, 1) 감각-운동 단계, 2) 운동요소 통합 단계, 3) 구조적 움직임 단계, 4) 기능적 구강 운동 단계, 5) 통합적 구강 운동 단계로 구분하였다. 본론 : OMFT는 총 3개의 기법, 10개 범주, 50개 하위 항목으로 구성되어 있다. 자세히 살펴보면, 1) 준비 기법: 감각 인식 및 적응, 치료 적응과 목의 움직임을 통한 준비 과정으로 2범주, 12하위 항목, 2) 핵심기법: 얼굴, 입술, 볼, 잇몸, 혀, 턱 등의 구강 구조물에 치료사의 직접적인 자극을 통한 구강 운동 기술 촉진과 씹기 기능 증진을 목적으로 7개 범주, 30개 하위 항목, 3) 응용기법: 음식 섭취와 삼킬 때 도움을 줄 수 있는 치료적 접근법 1개 범주, 8개 하위 항목으로 구성된다. 결론 : 본 논문은 OMFT의 실제적 적용을 위한 구강 관련 구조물의 기능 증진을 목표로 하는 준비 기법과 핵심 기법 및 섭식을 포함한 구강 기능의 종합적인 접근을 고려한 응용 기법에 대하여 소개하는 것을 목적으로 한다.

키넥트 센서를 이용한 자기통제 피드백이 가능한 가정용 훈련프로그램 개발 (Development of Home Training System with Self-Controlled Feedback for Stroke Patients)

  • 뇌졸중 환자들은 운동장애, 감각장애, 인지장애 등 다양한 후유증을 경험하게 되며 이를 극복하기 위하여 많은 재활훈련을 받게 된다. 이러한 재활훈련은 병원에서만 이루어 질 때 보다 일상 속에서 지속적으로 수행될 때 그 효과가 극대화 된다. 이를 위해서 다양한 가정용 훈련프로그램이 보급되었지만 이를 수행함에 있어서 적절한 피드백에 제공되어 지는 프로그램 부족한 실정이다. 따라서 본 연구에서 3차원 깊이 측정이 가능한 키넥트 센서를 이용하여 사용자에게 필요한 피드백을 제공 받을 수 있는 가정용 운동프로그램을 개발하고자 한다. 본 연구에서 제안된 프로그램은 사용자가 원할 때만 수행지식(knowledge of performance; KP) 피드백과 결과지식(knowledge of result; KR) 피드백을 선택적으로 제공하며 이러한 사용자 주도적인 선택적 피드백은 사용자의 운동효과를 극대화 시킬 것이다.

뇌성마비 아동에서 대동작기능평가(GMFM)와 보행의 시공간적 변수와의 관계 (The Relevance Between Gross Motor Function Measurement (GMFM) and the Spatiotemporal Parameters of Gait in Children With Cerebral Palsy)

  • This paper presents the relevance between GMFM and the spatiotemporal parameters of gait in children with cerebral palsy. Twenty-one children ($73.11{\pm}30.06$ months) with cerebral palsy participated in this study. GMFM was performed and spatiotemporal parameters of gait were measured by foot print gait analysis. A correlation analysis was used to investigate the correlation between GMFM scores and spatiotemporal parameters of gait. A linear regression analysis was employed to find how much each gait spatiotemporal parameters could be predicted from GMFM scores. The total GMFM scores was significantly correlated with walking speed, cadence, and stride length. Dimensions D (standing) and E (walking, running, and jumping) were more significantly correlated with gait spatiotemporal parameters than dimensions A (lying and rolling), B (sitting), and C (crawling and kneeling). The GMFM scores were useful for predicting spatiotemporal parameters. However, it is difficult to predict the status of gait development using GMFM scores because GMFM scores and gait spatiotemporal parameters are only measured as quantities not qualities. In the field, it is easily found that many children with cerebral palsy are unable to walk in any way. Consequently, gait analysis cannot be performed in many cases. Therefore, it is more reasonable to investigate the influence of GMFM on spatiotemporal parameters, rather than vice versa.

한글판 아동 균형척도(Pediatric Balance Scale)의 측정자간 신뢰도 (Inter-Examiner Reliability of the Korean Version of the Pediatric Balance Scale)

  • The Pediatric Balance Scale (PBS) is a modified Berg's Balance Scale developed as a balance measure for school-aged children with mild to moderate motor impairments. The purpose of this study was to determine the inter-examiner reliability of the Korean version of PBS when applied to children with developmental delays. In this study, PBS was administrated to a total of 79 children with developmental delays (17 with global developmental delay, 31 with cerebral palsy, and 31 with mental retardation) in the Seoul Community Rehabilitation Center. Two pediatric trained physical therapists with longer than 13 year of clinical experience scored the children's performance blind, while replaying videotaped data. The inter-examiner reliability was statistically determined by intraclass correlation coefficients (ICCs). The results of this study revealed that the Korean version of PBS seems to have high inter-examiner reliability when applied to children with movement disorders such as global developmental delay (ICC=.96) and cerebral palsy (ICC=.97); however, it has relatively lower inter-examiner reliability (ICC=.78) for children with developmental delay secondary to mental retardation. therefore, the results support that the Korean version of the PBS could be a useful clinical measurement to assess the balance skills for children with developmental delay who have an adequate level of cognition to enable them to fol1ow the verbal instructions to complete the test.

고유수용성 신경근 촉진법이 편마비 환자의 균형능력에 미치는 영향 (The Effect of Proprioceptive Neuromuscular Facilitation on Balance Ability in Patients with Hemiparetic)

  • Purpose : The aim of this study was to determine the effect of proprioceptive neuromuscular facilitation(PNF) on balance ability in poststroke hemiparetic subjects. Methods : The subjects of this study were 12 patients with hemiplegia who volunteered to participate in the experiment which was carried out over the course of 4 weeks. The Proprioceptive Neuromuscular Facilitation is applied to group with three positions (sidelying, half-standing, modified plantigrade). The tests between before and after the intervention were measured by FSST (Four Square Step Test), FICSIT-4 (Frailty and Injuries: Cooperative Studies of Intervention Techniques), BBS(Berg Balance Scale). The data were analyzed using paired t-test and Wilcoxon signed rank test to determine the statistical significance. Results : The results of this study were summarized as follows: 1. After intervention, the score of BBS and FICSIT-4 significantly were increased compared with before intervention. 2. After intervention, the time of FSST were reduced significantly compared with before intervention. Conclusions : According to above results, Proprioceptive Neuromuscular Facilitation improved with balance ability of patients with hemiplegia. This study provided basic data for effects Proprioceptive Neuromuscular Facilitation on balance ability. In conclusion, Proprioceptive Neuromuscular Facilitation with other interventions is effective way to improve balance ability of patients with hemiplegia.

Epigenetic Regulation in the Brain after Spinal Cord Injury : A Comparative Study

  • Objective : After spinal cord injury (SCI), functional and structural reorganization occurs at multiple levels of brain including motor cortex. However, the underlying mechanism still remains unclear. The current study was performed to investigate the alterations in the expression of the main regulators of neuronal development, survival and death, in the brain following thoracic contusive SCI in a mouse model. Methods : Eight-week-old female imprinting control region mice (n=60; 30-35 g) were used in this study. We analyzed the expression levels of regulators such as brain-derived neurotrophic factor (BDNF), glial cell line-derived neurotrophic factor (GDNF), nerve growth factor (NGF) and histone deacetylase (HDAC) 1 in the brain following thoracic contusive SCI. Results : The expression of BDNF levels were elevated significantly compared with control group at 2 weeks after injury (p<0.05). The expression of NGF levels were elevated at 2, 4 weeks compared with control group, but these difference were not significant (p>0.05). The GDNF levels were elevated at 2 week compared with control group, but these differences were not significant (p>0.05). The difference of HDAC1 levels were not significant at 2, 4 and 8 weeks compared with control group (p>0.05). Conclusion : These results demonstrate that the upregulation of BDNF may play on important role in brain reorganization after SCI.
